1.Efficacy and safety of ruxolitinib in the treatment of myelofibrosis

OBJECTIVE To explore the efficacy and safety of ruxolitinib in the treatment of myelofibrosis (MF). METHODS A retrospective collection of data was conducted on 42 MF patients who were treated with ruxolitinib in a standardized manner for more than 6 months in the Third People’s Hospital of Bengbu from September 2018 to April 2024. The clinical symptom scores, spleen size reduction, and MF grading of the patients before and after treatment were analyzed. Additionally, the occurrence of adverse reactions with a causality assessment result of “definite”“probable” or “possible” was recorded. The patients’ survival status was followed up. RESULTS After 6 months of treatment, both clinical symptom scores and the total score were significantly decreased than before treatment (P<0.05). The length and thickness of the spleen were significantly shorter than before treatment (P<0.05). MF classification in 5 patients decreased by 1 level compared with baseline, 1 case was level 2 and dropped to level 0, 14 patients remained stable. The main adverse reactions were anemia (26 cases), thrombocytopenia (14 cases), infection (11 cases), and gastrointestinal discomfort (9 cases). Thirty-nine patients survived, with a survival rate of 92.86%. CONCLUSIONS Ruxolitinib can effectively improve the clinical symptoms of patients with MF, shrink the spleen, stabilize and even improve MF grading, and holds promise for bringing long-term survival benefits to MF patients. Adverse reactions are mainly anemia, thrombocytopenia, infection and gastrointestinal discomfort.
2.Efficacy of metoprolol versus ivabradine in treatment of POTS in elderly patients after COVID-19 infection

Objective To explore the difference in efficacy of metoprolol versus ivabradine in the treatment of postural orthostatic tachycardia syndrome(POTS)in the elderly after COVID-19 infection.Methods A total of 110 patients diagnosed with POTS at our department from Decem-ber 1,2022 to January 31,2023 were included.According to their drug regimen,they were divided into metoprolol group(62 patients)and ivabradine group(48 patients).On the 28th day of out-patient follow-up,the resting heart rate,heart rate of 10 min of standing,symptom disappearance rate,hospitalization rate,and mortality rate were compared between the two groups.Results On the 28th day of treatment,the resting heart rate and postural heart rate for 10 min were decreased in both groups when compared with the levels at initial diagnosis(P<0.01).And there were no significant differences in the two types of heart rate between the two groups on the 28th day(71.0±7.0 vs 72.1±7.0,P=0.401;76.5±7.2 vs 77.4±7.6,P=0.573).No obvious differences were observed between the two groups in symptom disappearance rate,hospitalization rate,or mortality rate(88.7%vs 89.6%,3.2%vs2.1%,0%vs 0%,P>0.05).Conclusion Metoprolol and ivabradine can effectively treat POTS in the elderly patients after COVID-19 infection.
3.Posttraumatic growth status and latent profile analysis of nurses at psychiatric department

Objective To explore the current status of posttraumatic growth (PTG) among nurses at psychiatric department and analyze its latent profiles and population characteristics. Methods A total of 357 nurses from psychiatric departments of five tertiary Grade A hospitals were selected as the research subjects using the convenience sampling method. The PTG and professional quality of life were studied using the Posttraumatic Growth Inventory and the Chinese version of the Compassion Fatigue Scale. Results The PTG score of the nurses was (56.6±23.2). The scores of compassion satisfaction, burnout, and secondary traumatic stress among nurses were (32.6±7.2), (26.9±5.9), and (26.0±5.4), respectively. The result of potential profile analysis showed that the nurses could be divided into three latent profiles based on PTG levels: low PTG group (34.4%), moderate PTG group (44.0%), and high PTG group (21.6%). The results of multinomial logistic regression analysis showed that the nurses who slept 7-8 hours per day were at higher risk of being in the high PTG group compared with those who slept more than eight hours per day (P<0.05). Psychiatric nurses who took regular exercise were at higher risk of being in the high PTG group compared with those who took irregular exercise (P<0.05). The nurses who had high job satisfaction scores were at higher risk of being in the high PTG group compared with those who had low job satisfaction scores (P<0.01). The nurses with higher compassion satisfaction scores increased the risk of being in the high PTG group compared with those with lower compassion satisfaction scores (P<0.01). The nurses with higher burnout scores increased the risk of being in the low PTG group compared with those with lower burnout scores (P<0.01). Conclusion The PTG characteristics of the nurses exhibit heterogeneity and can be categorized into three distinct profiles. Sleep duration, regular exercise, job satisfaction, compassion satisfaction, and burnout are influencing factors for the PTG latent profiles of nurses working at psychiatric department.
4.Comfort efficacy of gene-transfected pig skin in arc burn patients

Objective To observe the comfort effect of gene-transfected pig skin in patients with arc burn wound.Methods A total of 56 patients with electric arc burn treated in our hospital from June 2020 to December 2022 were enrolled and then randomly divided into treatment group and control group,with 28 cases in each group.Both groups were treated with debridement,including removing pollutants and necrotic skin,and cleaning the wounds with normal saline.After debridement,the shallow second-degree burn wounds in the control group were covered with silver ion functional antibacterial dressing and wrapped with sterile cotton pad,while those in the treatment group were covered with thawed and softened gene-transfected pig skins.For the deep second-degree burn wounds,after simple debridement,the wounds were treated with scab grinding in 48 h after burn when the condition of the patients was stable.After this,the wounds in the control group and the treatment group were treated as the shallow wounds,respectively.Visual Analogue Scale(VAS),State Anxiety Inventory(SAI)score,treatment process satisfaction,patient comfort and time for wound healing were observed and compared between the 2 groups.Results There was no significant difference in the VAS score and SAI score between the 2 groups at the first dressing change(P>0.05),but the VAS score and SAI score were obviously lower in the treatment group than the control group at the second and third dressing changes(P<0.05).The treatment group obtained notably better comfort level and shorter time for wound healing than the control group(P<0.05).Conclusion Gene-transfected pig skin shows good efficacy in the wound surface of arc burn,which can shorten the wound healing time,reduce the pain of dressing change and improve the satisfaction and comfort of patients.
5.Development and reliability and validity tests of Oral Health Caring Ability Assessment Scale for Caregivers of Infants and Young Children

Objective To develop an Oral Health Caring Ability Assessment Scale for Infants and Young Children Caregivers and test its reliability and validity,so as to provide a scientific tool for assessing the oral health caring ability of them.Methods Based on the IMB Skills Model,the initial version of the scale was formed through literature review,expert consultation,and pre-investigation.From May to July 2023,a total of 218 caregivers of infants and young children who were treated in the child health department of a tertiary A specialized hospital in Qingdao were selected by convenience sampling for project analysis and reliability and validity test.From August to September 2023,we selected 330 caregivers of infants and young children who were treated in the child health department of 3 tertiary A general hospitals in Shandong and Hunan Provinces by convenience sampling to further verify the stability of the scale structure.Results The Oral Health Caring Ability Assessment Scale for Caregivers of Infants and Young Children included 4 dimensions,including feeding knowledge,oral cleaning and examination knowledge,oral health caring attitude and oral health caring behavior skills,with a total of 27 items.According to exploratory factor analysis,4 common factors were extracted and the cumulative variance contribution rate was 62.022%.The Cronbach's α coefficient of the questionnaire was 0.938;the split-half reliability was 0.774;the test-retest reliability was 0.990.The content validity index at the scale level was 0.957,and the content validity index at the item level was 0.833-1.000.Confirmatory factor analysis showed that the model fitted well and the factor structure of the scale was stable.Conclusion The Oral Health Caring Ability Assessment Scale for Caregivers of Infants and Young Children has good reliability and validity,which can be used to assess the level of oral health caring ability for caregivers of infants and young children accurately.
6.Establishment of chimeric rabbits with FOXN1 gene knockout

Objective To establish F0 generation chimeric rabbits with FOXN1 gene knockout and explore method for the in vivo conservation of immunodeficient rabbits in a conventional housing environment.Methods Initially,CRISPR/Cas9 technology was employed to inject constructed sgRNA and Cas9 protein into a single cell from rabbit two-cell stage embryos to obtain chimeric embryos with FOXN1 gene editing.The embryos were subsequently transferred into surrogate does.Finally,the F0 generation offsprings were genotyped using PCR and Sanger sequencing,and their growth and development in a conventional housing environment were observed.Results The PCR and Sanger sequencing result confirmed the successful establishment of himeric rabbits with FOXN1 gene knockout.On observation,the chimeras exhibited normal growth and development in a conventional environment without any immunodeficient phenotypes.Conclusions This study established a preliminary chimeric rabbit model with FOXN1 gene knockout that grows and develops normally in standard laboratory environments.This lays the foundation for the further breeding of FOXN1 immunodeficient rabbits in the future.
7.Performance and application of droplet digital polymerase chain reaction in rapid diagnosis of bloodstream infections

Objective To validate the performance of droplet digital polymerase chain reaction (ddPCR) reagents using clinical and standard strains, and to evaluate the effectiveness and practicability of ddPCR technology in clinical applications. Methods The concordance rate, specificity, precision, and lower limit of detection of the ddPCR kit were validated using clinical and standard strains. Blood samples from 74 patients with suspected bloodstream infections were collected, and both ddPCR and blood culture methods were used to determine the pathogens in the patient's blood samples. Results The average detection time of ddPCR for pathogens of bloodstream infection was 3.5 hours, which was able to complete the detection of over a dozen common pathogens simultaneously. The concordance rate, specificity, precision, and lower limit of detection of the ddPCR kit for bloodstream infection pathogens all met clinical requirements. Among the 74 patients with suspected bloodstream infections, the positive detection rate using the ddPCR method was 64.86%, while was 40.54% using blood culture, with a statistically significant difference (
8.The mediating and moderating effects of job crafting and proactive personality between head nurse empowerment and nurse voice behavior

Objective:To explore the influence of head nurse empowerment on nurses′ voice behavior and the mediating and moderating effects of nurses′ job crafting and proactive personality.Methods:A total of 805 nurses from the 4 grade A hospitals in Shandong province from February to March 2022 were selected by convenience sampling method, the Nurse Managers′ Empowering Behavioral Scale For Staff Nurses, Job Crafting Scale, Voice Behavior Scale and Proactive Personality Scale were used for cross-sectional survey. The mediating effect was analyzed by SPSS 26.0 and Process 3.4 macro.Results:The total score of nurses′ perception of head nurse empowerment was (4.01 ± 0.57) points, the total score of nurses′ job crafting was (4.19 ± 0.66) points, the total score of nurses′ voice behavior was (4.12 ± 0.62) points, and the total score of nurses′ proactive personality was (6.07 ± 0.90) points. Head nurse empowerment, nurse job crafting, voice behavior and proactive personality were positively correlated with each other ( r values were 0.483-0.824, all P<0.01). The mediating effect of job crafting was 0.541, accounting for 86.4% of the total effect; proactive personality had a positive moderating effect on the first half path of the mediation model ( ? R 2=0.163, P<0.01). Conclusions:Head nurse empowerment influenced voice behavior of nurses directly and indirectly through the mediating role of job crafting, proactive personality can moderate the effect of head nurse empowerment on nurse job crafting. It is suggested that the head nurse appropriately delegate power to the nurses and actively authorize them, identify nurses with high proactive personality, stimulate their job remodeling and voice behavior, and improve the quality of nursing work.

Hip fractures are among the most common fractures in the elderly, presenting to be a leading cause of disability and mortality. Surgical treatment is currently the main treatment method for hip fractures. The incidence of perioperative malnutrition is increased after hip fractures in the elderly due to the comorbidities, decreased basal metabolic rate, accelerated protein breakdown, weakened anabolism and surgical stress. However, malnutrition not only increases the incidence of postoperative complications, but also leads to increased mortality, indicating an important role of perioperative nursing management of nutrition for the elderly patients with hip fractures. At present, there still lacks scientific guidance and application standards on perioperative nursing management of nutrition for the elderly patients with hip fractures. Therefore, the Orthopedic Nursing Committee of Chinese Nursing Association and the Editorial Board of Chinese Journal of Trauma organized relevant experts to formulate the Expert consensus on perioperative nursing management of nutrition for elderly patients with hip fractures ( version 2023) according to evidence-based medical evidences and their clinical experiences. Fourteen recommendations were made from aspects of nutrition screening, nutrition assessment, nutrition diagnosis, nutrition intervention and nutrition monitoring to provide guidance for perioperative nursing management of nutrition in elderly patients with hip fractures.

Objective To explore the effects of warming-yang and tonifying-qi needling combined with electromyographic biofeedback therapy(EMGBFT)on brain image structure,surface myoelectric characteristics and neurotransmitters in patients with stroke rehabilitation.Methods A prospective research method was conducted in which 200 stroke rehabilitation patients admitted to the Third Hospital of Xingtai City from February 2021 to February 2022 were selected as the study subjects.According to the random principle,the patients were divided into a control group and an study group,with 100 cases in each group.Both groups received routine treatment for stroke,while the control group received a combination of EMGBFT.The study group received a combination of warming-yang and tonifying-qi acupuncture based on the control group,and both groups continued to receive treatment for 9 weeks.Observe the clinical efficacy of two groups of patients and compare the differences in National Institutes of Health Stroke Scale(NIHSS)scores,traditional Chinese medicine symptom scores,brain image structures,serum neurotransmitter levels,and surface electromyography levels before and after treatment,and observe the occurrence of adverse reactions.Results The effective rate of treatment in the study group was significantly higher than that in the control group(97.00% vs.87.00%,P<0.05).After treatment,NIHSS score,traditional Chinese medicine symptom score,root-mean-square(RMS)of biceps and triceps,and synergistic contraction rate(SCR)were significantly lower than those before treatment in both groups,and the levels of cerebral blood flow(CBF)in the thalamus and frontal lobe,fractional anisotropy(FA),norepinephrine(NE),5-hydroxytryptamine(5-TH),and dopamine(DA)were significantly higher than those before treatment.After treatment,the NIHSS score(4.18±1.09 vs.6.89±1.54),traditional Chinese medicine symptom score(5.41±1.08 vs.9.46±1.55),and biceps RMS(μV:9.76±3.51 vs.16.36±3.44),triceps brachii RMS(μV:6.79±1.83 vs.10.61±2.87),and SCR[(28.08±8.73)% vs.33.08±9.31)%]were significantly lower than those control group(all P<0.05),the CBF of the thalamus(mL·kg-1·min-1:278.97±86.32 vs.233.63±84.62),and the CBF of the frontal lobe(mL·kg-1·min-1:299.31±75.54 vs.262.81±87.18),FA(0.57±0.18 vs.0.48±0.14),serum 5-HT(ng/L:352.83±38.93 vs.306.71±32.54),NE(ng/L:160.83±17.25 vs.122.81±12.41),DA(μg/L:9.23±0.92 vs.7.36±0.71)were significantly higher than those of the control group(all P<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group(3.00% vs.14.00%,P<0.05).Conclusion Based on EMGBFT,the combination of warming-yang and tonifying-qi acupuncture can synergistically improve the clinical symptoms and brain image structure in stroke rehabilitation patients,and increase their serum neurotransmitter levels,with fewer adverse reactions,which is worthy of promotion.
