1.Expression of Cellular Receptors in the Ischemic Hemisphere of Mice with Increased Glucose Uptake

Many previous studies have shown reduced glucose uptake in the ischemic brain. In contrast, in a permanent unilateral common carotid artery occlusion (UCCAO) mouse model, our pilot experiments using 18F-fluorodeoxyglucose positron emission tomography (FDG PET) revealed that a subset of mice exhibited conspicuously high uptake of glucose in the ipsilateral hemisphere at 1 week post-occlusion (asymmetric group), whereas other mice showed symmetric uptake in both hemispheres (symmetric group). Thus, we aimed to understand the discrepancy between the two groups. Cerebral blood flow and histological/metabolic changes were analyzed using laser Doppler flowmetry and immunohistochemistry/Western blotting, respectively. Contrary to the increased glucose uptake observed in the ischemic cerebral hemisphere on FDG PET (p<0.001), cerebral blood flow tended to be lower in the asymmetric group than in the symmetric group (right to left ratio [%], 36.4±21.8 vs. 58.0±24.8, p=0.059). Neuronal death was observed only in the ischemic hemisphere of the asymmetric group. In contrast, astrocytes were more activated in the asymmetric group than in the symmetric group (p<0.05). Glucose transporter-1, and monocarboxylate transporter-1 were also upregulated in the asymmetric group, compared with the symmetric group (p<0.05, respectively). These results suggest that the increased FDG uptake was associated with relatively severe ischemia, and glucose transporter-1 upregulation and astrocyte activation. Glucose metabolism may thus be a compensatory mechanism in the moderately severe ischemic brain.

Transcervical resection of endometrium is under-utilized in the treatment of dysfunctional uterine bleeding, uterine myoma and menorrhagia. The procedure is similar to transurethral resection of prostate in men with a possibility of substantial absorption of irrigation fluid. Absorption of a large volume of fluid can cause excessive intravascular volume, hyponatremia, cerebral edema and death. Severe hyponatremia leading to extrapontine myelinolysis is an extremely rare complication of this procedure. So, We report a case of developed extrapontine myelinolysis after hysteroscopic myomectomy which, however, showed complete recovery.

3.A Case of Retroperitoneal Liposarcoma Presenting as Pelvic Mass.

Liposarcoma is the most common soft tissue sarcoma in adults and accounts for 9.8% to 16% in all soft tissue sarcomas. The common sites include limbs, buttocks, and retroperitoneum, et al. The early diagnosis of retroperitoneal liposarcoma may be difficult because of due to the late onset of symptoms and the tumors are frequently noted in a large size with the involvement of adjacent structures. Surgical therapy remains the most effective modality for the treatment of retroperitoneal liposarcoma. But, complete surgical resection of these tumors is often challenging and, at times, may be impossible. So, adjuvant radiation therapy is necessary for the improvement of prognosis in many cases. We report a case of retroperitoneal liposarcoma which was treated with complete surgical resection followed by adjuvant radiation therapy.

Female phenotype of a 46,XY male may originates from male pseudohermaphroditism due to 17alpha-hydroxylase deficiency. Lack of cortisol increases adrenocorticotropic hormone (ACTH) and mineralocorticoid production, leading to low renin hypertention and hypokalemia. A 41-year-old phenotypic female presented primary amenorrhea and hypertension. In the hormonal profile, the levels of serum estradiol, testosterone, rennin, and cortisol were decreased and ACTH and deoxycorticosterone were increased. Laparoscopic bilateral gonadectomy was performed, and corticosteroid, antihypertensive drugs, and estrogen were administered. We report this case with a brief review of the literatures.

5.Hydrohemothorax and Subclavian Artery Laceration during Internal Jugular Vein Cannulation: A case report.

Central venous cannulation via an internal jugular vein has become a common procedure in the management and monitoring of severely ill patients. Although complications such as carotid artery puncture or pneumothorax have been reported, hemothorax is relatively uncommon. We describe a case of hydrohemothorax and subclavian artery laceration which occurred during right internal jugular vein cannulation. A 44-year-old female patient was admitted for laparoscopic adrenalectomy under general anesthesia. For central venous pressure monitoring, central venous cannulation performed. However, dilator overinsertion injured the right subclavian artery. This led to a massive ipsilateral hydrohemothorax requiring thoracotomy for subclavian artery repair. This case suggests that central venous cannulation should be done carefully and improves awareness of the potential for dilator induced injury.

6.Change of Blood Pressure Variability before and during Sevoflurane Anesthesia.

BACKGROUND: Relatively little is known about the effects of general anesthesia on blood pressure variability (BPV). This study was designed to evaluate the changes of high frequency (HF) and low frequency (LF) of BPV before and during general anesthesia with sevoflurane. METHODS: Beat-to-beat blood pressure was recorded at conscious baseline and during general anesthesia at 2% end-tidal sevoflurane in 17 healthy living-liver transplantation donors. BPV estimated by power spectra of systolic (SBP) and mean blood pressure (MBP) was calculated. RESULTS: Both LF power of SBP and MBP were diminished to 96.2% and 97.1% during sevoflurane anesthesia (5.5 +/- 2.8 to 0.2 +/- 0.2 mmHg2, 6.6 +/- 3.7 to 0.2 +/- 0.2 mmHg2, P < 0.001 for both). However, there were no significant changes of HF power of SBP and MBP during sevoflurane anesthesia. CONCLUSION: Sevoflurane anesthesia reduced noticeably LF power, which was associated with sympathetic vasomotor activity, but not HF power, which represents mostly the mechanical effect of respiration on blood pressure, of BPV.

7.The Effect of Low-dose Atropine on Baroreflex Sensitivity Assessed by Transfer Function Analysis.

BACKGROUND: The arterial baroreflex is a key mechanism for maintaining blood pressure homeostasis. Low-dose atropine (LDA) causes bradycardia, either by acting on the sinoatrial node or due to its effect on central muscarinic receptors, which increases vagal activity. We evaluated the effect of LDA on baroreflex sensitivity (BRS) in healthy awake subjects. METHODS: We assessed changes in RR interval (RRI) and systolic blood pressure (SBP), power spectral densities of heart rate variability (HRV) and systolic blood pressure variability (SBPV), and spontaneous BRS by using transfer function analysis before and after LDA (2microgram/kg) in 17 healthy volunteers. RESULTS: LDA induced not only bradycardia but also increased of the high-frequency (HF) component of HRV, RMSSD (root mean squared successive difference interval), and pNN50 (percentage of sinus cycles differing from the preceding cycle by > 50 ms). The HF and LF components of SBPV remained unchanged. Spontaneous BRS determined by transfer function analysis increased significantly (P < 0.05), and changes in BRS were significantly associated with changes in the HF component of HRV (P < 0.05). CONCLUSIONS: LDA increased vagal cardiac function and arterial baroreflex in awake subjects. This result suggests that increased vagal cardiac function by LDA application is related to baroreflex increase.

Ewing sarcoma is an uncommon bone malignancy of childhood. Although Ewing sarcoma is mostly a tumor of bone, it may also arise from soft tissues (extraskeletal Ewing sarcoma). We report a case of extraskeletal Ewing sarcoma that arose in the retroperitoneum of a 18-month-old girl and presented with right leg pain and gait disturbance. 10.Multiple Rhabomyomas in left Ventricular Outflow Tract Combined with Tuberous Sclerosis: A Case Report.

Cardiac rhabdomyoma is a rare type of benign tumor affecting the heart. There are a few previous reports of intracardiac rhabdomyomas causing ventricular arrythmia. We describe a 1-year-old female tuberous sclerosis patient who was presented with a ventricular tachycardia. Diagnostic echocardiography revealed two masses in the left ventricular outflow tract originating from the ventricular septum. The masses were surgically resected through aortotomy using cardiopulmonary bypass and the masses appeared benign. The pathology was that of a cardiac rhabdomyomas. Postoperative course was uneventful and the ventricular tachycardia was controlled.
