Todo Yoshimasu was one of the Kampo doctors of the Koho school in the Edo period, and created the basic structure of the modern traditional Japanese Kampo medicine. His achievements are still recognized today. His oldest son, Nangai Yoshimasu, is also credited with inheriting and developing the medical techniques established by his father. Todo’s descendants continued to be involved in the medical profession. In this paper, we first report that the building of the Yoshimasu clinic still exists in Tsuwara, Kurayoshi City, Tottori Prefecture, where Shiho Yoshimasu, the fifth-generation descendant of Todo, and his son Ryozo practiced medicine. We also found the tombstones there for Tetsutaro (Shiho), Yutaro, Ryozo, Tamenori, and Teruo of the Yoshimasu family. In conclusion, one of the origins of the Yoshimasu’s, a well-known family of doctors in Japan, is in Tottori. Therefore, Tsuwara, Kurayoshi City, Tottori Prefecture is an important place to describe Todo Yoshimasu. Here, we revealed the Yoshimasu family in Tottori based on new materials.

Purpose:
The study aimed to compare the diagnostic performance of washout-parametric imaging (WOPI) with that of conventional contrast-enhanced ultrasound (cCEUS) in differentiating focal liver lesions (FLLs).
Methods:
A total of 181 FLLs were imaged with contrast-enhanced ultrasound using Sonazoid, and the recordings were captured for 10 minutes in a prospective setting. WOPI was constructed from three images, depicting the arterial phase (peak enhancement), the early portal venous phase (1-minute post-injection), and the vasculo-Kupffer phase (5 or 10 minutes post-injection). The intensity variations in these images were color-coded and superimposed to produce a single image representing the washout timing across the lesions. From the 181 FLLs, 30 hepatocellular carcinomas (HCCs), 30 non-HCC malignancies, and 30 benign lesions were randomly selected for an observer study. Both techniques (cCEUS and WOPI) were evaluated by four off-site readers. They classified each lesion as benign or malignant using a continuous rating scale, with the endpoints representing "definitely benign" and "definitely malignant." The diagnostic performance of cCEUS and WOPI was compared using the area under the receiver operating characteristic curve (AUC) with the DeLong test. Interobserver agreement was assessed using the intraclass correlation coefficient (ICC).
Results:
The difference in average AUC values between WOPI and cCEUS was 0.0062 (95% confidence interval, -0.0161 to 0.0285), indicating no significant difference between techniques. The interobserver agreement was higher for WOPI (ICC, 0.77) than cCEUS (ICC, 0.67).
Conclusion
The diagnostic performance of WOPI is comparable to that of cCEUS in differentiating FLLs, with superior interobserver agreement.

8.Home Care in the Era of COVID-19 —Results from the Bereaved Families of Terminal Cancer Patients Survey—

The purpose of this study was to examine the impact of COVID-19 on home care at the end of life and the satisfaction of bereaved families. A questionnaire survey was conducted on 100 bereaved families of terminal cancer patients who were receiving home care. The effects of the COVID-19 on at-home medical treatment and the rate of satisfaction of bereaved families were examined. The response rate for this survey was 72.0%. Of the respondents, 52.8% of the bereaved families answered that the COVID-19 had an effect on their decision to choose home care. The rate of satisfaction of bereaved families was 98.6%. Even for terminal cancer patients who chose home care in the era of COVID-19 at our hospital, we were able to achieve high level of satisfaction for bereaved families.
9.Chronic Encapsulated Intracerebral Hematoma Associated with Cavernous Malformation.

Chronic encapsulated intracerebral hematoma (CEIH) is a rare cerebrovascular disease that behaves as a slowly expanding lesion with a gradual onset. It is well established that CEIH is associated with arteriovenous malformations; however, CEIH associated with cavernous malformation (CM) is extremely rare. We herein report a case of CEIH associated with CM, and discuss its pathogenesis. A 12-year-old female was admitted to our hospital because of a one week history of progressive headache and nausea. Brain computed tomography scan and magnetic resonance imaging showed an intracerebral hematoma surrounded by edema in the right frontal lobe. One week later, her headache and nausea worsened, and a brain computed tomography scan revealed the enlargement of hematoma. A right frontal craniotomy was performed. The capsule, mass, and hematoma were totally removed. Histological examination confirmed the diagnosis of CEIH associated with CM. Immunohistochemical analysis revealed increased expression of vascular endothelial growth factor (VEGF) and the VEGF receptor-1 in the endothelium and fibroblasts. Our findings suggest that the activated VEGF pathway might have positively contributed to development of CEIH in the present patient.

10.Effect of Traditional Japanese Herbal (Kampo) Medicine, Goreisan, for Disdialysis and Muscle Cramps in Patients undergoing Maintenance Hemodialysis

Goreisan, a well-known hydrostasis modulating formula, has been traditionally used against edema, thirst, spontaneous sweating, and oliguria. Patients undergoing maintenance hemodialysis (HD) have a condition of chronic water metabolism abnormality. Acute changes in water metabolism due to HD therapy frequently cause disdialysis syndrome (including muscle cramps), therefore, the prevention and treatment of disdialysis syndrome is of importance. In this study, 20 patients with history of disdialysis syndrome undergoing HD were administered goreisan (5.0 g/day) and its effect was investigated. After 8 weeks of goreisan therapy, diagnostic suitai syndrome criteria (pre-treatment 40.9 ± 8.7, post-treatment 33.2 ± 7.8 points), total 10% NaCl dosages (20 mL/syringe) to treat muscle cramps during HD (pre 2.9 ± 0.6, post 1.9 ± 0.6 syringes/week), serum brain natriuretic peptide (BNP) concentrations after HD (pre 165.4 ± 48.3, post 148.4 ± 39.0 pg/mL), and systolic blood pressures (pre 90.8 ± 15.0, post 100.1 ± 13.5 mmHg) indicted statistical improvement (p < 0.01).These effects were observed not only in patients with no diabetes mellitus (DM), but also in patients with DM.These results indicate the possibility that goreisan is effective for prevention of disdialysis syndrome and muscle cramps in patients under HD.
