1.Relief Status of Persons Injured by COVID-19 Vaccination under the Immunization Health Damage Relief Program

Objective: The Immunization Health Damage Relief Program was established to provide "prompt and wide-ranging" relief for health damage caused by routine vaccination.Methods: In this study, utilizing the report of the Subcommittee on Infectious Diseases and Immunization, we investigated the relief status of persons who received the new coronavirus vaccine. The study period was three years, from 2021 to 2023, and the number of cases accepted, the number of certifications, the number of denials, and the number of delays were analyzed.Results: Most of the claims were for medical expenses and medical benefits, and the remaining claims were for lump-sum death benefits and funeral expenses. The approval rate was 84.9%, and the denial rate was 14.1%. The scrutiny rate was less than 30% since July 2022, but with the establishment of a new study group on January 2023, the rate rose to approximately 70%. Initially, none of the lump-sum death payments were approved, but later the approval rate increased, and there was a period when almost all the lump-sum death payments were approved.Conclusions: The Ministry of Health, Labor and Welfare (MHLW) is responding as quickly as possible to the increasing number of applications, but the number of health relief applications is unprecedented in the history of vaccines. For the victims and their families, the situation does not appear to be satisfactory.
2.Regarding the Relief Status for HPV Vaccine Side Effects under Immunization Health Damage Relief Program

Immunization Health Damage Relief Program is a system created with the aim of providing “quick and wide-ranging” relief for health damage caused by routine vaccinations. In this study, we investigated the current status of payments awarded for side effects of the human papilloma virus (HPV) vaccine over the 10-year period from 2013 to 2023. We obtained the data for this study from reports of the subcommittees of the Vaccination Health Damage Relief System, such as the results of deliberations by the Infectious Diseases and Immunization Review Subcommittee. The certification rate under this system was approximately 50%, which is the same as the certification rate of the Adverse Drug Reaction Relief System. The types of diseases and disorders reported in the system were diverse and had multiple symptoms. The number of applications increased in the final year of the study because of the recommendation for reopening the system for applications in 2022.
3.Peroral cholangioscopy for the evaluation of bile duct stricture in hepatocellular carcinoma on a preoperative examination

Objective: Bile duct tumor thrombosis in hepatocellular carcinoma (HCC) is a relatively rare event with a poor prognosis. Furthermore, bile duct tumor thrombus in HCC may be misdiagnosed when only imaging modalities are used. The efficiency of peroral cholangioscopy (POCS) in evaluating bile duct lesions has been reported.Patients: We present three cases of HCC with bile duct strictures in which POCS was performed as a preoperative evaluation.Results: In these three cases, diagnosing whether the lesion was a bile duct tumor thrombus on CT and endoscopic retrograde cholangiopancreatography was difficult. We performed POCS in three cases and were able to diagnose the presence of bile duct tumor thrombus of HCC, including differentiation from extrinsic compression of the bile duct.Conclusion: POCS for HCC with bile duct features is useful for the preoperative diagnosis of bile duct tumor thrombus, especially in cases where the surgical procedure depends on the presence of bile duct tumor thrombus.
4.Endovascular occlusion of giant serpentine aneurysm: A case report and literature review

Giant serpentine aneurysms (GSAs) are a rare subgroup of intracranial aneurysms. Separate inflow and outflow flow due to intraluminal thrombosis is the most distinguishing feature of GSAs. In treating these lesions, surgical clipping and ligation were the main treatments in the past, but bypass for revascularisation and endovascular therapies (EVTs) for deconstructive purposes are more prominent today. A 51-years-old male patient presented with headache and mild right hemiparesis. He had a GSA arising from the left fetal type posterior cerebral artery (fPCA) that was out of follow-up for six years. Radiological images revealed midline shifting and mesencephalon compression. We performed endovascular parent artery coil occlusion. The symptoms of the patient improved at the first-month follow-up. Even if there is a mass effect in GSAs, deconstructive EVT is a safe and feasible method for managing these lesions.
5.MLH1 promoter hypermethylation predicts poorer prognosis in mismatch repair deficiency endometrial carcinomas

Objective:
The antitumor effects of anti-PD-1 antibody against mismatch repair deficiency (MMR-D)-associated cancers have been reported. MMR-D is found in approximately 20%–30% of endometrial carcinomas (ECs) and frequently occurs due to MLH1 promoter hypermethylation (MLH1-PHM). ECs with MLH1-PHM are classified according to the molecular screening of Lynch syndrome (LS), but few detailed reports are available. The purpose of this study was to clarify the clinical features of EC with MLH1-PHM.
Methods:
Immunohistochemistry of MMR proteins (MLH1, MSH2, MSH6, and PMS2) was performed on specimens from 527 ECs treated at our university hospital from 2003 to 2018. MLH1 methylation analysis was added to cases with MLH1/PMS2 loss. ECs were classified as follows: cases that retained MMR proteins as “MMR-proficient;” cases with MLH1/PMS2 loss and MLH1-PHM as “met-EC;” and cases with other MMR protein loss and MLH1/PMS2 loss without MLH1-PHM as “suspected-LS.” The clinical features, including long-term prognosis, of each group, were analyzed.
Results:
Accordingly, 419 (79.5%), 65 (12.3%), and 43 (8.2%) cases were categorized as “MMR-proficient,” “suspected-LS,” and “met-EC,” respectively. Significantly, “met-EC” had a lower proportion of grade 1 tumors (37.5%) and a higher proportion of stage III/IV tumors (37.2%) than the other groups. The overall and progression-free survival of “met-EC” were significantly worse than those of “suspected-LS” in all cases.
Conclusion
In ECs with MMR-D, “met-ECs” were a subgroup with a poorer prognosis than “suspected-LS.” “Met-ECs” would be the main target for anti-PD-1 antibody treatment, and its clinical susceptibility should be verified individually.
6.A study of the triage function of referrals in an urban clinic

Introduction : To promote disease management in the community, general physicians should refer their patients to specialists in a timely and appropriate manner. In this study, we propose an indicator for evaluation of such referrals.
Methods : We analyzed all referrals in an urban clinic from September 1, 2011 to August 31, 2012. Symptoms and diagnoses documented by general physicians were collected from medical records, and the final diagnoses by specialists were collected from their reports. The symptoms and diagnoses were classified using the International Classification of Primary Care second edition (ICPC-2). Referral rates, hospitalization rates, and place of referral were analyzed.
Results : The average number of encounters in the candidate clinic was 1402 per month, and the mean number of referrals was 23 (1.6% of encounters). Of patients who received a referral, 6.75 (29.1%) were admitted to hospitals. The symptoms and diagnoses of the referred patients were distributed across all chapters (A to Z) of ICPC-2. Diagnoses of admitted patients included pneumonia (R81) (24%), urinary tract infection (U70 and U71) (9%), and acute gastroenteritis with dehydration (D73 with T11) (9%).
Conclusion : We identified the referral rates, hospitalization rates, and distribution of referral patients as indicators of the triage function of primary care physicians. These should be evaluated further as potential indicators of “the quality of medical care.”
7.Construction Case with an Electronic Hospital Formulary to Be Able to Do Rapid Retrieval Using a Portable Terminal iPhone®

Objective: The hospital is changing its formulary reference from paper-based to intranet. There was concern that both paper-based and intranet versions of the formulary would be necessary. Revising the paper-based hospital formulary each time package inserts are revised is difficult. For your review we report on the creation of the iPhone® electronic formulary which enables rapid off-line formulary retrieval and easy updates while at the same time providing low cost service in a light device.
Methods: The CSV (Comma Separated Value) of the hospital formulary dictionary was made using a standard personal computer. The CSV data file was converted using JAMES2DIC into a HTML file format. Next, the converted HTML file is transformed into the EPWING (Electronic Publishing WING) format using EBStudio. Finally, we forward the EPWING dictionary file from the personal computer to the iPhone®. The retrieval becomes possible by using EBPocket for iOS of EPWING/electronic book viewer software for the iPhone®. The number of items was assumed to be 29 items thought for a lot of inquiries to exist.
Results: We compared the paper-based formulary with the iPhone® electronic formulary. As a result, the iPhone4® electronic formulary shortened the retrieval time, was smaller, lighter, and excellent at a lower price.
Conclusion: The iPhone4® electronic formulary enables the user to perform complex full-text searches and retrieve information at a much higher speed than is possible with paper based formularies. It has the additional advantage of seamless integration and deployment of formulary additions or reference material revisions. We believe we have successfully created a practical electronic formulary.
8.Simultaneous Operation for Lung Cancer and Thoracic Aortic Aneurysm with Thoracic Endovascular Repair

A 67-year-old man was given a diagnosis of lung cancer and thoracic aortic aneurysm (TAA). We first performed thoracic endovascular repair (TEVAR), and then right lower lobectomy for lung cancer. TEVAR shortened the operation time and yielded less operative damage. Therefore, TEVAR can be an effective choice for simultaneous surgery of TAA and lung cancer.
9.Cesium Implant for Tongue Carcinoma with a Thickness of 1.5 cm or More: Cases Successfully Treated with a Modified Manchester System.

PURPOSE: Deciding on treatment carcinoma of the tongue when the tumor has a thickness of 1.5 cm or more is difficult. Surgery often requires wide resection and re-construction, leading to considerable functional impairment. A cesium implant is an attractive option, but according to the Manchester System, a two plane implant is needed. MATERIALS AND METHODS: According to the textbook, a tumor is sandwiched between the needles, which are implanted at the edge of the tumor. This may cause an unnecessarily high dose to the outer surface of the tongue, which sometimes leads to a persistent ulcer. To avoid this complication, we invented a modified implantation method, and applied the method to five consecutive patients. RESULTS: With a minimum follow-up of 2 years, all primary tumors in 5 consecutive patients have been controlled. No complications occurred in soft tissue of the tongue or in the mandible. CONCLUSION: Our modified Manchester System was feasible and effective for tumors that has a thickness of 1.5 cm or more.
10.A Successful Treatment for Myonephropathic Metabolic Syndrome and Delayed Intestinal Ischemia after Operation of Acute Type B Aortic Dissection with Bilateral Lower Limb Ischemia

A 20-year-old man suddenly complained of back pain and bilateral lower limb weakness. Computed tomography showed acute type B aortic dissection. The patent false lumen extended from distal arch to the left common iliac artery. The true lumen was severely compressed by the false lumen and both legs were ischemic. He underwent emergency fenestration of the abdominal aorta and stenting of the left iliac artery. Although the lower limbs ischemia was improved, he developed myonephropathic metabolic syndrome and received plasma exchange, continuous hemodialysis and endotoxin absorption therapy. Thirteen days after the operation, intestinal ischemia occurred and he underwent emergency bowel resection with creation of a stoma. Development of dissection to the superior mesenteric artery (SMA) and the malperfusion of SMA by severe compression of the true lumen were thought to cause intestinal ischemia.
