1.Changes and clinical significance of gastrin 17 in diabetic nephropathy

Objective:To investigate the changes and clinical significance of gastrin 17 (G-17) in patients with diabetic nephropathy (DN).Methods:One hundred and twenty-four DN patients admitted to Hefei Second People′s Hospital from July 2018 to December 2020 were selected as the DN group, and divided into Ⅰ-Ⅱstage subgroup (68 cases) and Ⅲ-Ⅴ stage subgroup (56 cases) according to the stage of DN.Inaddition, 100 cases of type 2 diabetes mellitus(T2DM) patients without DN were selected as the T2DM group, and 100 healthy subjects who examined during the same period were selected as the control group. The levels of G-17, serum creatinine (SCr), evaluated glomerular filtration rate (eGFR) and other index in each group were detected. The normal level of G-17 was 1-7 pmol/L. G-17>7 pmol/L and ≤ 15 pmol/L was as marginal rising, and G-17>15 pmol/L was as rising.Results:The marginal rising rate of G-17 in the DN group was higher than that in the T2DM group: 43.5%(54/124) vs. 23.0%(23/100); the rising rate of G-17 in the DN group was higher than that in the T2DM group and the control group: 21.0%(26/124) vs. 7.0%(7/100), 4.0%(4/100), and the differences were statistically significant ( P<0.05). The marginal rising rate and rising rate of G-17 in Ⅲ-Ⅴstage subgroup were both higher than those in the Ⅰ-Ⅱ stage subgroup and the T2DM group: 58.9%(33/56) vs. 30.9%(21/68), 23.0%(23/100); 32.1%(18/56) vs. 11.8%(8/68), 7.0%(7/100), and the differences were statistically significant ( P<0.05). The marginal rising rate and rising rate of G-17 in DN patients with a disease course of ≥3 years was higher than that in patients with a disease course of <3 years and the T2DM group: 53.0%(44/83) vs. 24.4%(10/41), 23.0%(23/100); 27.7%(23/83) vs. 7.3%(3/41), 7.0%(7/100), and the differences were statistically significant ( P<0.05). Correlation analysis showed that G-17 was positively correlated with SCr ( r = 0.367, P<0.001) and negatively correlated with eGFR ( r = -0.619, P<0.001) in DN patients. Conclusions:The level of G-17 in ND patients is significantly increased, which is closely related to DN staging and can provide an auxiliary indicator for screening renal function in patients with T2DM.
2.Direct determination of arsenic in urine by inductively coupled plasma mass spectrometry

Objective:To establish a direct and rapid method for direct determination of arsenic in urine by inductively coupled plasma mass spectrometry (ICP-MS).Methods:The newly collected urine samples were diluted directly with pure water without pretreatment. The total arsenic content in urine was determined directly by ICP-MS. The method was studied from the aspects of linear range of standard curve, correlation coefficient, detection limit, precision and accuracy.Results:The concentration of arsenic in urine was in the range of 0 - 200 μg/L, the ratio of arsenic to germanium was in good linear relationship with arsenic concentration, the correlation coefficient was 0.999 5 - 0.999 9 ( n = 6), the lowest qualitative and quantitative detection limits of arsenic in urine were 0.66 and 1.94 μg/L(the sampling volume was 0.50 ml), respectively. Five urine samples with different arsenic concentrations were tested for intrabatch and inter batch precision with RSD ranging from 1.51% to 6.84% and 1.85% to 5.03%, respectively. The total arsenic of urine samples from the Center for Endemic Disease Control, Chinese Center for Disease Control and Prevention was determined with this method, and the results were within the range of the published consensus value. The recovery rate of 4 urine samples with arsenic concentration range 3.19 - 89.36 μg/L was 99.25% - 103.67%, and the total average recovery rate was 101.51%. Conclusions:Application of ICP-MS method to detect arsenic content in urine, urine samples can be directly injected to realize the automation of injection, detection process and result analysis. The test parameters such as sensitivity, precision and accuracy of the method meet the requirements of the development of biological sample detection method and are suitable for rapid and direct determination of total arsenic in urine.
3.Diagnosis value of ADC values combination with MRI signs on breast lump lesions with the type of TICⅡ

Objective To explore the significance of ADC value combined with MRI signs on identifying benign and malignant breast lump lesions with the type of TICⅡ.Methods 187 patients with breast lump lesions of TICⅡ,which were confirmed by surgical pathology,proceeded MRI.The ADC value,early-phase enhancement rate,length of lesions,lobulated appearance and spiculation, inverted nipple,thickening of the skin and the length of ipsilateral axillary lymph nodes were analyzed and recorded.T-tests was used to analyzed the measurement data,χ2test was used to analysis the attribute data.The ROC curves were used to evaluate the diagnostic efficiency of ADC value and MRI signs.Results The ADC value was (1.418±0.299)×10-3mm2/s and(0.860±0.142)× 10-3mm2/s (P<0.01)for breast benign and malignant lesions respectively,while the early-phase enhancement rate were (170.387± 74.580)% and (160.778±39.786)%(P=0.258)respectively.When ADC values were used for differential diagnosis of breast benign and malignant lesions with a threshold of 1.017×10-3mm2/s,the area under the sensitivity and specificity were 89.0% and 98.0% The sensitivity and specificity of lobulated appearance,spiculation,inverted nipple and thickening of the skin were 92.6% and 33.3%, 88.2% and 88.2%,20.5% and 94.1%,35.3% and 88.2%,respectively.When the 1.0 cm was used as the threshold of the length of ipsilateral axillary lymph,its sensitivity and specificity were 47.8% and 80.4%.The ROC curve of early-phase enhancement rate showed no diagnostic capability(P>0.05).Conclusion ADC value combined with MRI features are helpful to improve the sensitivity and specificity in breast lesions with the type of TICⅡ.
4.Impact of Wildtype p53 Induced Phosphatase 1 Gene Lacking on Heart Function in Experimental Mice

Objective: To explore the impact of knocking out wildtype p53 phosphatase 1 gene on heart function with the changes of cardiac tissue mRNA and protein expressions in experimental mice. Methods: Our research included in 2 groups: Wildtype (WT) mice group and Wip1 knockout (Wip1-KO) mice group. n=10 in each group. The heart function, ratio of heart weight to body weight (HW/BW) were examined and compared between 2 groups; cardiac tissue morphology was observed by HE staining; mRNA expressions of ANP, BNP, MCP-1 andα-SMA were determined by RT-PCR and protein expressions of Bcl-2, Bax and c-caspase3 were measured by Western blot analysis. Results: Compared with WT mice group, Wip1-KO mice group showed decreased Wip1 mRNA expression,P<0.05, decreased LVEF, LV fraction shortening and increased left ventricular end systolic diameter (LVESD), allP<0.05; Wip1-KO mice group had reduced BW and elevated ratio of HW/BW, bothP<0.05 even the heart weight was similar between 2 groups. There was no difference in cardiac tissue morphology between 2 groups; mRNA expressions of ANP, BNP, MCP-1 and α-SMA were similar between 2 groups,P>0.05; apoptosis related protein expressions of Bax/Bcl-2 and c-caspase3 were similar between 2 groups,P>0.05. Conclusion: Wip1 gene knockout may impair the heart function in experimental mice, while the relevant mechanism should be further investigated.
5.Efficacy of lumbar discectomy, radiofrequency ablation and annuloplusty performed with Disc-FX system under guidance of C-arm in treatment of discogenic low back pain

Objective To evaluate the efficacy of lumbar discectomy,radiofrequency ablation and annuloplusty performed with Disc-FX system under the guidance of C-arm in treatment of discogenic low back pain.Methods Twenty-eight patients with discogenic low back pain,with VAS score greater than 7,aged 27-73 yr,underwent lumbar discectomy,radiofrequency ablation and annuloplusty performed using Disc-FX system under the guidance of C-arm.The patients were followed up for 2 months after surgery and pain was assessed using VAS score.VAS scores were recorded before surgery,and at 7 and 14 days and 1 and 2 months after surgery.The effective analgesia was recorded according to VAS scores.The therapeutic effect was evaluated according to Macnab standard 2 months after surgery.Results VAS scores were significantly lower at each time point after surgery than before surgery (P < 0.05).The rate of effective analgesia was 93 % at 2 months after surgery.The excellent and good rate of the therapeutic effect evaluated was 93 %.One female patient developed injury to lumbar venous plexus,and no patients developed infection of intervertebral disk,nerve root injury or spinal cord injury.Conclusion The excellent and good rate is 93 % when lumbar discectomy,radiofrequency ablation and annuloplusty performed using Disc-FX system is used for treatment of discogenic low back pain under the guidance of C-arm,with fewer side effects.
6.Effects of implantation of iodine-125 seeds in the intervertebral foramina on histological changes in dorsal root ganglion and pain threshold in rats

Objective To evaluate the histologic changes of dorsal root ganglion (DRG) and pain threshold following implantation of iodine-125(125I) seeds in the lumbar intervertebral foramina in rats.Methods Thirtysix adult male SD rats aged 6 months weighing 220-250 g were randomly assigned into 3 groups (n =12 each):group sham operation (group A) and groups low and high radioactive 125I seeds (groups B and C).125I seeds of 0.4 and 0.8 mCi were implanted in L4,5 and L5,6 intervertebral foramina in groups B and C respectively.Paw withdrawal threshold to von Frey filament stimulation (MWT) and paw withdrawal latency to thermal nociceptive stimuli (TWL) were measured 2 days before (T1,baseline) and at 2,7,14,30 and 60 days after implantation (T2-6) in 6 animals in each group.The remaining 6 animals in each group were sacrificed at 14 and 60 days after 125I seeds implantation for microscopic examination of L5 DRG.Results MWT and TWL were significantly decreased after 125I seeds implantation at T4 compared with the baseline values at T1 in both groups,and returned to the baseline in group B and were significantly increased in group C at T6.The neurons were severely injured in DRG in groups B and C in a time-and dose-dependent manner.Conclusion Implantation of 125I seeds in intervertebral foramen can induce radiation resection of DRG.
7.Efficacy of pregabalin in patients with posst-herpetic neuralgia: a meta-analysis

Objective To systematically review the efficacy of pregabalin in patients with post-herpetic neuralgia.Methods A comprehensive search was undertaken to identify all randomized placebo-controlled trials of pregabalin in patients with post-herpetic neuralgia.Medline,Web of Science,Cochrane Library,Wanfang Database and CNKI were searched from January 1966 up to December 2008.The modified Jadad scale was used for quality assessment.Numerical rating scale ( NRS),effective analgesia rate and the incidence of adverse effects were taken as main outcomes.Meta-analysis was conducted using the Cochrane Collaboration Review Manager 4.2 software.Results A total of four studies involving 1024 patients were included in this meta-analysis.The modified Jadad scale scores for the 4 studies were ≥ 4.The NRS scores were significantly lower,while effective analgesia rate was higher in groups 150,300,600 mg/d than placebo-controlled group (P < 0.05).There were no significant differences in NRS scores and effective analgesia rate between 150 mg/d and 300 mg/d groups.The NRS scores were significantly lower in 600 mg/d group than in 300 mg/d group,but there were no significant differences in effective analgesia rate between 600 mg/d and 300 mg/d groups.The most frequent adverse effects were dizziness,somnolence,peripheral edema,and headache.Most of the adverse effects were mild or moderate in intensity.The occurrence of adverse effects appeared to be dose-related.Conclusion Pregabalin is effective and safe in patients with post-herpetic neuralgia,but the efficacy of reducing pain is not a dose-dependent manner.
8.Efficacy of destruction of dorsal root ganglia with local doxorubicin Injection under the guidance of CT for postherpetic neuralgia involving thoracic back region

Objective To evaluate the efficacy of destruction of dorsal root ganglia with local doxorubicin injection guided by CT for postherpetic neuralgia involving thoracic back region. Methods One hundred and fifty patients suffering from postherpetic neuralgia in thoracic back region were randomly divided into 3 groups ( n = 50 each): group A oral medicine; B and C groups undergoing local injection under the guidance of C-arm and CT respectively + oral medicine. Three spinal segments severely affected by herpes virus were chosen for paravertebral puncture. 1% lidocaine 1 ml was injected at each segment as test dose. Fifteen minutes later doxorubicin 3.3 mg and betamethasone compound 4.7 mg were injected at each segment if no side-effect occurred. All 3 groups were given oral medicine according to the intensity of pain after local injection. The number of patients who exited from the study because of the side effects of oral medicine was recorded. VAS, sleep interference score (SIS) and a short form of McGill pain questionnaire (SF-MPQ) were used to evaluate the efficacy of the treatment the day before (baseline), 24 h, 1 week, 1, 3 and 6 months after local injection. The dosage of oxycodone extended-release tablets and gabapentin was recorded, and also the incidence of pneumothorax within 12 h after local injection. Results The exit rate, VAS, SIS and SF-MPQ scores, dosage of oxycodone extended-release tablet and gabapentin were significantly lower in B and C groups than in group A, but there was no significant difference between the 2 groups. The incidence of pneumothorax was 10% in group B but no pneumothorax developed in group C.Conclusion Destruction of dorsal root ganglia with local doxorubicin injection guided by CT is more effective for the treatment of postherpetic neuralgia.
