1.Discussion on the indications and timing of targeted therapy and immunotherapy before and after liver transplantation for hepatocellular carcinoma

Liver transplantation is one of the main treatments of early hepatocellular carcinoma (HCC). The recurrence of HCC after liver transplantation severely affects the long-term survival rate of the recipients. Targeted therapy and immunotherapy play a critical role in HCC downstaging, preventing disease progression, reducing recurrence rate, prolonging the survival and improving the quality of life. However, no consensus has been reached on the application of targeted therapy and immunotherapy in recipients undergoing liver transplantation for HCC, including indications, timing and dosage. In this article, clinical research progresses on the indications and timing of targeted therapy and immunotherapy before and after liver transplantation for HCC were reviewed, aiming to provide reference for prolonging the survival of recipients after liver transplantation for HCC.
2.Research advances on the application of natural and recombinant collagen in wound repair.

Collagen is a macromolecular protein constituting the extracellular matrix of animal connective tissue, which has been widely used and developed in fields of biomedicine, tissue engineering, food, and cosmetics. Due to its advantages such as abundant sources and good biocompatibility, low immunogenicity, and degradability, collagen can be used as a dressing or tissue engineering scaffold for wound repair. According to the source of materials, collagen can be divided into natural collagen and recombinant collagen. Natural collagen is mainly extracted directly from mammals and fish; recombinant collagen is obtained based on genetic engineering technology, and its sources include recombinant expression systems of microorganisms, animals, and plants. This paper summarizes the sources of collagen, and the roles, advantages, and disadvantages of different sources of collagen in wound repair, the particularity and superiority of collagen combined with three-dimensional printing technology in wound repair, the impact of market norms of China's collagen industry on the field of wound repair, and explains the precautions for the development of collagen-related products, aiming to provide new ideas for selecting a suitable source of collagen for wound repair.

3.Risk factors for pertussis among children hospitalized for pertussis during 2016-2017,in Guizhou Province of China:a case-control study

Aim:Pertussis is a respiratory tract infection,the vaccine for which was introduced in the Expanded Programme on Immunization (EPI) in Guizhou Province of China (hereafter referred as Guizhou) in the 1980s.This vaccine rapidly decreased incidence rates of pertussis in the province,however,despite the wide high coverage of the diphtheria,tetanus and acellular pertussis (DTaP) combined vaccine,there has been a resurgence of pertussis since 2014.Even with this recent increase in disease transmission,risk factors for pertussis infection have not been evaluated in Guizhou.We aimed to provide information on pertussis risk factors and insight on designing targeted pertussis control policies and measures through this study.Methods:A 1∶2 matched retrospective case-control study was conducted between 2016 and 2017,involving infants and children younger than 6 years old and parents of the participants.The enrolled cases included clinical and laboratory confirmed pertussis cases according to the WHO-recommended pertussis definition.Controls were selected from children in the same neighborhood who were not diagnosed with pertussis prior to our investigation and did not exhibit any clinical manifestations of pertussis.Results:The household size[Matched odds ratio (ORm) =1.4,95% confidence interval (CI):1.1-1.7]and house-hold members with antecedent cough (ORm =3.6,95% CI:1.8-7.2) were significantly associated with their child's pertussis onset.The parents' occupations were significantly associated with their child's pertussis onset(ORm =9.4,95% CI:1.6-54.8,for mother side;ORm =4.5,95% CI:1.2-16.5,for father side),when they worked in the business and/or service industry.Having family members with a history of cough was an independent risk factor for pertussis[Adjusted odds ratio (AOR) =43.6,95% CI:2.7-694.0].Besides,the parents' demographic characteristics and DTaP doses were not found to be independent factors.Conclusion:Household exposure is an important risk factor for pertussis infection in infants and young children and should therefore be considered as a major factor during formulation of pertussis control policies and measures.
4.Screening and analysis of key genes in follicular lymphoma based on bioinformatics method

Objective:To screen out and analyze the key genes of follicular lymphoma (FL) according to transcriptome data in Gene Expression Omnibus (GEO) database.Methods:Transcriptome datasets GSE32018 and GSE55267 were collected by using GEO database. R software was used for variance analysis to screen differentially expressed genes. FunRich 3.13 software was used to analyze common differential genes. The biological processes and pathways were analyzed with Cytoscape 3.7.2 software to screen potential genes related to the pathogenesis of FL. By analyzing clinical data from Oncomine database for survival analysis, the screened differential genes were verified.Results:A total of 141 up-regulated genes and 199 down-regulated genes were identified by differentially analyzing GSE32018 and GSE55267 datasets. Finally, 12 key genes including CXCL8, KRT19, CYCS, CDKN3, SFN, RRM2, FN1, APOE, CXCL12, VWF, GATA3 and TIMP1 were screened out; CYCS, CXCL8 and CXCL12 were closely related with the early survival rate of patients. Overexpression of CXCL12 and low expression of CYCS were found to be associated with poor prognosis of FL patients. CXCL8 expression was decreased in lymphoma tissues, but the relatively high expression of CXCL8 in survival analysis showed shortened overall survival, which might be related to the early development of FL. GO, KEGG and Reactome pathways were screened out including GO: 0001892, KEGG: 04115, R-HSA: 2559582, GO: 0060968, R-HSA: 6785807, GO: 0043627, GO: 0001936 and GO: 0043062.Conclusion:The selected genes CYCS, CXCL8 and CXCL12 may provide more effective biomarkers for the treatment of FL.
5.Effect of self-management efficacy on compliance of taking capecitabine in discharged patients with colorectal cancer

Objective To explore the effect of self-management efficacy on compliance of taking capecitabine in discharged patients with colorectal cancer. Methods A total of 82 patients with colorectal cancer who were treated in the First Affiliated Hospital with Nanjing Medical University from January 2014 to December 2016 were selected by convenient sampling method. The general data, drug compliance and self-management efficacy of patients were investigated and the relationship between drug compliance and self-management efficacy was analyzed. Results Compliance score of taking capecitabine for 82 patients with colorectal cancer was (6.42±1.65) points; among them 5 cases were excellent (6.10%), good in 54 cases (65.85%) and poor in 23 cases (28.05%). Compliance of taking capecitabine in discharged patients with colorectal cancer was positively correlated with the scores of all dimensions and total score of self-management efficacy. Self-management efficacy was an independent factor affecting the compliance of patients with capecitabine, which could explain 13.4% of the overall variation of compliance. Conclusions Compliance of taking capecitabine in discharged patients with colorectal cancer is unsatisfied. Education on self-management efficacy from medical staff could improve the patients' medication adherence and their quality of life.
6.Chemotherapy drugs affect terminal lung cancer EGFR mutation status

Objective To compare the advanced non -small cell lung cancer (NSCLC)patients before and after chemotherapy peripheral blood EGFR mutation status,we understand whether the chemotherapy drug impacts the EGFR mutation status of the advanced NSCLC patients,so as to improve the precision of EGFR TKIs -drug use. Methods To collect the peripheral blood of 30 cases of advanced NSCLC before chemotherapy and after chemotherapy for 6 cycles.DHPLC technique was used to detect the EGFR mutation states of EGFR exon 19 and in exon 21.Results In 30 patients,chemotherapy prior EGFR mutation positive rate was 53.3% (16 /30).After 6 cycles of chemotherapy, the EGFR mutation positive rate was 36.6% (11 /30),the consistent rate was 56.6 (17 /30)before and after chemo-therapy,inconsistent rate was 53.4% (13 /30).10 cases from positive to negative before chemotherapy,3 cases from negative into positive before chemotherapy with statistical significance (P =0.046).Six EGFR19 exons changed, change rate of 20%,8 EGFR21 exons shift changed at a rate of 26%.EGFR19,21 shift in 1 case,with no statistical significance(P =0.39,P >0.05).Conclusion (1)The late NSCLC patients before and after peripheral blood of EGFR mutation status change,so before starting the targeted therapy we must recive real -time detection of peripheral blood EGFR mutation status,so as to decide whether to choose EGFR TKIs targeted drug therapy.(2)EGFR21 exons transformation rate is higher than EGFR19 exons conversion rate,but with no statistical difference,this phenomenon may be related to EGFR19 exons patients who with EGFR mutations -TKIs treatment efficiency is higher.
7.Qualitative research on work stress of new nurses in children′s hospital

Objective To explore the main sources of work stress of new nurses in children′s hospital, to analyze the causes, and to provide reference for managers to find out reasonable ways of reducing work stress in new nurses. Methods Phenomenological method was used to explore stress experience of new nurses. A total of 10 new nurses were interviewed by semi-structured talk. Topics were refined by reading, analyzing, reflecting, classifying and refining the interview data. Results Four main stress topics of new nurses included professional knowledge and skills, nature of work, performance evaluation and interpersonal relationship. Conclusions New nurses have great stress at work. Managers should find reasonable ways to relieve their stress, provide help for them, make them adapt to the new environment and work as soon as possible, enhance the sense of ownership and provide better service for patients.
8.Nursing of transfusion port implanted by axillary vein

Objective To evaluate the clinical application and nursing effect of implantable venous access ports via the axillary vein.Methods From September 2014 to December 2015,1 263 cases of malignant tumor patients were implanted implantable venous access ports via the axillary vein under ultrasound guidance in Oncology Department of our hospital.The effects of the application of implantation method and nursing were observed.Results The 1 325 cases underwent implantable venous access ports via the axillary vein with 1 263 cases catheterization succeeded,and the success rate is 95.3%.For 62 failed cases,the internal jugular vein catheterization were carried out instead.The average time of implantation time was 10.5 minutes.For the 3 cases of catheter blockage,they were reused after recanalization using thrombolysis under negative pressure with urokinase;for the 2 cases localized infection,the ports were removed and the drug were injected through PICC;no serious complications,namely thrombosis,pinch-off syndrome,catheter disconnection,port body flips et al,happened.Conclusions Implantable venous access ports via the axillary vein with the features of high success ratio,short operating time,can be used as a new selection of port implantation.By establishing the standardization and homogenization of the whole catheter management system postoperatively,strengthening catheter maintenance training for nursing staff,and focusing on the details of the process of operation and maintenance,the complications of infusion port can be reduced,so as to keep the port using for a longer time.
9.Effect of open-ended venous port with different sealing fluids to seal tube

Objective To evaluate the effect of tube sealing using pre-flushed flushing syringes ( flush) and heparin saline on open-ended port for venous transfusion during continuous infusion. Methods During the period of continuous intravenous infusion, a total 90 tumor patients were randomly divided into 2 groups. They used open-ended port for venous transfusion and was sealed by using pre-flushed flushing syringes and 100 U/ml heparin saline respectively. The catheter blockage rate, dispensing time, sealing tube time and cost of single sealing tube were compared between two groups.Results During the period of continuous infusion, the catheter occlusion rate of flush group and the heparin saline group were 1.84% and 1.47% (χ2=0.2642,P>0.05); on the dispensing and seal tube time,they were (11.5±2.9) seconds and (10.5±2.6) seconds in the flash group, and (37.1±2.9) seconds and (24.6±3.3) seconds in the heparin saline group (t=41.982,22.797;P<0.01). The cost for flush sealing is 8.33 yuan lower than that of heparin saline sealing.Conclusions During the period of continuous intravenous infusion, sealing tube by using flush on open-ended infusion port is safe and effective. The workload of the nurses and cost is reduced, it is worth to be promoted.
10.Assistant Effects of Longman Ampelopsis Grossedentata on Serum Lipid

Objective:To study the effect of Longman Ampelopsis grossedentata on the level of serum lipid. Methods:The exper-imental rats were given high-fat diet during the experiment. The rats were randomly divided into the model group, high dose group (37. 80 g·kg-1), medium dose group (12. 60 g·kg-1), low dose group (6. 30 g·kg-1) and lipid-lowering drug group (180 mg· kg-1 ) . All the groups were administrated continuously for 30 days, and the effect on body weight, serum TC, TG and HDL-C were measured. Results:Compared with that of the model group, the increase of body weight in the other groups was not significant ( P>0. 05), and all the rats had no obvious poisoning symptom. TC and TG were higher after the high-fat diet administration than those be-fore the experiment in the model group, and the differences were statistically significant (P<0. 05). After the 30-day treatment, the levels of TC and TG in the high-dose group were significantly decreased compared with those of the model group (P<0. 05). Conclu-sion:Longman Ampelopsis grossedentata shows assistant therapeutic effect on serum lipid.
