1.Improving a follow-up plan based on the tumor deposits status and recurrence pattern of stage Ⅱ/Ⅲ gastric cancer patients after radical gastrectomy

Objective:To investigate the relationship between cancer node status identified after radical resection and patterns of recurrence in stage Ⅱ/Ⅲ gastric cancer patients for developing personalized follow-up plans in node-positive patients.Methods:A retrospective analysis was conducted on 706 stage Ⅱ/Ⅲ gastric cancer patients who underwent curative intent surgery at Henan Cancer Hospital from Sep 2015 to Sep 2018. Patients were categorized into node-positive (TDs +) and node-negative (TDs -) groups based on their cancer node status. Clinical-pathological characteristics and recurrence patterns were compared between the groups to formulate an optimized follow-up plan. Results:Among the patients, there were 112 TDs + and 594 TDs - cases. TDs - patients exhibited significantly better overall survival and disease-free survival rates at 1 and 3 years compared to the TDs + group (92.4%,72.2% vs. 79.4%,38.8%;87.9%,68.6% vs. 66.7%,35.9%, all P<0.05). The cumulative recurrence rates of peritoneal and distant metastases were higher in TDs + patients compared to TDs - patients, with statistical significance (33.0% vs. 14.5%,21.4% vs. 10.8%, χ 2=21.285,8.851 all P<0.05). TDs + patients experienced significantly earlier median time to distant metastasis compared to TDs - patients (9.0 months vs. 15.4 months , Z=-2.294 P=0.022). The risk of peritoneal metastasis recurrence was higher in the TDs + group, showing a bimodal recurrence pattern at 8.5 and 15.0 months post-surgery. TDs + patients also had a higher risk of distant metastasis recurrence, with a single peak at 6 months. Conclusion:Postoperative recurrence patterns differ between TDs + and TDs - patients, and an optimized follow-up plan can enhance early detection of recurrence.
2.Efficacy of sandwich cheiloplasty on macrocheilia secondary to port-wine stain

Objective:To analyze the curative effect of sandwich cheiloplasty for the treatment of macrocheilia secondary to port-wine stain (PWS).Methods:A total of 43 patients, including 20 males and 23 females, aged 37.0 years (ranged from 4 to 69 years), who underwent sandwich cheiloplasty for the treatment of macrocheilia secondary to PWS from March 2008 to March 2022 were retrospectively enrolled. According to the location of the lesions, they were divided into two groups: upper lip group (21 cases) and lower lip group (22 cases). Pathological examinations were performed in all excised specimens. Postoperative attentions should be paid to keeping incisions locally clean, observing the blood supplying, and taking care of fluid accumulation or incision split. After discharge, the lip appearance and function were evaluated every six months in the outpatient department, and the long-term efficacy was further divided into three levels: grade Ⅰ (poor), grade Ⅱ (moderate), and grade Ⅲ (good). The comparison of grade data named long-term efficacy between the two groups was conducted by Mann-Whitney U test.Results:All 43 patients underwent sandwich cheiloplasty, of which 40 patients received one operation, whereas the other 3 received two due to recurrences. One-stage incision healing was achieved in 43 cases, whereas partial mucosal necrosis appeared in 2 cases, and slight incision dehiscence occurred in 1 case, which healed well after local dressing change. The pathological examination results of 43 excised tissue specimens all showed capillary malformations. After follow-up for 1 to 10 years, long-term efficacy evaluation was made up as follows: 36 cases were evaluated as grade Ⅲ, 7 cases as grade Ⅱ, and 0 case as grade Ⅰ. There were 18 cases of grade Ⅲ, and 3 cases of grade Ⅱ in group A, compared to 18 cases of grade Ⅲ, and 4 cases of grade Ⅱ in group B. By rank sum test, there was no significant difference in overal efficacy between the two groups ( Z=0.342, P>0.05). Conclusions:Sandwich cheiloplasty for the treatment of macrocheilia secondary to PWS effectively removes malformed vascular lesions, and consequently, it can achieve a good long-term therapeutic effect.
3.Study on the mechanism of Wenyang huazhuo tongluo formula contained serum inhibiting the proliferation of Th17 cells in scleroderma patients

OBJECTIVE To study the effects of Wenyang huazhuo tongluo formula conta ined serum on the expression and methylation of retinoic acid related nuclear orphan receptor (RORγt),and to explore the mechanism of its regulation of Th 17 cell proliferation in the treatment of scleroderma. METHODS Wistar rats were given 15,30,60 g/(kg·d)Wenyang huazhuo tongluo formula intragastrically to prepare different doses of drug-contained serum ,and peripheral blood of scleroderma patients were collected to sort Th 17 cells. Using blank serum as blank control ,methyltransferase inhibitor decitabine (DCA)as positive control , Th17 cells were treated with different concentrations of drug-contained serum ,and then the proliferation of Th 17 cells was detected by CCK- 8;mRNA expressions of RORγt and IL-17 were detected by qRT-PCR ,and the protein expression of RORγt was detected by Western blot assay ;the protein expression of IL- 17 in the cell supernatant was detected by ELISA ,and the methylation level of RORγt gene promoter was detected by methylation-specific PCR. The transcriptional activity of RORγt gene promoter was detected by dual-luciferase assay. RESULTS Compared with blank control ,Wenyang huazhuo tongluo formula contained serum and DCA could inhibit the proliferation of Th 17 cells(P<0.05),reduced the mRNA and protein expressions of RORγt and IL-17,enhanced the methylation level of RORγt gene promoter and attenuated , the transcription activity of RORγt gene promoter. Except for mRNA expression of Th 17 and the methylation level of RORγt gene promoter in drug-contained serum low-dose group ,there were statistical significance in above indexes of other groups(P<0.05). CONCLUSIONS Wenyang huazhuo tongluo formula can promote the methylation lev el of RORγt gene,and inhibit the expression of RORγt and the secretion of IL-17,so as to inhibit the proliferation of Th 17 cells.
4.Association of the interaction between serum uric acid and diastolic blood pressure on the risk of tophi

Objective:To analyze the influencing factors of gout patients and explore whether there is an interaction between serum uric acid and diastolic blood pressure affecting the onset of tophi.Methods:A total of 4 798 gout patients were retrospective selected in the outpatient Department of the Gout Clinical Medical Center of the Affiliated Hospital of Qingdao University from September 2016 to May 2020. It was divided into tophi group and non-tophi group to compare the differences in indicators. A logistic regression model was used to analyze the influencing factors of tophi, and an interaction model was constructed to analyze the interactions.Results:Multivariate logistic regression analysis showed significant associations between age, diastolic blood pressure, alcohol consumption history, gout family history, blood uric acid, urea nitrogen, and creatinine clearance and tophi formation. The results of blood uric acid-related interaction analysis showed a significant interaction between blood uric acid and diastolic blood pressure( Pinteraction=0.014), and the risk of developing tophi in low diastolic blood pressure and high diastolic blood pressure group increased by 34.4%( OR=1.344, 95% CI 1.105-1.635, P=0.003) and 95.4%( OR=1.954, 95% CI 1.558-2.450, P<0.001) in the high blood uric acid group compared with the low blood uric acid group. The results of diastolic blood pressure and blood uric acid subgroup analysis showed that there was no statistical difference in the risk of developing tophi in people with low uric acid levels( P=0.238), but in people with high uric acid levels, the risk of developing tophi was 67%( OR=1.670, 95% CI 1.379-2.022, P<0.001) higher than that in the low uric acid group. Conclusion:Age, diastolic blood pressure, combined alcohol consumption history and gout family history, blood uric acid, renal function are related to the occurrence of tophi. High uric acid and high diastolic blood pressure have interaction on the occurrence of tophi. Attention and proactive intervention shall be applied to this group of patients.
5.A Nomogram model involving preoperative inflammatory markers for predicting postoperative overall survival in patients with stage Ⅰ-Ⅲ gastric cancer

Objective:To establish a nomogram to predict overall survival of patients with stage Ⅰ-Ⅲ gastric cancer (GC) based on preoperative inflammatory markers.Methods:Clinicopathological and follow-up data of 1 035 patients with stage Ⅰ-Ⅲ gastric cancer operated at He'nan Cancer Hospital between May 2015 and Oct 2016 were retrospectively collected. A nomogram was established based on prognostic factors. Harrell's concordance index (C-index), receiver operating characteristic (ROC) curve, calibration curve and decision curve analysis (DCA) were used to verify the performance of the model according to differentiation, calibration and clinical utility.Results:A total of 1 035 patients were enrolled . The median follow-up time was 41.9 months; According to the optimal cutoff value, 170 were with elevated neutrophil-to-lymphocyte ratio (NLR) and 865 with a reduced ratio; 562 in elevated platelet-to-lymphocyte ratio (PLR) vs. 473 in the reduced group; fibrinogen/albumin ratio (FAR) elevated in 108 group vs. 972 in the reduced group; 180 in the prognostic nutritional index score (PNI) elevated group and 855 in the reduced group. Two hundred and sixty-seven patients were categorized at stage Ⅰ, 334 at stage Ⅱ ,434 at stage Ⅲ. Multivariate regression analysis showed tumor location, vascular tumor thrombus, pTNM stage, FAR, PNI and NLR were independent prognostic factors (all P<0.05). The C-index of the nomogram was 0.723 (95% CI: 0.710 -0.736) and had better clinical utility than the American Joint Committee on Cancer (AJCC) 8th TNM staging system 0.693 (95% CI, 0.681 -0.705). The calibration curve of the nomogram showed that the predicted survival rate was consistent with the actual survival rate in GC patients. Compared to AJCC 8th pTNM staging system, the DCA curve indicate that the nomogram has a higher net income. Conclusion:The nomogram predicting overall survival of patients with stage Ⅰ-Ⅲ gastric cancer is established and verified , which provides better individual prediction than TNM staging system.
6.Electro-acupuncture promotes repair of rotator cuff injury in rats.

OBJECTIVE:
To observe the therapeutic effect of electro-acupuncture on tendon healing and functional recovery of rotator cuff injury in rats and explore the therapeutic mechanism of electro-acupuncture.
METHODS:
Ninety SD rats were randomly divided into electro-acupuncture group, model group and blank control group, and models of rotator cuff injury were established in the former two groups.The rats in electro-acupuncture group was treated with electro-acupuncture after the operation, and those in the other two groups received no treatment.The right forefoot thermal withdrawal latency (TWL), the contents of IL-1β, IL-6 and TNF-α in the synovial fluid and the maximum tension load of supraspinatus tendon were measured at 2, 4 and 8 weeks after the operation.
RESULTS:
TWL in the model group was significantly lower than that in the blank control group and electro-acupuncture group at 2, 4 and 8 weeks after the operation (
CONCLUSIONS
Electro-acupuncture treatment not only effectively reduces the expression of inflammatory factors to relieve pain, but also promotes the repair of damaged tissue to improve the biomechanical properties of rotator cuff in the rat models.

7.Clinical application of autologous freezing fat granule injection grafting in facial rejuvenation

Objective:To explore the clinical application of autologous freezing fat granule injection grafting in facial rejuvenation.Methods:A total of 64 cases of facial skin soft tissues ageing atrophy were treated by transplantation of autologous purification freezing lipochondria. Autologous fat was obtained from patient's abdomen or thighs, centrifugated at low velocity and low pressure to remove the oil and fluid, then stored the lipochondria in -20℃. Rewarming the fat under 37 ℃ for 1 hour, we observed the integrity of the adipocyte and detected the vitality of the fat. Then the purified autologous fat was injected into the recipient site of the face.Results:The fat cell membrane and cell nucleus were clear and integrity after stored in -20℃ for 24 weeks, and the vitality of the fat was (88.89±1.23)%. 21 cases gained satisfactory clinical results by injecting once and 35 cases with 2 times injections, 8 cases with 3 times injections, the effects were satisfactory and there was no complication by follow-up from 6 to 24 months. 82.81% patients and doctors were satisfactory with the curative effect, and 1.56% patients and doctors were unsatisfactory.Conclusions:The effects are satisfactory of autologous purified freezing microparticle fat injected transplant. It has low absorptivity, can duplicate injection, and accept easily by people. It is a good method for facial rejuvenation and worth to spread in the clinical practice.
8.MR imaging study of arachnoid granulation in patients with cerebral venous sinus thrombosis

Objective:To study the changes and imaging features of arachnoid granulation (AG) in patients with cerebral venous sinus thrombosis (CVST).Methods:The clinical and imaging data of 35 patients with idiopathic CVST confirmed by MR imaging combined with CE-MRV or DSA in our hospital from January 2013 to December 2018, and 35 healthy controls collected at the physical examination center at the same time period were detected. The sizes and numbers of AG were compared between the two groups, and the MR imaging features of CVST and AG in patients with CVST at different courses were compared.Results:(1) In 35 patients with CVST, AG was found in 14 patients, and the average diameter was (5.19±2.40) mm, ranged from 1.83-11.77 mm; in the control group, AG was found in 23 patients, and the average diameter was (4.45±2.03) mm, ranged from 1.45-9.87 mm; no statistical difference was noted in diameter between the two groups ( t=2.121, P=0.221), and the number of AG showed statistical difference between the two groups (χ 2=4.644, P=0.031). (2) In 8 patients with CVST at acute phase, CVST showed hypointense on T2WI and FLAIR; in 20 patients with CVST at subacute phase, CVST showed hyperintense on T2WI and FLAIR; in 7 patients with CVST at chronic phase, CVST showed hyperintense on T2WI and FLAIR. At any stage of venous sinus thrombosis, AG showed high signal on T2WI and low signal on FLAIR. Conclusion:AG detection rate in CVST patients is lower than that in normal controls; MR imaging T2WI and FLAIR sequences could effectively distinguish CVST with AG.
9.MRI T2WI and 3D contrast enhanced magnetic resonance venography in diagnosis of transverse sinus arachnoid granulations: a comparative study

Objective:To explore the diagnostic values of MRI T2WI and 3D contrast enhanced magnetic resonance venography (3D CE-MRV) in transverse sinus arachnoid granulations (AGs).Methods:Clinical data were obtained from 811 patients who were subjected to non-contrast MR imaging and 3D CE-MRV between January 2016 and April 2019 in our hospital. And the number of patients whose transverse sinus AGs could be shown by T2WI and MRV, the number, size, and shape of AGs, and the number of AGs-related veins were recorded.Results:MRI T2WI showed 242 AGs in 184 patients (22.7%), and 3D CE-MRV showed 138 AGs in 119 patients (14.7%); MRI T2WI had a significantly higher percentage in showing AGs patients as compared with 3D CE-MRV ( P<0.05). Two hundred and forty-two AGs showed by MRI T2WI were divided into 3D CE-MRV observation group ( n=138) and 3D CE-MRV non-observation group ( n=104); the diameter of AGs in the 3D CE-MRV non-observation group ([5.30±2.10] mm) was significantly smaller than that in the 3D CE-MRV observation group ([6.14±2.03] mm, P<0.05). MR T2WI showed that 231 AGs were like circles or ellipses and 11 AGs were like fractal; while 3D CE-MRV showed that 138 AGs were like circles or ellipses and 0 AG were like fractal. MRI T2WI showed that 289 veins were closely related to AGs, and 3D CE-MRV showed that 97 veins were closely related to AGs. MRI T2WI could demonstrate significantly higher proportion of non-labbe veins which were closely related to AGs than 3D CE-MRV ( P<0.05). Conclusion:As compared with 3D CE-MRV, T2WI can find larger number of AGs in the transverse sinus, and better display the shape of AGs and the vein adjacent to them.
10. Retrospective analysis of classification and treatment of microcystic lymphatic malformations of tongue

Objective:
To explore the clinical classification of microcystic lymphatic malformations of tongue and observe the treatment of microcystic lymphatic malformations of tongue by retrospective analysis, in order to provide reference for clinical practice.
Methods:
From October 2005 to October 2015, the complete data of 220 cases of microcystic lymphatic malformations of tongue (115 males and 105 females) received and treated in Provincial Special Department of Vascular Anomalies, Linyi Tumor Hospital was analyzed retrospectively. The age ranged from 8 months to 52 years old, with a median age of 16 years old. All patients were followed up for 3 years, and according to their clinical manifestations, they were divided into three types: localized type of 23 cases, diffuse type of 161 cases, and megaloglossia type of 36 cases. Injection with pingyangmycin merely was performed on 58 cases, whereas merely surgery on 20 cases, injection with pingyangmycin combined with high frequency electrocoagulation on 55 cases, and surgery combined with injection with pingyangmycin on 87 cases. The therapeutic effect was evaluated according to the grade 4 standard. The χ2 test was used for statistical analysis of count data. Rank sum test was used for statistical analysis of ranked data.
Results:
The percentage of surgery merely of localized type was 87.0% (20/23), significantly higher than that of other types of lesions [0% (0/197)] (χ2=178.060,
