1.Neuroprotective effect of Wendan Decoction on a mouse model of sleep disorders via IKKβ/NF-κB pathway

AIM To investigate the effect of Wendan Decoction on nerve injury in a mouse model of sleep disorders and its mechanism.METHODS A mouse model of insomnia was established by the modified multiple platform sleep deprivation method.After successful modeling,the mice were randomly divided into the model group,the estazolam tablet group(0.15 mg/kg)and the low-dose and high-dose Wendan Decoction groups(12.5,50 g/kg),with 6 mice in each group,in contrast to the 6 mice of the control group.After 7 days of drug intervention,the mice had their changes of cerebral cortex,hippocampal CA1 area and hypothalamus observed by HE staining;their neuronal damage observed by Nissl staining;their levels of neurofilament light chain(NEFL),neuron-specific enolase(NSE),S100 calcium-binding protein B(S100B),tumor necrosis factor(TNF-α),interleukin-6(IL-6)and interleukin-1β(IL-1β)in brain tissue and serum detected by ELISA;their cerebral expression of glial fibrillary acidic protein(GFAP)detected by immunohistochemical method;and their cerebral expressions of GFAP,phosphorylated IκB kinase β(p-IKKβ)and phosphorylated nuclear transcription factor-κB(p-NF-κB)detected by Western blot.RESULTS Compared with the model group,the high-dose Wendan Decoction group displayed increased number of neurons,complete and neatly arranged structure;decreased number of neurons with nuclear shrinkage and deformation;increased Nissl bodies,decreased levels of NEFL,NSE,S100B,TNF-α,IL-6 and IL-1β in serum and brain tissue(P<0.01);decreased cerebral expression of GFAP(P<0.01);and decreased phosphorylation levels of cerebral p-IKKβ and p-NF-κB(P<0.01).CONCLUSION Wendan Decoction can reduce the nerve damage and the expression of proinflammatory mediator in sleep disorders mice,and the mechanism may be related to the inhibited activation of IKKβ/NF-κB pathway.
2.To Investigate the Effects of Anmeidan on Neurotransmitters in Sleep Deprived Rats Based on the Regulation of Astrocytes

Objective To investigate the effects of Anmian Dan on neurotransmitters in the brain of model rats,which were sleep deprived by multi-platform water environment.Methods Fifty SD rats were randomly and evenly divided into 5 groups with 10 rats in each group,which were blank control group(Control group),Model group(Model group),Estazolam group(Estazolam group),low dose group(AMD-L group)and high dose group(AMD-H group).The rats were subjected to sleep deprivation in a multi-platform water environment for 20 hours per day for 21 days.The movement distance and movement time of rats at different time points were recorded by autonomous activity analyzer to evaluate the changes of autonomous activity.The contents of glutamic acid(Glu)and gamma-aminobutyric acid(GABA)were detected by ELISA,and the mRNA expression levels of NDRG2,GLT-1,GAD65 and GAD67 were detected by Real-time PCR.Western blot was used to detect the expressions of NDRG2,p-PI3K,p-Akt,GLT-1,GAD65 and GAD67.Results The Model group was more active than the Control group,and the concentration of GABA in the cortex of the Model group was decreased and the concentration of Glu was increased.The mrna and protein expression levels of NDRG2 in Model group were higher than those in Control group(P<0.01),but the mrna and protein expression levels of GLT-1,GAD65 and GAD67 in model group were lower than those in Control group(P<0.01).The protein expression levels of P-PI3K and P-AKT in the cortex of model group were significantly decreased(P<0.01).Compared with Model group,Anmeidan could reduce the autonomic activity of sleep deprived rats,increase the concentration of GABA,decrease the concentration of Glu in cortex(P<0.05),and increase the mrna relative expression levels and protein expression levels of GLT-1,GAD65 and GAD67(P<0.05).The expression levels of P-PI3K and P-Akt were increased(P<0.01),and mrna and protein expression levels of NDRG2 were decreased(P<0.01).Conclusion Anmian Dan may regulate the activity of astrocytes and affect the levels of neurotransmitters GABA and GLU in the brain through the PI3K/AKT signaling pathway,thus playing a role in improving the circadian rhythm disturbance in sleep-deprived rats.

After more than six years of practice, the National Metabolic Management Center(MMC) has developed distinctive approaches to manage metabolic patients and has achieved a series of positive outcomes, continuously advancing the standardized diagnosis and treatment model. In order to further improve the efficiency, based on the first edition, the second edition guideline was composed by incorporating experience of the past six years in conjunction with the latest international and domestic guidelines.
4.Effect of Anmeidan on Serum Levels of BDNF, GFAP, and Irisin in Patients with Chronic Insomnia

ObjectiveTo explore the effect of Anmeidan on the sleep quality and serum levels of brain-derived neurotrophic factor (BDNF), glial fibrillary acidic protein (GFAP), and irisin in the patients with chronic insomnia. MethodA multicenter, randomized, double-blind, placebo-controlled clinical study was carried out, including 480 patients with chronic insomnia (deficiency syndrome) in Wuhan (Hubei), Guangzhou (Guangdong), and Lanzhou (Gansu). They were randomized into an observation group and a control group at a ratio of 1∶1. The observation group was orally administered with Anmeidan granules at a dose of 11 g, 3 times per day, and the control group with Anmeidan simulant at a dose of 11 g, 3 times per day, Both groups of patients received sleep education after enrollment. After 4 weeks of medication, the Athens insomnia scale (AIS) scores, Spiegel scale scores, and serum levels of BDNF, GFAP, and irisin were compared between the two groups as well as between before and after treatment. ResultA total of 480 adult patients with chronic insomnia were enrolled in this study, with 64 patients falled off. Finally, the 415 patients were included in the analysis, including 213 patients in the observation group and 202 patients in the control group. There was no difference in age or sex between the two groups of patients. Compared with before treatment, the treatment in both groups decreased the AIS and Spiegel scores (P<0.01). After treatment, the observation group had lower AIS and Spiegel scores than the control group (P<0.01). The treatment in the observation group slightly lowered the level of BDNF, elevated the level of irisin (P<0.05), and lowered the level of GFAP (P<0.05) in the serum. After treatment, the observation group showed higher level of irisin (P<0.05) and lower levels of BDNF and GFAP in the serum than the control group. ConclusionAnmeidan may improve the sleep quality of patients with chronic insomnia by elevating the irisin level and lowering the GFAP level in the serum.

Objective: To understand the current status of diagnosis and treatment of chronic lymphocytic leukemia (CLL) /small lymphocytic lymphoma (SLL) among hematologists, oncologists, and lymphoma physicians from hospitals of different levels in China. Methods: This multicenter questionnaire survey was conducted from March 2021 to July 2021 and included 1,000 eligible physicians. A combination of face-to-face interviews and online questionnaire surveys was used. A standardized questionnaire regarding the composition of patients treated for CLL/SLL, disease diagnosis and prognosis evaluation, concomitant diseases, organ function evaluation, treatment selection, and Bruton tyrosine kinase (BTK) inhibitor was used. Results: ①The interviewed physicians stated that the proportion of male patients treated for CLL/SLL is higher than that of females, and the age is mainly concentrated in 61-70 years old. ②Most of the interviewed physicians conducted tests, such as bone marrow biopsies and immunohistochemistry, for patient diagnosis, in addition to the blood test. ③Only 13.7% of the interviewed physicians fully grasped the initial treatment indications recommended by the existing guidelines. ④In terms of cognition of high-risk prognostic factors, physicians' knowledge of unmutated immunoglobulin heavy-chain variable and 11q- is far inferior to that of TP53 mutation and complex karyotype, which are two high-risk prognostic factors, and only 17.1% of the interviewed physicians fully mastered CLL International Prognostic Index scoring system. ⑤Among the first-line treatment strategy, BTK inhibitors are used for different types of patients, and physicians have formed a certain understanding that BTK inhibitors should be preferentially used in patients with high-risk factors and elderly patients, but the actual use of BTK inhibitors in different types of patients is not high (31.6%-46.0%). ⑥BTK inhibitors at a reduced dose in actual clinical treatment were used by 69.0% of the physicians, and 66.8% of the physicians had interrupted the BTK inhibitor for >12 days in actual clinical treatment. The use of BTK inhibitors is reduced or interrupted mainly because of adverse reactions, such as atrial fibrillation, severe bone marrow suppression, hemorrhage, and pulmonary infection, as well as patients' payment capacity and effective disease progression control. ⑦Some differences were found in the perceptions and behaviors of hematologists and oncologists regarding the prognostic assessment of CLL/SLL, the choice of treatment options, the clinical use of BTK inhibitors, etc. Conclusion: At present, a gap remains between the diagnosis and treatment of CLL/SLL among Chinese physicians compared with the recommendations in the guidelines regarding the diagnostic criteria, treatment indications, prognosis assessment, accompanying disease assessment, treatment strategy selection, and rational BTK inhibitor use, especially the proportion of dose reduction or BTK inhibitor discontinuation due to high adverse events.

6.Exploration of Potential Targets and Experimental Verification of Wendan Decoction in Treating Insomnia Based on Network Pharmacology

Objective A network-based pharmacological analysis approach to explore and validate the potential targets of Wendan decoction for the treatment of insomnia.Methods Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform was used to screen active components and corresponding targets of Wendan Decoction.Take the protein corresponding to the target intersection.protein-protein-interaction-network analysis of the interaction between target proteins.Through Database for Annotation,Visualization and Integrated Discovery database.Gene ontology and Kyoto Encyclopedia of Genes and Gnomes(KEGG)were used to analyze the biological process,cellular component,molecular function,and pathway enrichment of Wendan Decoction.The insomnia-related targets were searched through Human gene database,and the common targets of Wendan Decoction and insomnia were obtained.Building a common targets protein-protein interaction network.Cytoscape(visualization software)analyzes the core genes of the common targets,performs biological function and pathway enrichment analysis of the common targets,integrates the biological functions and signal transduction involved in the core genes,and identifies the core targets of Wendan Decoction treatment insomnia.Validation of core target mRNA expression in vivo.Results 97 active components of Wendan Decoction were screened,and 266 targets were identified.There were 2587 insomnia-related targets and 119 targets in total.The core genes in the common targets were AKT1(AKT Serine/Threonine Kinase 1),TNF(Tumor Necrosis Factor),IL-6(Interleukin 6),TP53(Tumor Protein P53),VEGFA(Vascular Endothelial Growth Factor A),CASP3(Caspase 3),MMP9(Matrix metallopeptidase 9),and MAPK3(Mitogen-Activated Protein Kinase 3).PI3K-AKT signaling pathway,apoptosis and p53 signaling pathway play important roles in the treatment of insomnia with Wendan decoction.Compared with the control group,the expression levels of AKT1 and MAPK3 in the brain tissue of the model group were significantly decreased(P<0.05),and the expression levels of TP53,VEGFA,Caspase-3 and TNF were significantly increased(P<0.05).Compared with the model group,the expression levels of AKT1 and MAPK3 in the brain tissue of mice in the high-dose group increased,and the expression levels of P53,VEGFA,Caspase-3 and TNF decreased.Conclusion The potential targets of Wendan decoction in treating insomnia are related to cell proliferation and apoptosis regulated by AKT1,MAPK3,TP53,VEGFA,Caspase-3 and TNF.
7.Lifestyle improvement and the reduced risk of cardiovascular disease: the China-PAR project.

BACKGROUND:
The benefits of healthy lifestyles are well recognized. However, the extent to which improving unhealthy lifestyles reduces cardiovascular disease (CVD) risk needs to be discussed. We evaluated the impact of lifestyle improvement on CVD incidence using data from the China-PAR project (Prediction for Atherosclerotic Cardiovascular Disease Risk in China).
METHODS:
A total of 12,588 participants free of CVD were followed up for three visits after the baseline examination. Changes in four lifestyle factors (LFs) (smoking, diet, physical activity, and alcohol consumption) were assessed through questionnaires from the baseline to the first follow-up visit. Cox proportional hazard models were used to estimate hazard ratios (HRs) and corresponding 95% confidence intervals (CIs). The risk advancement periods (RAPs: the age difference between exposed and unexposed participants reaching the same incident CVD risk) and population-attributable risk percentage (PAR%) were also calculated.
RESULTS:
A total of 909 incident CVD cases occurred over a median follow-up of 11.14 years. Compared with maintaining 0-1 healthy LFs, maintaining 3-4 healthy LFs was associated with a 40% risk reduction of incident CVD (HR = 0.60, 95% CI: 0.45-0.79) and delayed CVD risk by 6.31 years (RAP: -6.31 [-9.92, -2.70] years). The PAR% of maintaining 3-4 unhealthy LFs was 22.0% compared to maintaining 0-1 unhealthy LFs. Besides, compared with maintaining two healthy LFs, improving healthy LFs from 2 to 3-4 was associated with a 23% lower risk of CVD (HR = 0.77, 95% CI: 0.60-0.98).
CONCLUSIONS
Long-term sustenance of healthy lifestyles or improving unhealthy lifestyles can reduce and delay CVD risk.
8.Determination of methyl isobutyl ketone in urine by headspace coupled with gas chromatography-mass spectrometry.

Objective: To establish a method for the determination of methyl isobutyl ketone (MIBK) in urine samples by headspace-gas chromatography-mass spectrometry. Methods: Automatic headspace sampling technique was adopted to optimize the headspace conditions (headspace bottle heating temperature and equilibration time) and gas chromatographic conditions. A total of 5 ml samples were taken and added with 3.0 g ammonium sulfate into a 20 ml headspace bottle. After heated at 60 ℃ for 30 mins, gas from the upper part of headspace bottle was injected into gas chromatography with an injection volume of 100 μl. The target was separated by HP-5MS UI (30 m×0.25 mm×0.25 μm) capillary column and then detected by mass spectrometry detector. The retention time and external standard method were used for qualitative and quantitative analysis of MIBK in samples, respectively. Results: The standard curve of MIBK showed significant linearity between 20.0-1 000.0 μg/L. The standard curve was y=62.9x-652.5, and the correlation coefficient r=0.9998. The detection limit of MIBK was 5.0 μg/L and the quantification limit of MIBK was 16.0 μg/L. The average recovery rate was 95.3%~100.2% at three spiked concentrations of low (50.0 μg/L) , medium (200.0 μg/L) and high (500.0 μg/L) . The intra-day and inter-day precisions were 1.7%~3.8% (n=6) and 1.2%~4.0% (n=6) respectively. This method was stable for the determination of MIBK, and the urine could be kept 14 d at -20 ℃ without significantly loss. Conclusion: This method is proved to be simple, practical and highly sensitive. It can satisfy the request for the determination of urine samples of workers exposed to MIBK.

9.Meta-analysis on the contents of trace elements in workers with occupational exposure to lead.

Objective: To quantitatively evaluate the content differences of trace elements in workers with occupational exposure to lead. Methods: In January 2021, relevant literatures on the contents of trace elements in workers with occupational exposure to lead published from 1990 to 2020 were searched through CNKI, Wanfang, VIP, PubMed, web of science and Embase. Screened and extracted the literatures, and evaluated the quality of the included literatures with Newcastle Ottawa Scale. Meta analysis was performed with Review Manager 5.3 software, and standardized mean difference (SMD) and its 95% confidence interval were used as effect indicators. Results: A total of 20 literatures were included, and the quality scores were 5-7. The results of Meta-analysis showed that compared with the control group, the contents of blood zinc (SMD=-1.01, 95%CI: -1.53, -0.49) , hair zinc (SMD=-0.17, 95%CI: -0.33, -0.01) , hair copper (SMD=-0.50, 95%CI: -1.01, 0) , hair iron (SMD=-3.91, 95%CI: -5.80, -2.03) and hair manganese (SMD=-1.09, 95%CI: -2.02, -0.15) in occupational lead exposure group were significantly lower (P<0.05) . Compared with the control group, the content of cobalt in hair of occupational lead exposure group (SMD=1.41, 95%CI: 0.72, 2.10) was higher, and the difference was statistically significant (P<0.05) . There was no significant difference in the contents of blood chromium, blood copper, blood iron, blood manganese, blood selenium and hair nickel between the two groups (P>0.05) . Conclusion: Workers with occupational exposure to lead have abnormal trace elements.

10.Anmeidan Improves Learning and Memory Ability of Sleep-deprived Rats by Regulating Astrocyte Activity

ObjectiveTo explore the effect of Anmeidan (AMD) on the learning and memory ability of sleep-deprived rats and the mechanism. MethodA total of 50 SD rats were randomized into control group, model group, low-dose AMD group (4.55 g·kg-1·d-1), high-dose AMD group (18.18 g·kg-1·d-1), and estazolam group (0.09 mg·kg-1·d-1). Insomnia was induced in rats with the self-made sleep deprivation box (21 days). The learning and memory ability of rats was measured by Morris water maze. Immunofluorescence method was employed to detect the number of cells expressing N-myc downstream-regulated gene 2 (NDRG2) and glial fibrillary acidic protein (GFAP) in hippocampus of rats, real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) to determine the mRNA expression of hippocampal NDRG2, glial glutamate transporter-1 (GLT-1), and GluNR2A and GluNR2B N-methyl-D-aspartate (NMDA) receptor subunits, and Western blot to examine the protein expression of NDRG2 and GLT-1 in hippocampus. ResultCompared with control group, the model group showed increase in the latency to reach the platform and total swimming distance, significant decrease in the total distance moved in the target quadrant, time in target quadrant, and times of crossing the platform (P<0.01), rise in the number of cells expressing NDRG2 and GFAP in the hippocampal CA1 region (P<0.01) and the mRNA level of NDRG2 and GluNR2B, reduction in the mRNA level of GLT-1 and GluNR2A, elevation in NDRG2 protein expression (P<0.01), and decrease in GLT-1 protein expression (P<0.01). In contrast to the model group, low-dose and high-dose AMD improved the learning and memory levels of sleep-deprived rats (P<0.01), reduced the number of cells expressing NDRG2 and GFAP (P<0.01), significantly decreased the mRNA expression of NDRG2 and GluNR2B, increased the mRNA expression of GLT-1 and GluNR2A, reduced NDRG2 protein level (P<0.05, P<0.01), and raised GLT-1 protein level (P<0.01). ConclusionAMD can improve the learning and memory ability of sleep-deprived rats. The mechanism is the likelihood that it regulates astrocyte activity, thereby affecting the neurotransmitter level and synaptic plasticity in the brain.
