4.Correlation between cervical vertigo and vestibular function evaluated by vestibular evoked myogenic potentials.

OBJECTIVE:
To analyze the relationship between cervical vertigo and vestibular function evaluated by vestibular evoked myogenic potentials(VEMPs) and analyze the correlations between cervical vertigo and vestibular dysfunction, discuss the related factors of cervical vertigo and guide the clinical treatment of patients with cervical vertigo.
METHODS:
A total of 75 patients with cervical vertigo as the main complaint in the outpatient clinic of the Second Hospital of Shanxi Medical University from August 2019 to July 2020 were set as the diseased group, and 60 patients without cervical and vestibular related diseases in the hospital were selected to set as non-diseased group. The age of diseased group was 12 to 70 years with an average of (46.40±10.91) years, including 25 males and 50 females;and the age of non-diseased group was 22 to 60 years with an average of(43.78±7.75) years, including 19 males and 51 females. VEMPs were performed in the two groups. The data of VEMPs were collected and the results were compared and analyzed. The patients with abnormal cervical myogenic vestibular evoked myogenic potential (cVEMP) were divided into light, moderate and severe groups. The correlation between VEMPs and cervical vertigo and its severity were analyzed by statistical method.
RESULTS:
(1)The severity of cervical vertigo in diseased group:33 cases of mild, 34 cases of moderate, 8 cases of severe; cVEMP examination:62 cases were positive and 13 cases were negative, including 13 cases of mild, 33 cases of moderate, 16 cases of severe. The cVEMP of non-diseased group:4 cases were positive and 56 cases were negative.(2) The level of cVEMP in diseased group was higher than that in non-diseased group (P<0.001). It can be considered that there was a correlation between cervical vertigo and vestibular function.(3)The correlation between the level of cVEMP and the level of cervical vertigo in diseased group was analyzed. The Spearman rank sum test was used, and the correlation coefficient was 0.687, which was statistically significant (P<0.05). And it can be considered that the two indicators have a high degree of correlation.
CONCLUSION
It is feasible to evaluate the relationship between cervical vertigo and vestibular function by VEMPs. For patients with cervical vertigo, the higher the severity, the greater the positive rate of VEMPs, which indicates that it has a greater impact on vestibular function. The treatment of patients with cervical vertigo should be the combination of cervical rehabilitation and vestibular function.

5.Effect of Maxing Shigantang on Airway Injury and EGFR Expression in Asthma Model Rats

ObjectiveTo explore the optimal formula of Maxing Shigantang in regulating epidermal growth factor receptor(EGFR)expression and alleviating airway injury in asthmatic rats and to reveal the underlying mechanism. MethodSD male rats were randomly divided into normal group, model group, dexamethasone group (5×10-4 g·kg-1) and Maxing Shigantang 1∶0.5, 1∶1, 1∶2 groups (group A, B, C, 10 g·kg-1), with 8 rats in each group. The other groups except the normal group received nebulization of 2% acetylcholine chloride and 0.4% histamine phosphate for the modeling of asthma. One hour before modeling, the normal group and the model group were given the same amount of normal saline, and the other groups were given the same amount of corresponding drugs, once a day for 7 days. On the 7th day, the model was established and the incubation period of asthma was recorded. The rats were then immediately anesthetized, and arterial blood and tracheal tissue were collected. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the levels of interleukin-2 (IL-2), interleukin-4 (IL-4), and tumor necrosis factor-α (TNF-α) in serum. Pathological sections were prepared for the observation of the pathological changes of tracheal tissues and the ultrastructure of epithelial cells in each group. Terminal-deoxynucleotidyl transferase-mediated nick-end labeling (TUNEL) was adopted to detect epithelial cell apoptosis, and in situ hybridization and Western blot were employed to determine the mRNA and protein levels of epidermal growth factor receptor (EGFR), respectively. ResultCompared with the model group, groups A, B and C prolonged the incubation period of asthma (P<0.05,P<0.01). Compared with the control group, the model group showed declined IL-2 level (P<0.01), risen IL-4 and TNF-α levels (P<0.05,P<0.01), increased airway pathology score, collagen volume fraction, and airway epithelial cell apoptosis index (P<0.01), and up-regulated mRNA and protein levels of EGFR in trachea tissue (P<0.01). Compared with the model group, group A showed increased IL-2 level (P<0.05) and declined IL-4 (P<0.05,P<0.01) level, and group B showed declined IL-4 level (P<0.05). The level of TNF-α in groups A, B, and C declined compared with that in the model group (P<0.01). Maxing Shigantang repaired the tracheal tissue to different degrees (P<0.05). Among the three groups, group A inhibited tracheal fibrosis (P<0.05) and had the most significant effect of repairing the ultrastructural changes of airway epithelial cells. Groups A, B and C all inhibited the apoptosis of airway epithelial cells (P<0.05). All the three groups inhibited the up-regulation of EGFR mRNA level (P<0.05,P<0.01), and groups B and C inhibited the up-regulation of EGFR protein level (P<0.05,P<0.01). ConclusionMaxing Shigantang can inhibit the abnormal changes of airway epithelial structure, alleviate airway injury, and can down-regulate the expression of EGFR in the tracheal tissue of asthma model rats. In this study, the optimal compatibility of Maxing Shigantang to repair airway epithelial injury in asthmatic rats was group A, with the Ephedrae Herba-Armeniacae Semen Amarum-Glycyrrhizae Radix et Rhizoma-Gypsum Fibrosum ratio of 1∶0.5∶4∶1.
6.Effects and Mechanism of Xiaozheng Decoction on the Proliferation and Migration of Uterine Leiomyoma Cells in Rat

OBJECTIVE:To study the effects a nd mechanism of X iaozheng decoction on the proliferation and migration of uterine leiomyoma cells in rat. METHODS :Female SD rats were randomly divided into normal control group ,model group , chemical medicine positive control group (Mifepristone tablets ,2.25 mg/kg),TCM positive control group (Guizhi fuling capsules , 200 mg/kg),Xiaozheng decoction low-dose ,medium-dose and high-dose groups (1.4,2.8,5.6 g/kg),with 15 rats in each group. Except for normal control group ,other groups were given intramuscular injection of estrogen and progesterone to induce uterine leiomyomas model. On the second day after modeling ,rats in administration groups were given relevant medicine intragastrically ; normal control group and model group were given constant volume of normal saline intragastrically ,once a day ,for consecutive 4 months. After last administration ,the uterus was removed and its morphology was observed ;the uterine coefficient was calculated. Uterine leiomyoma cells or uterine smooth muscle cells were isolated and cultured. The proliferation rate and migration rate of cells were detected by MTT method and cell scratch test ;flow cytometry was used to detect the cell apoptosis rate ;mRNA expression of HMGB 1 were detected by RT-qPCR. The expression of phosphorylated protein kinase B (p-Akt),nuclear factor κB inhibitor α (IκBα)and phosphorylated transforming growth factor β-activated kinase 1(p-TAK1)were detected by ELISA ;the protein expression of HMGB 1,phospholipid 3 kinase(PI3K),p-Akt in cytoplasm and nuclear factor κB p65(NF-κB p65)in nucleus were detected by Western blot assay. RESULTS :Compared with normal control g roup,the myometrium of the model group was significantly thickened ,the number of uterine smooth muscle cells were significantly increased and the sizes were 83777930。E-mail:jianghua_ouyang@126.com different,the arrangement of muscle fibers in so me areas was disordered,the uterine coefficient ,and the relative expression of HMGB 1 mRNA and protein were increased significantly (P< 0.01). Compared with model group ,the thickening of uterine myometrium and other symptoms were improved to different extents in Xiaozheng decoction groups and positive control groups ;the uterine coefficient ,cell proliferation rate ,migration rate ,mRNA and protein expression of HMGB 1,the expression of p-Akt and IκBα,protein expression of PI 3K and p-Akt (except for Xiaozheng decoction low-dose group )in cytoplasm were all decreased significantly (P<0.05 or P<0.01);the cell apoptosis rate ,the expression of p-TAK 1,protein expression of NF-κB p65 in nucleus were all increased significantly (P<0.05 or P<0.01). The effects of Xiaozheng decoction showed a dose-dependent trend. CONCLUSIONS :Xiaozheng decoction can inhibit the proliferation and migration of uterine leiomyoma cells by down-regulating the expression of HMGB 1,PI3K and p-Akt ,up-regulating the expression of NF-κB p65,so as to promote cell apoptosis.
7.Single-cell analysis of angiotensin-converting enzyme II expression in human kidneys and bladders reveals a potential route of 2019 novel coronavirus infection.

BACKGROUND:
Since 2019, a novel coronavirus named 2019 novel coronavirus (2019-nCoV) has emerged worldwide. Apart from fever and respiratory complications, acute kidney injury has been observed in a few patients with coronavirus disease 2019. Furthermore, according to recent findings, the virus has been detected in urine. Angiotensin-converting enzyme II (ACE2) has been proposed to serve as the receptor for the entry of 2019-nCoV, which is the same as that for the severe acute respiratory syndrome. This study aimed to investigate the possible cause of kidney damage and the potential route of 2019-nCoV infection in the urinary system.
METHODS:
We used both published kidney and bladder cell atlas data and new independent kidney single-cell RNA sequencing data generated in-house to evaluate ACE2 gene expression in all cell types in healthy kidneys and bladders. The Pearson correlation coefficients between ACE2 and all other genes were first generated. Then, genes with r values larger than 0.1 and P values smaller than 0.01 were deemed significant co-expression genes with ACE2.
RESULTS:
Our results showed the enriched expression of ACE2 in all subtypes of proximal tubule (PT) cells of the kidney. ACE2 expression was found in 5.12%, 5.80%, and 14.38% of the proximal convoluted tubule cells, PT cells, and proximal straight tubule cells, respectively, in three published kidney cell atlas datasets. In addition, ACE2 expression was also confirmed in 12.05%, 6.80%, and 10.20% of cells of the proximal convoluted tubule, PT, and proximal straight tubule, respectively, in our own two healthy kidney samples. For the analysis of public data from three bladder samples, ACE2 expression was low but detectable in bladder epithelial cells. Only 0.25% and 1.28% of intermediate cells and umbrella cells, respectively, had ACE2 expression.
CONCLUSION
This study has provided bioinformatics evidence of the potential route of 2019-nCoV infection in the urinary system.

This study was designed to determine the metabolites of Zhali Nusi Prescription(ZLNSP) in rats. The ultra-high performance liquid chromatography-LTQ Orbitrap mass spectrometric(UHPLC-LTQ-Orbitrap-MS) and mass defect filter techniques were applied to analyze the metabolites of ZLNSP in rat plasma, bile, urine and feces. The biological samples were analyzed by ACQUITY UPLC BEH T_3 column(2.1 mm×100 mm,1.7 μm), with 0.1% formic acid water(A)-acetonitrile(B) as mobile phase, and the biological samples were analyzed in negative ion mode by electrospray ionization mass spectrometry(ESI-MS). An analytical method for biological samples of rats was established, and 8 prototype components and 36 metabolites were identified. The results showed that the metabolic pathways of the main components of ZLNSP in rats included methylation, glucuronidation, sulfation and so on. It provi-ded information for the therapeutic effect of ZLNSP in vivo.

10. Advances in the research of Fournier gangrene

Fournier gangrene is a relatively rare clinical critical disease, and its clinical symptoms are not specific and easily unrecognized by some clinicians. It has the features of acute onset, quick development, severe illness, and often accompanied by infection shock which is seriously life-threatening. It is difficult in treatment with high medical costs and long length of hospitalization, which increases pain for patients and relatives and brings heavy economic and psychological burden on patients, society, and medical workers. By reviewing the literature home and abroad and combined with clinical practice, I summarize the researches on concept, epidemiology, clinical manifestation, diagnosis and treatment of Fournier gangrene, in order to provide reference for vast number of clinical workers.
