1.Exploring Vascular Recruitment in Tracheobronchial Adenoid Cystic Carcinoma from Perspective of Abnormal Collateral

Tracheobronchial adenoid cystic carcinoma (TACC) is a low-grade malignant tumor originating from the airway mucosa. Despite its slow progression,it is characterized by high invasiveness,frequent recurrence,and a strong tendency for metastasis. Preclinical studies have shown that vascular-targeted therapy holds significant potential. However,an effective systemic treatment for TACC has not been established yet. This study explored TACC from the perspective of "Feiji" in traditional Chinese medicine (TCM) as the starting point. It deeply investigated the mechanisms of abnormal collaterals and tumor vascular recruitment and further elaborated on the theoretical connection between abnormal collaterals and tumor vascular recruitment. Firstly,collateral hyperactivity led to disordered and erratic pulmonary collaterals. Their abnormal structures were similar to the disorderly and tortuous nature of tumor (pseudo)angiogenesis. This resulted in imbalances in the functions of circulation,perfusion,and reverse injection of the pulmonary collaterals,and then led to unrestrained collateral dysfunction and the accumulation of pathogenic factors. Secondly,the remodeling of the extracellular matrix (ECM) and epithelial-mesenchymal transition (EMT) in TACC were critical processes in vascular co-option (VCO),representing the micro-level manifestation of the displacement of nutrient and defense. During this process,ECM remodeling made TACC cells more likely to hijack normal blood vessels,creating a complex vascular microenvironment conducive to tumor growth. In terms of treatment,this study proposed a TCM strategy of "regulating collaterals to expel pathogenic factors and nourishing collaterals to strengthen the healthy Qi",and listed potential TCM. These were intended to regulate the Qi and blood in the collaterals,repair the functions of abnormal collaterals,and intervene in the vascular recruitment process of TACC. Future research should focus on improving the TCM clinical syndrome characteristics of TACC. Through modern molecular biology techniques,it is necessary to deeply analyze the micro-level pattern of vascular recruitment in TACC. This would enrich the understanding of the profound connection between abnormal collaterals and tumor vascular recruitment,providing empirical evidence for TCM-targeted therapies for vascular recruitment in TACC.
2.Discussion on the Treatment of Heart Failure from the Theory of "Sweat Pore-Collaterals-Zang and Fu Organs" Based on Mitochondrial Function

Mitochondria are the main site of energy metabolism in cardiomyocytes, and at the same time mediate apoptosis and immune response, so mitochondrial dysfunction is closely related to the development of heart failure. Combined with the pathogenesis of mitochondrial dysfunction and heart failure, it is proposed that the mitochondrial function is similar to "sweat pore - collaterals - zang and fu organs", according to which the treatment of heart failure is based on the theory of "sweat pore - collaterals - zang and fu organs". It is believed that the core mechanism of heart failure is qi deficiency, and qi deficiency leads to the weakness of the sweat pore opening and closing, or even the sweat pore closure, then resulting in qi deficiency and blood stasis, collaterals stagnation fail to flourish, and qi, blood, and body fluids can not infiltrate and nourish zang-fu organs, so that the heart fail to be nourished, and the disease will develop. The treatment should be based on the method of boosting qi and opening sweat pore, using acridity to unblock the collaterals, and invigorating blood and draining water, with medicinal of boosting qi, invigorating blood, and draining water as treatment.
3.Exploring the factors affecting music-related quality of life in post-lingual deaf adults with cochlear implants

Objective:To observe the feelings of listening to music and the importance of music in the daily life of post-lingual deaf adults with cochlear implants, and to explore the relevant influencing factors.Methods:This was a cross-sectional survey study. From January 2021 to August 2021,the Music-Related Quality of Life Scale was used to evaluate the music needs and music experiences of 63 post-lingual deaf adults who met the inclusion criteria, including 27 males and 36 females, aged (40.7±12.3) years, at the time of surgery (36.8±13.1) years, and with a preoperative hearing aid ineffective time of (3.9±5.8) years. Indicators analyzed included age, duration of ineffective preoperative hearing aid wear, preoperative music preference, duration of postoperative cochlear implant use, current hearing aid modality, and auditory rehabilitation outcomes. Whether the six factors mentioned above constituted an influence on the subjects′ music listening was investigated using SPSS 25.0 statistical software.Results:All of the observations in the scale were correlated with a single factor. The two sub-dimensions of music experience section were related to the effect of auditory rehabilitation. In the importance section, the effect of auditory rehabilitation was the influential factor of the dimension of "participation importance", and the preoperative enjoyment of music was the relevant influential factor of the dimension of "perceived importance". There was a significant difference between the groups when they were grouped by the above factors ( P value<0.05), while there was no statistically significance between the groups when they were grouped by other factors ( P value>0.05). Conclusions:Post-lingual deaf adults show the need and attempt to listen to music after cochlear implantation. The effectiveness of auditory rehabilitation and the degree of music preference preoperatively are two important factors that influence music listening in implant recipients. Once the level of auditory communication has been restored to a certain degree, it is important to pay more attention to the needs of music for implant recipients and train them in time, especially for those with music preferences preoperatively.
4.Diabetic retinopathy detection algorithm based on transfer learning

Objective To investigate a diabetic retinopathy ( DR ) detection algorithm based on transfer learning in small sample dataset. Methods Total of 4465 fundus color photographs taken by Gaoyao People ' s Hospital was used as the full dataset. The model training strategies using fixed pre-trained parameters and fine-tuning pre-trained parameters were used as the transfer learning group to compare with the non-transfer learning strategy that randomly initializes parameters. These three training strategies were applied to the training of three deep learning networks:ResNet50,Inception V3 and NASNet. In addition,a small dataset randomly extracted from the full dataset was used to study the impact of the reduction of training data on different strategies. The accuracy and training time of the diagnostic model were used to analyze the performance of different training strategies. Results The best results in different network architectures were chosen. The accuracy of the model obtained by fine-tuning pre-training parameters strategy was 90. 9%,which was higher than the strategy of fixed pre-training parameters (88. 1%) and the strategy of randomly initializing parameters ( 88. 4%) . The training time for fixed pre-training parameters was 10 minutes,less than the strategy of fine-tuning pre-training parameters ( 16 hours ) and the strategy of randomly initializing parameters (24 hours). After the training data was reduced,the accuracy of the model obtained by the strategy of randomly initializing parameters decreased by 8. 6% on average,while the accuracy of the transfer learning group decreased by 2. 5% on average. Conclusions The proposed automated and novel DR detection algorithm based on fine-tune and NASNet structure maintains high accuracy in small sample dataset,is found to be robust,and effective for the preliminary diagnosis of DR.
5.A triage strategy in advanced ovarian cancer management based on multiple predictive models for R0 resection: a prospective cohort study.

OBJECTIVE: To present the surgical outcomes of advanced epithelial ovarian cancer (AEOC) since the implementation of a personalized approach and to validate multiple predictive models for R0 resection. METHODS: Personalized strategies included: 1) Non-invasive model: preoperative clinico-radiological assessment according to Suidan criteria with a predictive score for all individuals. Patients with a score 0–2 were recommended for primary debulking surgery (PDS, group A), or otherwise were counseled on the choices of PDS, neoadjuvant chemotherapy (NAC, group B) or staging laparoscopy (S-LPS). 2) Minimally invasive model: S-LPS with a predictive index value (PIV) according to Fagotti. Individuals with a PIV < 8 underwent PDS (group C) or otherwise received NAC (group D). Intraoperative assessment (with Eisenkop, peritoneal cancer index [PCI], and Aletti scores) and surgical results were prospectively collected. RESULTS: Between September 2015 and August 2017, 161 pathologically confirmed epithelial ovarian cancer patients were included. A total of 52 (32.3%) patients had a predictive score of 0–2, and 109 (67.7%) patients had a score ≥ 3. Among these individuals, 41 (25.5%) patients received S-LPS. Finally, 110 (68.3%) patients underwent PDS (A+C), and 51 (31.7%) patients received NAC (B+D). The R0 resection rates in PDS and NAC patients were 56.4% and 60.8%, respectively. The area under the curve (AUC) of Suidan criteria was 0.548 for group (A+C). The AUC of Fagotti score was 0.702 for group C. The AUC of Eisenkop, PCI, and Aletti scores were 0.808, 0.797, and 0.524, respectively. CONCLUSION: The Suidan criteria were not effective in these AEOC patients. S-LPS was helpful in decision-making for PDS and should be endorsed in the future.

6.Role of Th1 cell and its related cytokines IFN-γin the regulation of immune function in patients with AIDS

Objective To investigate the regulation of immune function in patients with acquired immune deficiency syndrome (AIDS) by the secretion of T cell 1 (Th1) gamma interferon gamma (IFN-γ) in patients with acquired immune deficiency syndrome. Methods From January 2011 to January 2014, 20 cases of laboratory-confirmed AIDS patients were collected for research. Patients with AIDS received highly active anti-retroviral therapy (HAART group), while 20 healthy volunteers were selected as control group. Before receiving highly active anti-retroviral treatment in six months and 12 months , the patients with AIDS were collected in blood study of peripheral blood mononuclear cells (PBMC) in separated culture group and IFN-γ co-culture group for testing culture differential expression of cytokines and immune cells in solution. Results Before treatment in 6 months and 12 months , the supernatants in the cultured alone group and IFN-γ co-culture group by IL-12, IL-21 levels were significantly lower than that in the healthy group (P < 0.05); After treatment in 6 months and 12 months , the supernatants in the IFN-γ co-culture group by IL-12 and IL-21 levels were significantly higher than that in the cultured alone group (PP < 0.05). Before treatment in 6 months and 12 months,the level of IP-10 and CD4 + CD25 + Foxp3Treg cells in the cultured alone group and IFN-γ co-culture group, were significantly higher than the healthy group (P < 0.05); After treatment in 6 months and 12 months, the supernatant in the IFN-γ co-culture group was significantly higher than the average in the cultured alone group in IP-10; CD4 + CD25 + Foxp3Treg cells in the IFN-γ co-culture group were significantly lower than that in the cultured alone group (P < 0.05). Conclusion Patients with AIDS received highly active anti-retroviral therapy can improve the immunity. IFN-γ may further stimulate the secretion of immune cytokine in patients with AIDS.

Objective To explore the relationship between cervical lesions in patients with high -risk human papilloma virus(HPV)infection and serum interleukin -1 beta(IL -1 ),interleukin 2(IL -2),interleukin 10(IL -10).Methods 180 cases of cervical lesions were treated in our hospital from August 2013 to August 2015. Among them,105 cases were infected with HPV,and 75 cases were not infected by HPV.The serum levels of IL -2, IL -10 and IL -1βwere observed in two groups,and compared the levels of serum IL -2,IL -10 and IL -1βin patients with different DNA hr -HPV load,the correlation between DNA hr -HPV load and IL -1β,IL -2 and IL -10 levels was analyzed.Results The levels of IL -1β,IL -10,IL -2 were (0.85 ±0.23)ng/L,(182.35 ± 10.02)ng/L,(38.97 ±5.23)ng/L in patients with HPV infection,the levels of IL -1β,IL -10,IL -2 in patients without HPV infection were (0.62 ±0.18)ng/L,(305.42 ±11.13)ng/L,(25.18 ±3.16)ng/L.The levels of IL -1β,IL -10 were significantly higher in HPV group than in without HPV infection group,the level of IL -2 in HPV group was significantly lower than that of uninfected HPV group,and the differences were statistically significant (t =7.222,20.328,-7.558,all P <0.01).There were significant differences in the load of DNA hr -HPV between different pathological changes of cervical lesions,and the DNA hr -HPV load increased significantly with the increase of the degree of disease(t =6.214,19.097,33.906,6.952,6.274,all P <0.05).IL -1βand IL -10 levels from high to low were DNA hr -HPV load volume >1 000,100 -1 000 and <100,IL -2 levels from high to low were DNA hr -HPV load <100,100 -1 000 and >1 000.DNA hr -HPV load was positively correlated with serum IL -1βand IL -10 levels in patients with cervical lesions and HPV infection(r =0.452,0.422,P =0.035,0.019),and nega-tively correlated with IL -2 level(r =-0.398,P =0.027).Conclusion hr -HPV infection is closely correlated with serum IL -1β,IL -2,IL -10 levels in cervical lesions patients,IL -1,IL -2 and IL -10 can be used as impor-tant index for clinical monitoring.

The central nervous system (CNS) plays a key regulatory role in glucose homeostasis. In particular, the brain is important in initiating and coordinating protective counterregulatory responses when blood glucose levels fall. This may due to the metabolic dependency of the CNS on glucose, and protection of food supply to the brain. In healthy subjects, blood glucose is normally maintained within a relatively narrow range. Hypoglycemia in diabetic patients can increase the risk of complications, such as heart disease and diabetic peripheral neuropathy. The clinical research finds that the use of traditional Chinese medicine (TCM) has a positive effect on the treatment of hypoglycemia. Here the authors reviewed the current understanding of sensing and counterregulatory responses to hypoglycemia, and discuss combining traditional Chinese and Western medicine and the theory of iatrogenic hypoglycemia in diabetes treatment. Furthermore, the authors clarify the feasibility of treating hypoglycemia on the basis of TCM theory and CNS and have an insight on its clinical practice.
9.The characteristics of ABL1 gene mutations in 210 cases of Chinese patients who have imatinibresistant in chronic myeloid leukemia and Ph positive acute lymphoblastic leukemia

ObjectiveTo understand the characteristics of mutations in BCR-ABL1 kinase domain mutation,these chronic myeloid leukemia (CML) and Ph positive acute lymphoblastic leukemia (ALL)patients who got imatinib treatment had poor effect.MethodsTotally 177 CML patients and 33 Ph( + )ALL patients were selected at Beijing Dao-Pei Hospital from Sep.2007 to Dec.2010.All of them were Chinese patients.Totally 243 bone marrow or peripheral blood specimens were collected from the patients,who had early effect,then resistance emergenced,or for more than 3 months of poor efficacy.Extracted total RNA from the specimens' nuclear cells,reversed transcription to cDNA.Amplified the whole span of BCRABL1 fusion kinase gene by nest PCR (from 242 to 493 amino acid coding sequence),used the type AB3130XL gene sequencing instrument determinate the gene sequence of ABL1 kinase region and then used the Variant Reporter V1.0 software to analyze the results of gene mutations.ResultsThirty-two kinds of different mutations were detected of ABL1 gene mutations,accounting for 34.2% (83/243 cases).Among them,the T315I was 12% (10/83),mutation rate was the highest,followed by Y253H was 11% (9/83),G250E was 7% (6/83),E255K was 7% (6/83),M351T was 6% (5/83),E459K was 5% (4/83) ;Q252H,D276G,F317L,E355G,F359V,H396R were all 4% (3/83).Three cases of insertion mutations were found,including 2 cases of 357-358insk,1 case of V304RfsX17.Seven patients had found existence two or more point mutations.The multiple drug resistance mutations might exist in the same leukemia clone.The same individual was not only contain common resistance mutations,but also rare point mutations,insertion mutations.The mutations might be lead to loss of kinase activity.ConclusionsUnder the imatinib drugs pressure,the ABL1 gene mutation in leukemia cells appears randomly,and results in different resistant clones.Different resistant clones can coexist in the same patients in vivo; resistant clones not only contain point mutations,but also contain inserted deletion mutations.
