1.Quality evaluation of Xintong granules based on HPLC fingerprint and quantitative analysis of multi-components by single-marker method

OBJECTIVE To establish the HPLC fingerprint of Xintong granules and the quantitative analysis of multi- components by single-marker method (QAMS) to determine the contents of 7 components, so as to provide a scientific basis for their quality control. METHODS HPLC method was used to establish the fingerprints for 10 batches of Xintong granules (No. S1- S10), and similarity evaluation, cluster analysis (CA) and partial least squares-discriminant analysis (PLS-DA) were performed. At the same time, the contents of seven components, including puerarin, daidzin, calycosin-7-O- β -D-glucoside, stilbene glycoside, naringin, icariin and tanshinone ⅡA, were determined by QAMS method, and were compared with the results of external standard method. RESULTS A total of 18 common peaks were marked and 7 peaks were identified in the HPLC fingerprints for 10 batches of Xintong granules, namely puerarin (peak 4), daidzin (peak 7), calycosin-7-O-β-D-glucoside (peak 9), stilbene glycoside (peak 10), naringin (peak 12), icariin (peak 17), and tanshinone ⅡA (peak 18); the similarities among them were more than 0.990, and CA and PLS-DA results showed that S4-S5,S8-S10,S1-S3 and S6-S7 were clustered into three categories, respectively. Using naringin as the internal standard, the contents of puerarin, daidzin, calycosin-7-O-β-D-glucoside, stilbene glycoside, icariin and tanshinone ⅡA were determined to be 7.868 1-10.181 2, 1.709 2-2.374 1, 0.285 2-0.326 3, 1.024 1- 1.523 9, 0.140 2-0.290 4, and 0.077 1-0.219 4 mg/g, respectively, by the QAMS. These results showed no significant differences compared to those obtained by the external standard method. CONCLUSIONS Established HPLC fingerprint and QAMS method are convenient, stable and accurate, which can provide a basis for the quality evaluation of Xintong granules.
2.Intraperitoneal chemotherapy for colorectal cancer peritoneal metastasis

Peritoneal metastasis is one of the common site of colorectal cancer metastasis and associated with a poor prognosis. The core strategy for colorectal cancer peritoneal metastasis primarily revolves around a comprehensive treatment approach with cytoreductive surgery and systemic chemotherapy as the mainstay, supplemented by intraperitoneal chemotherapy. As an important supplement to treatment, intraperitoneal chemotherapy has broad application prospects. The main modalities are hyperthermic intraperitoneal chemotherapy (HIPEC), neoadjuvant intraperitoneal and systemic chemotherapy (NIPS), early postoperative intraperitoneal chemotherapy (EPIC), sequential postoperative intraperitoneal chemotherapy (SPIC), normothermic intraperitoneal chemotherapy (NIPEC) and pressurized intraperitoneal aerosol chemotherapy (PIPAC). To promote the standardized application of intraperitoneal chemotherapy, further research on the mechanisms underlying peritoneal metastasis of colorectal cancer, selection of effective intraperitoneal chemotherapy agents, determination of optimal timing and administration protocols, exploration of the feasibility of sequential intraperitoneal chemotherapy and conduction of valuable basic and clinical research are currently needed. This paper will review the development and origins of intraperitoneal chemotherapy, treatment modalities, as well as the current application status and prospects of various treatment approaches in the context of peritoneal metastasis of colorectal cancer.
3.The value of mammogram and MRI parameters combined with peripheral blood RLR in predicting recurrence and metastasis after modified radical breast cancer surgery

Objective:To investigate the value of mammography and MRI parameters combined with red cell distribution width to lymphocyte ratio (RLR) in predicting recurrence and metastasis of breast cancer after modified radical surgery.Methods:104 female breast cancer patients who received improved radical surgery in Jiande First People’s Hospital from Jun. 2021 to Dec. 2023 were included as the study objects. The Japanese MGU-1000D MAMMOREX Pe.ru.ru DIGITAL mammography machine was used to examine each subject before surgery, and the X-ray signs of the primary tumor lesion were recorded, including maximum diameter, breast density, calcification, mass morphology, vascular signs and tumor margin. The primary tumor was evaluated by MRI using a MAGNETOM Verio magnetic resonance imaging machine and a dedicated breast examination coil, the apparent diffusion coefficient (ADC) value was measured and automatically calculated, and the RLR was recorded.Results:There were 31 patients with recurrence and metastasis and 73 patients without recurrence or metastasis. The proportion of patients with clinical stage III and Ki67 level > 14% in postoperative recurrence and metastasis group was significantly higher than that in patients without recurrence and metastasis. The proportion of patients with calcification, vascular thickening, increase and burr at the edge of mass in the recurrence and metastasis group was significantly higher than that in the patients without recurrence or metastasis. The ADC value of patients with recurrence and metastasis after breast cancer surgery was 0.93±0.12, and that of patients without recurrence and metastasis was 1.08±0.15, the former was significantly lower than the latter, and the difference was statistically significant ( t=5.64, P<0.001). The RLR of peripheral blood in patients with recurrence and metastasis after breast cancer surgery was 21.36±2.39, and that of patients without recurrence and metastasis was 19.93±2.14, the former was significantly higher than the latter, and the difference was statistically significant ( t=4.37, P<0.001). Multivariate Logistic regression analysis showed that axillary lymph node metastasis, vascularization, thickening, burr on tumor edge, small ADC value and large peripheral blood RLR were independent risk factors for recurrence and metastasis after breast cancer surgery ( P<0.05). ROC curve results showed that the area under the curve of the logistic regression model based on the above factors was 0.860 (0.798-0.921), the sensitivity was 80.4%, and the specificity was 81.8%. Conclusion:Mammogram and MRI parameters combined with peripheral blood RLR level have certain value in predicting recurrence and metastasis after modified radical breast cancer surgery.

Traumatic supraorbital fissure syndrome (TSOFS) is a symptom complex caused by nerve entrapment in the supraorbital fissure after skull base trauma. If the compressed cranial nerve in the supraorbital fissure is not decompressed surgically, ptosis, diplopia and eye movement disorder may exist for a long time and seriously affect the patients′ quality of life. Since its overall incidence is not high, it is not familiarized with the majority of neurosurgeons and some TSOFS may be complicated with skull base vascular injury. If the supraorbital fissure surgery is performed without treatment of vascular injury, it may cause massive hemorrhage, and disability and even life-threatening in severe cases. At present, there is no consensus or guideline on the diagnosis and treatment of TSOFS that can be referred to both domestically and internationally. To improve the understanding of TSOFS among clinical physicians and establish standardized diagnosis and treatment plans, the Skull Base Trauma Group of the Neurorepair Professional Committee of the Chinese Medical Doctor Association, Neurotrauma Group of the Neurosurgery Branch of the Chinese Medical Association, Neurotrauma Group of the Traumatology Branch of the Chinese Medical Association, and Editorial Committee of Chinese Journal of Trauma organized relevant experts to formulate Chinese expert consensus on the diagnosis and treatment of traumatic supraorbital fissure syndrome ( version 2024) based on evidence of evidence-based medicine and clinical experience of diagnosis and treatment. This consensus puts forward 12 recommendations on the diagnosis, classification, treatment, efficacy evaluation and follow-up of TSOFS, aiming to provide references for neurosurgeons from hospitals of all levels to standardize the diagnosis and treatment of TSOFS.
6.Effect of electroacupuncture on expression of V2R,AQP2 and ENaC-α in cochlea of guinea pigs with endolymphatic hydrops

Objective To investigate the effect of electroacupuncture(EA)on the expression of vasopressin type 2 receptor(V2R),aquaporin-2(AQP2)and epithelial sodium channel-α(ENaC-α)in guinea pigs with arginine vasopressin(AVP)-induced endolymphatic hydrops(EH).Methods Male albino guinea pigs were randomly divided into normal control,EH model and EA groups(n=12 guinea pigs in each group).The EH model was established by continuous injection intraperitoneally of AVP,4μg/(kg·d)for 7 days,and then 6μg/(kg·d)for 3 days,totally 10 days.EA(100Hz,1mA)was applied to Baihui(GV 20)and unilateral Tinggong(SI 19)for 20 min,once a day for continuous 10 days.Cochlear hydrops severity was measured using hematoxylin-eosin(HE)staining,and then the ratio of scala media(SM)area to SM + scala vestibuli(SV)area(R value)was calculated.The expression level of V2R,AQP2 and ENaC-α in the cochlea was detected by western blot.Results Following modeling,R value,the V2R,AQP2 and ENaC-α protein expression were significantly increased in comparison with the normal group(all P<0.01).After EA intervention,R value,the V2R,AQP2 and ENaC-α protein expression were notably down-regulated in comparison with the model group(all P<0.01).Conclusion EA can relieve the cochlear hydrops of EH guinea pigs,which may be related to its effect in down-regulating the expression of V2R,AQP2 and ENaC-α protein in the cochlea.

Chronic refractory wound (CRW) is one of the most challengeable issues in clinic due to complex pathogenesis, long course of disease and poor prognosis. Experts need to conduct systematic summary for the diagnosis and treatment of CRW due to complex pathogenesis and poor prognosis, and standard guidelines for the diagnosis and treatment of CRW should be created. The Guideline forthe diagnosis and treatment of chronic refractory wounds in orthopedic trauma patients ( version 2023) was created by the expert group organized by the Chinese Association of Orthopedic Surgeons, Chinese Orthopedic Association, Chinese Society of Traumatology, and Trauma Orthopedics and Multiple Traumatology Group of Emergency Resuscitation Committee of Chinese Medical Doctor Association after the clinical problems were chosen based on demand-driven principles and principles of evidence-based medicine. The guideline systematically elaborated CRW from aspects of the epidemiology, diagnosis, treatment, postoperative management, complication prevention and comorbidity management, and rehabilitation and health education, and 9 recommendations were finally proposed to provide a reliable clinical reference for the diagnosis and treatment of CRW.
8.Prevention and treatment of complications after open wedge high tibial osteotomy for knee arthritis

Open wedge high tibial osteotomy (OWHTO) is one of the effective treatments of medial knee arthritis and an important step in the treatment of knee arthritis. However, with the popularization of OWHTO, more and more of its related complications have been reported, including under- and over-correction of the alignment, tilted joint line, fracture of the cortical hinge, and changes in the posterior tilt of the tibial plateau. This paper reviewed the domestic and foreign research literature on the OWHTO treatment of knee arthritis to analyze the incidence of intraoperative and postoperative complications, treatment strategies and prevention methods concerning the complications in the treatment of OWHTO. A comprehensive understanding of the prevention and treatment methods of the complications can help avoid and reduce some of the complications so that OWHTO can be popularized more widely.

To screen and evaluate the TCM in the treatment of bronchial asthma. Methods Retrieved the journals from China National Knowledge Infrastructure, the Vip Database, Wanfang Data and the Chinese Biomedical Literature Database (Sino Med) . The time limitation ran from January 1, 1987 to December 31, 2016. Extracted the indicators of the assessment based on the evaluating and screening system of country feasible TCM technology. Results a total of 150 treatment techniques were screened.They mostly were Chinese compound formula or point-application therapy. The score was between 24.857 and 34.304. Application of evaluating and screening system of country feasible TCM technology can screen and evaluate the feasible technologiesin the treatment of remission phase of bronchial asthma.
10.Study on the Medication Regularity of Traditional Chinese Medicine in the Treatment of Urinary Tract Infection Based on the Ancient and Modern Consilia Cloud Platform

Objective: To research the medication regularity of Chinese medicine on treating urinary tract infection based on the Ancient and Modern Consilia Cloud Platform to provide references for clinical treatment of urinary tract infection.Methods: The Ancient and Modern Consilia Cloud Platform was used to screen the consilia of Traditional Chinese Medicine for the treatment of urinary tract infection, the materials mainly come from books, journal articles, famous medical studios in the modern Chinese medicine case database. Screening, cleaning, standardizing consilia, and the standardized consilia database was set up. Statistical analysis, cluster analysis and complex networks were used to analysed the core prescription, the herb medicine frequency, the herb medicine four gas, the herb medicine five flavors, the herb medicine channel tropism and other regularity of Chinese medicine treatment of urinary tract infection, and displayed visually. Results: A total of 501 prescriptions were included, a total of 245 herb medicine were involved, and liquorice, poria, dianthus superbus and rehmannia were used more frequently. The tastes are cold, neutral, sweet taste, and bitter taste. Their channel tropisms mainly belong to lung and liver. The core prescriptions are based on Bazhengsan and Zhibaidihuangwan. Conclusion: Heat, dampness and diuretic herbs are used in the treatment of urinary tract infections in Chinese medicine, and nourishing yin and kidney, and relieving the stagnation of liver at the same time.Asthenic and sthenia syndrome, appearance and essence are also taken into account.
