Thymocyte selection-associated high mobility group box (TOX), a member of the high mobility group protein super-family, plays an important role in T cell development, functional maintenance, and exhaustion. It has been recently found that TOX exerts critical immunoregulatory functions during pathogen infections, and TOX expression is strongly associated with the intensity and tolerance of host immune responses. This review systematically summarizes the structural and functional features of TOX and focuses on its expression dynamics, mechanisms of action, and immunomodulatory effects during viral, bacterial, and parasitic infections, which provides a theoretical support to better understanding of the role of TOX in infectious diseases and provides new insights into development of potential immunotherapeutic strategies targeting TOX.
2.Effects of matrine on inflammation,oxidative stress and wound healing in atopic dermatitis

Objective To evaluate the effects of matrine on the inflammatory,oxidative and migratory responses of HaCaT keratinocytes in an in vitro atopic dermatitis(AD)model.Methods HaCaT cells were divided into the control group(no treatment)and the model group(0.05,0.1 and 0.2 mmol/L).HaCaT cells in the model group were stimulated with 10 μg/L tumor necrosis factor-α(TNF-α)/interferon-γ(IFN-γ)for 24 h.Cells in the model group were pretreated with 0.05,0.1 and 0.2 mmol/L matrine for 1 h and then stimulated with 10 μg/L TNF-α/IFN-γ for 24 h.MTT assay was used to assess the cytotoxicity of matrine.Enzyme-linked immunosorbent(ELISA)assay was performed to determine levels of cytokines(IL-6,IL-8 and IL-1β)and chemokines(TARC,MDC and RANTES).mRNA expression levels of antioxidant genes were detected by real-time quantitative PCR(RT-qPCR).Cell migration was analyzed by scratch closure assay.Immunohistochemistry was commended to analyze the nuclear translocation of NF-κB p65.Western blot assay was used to detect the phosphorylation levels of p38,ERK and p65 proteins.Results Compared with the model group,matrine 0.1 and 0.2 mmol/L significantly decreased the expression levels of IL-6,IL-1β,IL-8 and chemokines,and matrine significantly enhanced the expression levels of superoxide dismutase-1(SOD1),SOD2,catalase(CAT)and glutathione peroxidase(GPx),and promoted scratch wound healing(P<0.05).In addition,matrine 0.1 and 0.2 mmol/L inhibited the phosphorylation levels of p38,ERK and p65 proteins and the nuclear translocation of p65.Conclusion Matrine possesses anti-inflammatory and antioxidant properties,and stimulates wound closure in eratinocyte AD model,which may be related to the inhibition of the activation of MAPK and NF-κB pathways.
3.Visual analysis of application of blended teaching in the field of undergraduate nursing education

Objective:To explore the application status, research hotspots and frontiers of blended teaching mode in undergraduate nursing education at home and abroad, so as to provide reference for related research.Methods:In Chinese, CNKI, Wanfang Database and VIP, and in English, Web of Science database were used as search sources. CiteSpace was used to conduct a quantitative analysis of the retrieved literatures published from 2008 to 2022.Results:A total of 43 literatures in Chinese and 83 in English were retrieved, and the number of publications showed an overall increasing trend. The research hotspots in China focused on education reform, online learning and curriculum system evaluation, and the research frontiers were network teaching, moral education and humanistic care. Foreign research focused on the choice of education model and emphasizes interaction, and the research frontier were to carry out innovative blended teaching and pay attention to the learning experience and learning environment construction of nursing students.Conclusions:In the future, blended teaching should pay attention to carrying out innovative research and dynamic evaluation and feedback of students' satisfaction. In particular, it is necessary to strengthen the cultivation of blended learning competence among nursing teachers, aiming to promote the knowledge and skills of nursing students, and continuously optimize and innovate the application of blended learning in the field of undergraduate nursing education.
4.Effect of early glucose metabolism abnormality on skeletal muscle content of young men

Objective To investigate the effect of early glucose metabolism abnormality on skeletal muscle content of young men.Methods 88 males who underwent physical examinations in our hospital from September 2020 to September 2021 were divided into normal blood glucose group(NGT,n=47)and IGR group(n=41),according to their FPG and 2 hPG levels.FPG,FIns and visceral fat area(VFA)were measured.HOMA-IR,HOMA-β,skeletal muscle index(SMI)and body fat ratio(BFR)were calculated.Results Compared with NGT group,the males in IGR group showed elevated BMI,WC,FPG,2 hPG,HOMA-IR and VFA(P<0.05),but decreased HDL-C and SMI(P<0.05).Pearson correlation analysis showed that SMI was negatively correlated with BMI,WC,FPG,2 hPG,HOMA-IR,VFA and BFR in young men(P<0.05 or P<0.01).Logistic regression analysis showed that BMI was the influencing factor of FPG,while SMI was the influencing factor of 2 hPG.Conclusions In young men with abnormal glucose metabolism,the postprandial blood glucose significantly increase with the decrease of skeletal muscle content,and fasting blood glucose is mainly affected by BMI.
5.Network Meta-analysis of the effects of different types of exercises on children with attention deficit hyperactivity disorder

Objective:To evaluate the effects of different exercises on executive function, moter skills and core symptoms in children with attention deficit hyperactivity disorder (ADHD) using network Meta-analysis, and to provide evidence-based basis for the treatment of children with ADHD.Methods:The randomized controlled trails(RCTs)of exercises on children with ADHD were searched in Web of Science, PubMed, Cochrane Library, Embase, PsycINFO, CNKI, Wanfang Data, VIP and CBM databases.The retrieval time limit was from the establishment of each database to May 2023.The Cochrane quality review manual was used to evaluate the quality of the included literatures.Stata 17.0 software was used to perform network Meta-analysis.Results:Finally, twenty-four studies were enrolled including five types of exercise intervention, such as aerobic exercise, multicomponent exercise, exergaming, high intensity interval training (HIIT) and traditional low-to-moderate intensity exercise therapy (TRAD).The results of network Meta-analysis showed that: in terms of improving executive function(working memory, inhibitory control, and cognitive flexibility), motor skills and core symptoms of children with ADHD, the optimal exercise intervention was aerobic exercises (surface under the cumulative ranking were 78.9%, 73.0%, 88.7%, 77.9% and 69.9% respectively).Conclusion:Exercises can improve executive function, improve motor skills, and alleviate ADHD core symptoms.Aerobic exercise has a better effect on improving executive function, motor skills and core symptoms of ADHD children.
6.Study on optimization of the preparation for citrus oils β-cyclodextrin microspheres inclusion compound by using the response surface methodology

Objective:The response surface methodology (RSM) and Box-Behnken design were adopted to optimize the preparation for citrus oils β-cyclodextrin microspheres inclusion compound. And the physical characterization and heat stability were evaluated.Methods:The best preparation technology included inverse emulsion polymerization and saturated water solution method, with volatile oil weight ratio and microspheres, microspheres and water feeding ratio, inclusion temperature as impact factors, inclusion rate as the response value, establish regression model,. We explored the orange peel naphtha beta ring paste by microscopy, infrared spectroscopy (IR), differential scanning calorimetry (DSC) and heat stability test.Results:The best preparation technology included the essential oil with beta ring paste microspheres ( V: m), the ratio of 1:10, beta ring paste small ball and the ratio of water (m: V) for 1:15, and inclusion temperature for 41 ℃. The average encapsulation efficiency and the average rate of yield under optimized conditions were 62.21% and 85.24%, respectively. The physical characterization and thermal stability tests demonstrated that the β-cyclodextrin microsphere inclusion complex of volatile oil from Citrus was successfully prepared and the physical properties were stable. Conclusion:The preparation method of citrus oils β-cyclodextrin microspheres inclusion compound by using the response surface methodology.

Objective:Surgical site infection (SSI) is the most common infectious complication after emergency abdominal surgery (EAS). To a large extent, most SSI can be prevented, but there are few relevant studies in China. This study mainly investigated the current situation of SSI occurrence after EAS in China, and further explored risk factors for SSI occurrence.Methods:Multi-center cross-sectional study was conducted. Clinical data of patients undergoing EAS in 33 hospitals across China between May 1, 2019 and June 7, 2019 were prospectively collected, including perioperative data and microbial culture results from infected incisions. The primary outcome was the incidence of SSI after EAS, while the secondary outcomes were postoperative hospital stay, ICU occupancy rate, length of ICU stay, hospitalization cost, and mortality within postoperative 30 days. Univariate and multivariate logistic regression models were used to analyze the risk factors of SSI after EAS.Results:A total of 660 EAS patients aged (47.9±18.3) years were enrolled in this study, including 56.5% of males (373/660). Forty-nine (7.4%) patients developed postoperative SSI. The main pathogen of SSI was Escherichia coli [culture positive rate was 32.7% (16/49)]. As compared to patients without SSI, those with SSI were more likely to be older (median 56 years vs. 46 years, U=19 973.5, P<0.001), male [71.4% (35/49) vs. 56.1% (343/611), χ 2=4.334, P=0.037] and diabetes [14.3% (7/49) vs. 5.1% (31/611), χ 2=5.498, P=0.015]; with-lower preoperative hemoglobin (median: 122.0 g/L vs. 143.5 g/L, U=11 471.5, P=0.006) and albumin (median: 35.5 g/L vs. 40.8 g/L, U=9452.0, P<0.001), with higher blood glucose (median: 6.9 mmol/L vs. 6.0 mmol/L, U=17 754.5, P<0.001); with intestinal obstruction [32.7% (16/49) vs. 9.2% (56/611), χ 2=25.749, P<0.001], with ASA score 3-4 [42.9% (21/49) vs. 13.9% (85/611), χ 2=25.563, P<0.001] and with high surgical risk [49.0% (24/49) vs. 7.0% (43/611), χ 2=105.301, P<0.001]. The main operative procedure resulting in SSI was laparotomy [81.6%(40/49) vs. 35.7%(218/611), χ 2=40.232, P<0.001]. Patients with SSI experienced significantly longer operation time (median: 150 minutes vs. 75 minutes, U=25 183.5, P<0.001). In terms of clinical outcome, higher ICU occupancy rate [51.0% (25/49) vs. 19.5% (119/611), χ 2=26.461, P<0.001], more hospitalization costs (median: 44 000 yuan vs. 15 000 yuan, U=24 660.0, P<0.001), longer postoperative hospital stay (median: 10 days vs. 5 days, U=23 100.0, P<0.001) and longer ICU occupancy time (median: 0 days vs. 0 days, U=19 541.5, P<0.001) were found in the SSI group. Multivariate logistic regression analysis showed that the elderly (OR=3.253, 95% CI: 1.178-8.985, P=0.023), colorectal surgery (OR=9.156, 95% CI: 3.655-22.937, P<0.001) and longer operation time (OR=15.912, 95% CI:6.858-36.916, P<0.001) were independent risk factors of SSI, while the laparoscopic surgery (OR=0.288, 95% CI: 0.119-0.694, P=0.006) was an independent protective factor for SSI. Conclusions:For patients undergoing EAS, attention should be paid to middle-aged and elderly patients and those of colorectal surgery. Laparoscopic surgery should be adopted when feasible and the operation time should be minimized, so as to reduce the incidence of SSI and to reduce the burden on patients and medical institutions.

9. Bortezomib-based induction chemotherapy followed by autologous hematopoietic stem cell transplantation and maintenance in 200 patients with multiple myeloma: long-term follow-up results from single center

Objective:
To study the efficacy, safety and long-term outcomes of integrated strategy of bortezomib-based induction regimens followed by autologous hematopoietic stem cell (ASCT) and maintenance therapy in Chinese multiple myeloma (MM) patients.
Methods:
200 MM patients receiving integrated strategy of bortezomib--based induction regimens followed by ASCT and maintenance therapy were retrospectively and prospectively analyzed from December 1. 2006 to April 30. 2018.
Results:
The complete remission rates (CR) and better than very good partial remission rates (VGPR) after induction therapy, transplantation and maintenance therapy were respectively 31% and 75.5%, 51.8% and 87.7%,73.6% and 93.4%. There was no difference between 4 cycles and more than 5 cycles induction chemotherapy. The negative rate of MRD detection by flow cytometry was 17.6% and 38.2% respectively after induction and 3 months after transplantation. The negative rate of MRD gradually increased during the maintenance therapy. The success rate of high dose CTX combined with G-CSF mobilization was 95.5% and transplantation related mortality (TRM) was zero. The median time to progress (TTP) was 75.3 months and the median overall survival (OS) was 99.5 months. TTP of patients obtaining CR and negative MRD after induction were longer that those of no CR and positive MRD. TTP and OS of patients receiving triple-drug induction and ASCT in early stage were longer than those of double-drug induction and ASCT in late stage. LDH≥240 U/L, high risk cytogenetics, ISS II+III stage and HBsAg positive were prognostic factors at diagnosis. However, only MRD and high risk cytogenetics were independent prognostic factors after transplantation and maintenance therapy. The clinical characteristics of patients of TTP ≥6 years were listed below: light-chain type M protein, ISS I stage, normal level of hemoglobin and platelet, normal LDH, HBsAg negative, chromosome 17p-negative, good response and sustained good response.
Conclusions
Integrated strategy of bortezomib-based induction regimens followed by ASCT and maintenance therapy can significantly improve the short-term and long-term efficacy. The prognostic factors of TTP in different disease stages were different. Response to treatment, especially MRD, played a more important role in prognostic factors.
10.Application of Clinical Pathway teaching method on ESRD teaching practice

Objective To explore the clinical pathway teaching method application effection end-stage renal disease( ESRD) teaching practice. Methods From May 2013 to July 2014, a total of 80 nursing intern students in Nephrology Department, the Fifth Hospital of Wuhan, were divided into observation group (40 cases) and control group (40 cases) according to a random number table method. Conventional teaching method was applied in the control group whereas the observation group used clinical pathway teaching method. Four weeks after the internship,the quality of clinical teaching programs scores, nursing students theoretical knowledge, skills assessment excellent rates and patient satisfaction levels of both groups were compared. Results Statistical analysis of the quality of clinical teaching programs filled by nursing intern students showed that the observation group nursing students in the teachers′ teaching management, teaching attitude, knowledge level and teaching skill level four aspects were higher than those in the control group (P<0. 05). The average total score of the observation group was (91. 3 ± 4. 1) while the control group was (79. 6 ± 3. 2), and the difference was statistically significant (P <0. 01). Nursing students theoretical knowledge and skills excellent rates of the observation group were 55. 0% and 57. 5%, significantly higher than those ofthe control group ( 22. 5%, 17. 5%). Patients in the observation group of nursing students satisfaction rate was 76. 58%, significantly higher than these of the control group which was 51. 89% (P<0. 05). Patients′ nursing students satisfaction rate in the observation group was 76. 58%, higher than that of the control group (51. 90%), and the difference was statistically significant (P <0. 01). Conclusions Clinical pathway teaching method which can promote effective communication between teachers and students, strengthen teacher-student communication, and improve the learning efficiency of nursing intern students is also conducive to nursing intern students′ practice, improve their theoretical knowledge, operation skills, and comprehensive ability. Satisfaction of patients with end-stage renal disease can be improved as well.
