1.Research on the deep learning model based on the combination of intratumoral and peritumoral dynamic contrast-enhanced MRI for predicting axillary lymph node metastasis in breast cancer

Objective To explore the value of deep learning models in predicting axillary lymph node(ALN)metastasis of breast cancer based on intratumoral and peritumoral dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI).Methods A retrospective analysis was conducted on cases from Tianjin Medical University Cancer Hospital and Laoshan Branch of Affiliated Hospital of Qingdao University,involving a total of 850 lesions in 850 patients.The region of interest within the tumor was delineated at the largest area of the lesion on the first enhancement images and automatically expanded by 3 mm and 6 mm in a conformal fashion.Deep learning prediction models based on ResNet50 were developed via intratumoral,peritumoral,and intratumoral combined peritumoral models,respectively,and a comprehensive prediction model was developed by integrating semantic features of imaging reports.Cases from Tianjin Medical University Cancer Hospital were randomly divided into training and test cohorts in a 7∶3 ratio,while cases from Laoshan Branch of Affiliated Hospital of Qingdao University served as the external validation cohort.The area under the curve(AUC),accuracy,sensitivity,specificity,F1-score,and Brier-score were calculated,respectively.Results The model incorporating intratumoral,peritumoral(3 mm),and semantic features demonstrated the highest performance,with AUC of 0.801[95%confidence interval(CI)0.765-0.845],0.781(95%CI 0.745-0.817),and 0.752(95%CI 0.700-0.793)in the training cohort,test cohort,and external validation cohort,respectively,and there was no significant difference in AUC between combined model and intratumoral/peritumoral model,respectively,but it demonstrated the higher sensitivity and F1-score,and the lower Brier-score.Conclusion Incorporating peritumoral images into the conventional model based on intratumoral images enhanced the predictive ability of ALN metastasis in breast cancer.

Endodontic diseases are a kind of chronic infectious oral disease.Common endodontic treatment concepts are based on the removal of inflamed or necrotic pulp tissue and the replacement by gutta-percha.However,it is very essential for endodontic treatment to debride the root canal system and prevent the root canal system from bacterial reinfection after root canal therapy(RCT).Recent research,encompassing bacterial etiology and advanced imaging techniques,contributes to our understanding of the root canal system's anatomy intricacies and the technique sensitivity of RCT.Success in RCT hinges on factors like patients,infection severity,root canal anatomy,and treatment techniques.Therefore,improving disease management is a key issue to combat endodontic diseases and cure periapical lesions.The clinical difficulty assessment system of RCT is established based on patient conditions,tooth conditions,root canal configuration,and root canal needing retreatment,and emphasizes pre-treatment risk assessment for optimal outcomes.The findings suggest that the presence of risk factors may correlate with the challenge of achieving the high standard required for RCT.These insights contribute not only to improve education but also aid practitioners in treatment planning and referral decision-making within the field of endodontics.

Dental Caries is a kind of chronic oral disease that greatly threaten human being's health. Though dentists and researchers struggled for decades to combat this oral disease, the incidence and prevalence of dental caries remain quite high. Therefore, improving the disease management is a key issue for the whole population and life cycle management of dental caries. So clinical difficulty assessment system of caries prevention and management is established based on dental caries diagnosis and classification. Dentists should perform oral examination and establish dental records at each visit. When treatment plan is made on the base of caries risk assessment and carious lesion activity, we need to work out patient‑centered and personalized treatment planning to regain oral microecological balance, to control caries progression and to restore the structure and function of the carious teeth. And the follow-up visits are made based on personalized caries management. This expert consensus mainly discusses caries risk assessment, caries treatment difficulty assessment and dental caries treatment plan, which are the most important parts of caries management in the whole life cycle.

4.Establishment of HPLC Fingerprint and Content Determination of 5 Kinds of Isoflavone Components in Sojae Semen Nigrum

OBJECTIVE:To establ ish the fingerprint of Sojae Semen Nigrum and content determination method of 5 kinds of isoflavones,so as to provide reference for controlling its quality better. METHODS :HPLC method was adopted to establish the fingerprint and detect the contents of 5 kinds of isoflavones. The determination was performed on Phenomenex C 18 column with mobile phase consisted of acetonitrile- 0.12% formic acid solution (gradient elution )at the flow rate of 1 mL/min. The detection wavelength was set at 260 nm;the column temperature was 30 ℃ and sample size was 10 μL. Using daidzin as reference,HPLC fingerprints of 12 batches of samples were determined. The similarity of 12 batches of samples was evaluated by TCM Chromatographic Fingerprint Similarity Evaluation System (2012A) to confirm common peak. Cluster analysis and principal component analysis were performed by using SPSS 20.0 software and SIMCA 13.0 software. RESULTS :There were 19 common peaks in HPLC fingerprints of 12 batches of samples ,the similarity of which was higher than 0.94. Totally 5 components were identified,such as daidzin ,glycitin,genistin,daidzein,genistein. Cluster analysis showed that 12 batches of Sojae Semen Nigrum were clustered into 2 categories,i.e. S 1-S3 clustered into one category ,and S 4-S12 clustered into the other category. By principal component analysis ,the contribution rates of two principle components were 53.261% and 40.715%;accumulative contribution rate was 93.976%. The linear range of above 5 components were 5.97-191.00 µg/mL(r=0.999 9),1.05-33.46 µg/mL(r=0.999 9), 8.93-285.61 µg/mL(r=0.999 5),0.82-26.33 µg/mL(r=0.999 9),0.93-29.64 µg/mL(r=0.999 7),respectively. The limits of quantitation were 0.881 1,0.611 6,0.078 6,0.243 3,0.511 6 μg/mL,respectively. The limits of detection were 0.264 3,0.244 7, 0.021 4,0.124 8,0.106 7 μg/mL,respectively. RSDs of precision ,stability,reproducibility and durability tests were all lower than 5%. Recoveries were 95.15%-96.56%(RSD=0.51%,n=6),98.52%-103.45%(RSD=1.88%,n=6),95.37%-97.91% (RSD=0.95%,n=6),99.75%-102.00%(RSD=0.78%,n=6),100.26%-103.65%(RSD=1.21%,n=6). Among 12 batches of Sojae Semen Nigrum ,the contents of above 5 components were 0.178 3-0.265 9,0.021 7-0.096 2,0.288 5-0.597 2,0.014 1- 0.058 8,0.012 9-0.082 9 mg/g. CONCLUSIONS :Established HPLC fingerprint and content determination method of 5 kinds of isoflavones can be used for quality control of Sojea Semen Nigrum. The Isoflavone components are similar ,but the contents are different among Sojae Semen Nigrum from different producing areas.

Clinical characteristics, such as features of abdominal pain, etiologies, image data, levels of serum amylase and lipase, and physical development data were extracted from electronic medical records. The comparison between groups based on etiology or with normal control was performed with student′s t test. Results The age of first episode was (8.2±3.7) years. There were 61 (65%) children diagnosed with idiopathic CP, and 25 (27%) with anatomic abnormalities. The age of onset in the group with anatomic abnormalities was lower than that in the idiopathic CP group ((6.3 ± 3.5) vs. (8.9 ± 3.4) years, t=3.211, P=0.002). There were 51 (54%) patients with serum amylase elevation, 41(44%) patients with lipase elevation, and 35 (37%) with elevation in both. The questionnaire showed that 28 out of 30 children had moderate to severe abdominal pain. The patients′ weight standard score (SDS) was significantly lower than the overall average in normal control (-0.4±1.1 vs. 0, t=-3.308, P=0.001). Meanwhile, the mean level of insulin like growth factor‐1 (IGF‐1) SDS of 35 children was significantly decreased (-1.8 ± 1.8 vs. 0, t=-6.136, P<0.01). There were 69% (37/54) patients diagnosed by magnetic resonance cholangiopancreatography (MRCP) combined with magnetic resonance imaging (MRI), higher than that diagnosed by abdominal ultrasound (29%, 27/94). Conclusions Idiopathic CP and anatomic abnormalities were the two main etiologies. Normal level of serum amylase and lipase or negative finding of ultrasound cannot exclude CP, while MRCP and MRI should be considered to improve CP diagnostic rate. It is noteworthy that growth delay would happen in children with CP history.
7.Curative Effects of Laparoscopic Radical Prostatectomy for Prostate Cancer and Its Effect on Serum Hormones Levels

Objective:To analyze the curative effects of laparoscopic radical prostatectomy for the prostate cancer and the its effect on the level serum hormones levels.Methods:86 patients with prostate cancer were selected and divided into the control group and the observation group with 43 cases in each group according to the draw method.The control group was treated by open radical prostatectomy,while the observation group was treated by laparoscopic radical prostatectomy.The operation indicators,serum follicle stimulating hormone (FSH),luteinizing hormone (LH),dihydrotestosterone (DHT),free testosterone (FT),total testosterone (T),prostate specific antigen (T-PSA,F-PSA) levels,CD3+,CD4+,CD8+,CD4+/CD8+ before and after the surgery as well as the occurrence of postoperative complications were compared between two groups.Results:The operation time of observation group was significantly longer than that of the control group,and the blood loss,bowel function recovery time,hospital stay,pain scores of observation group was significantly lower or shorter than those of the control group (P<0.05).No significant difference was found in the serum FSH,LH,DHT,FT,T,T-PSA,F-PSA levels before and after the surgery between two group(P>0.05).The CD3+,CD4+,CD4+/CD8+ of observation group was significantly higher than those of the control group (P<0.05),the CD8+ of observation group was lower than that of the control group(P< 0.05).The incidence rate of postoperative complication in the observation group was significantly lower than that of the control group (P<0.05).Conclusion:Laparoscopic radical prostatectomy could play a similar effect with the open surgery on the tumor control,it could improve the level of serum testosterone,immune function with high safety.
8. Effect of Δ40p53 isoform on enhancing the pro-apoptotic function of p53 in tumor cells

Objective:
To investigate the effect of Δ40p53, an alternative spliced isoform of p53 lacking the N-ter minus, on the pro-apoptotic function of p53.
Methods:
The wild-type p53 was ectopically expressed in HCT116-p53-/- (endogenous Δ40p53 expression), HCT116-p53+ /+ (wild-type p53) and H1299 (p53-null) cells by adenoviral delivery, while Δ40p53 plasmid were transfected into these cells to overexpress Δ40p53. The levels of Δ40p53 and p53 mRNA were detected by reverse transcription-polymerase chain reaction (RT-PCR) and quantitative PCR. The expression of related proteins was deter mined by Western blotting. The interaction of p53 and Δ40p53 was observed by co-immunoprecipitation assay. Calcein-AM/propidium iodide (PI) staining and flow cytometry were used to detect the apoptotic rate of tested cells in each group.
Results:
HCT116-p53-/- cells expressed endogenous Δ40p53 isoform. Neither transcription nor protein expression of wild-type p53 was interfered by the increased expression of Δ40p53. Full length p53 and Δ40p53 could bind to each other. Calcein-AM/PI staining showed that the apoptotic rates of H1299-Control, HCT116-p53-/- -Control, H1299+ p53, HCT116-p53-/-+ p53, H1299+ oxaliplatin (Oxa), HCT116-p53-/-+ Oxa, H1299+ p53+ Oxa and HCT116-p53-/-+ p53+ Oxa groups were (2.50±0.47)%, (2.40±0.32)%, (5.20±0.58)%, (4.10±0.18)%, (22.40±1.73)%, (19.30±1.11)%, (29.90±1.15)% and (39.30±2.26)%, respectively.
