1.Pathological changes in the total knee joint during spontaneous knee osteoarthritis in guinea pigs at different months of age

BACKGROUND:The guinea pig is considered to be the most useful spontaneous model for evaluating primary osteoarthritis in humans because of its similar knee joint structure and close histopathologic features to those of humans. OBJECTIVE:To investigate the pathological process of spontaneous knee osteoarthritis in guinea pigs by analyzing the histopathology of the total knee joint of guinea pigs aged 1 to 18 months. METHODS:Eight healthy female Hartley guinea pigs in each age group of 1,6,10,14,16,and 18 months old were selected.The quadriceps femoris was taken for hematoxylin-eosin staining,and the total knee joint was stained with hematoxylin-eosin and toluidine blue.The histopathology of the cartilage,subchondral bone,synovium,meniscus,and muscles were observed under light microscope.Mankin's score and synovitis score were compared,and the correlation analysis was conducted. RESULTS AND CONCLUSION:As the guinea pig age increased,the Mankin's score increased(P<0.05),and the pathological score of synovitis also gradually increased(P<0.05),and there was a significant positive correlation between the two(r=0.641,P<0.001).The incidence rate of subchondral bone marrow lesion in 18-month-old guinea pigs was 50%,and the incidence of meniscus injury was 37.5%.In addition,osteophyte and narrowing of the joint space were observed,and only a few guinea pigs had inflammation in the quadriceps femoris.To conclude,guinea pigs develop significant cartilage defects,synovial inflammation,subchondral bone lesions,meniscus injury,osteophyte formation,and joint space narrowing as they age,all of which are similar to the pathological processes of primary knee osteoarthritis in humans,making it an ideal model of spontaneous knee osteoarthritis.
2.Prevalence and influencing factors of mild cognitive impairment in the elderly in Songjiang District, Shanghai

Background With the aging of China's population, cognitive impairment in the elderly is receiving increasing public attention. Screening and intervention of people with mild cognitive impairment (MCI) are of great significance to prevent and reduce the occurrence of cognitive impairment. Objective To understand the prevalence and explore potential influencing factors of MCI in the elderly in Songjiang District, Shanghai, and to provide scientific basis for promoting early screening of cognitive impairment and precise intervention of MCI in the elderly in this area. Methods A cross-sectional study design was adopted. From August to October 2022, using multi-stage random sampling, 1800 elderly residents aged 60 years and above were screened for cognitive impairment in 6 neighborhood/village committees in 6 towns in Songjiang District. The survey questionnaires included a sociodemographic questionnaire, a health status and lifestyle questionnaire, the Instrumental Activities of Daily Living (IADL), the Patient Health Questionnaire (PHQ-9), and the Mini-Mental State Examination (MMSE). Prevalence rates of MCI among the elderly by selected social demographic characteristics, health status, and lifestyle were estimated, and potential influencing factors of MCI were evaluated by binary logistic regression. Results A total of 209 elderly residents with MCI and 1591 healthy elderly residents were detected, and the prevalence of MCI in the elderly aged 60 and above was 11.6% in Songjiang District. Being physically active (OR=0.556, 95%CI: 0.399, 0.774) reduced the risk of MCI. Illiteracy (OR=1.810, 95%CI: 1.239, 2.644), primary school education level (OR=3.454, 95%CI: 2.342, 5.092), non-participation in social activities (OR=1.945, 95%CI: 1.360, 2.781), IADL damaged (OR=3.173, 95%CI: 2.137, 4.712), and depression (OR=1.957, 95%CI: 1.112, 3.443) increased the risk of MCI (P<0.05). Conclusion The prevalence of MCI among the elderly in Songjiang District is lower than the national average. Educational level, physical activity, participation in social activities, IADL, and depression may be the influencing factors of MCI in the elderly. It is recommended to carry out early screening, early detection, and early intervention for cognitive impairment in the elderly. Improving involvement in physical exercise and increasing participation in social activities are encouraged. Special attention should be paid to the needs of vulnerable groups such as low education level and disabled elderly during a community MCI intervention program.

4.Investigation and analysis of radon concentration during subway operation in Wuhan City, China

Objective To investigate the levels and variations of radon concentration in Wuhan subway and to provide data support for ensuring public radiation safety. Methods The typical sampling method was used to select 18 stations from Wuhan Metro Line 2, Line 3, and Line 7 for the measurement and evaluation of radon in the air. Using RAD7 electronic radon detector and continuous radon measurement method, radon concentrations in station platforms and station halls were investigated. Analysis of variance and t test were used for statistical analysis. Results The concentration of radon in Wuhan subway ranged from 8.8 to 16.2 Bq/m3, with an average of 11.5 Bq/m3, which is far lower than the limit set by Requirements for Radiological Protection for Radon in Public Underground Structure and Using Geothermal Water (WS/T 668-2019). There was no significant difference in radon concentration between station platform and station hall (t = −0.081, P > 0.05). There was no significant difference in radon concentration between different lines (F = 0.338, P > 0.05). On the whole, the radon concentration in summer was lower than that in winter, and there was no significant difference between months (F = 1.280, P > 0.05). Conclusion The radon concentrations in Wuhan subway result in low-dose radiation, and there is no significant health risk of radon exposure.
5.Determination of Eight Lignans in Schisandrae Sphenantherae Fructus on the Market from Different Habitats by UPLC-MS/MS

OBJECTIVE
To compare the contents of schizandrin A, schizandrin B, schizandrin C, schisandrol A, schisandrol B, schisantherin A, anwuligan, and schisanhenol in Schisandrae Sphenantherae Fructus on market from 12 habitats.
METHODS
The samples were pre-treated by 96-well fitration plates. The assay was performed on ACE EXCEL 1.7 C18-AMIDE(100 mm×2.1 mm, 1.7 μm) column with 0.1% formic acid-water(A) and methanol(B), gradient elution, flow speed was 0.4 mL·min–1. Ion source was electric spray ion source, positive ion monitoring mode, multireaction monitoring mode for quantitative analysis. Linear, instrument precision, stability, repeatability, average recovery were investigated.
RESULTS
The content of schisantherin A in 10 of 12 producing areas reached the standard of ≥0.2% of Schisandrae Sphenantherae Fructus in 2020 Edition of Chinese Pharmacopoeia.
CONCLUSION
UHPLC-MS/MS is suitable for simultaneous determination of multiple components in Schisandrae Sphenantherae Fructus. The Schisandrae Sphenantherae Fructus in the market basically meet the national legal standards.
6.Contamination of Legionella pneumophila in metro cooling tower water and its influencing factors in a city

Background The detection rates of Legionella pneumophila in metro cooling tower water in various cities are relatively high, while studies investigating the trends of Legionella pneumophila pollution and analyzing its influencing factors are still limited. Objective To investigate the status of Legionella pneumophila contamination in the water of metro cooling towers in a city, to analyze its influencing factors, and to provide a basis for preventing and controlling Legionella pneumophila contamination in metro cooling tower water. Methods After categorizing the metro lines into three groups (>10 years, >5-10 years, and ≤5 years) according to the operating years of each metro line in a city, we randomly selected one line from each group and the selection resulted a total of 58 cooling towers. We collected water samples from the cooling towers during the early, middle, and late stages of cooling tower operation to detect Legionella pneumophila and water quality related indicators. Information on the environment and hygiene management measures of the cooling towers was obtained through on-site investigation and questionnaire survey. Generalized mixed linear models were used to analyze the influencing factors of Legionella pneumophila contamination in cooling tower water. Results The detection rates of Legionella pneumophila in the early, middle, and late stages of cooling tower operation were 25.9%, 77.6%, and 81.0%, respectively. The detection rates in the middle and late stages of operation were significantly higher than that in the early stage (P<0.001). The results of generalized mixed linear regression showed that the total dissolved solids and conductivity were positively associated with positive Legionella pneumophila. For each unit increase in ln-transformed total dissolved solids and conductivity, their ORs (95%CIs) were 3.73 (1.19, 11.69) and 3.62 (1.07, 12.25), respectively. Nitrates, sulfates, and free residual chlorine were negatively associated with positive Legionella pneumophila. For each unit increase in ln-transformed nitrates and sulfates, their ORs (95%CIs) were 0.52 (0.34, 0.80) and 0.54 (0.36, 0.81), respectively. Compared with the group with 0-0.04 mg·L−1 of free residual chlorine, the ORs (95%CIs) for the groups with 0.05-0.30 mg·L−1 and ≥0.31 mg·L−1 of free residual chlorine were 0.22 (0.07, 0.74) and 0.01 (0.01, 0.19), respectively. Nitrates, sulfates, and free residual chlorine were negatively associated with the concentration of Legionella pneumophila. For each unit increase in ln-transformed nitrates and sulfates, the βs (95%CIs) of Legionella pneumophila concentration were −1.17 (−1.59, −0.74) and −1.11 (−1.63, −0.58), respectively. Compared with the group with 0-0.04 mg·L−1 of free residual chlorine, the β (95%CI) was −5.66 (−9.45, −1.87) for the group with ≥0.31 mg·L−1 of free residual chlorine. Conclusion The metro cooling tower water in the selected city is seriously contaminated by Legionella pneumophila, and getting further aggravated as the cooling tower annual operation time increases. Total dissolved solids, conductivity, nitrates, sulfates, and free residual chlorine are the major influencing factors for positive Legionella pneumophila, and nitrates, sulfates, and free residual chlorine are the major influencing factors for the concentration of Legionella pneumophila.
7.Investigation on polyurethane grouting waterproof material-induced occupational acute 1,2-dichloroethane poisoning in the construction industry

ObjectiveTo analyze the causes of occupational acute 1,2-dichloroethane (1,2-DCE) poisoning accident during the use of polyurethane grouting materials for waterproof plugging operation in the construction industry. Methods By combining the clinical symptoms of the patient, worksite survey of occupational health and workplace occupational hazards monitoring method, the cause of an occupational acute 1,2-DCE poisoning accident was investigated at a construction site during the use of polyurethane grouting material for waterproofing and plugging operations. Results The patient was engaged in waterproof grouting work using polyurethane grouting material. The main volatile organic components in the raw materials were 1,2-DCE, with traces of dichloromethane, methyl acetate and others. The result of post-incident on-site investigation showed that the short-term exposure concentration of 1,2-DCE in the workplace air was 578.70 mg/m3, which was more than 30 times higher than the national occupational health standard limit. The mass concentration of 1,2-DCE in the patient's blood was 230 μg/L. Combined with the patient's occupational hazard exposure history, clinical manifestations, worksite survey of occupational health, and laboratory test results, according to GBZ 39-2016 Diagnosis of Occupational Acute 1,2-Dichloroethane Poisoning, this incident was diagnosed as a severe occupational acute 1,2-DCE poisoning event caused by the use of inferior polyurethane grouting material. Conclusion The excessive concentration of 1,2-DCE in the workplace air is the main cause of this poisoning accident. Construction sites with confined space operations should improve various occupational health management systems, occupational health engineering protective facilities, and personal protective equipment must be provided for workers.
8.A double-blind randomised controlled clinical trial of Shenling Baizhu Granules in treating low anterior resection syndrome in rectal cancer

Objective This study aimed to assess the clinical efficacy of Shenling Baizhu Granules in treating low anterior resection syndrome(LARS)in rectal cancer.Methods The study employed a randomized,double-blind,placebo-parallel controlled,single-center,validity-tested clinical trial design.December 2019 to June 2022,the Department of Gastrointestinal Surgery and Integrated Traditional Chinese and Western Medicine of Peking University First Hospital recruited 110 patients who had undergone low anterior resection(LAR)for rectal cancer and subsequently developed LARS.These patients,meeting the enrollment criteria,were randomly assigned into the treatment group(55)and the control group(55)using the double-blind method principle.The randomization table was generated by SAS 9.2 software employing the double-blind method.The treatment group received oral Shenling Baizhu Granules,while the control group received oral placebo granules.Both groups commenced treatment on the 10th day after-surgery for 30 consecutive days.Patients were evaluated using LARS score,traditional Chinese medicine(TCM)symptom grading,and XU Zhongfa score before treatment,on the 15th day of treatment,and on the 1st day after treatment cessation.Results Out of 110 patients,107 were included in the full analysis set for efficacy analysis:55 patients in the treatment group and 55 patients in the control group.One case in the treatment group was excluded(against protocol),and two cases in the control group were excluded(one lost to follow-up,one against protocol).Baseline data between the two groups were consistent,with no statistically significant difference.Before treatment,LARS scores for the treatment and control groups were 33.0(31.0,36.0)and 34.0(32.0,37.0)respectively.Patients with TCM symptom scores of grades 2 to 3 accounted for 92.73%and 90.57%in the treatment and control groups,respectively,with no statistically significant difference.After 30 days of treatment,LARS scores for the treatment and control groups were 21.0(19.8,23.0)and 26.0(22.0,28.0)respectively.The percentage of patients with TCM symptom scores of grades 2 to 3 decreased to 33.33%in the treatment group and 66.04%in the control group,with a statistically significant difference.Shenling Baizhu Granules showed rapid improvement in watery or loose stools in post-operative rectal cancer patients.After 30 days of treatment,Shenling Baizhu Granules significantly improved appetite,stool consistency,abdominal distension,abdominal pain,and eructation symptoms in postoperative rectal cancer patients.Before treatment,the XU Zhongfa scores for the treatment and control groups were 3.0(2.0,4.3)and 4.0(2.0,4.0)respectively,with no statistically significant difference.After 30 days of treatment,the XU Zhongfa scores for the treatment and control groups were 7.0(6.0,8.0)and 6.0(5.0,7.0)respectively,with the treatment group significantly higher than the control group(P<0.01).Conclusion Shenling Baizhu Granules can effectively improve LARS symptoms in patients following LAR of rectal cancer within a short period of time.
9.Differences in Chemical Composition of Polygonatum cyrtonema Rhizome Before and After Processing Based on UPLC-Q-Exactive Orbitrap MS Analysis

ObjectiveUltra performance liquid chromatography-quadrupole-electrostatic field orbitrap high resolution mass spectrometry(UPLC-Q-Exactive Orbitrap MS) was used to analyze the differences in chemical components between raw products and wine-processed products of Polygonatum cyrtonema rhizome. MethodUPLC-Q-Exactive Orbitrap MS was used to analyze the chemical compositions of P. cyrtonema rhizome before and after processing, and the effective response ions were extracted after raw data processing, and the differential compounds before and after processing were screened combined with multivariate statistical analysis and according to the conditions of variable importance in the projection(VIP) value>1, P<0.05, fold change(FC)>2 or FC<0.5, based on the retention time, quasi-molecular ions, fragment ions and other information, the components were identified in combination with the control products and the literature, and the significant difference compounds were identified by clustering thermal analysis and relative quantitative analyzed, in order to clarify the change rule of the main components in P. cyrtonema rhizome before and after processing. ResultA total of 72 differential constituents between raw products and wine-processed products were identified, including 15 alkaloids, 12 organic acids, 12 amino acids, 6 flavonoids, 4 saccharides and 23 others. There were a total of 18 significantly different components, among which 13 compounds, including L-malic acid, lactic acid and 9,12,13-trihydroxy-10-octadecenoic acid, showed an increasing trend in content after wine processing, 5 compounds such as trans-3-indoleacrylic acid, L-arginine, D-tryptophan, showed a decreasing trend after processing. ConclusionThe chemical components of P. cyrtonema rhizome are significantly different before and after processing, mainly organic acids, saccharides, amino acids, flavonoids and alkaloids, which can lay the foundation for the in-depth study of the processing mechanism of Polygonati Rhizoma.
