Essential oils(EOs) from Chinese medicinals, which can be used as adjuvants and exert certain therapeutic effect, are directly used in Chinese medicine formulas. Conventional research strategy for EOs from Chinese medicinals is to compare the efficacy of the prescriptions before and after the addition of EOs, and the penetration-enhancing mechanisms of EOs remain unclear. In modern research on EOs from Chinese medicinals, the method for studying chemical penetration enhancers is often used, which fails to reflect the overall efficacy of EOs. This study clarified the property regularity of EOs from Chinese medicinals as transdermal penetration enhancers, and thereby proposed a research model which integrated the medicinal and adjuvant properties of EOs from Chinese medicinals via "component-delivery-effect" characterization route. The core concept is that constituents of EOs from Chinese medicinals and their delivery process play a key role in their external application. This research model is expected to serve as a reference for further research on EOs from Chinese medicinals for transdermal application.

OBJECTIVES:
To investigate the clinical treatment outcomes and the changes of the outcomes over time in extremely preterm twins in Guangdong Province, China.
METHODS:
A retrospective analysis was performed for 269 pairs of extremely preterm twins with a gestational age of <28 weeks who were admitted to the department of neonatology in 26 grade A tertiary hospitals in Guangdong Province from January 2008 to December 2017. According to the admission time, they were divided into two groups: 2008-2012 and 2013-2017. Besides, each pair of twins was divided into the heavier infant and the lighter infant subgroups according to birth weight. The perinatal data of mothers and hospitalization data of neonates were collected. The survival rate of twins and the incidence rate of complications were compared between the 2008-2012 and 2013-2017 groups.
RESULTS:
Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of severe asphyxia and smaller head circumference at birth (P<0.05). The mortality rates of both of the twins, the heavier infant of the twins, and the lighter infant of the twins were lower in the 2013-2017 group compared with the 2008-2012 group (P<0.05). Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of pulmonary hemorrhage, patent ductus arteriosus (PDA), periventricular-intraventricular hemorrhage (P-IVH), and neonatal respiratory distress syndrome (NRDS) and a higher incidence rate of bronchopulmonary dysplasia (P<0.05).
CONCLUSIONS
There is a significant increase in the survival rate over time in extremely preterm twins with a gestational age of <28 weeks in the 26 grade A tertiary hospitals in Guangdong Province. The incidences of severe asphyxia, pulmonary hemorrhage, PDA, P-IVH, and NRDS decrease in both the heavier and lighter infants of the twins, but the incidence of bronchopulmonary dysplasia increases. With the improvement of diagnosis and treatment, the multidisciplinary collaboration between different fields of fetal medicine including prenatal diagnosis, obstetrics, and neonatology is needed in the future to jointly develop management strategies for twin pregnancy.

Chinese Journal of Experimental Traditional Medical Formulae 2022;28(7):1-10
ObjectiveTo explore the optimal formula of Maxing Shigantang in regulating epidermal growth factor receptor(EGFR)expression and alleviating airway injury in asthmatic rats and to reveal the underlying mechanism. MethodSD male rats were randomly divided into normal group, model group, dexamethasone group (5×10-4 g·kg-1) and Maxing Shigantang 1∶0.5, 1∶1, 1∶2 groups (group A, B, C, 10 g·kg-1), with 8 rats in each group. The other groups except the normal group received nebulization of 2% acetylcholine chloride and 0.4% histamine phosphate for the modeling of asthma. One hour before modeling, the normal group and the model group were given the same amount of normal saline, and the other groups were given the same amount of corresponding drugs, once a day for 7 days. On the 7th day, the model was established and the incubation period of asthma was recorded. The rats were then immediately anesthetized, and arterial blood and tracheal tissue were collected. Enzyme-linked immunosorbent assay (ELISA) was employed to detect the levels of interleukin-2 (IL-2), interleukin-4 (IL-4), and tumor necrosis factor-α (TNF-α) in serum. Pathological sections were prepared for the observation of the pathological changes of tracheal tissues and the ultrastructure of epithelial cells in each group. Terminal-deoxynucleotidyl transferase-mediated nick-end labeling (TUNEL) was adopted to detect epithelial cell apoptosis, and in situ hybridization and Western blot were employed to determine the mRNA and protein levels of epidermal growth factor receptor (EGFR), respectively. ResultCompared with the model group, groups A, B and C prolonged the incubation period of asthma (P<0.05,P<0.01). Compared with the control group, the model group showed declined IL-2 level (P<0.01), risen IL-4 and TNF-α levels (P<0.05,P<0.01), increased airway pathology score, collagen volume fraction, and airway epithelial cell apoptosis index (P<0.01), and up-regulated mRNA and protein levels of EGFR in trachea tissue (P<0.01). Compared with the model group, group A showed increased IL-2 level (P<0.05) and declined IL-4 (P<0.05,P<0.01) level, and group B showed declined IL-4 level (P<0.05). The level of TNF-α in groups A, B, and C declined compared with that in the model group (P<0.01). Maxing Shigantang repaired the tracheal tissue to different degrees (P<0.05). Among the three groups, group A inhibited tracheal fibrosis (P<0.05) and had the most significant effect of repairing the ultrastructural changes of airway epithelial cells. Groups A, B and C all inhibited the apoptosis of airway epithelial cells (P<0.05). All the three groups inhibited the up-regulation of EGFR mRNA level (P<0.05,P<0.01), and groups B and C inhibited the up-regulation of EGFR protein level (P<0.05,P<0.01). ConclusionMaxing Shigantang can inhibit the abnormal changes of airway epithelial structure, alleviate airway injury, and can down-regulate the expression of EGFR in the tracheal tissue of asthma model rats. In this study, the optimal compatibility of Maxing Shigantang to repair airway epithelial injury in asthmatic rats was group A, with the Ephedrae Herba-Armeniacae Semen Amarum-Glycyrrhizae Radix et Rhizoma-Gypsum Fibrosum ratio of 1∶0.5∶4∶1.

Liver fibrosis is one of the main diseases with high morbidity and mortality worldwide. It is the common pathological results of several chronic irritant disease such as viral hepatitis ,alcohol abuse, autoimmune diseases, metabolic diseases and cholestatic liver diseases and will further develop into cirrhosis ,liver failure, portal hypertension and even death. The excessive accumulation of extracellular matrix (ECM) that leads to disorder of liver structure is the main factor in the development of liver fibrosis. MicroRNAs are a class of 2225 nt endogenous noncoding small RNAs. Sufficient studies have shown that the abnormal expression of microRNAs is closely related to the progression of liver fibrosis. In this review, we summarize the regulatory effects of microRNAs discovered in recent years on the activation ,proliferation apoptosis and senescence of HSCs in liver fibrosisand the underlying mechanisms, putting forward the prospect.

Journal of Southern Medical University 2018;38(2):168-173
OBJECTIVETo investigate the protective effects of paeoniflorin against PM2.5-induced damage in BEAS-2B cells and explore the possible mechanism.
METHODSWith a factorial design, this study was performed to observe the protective effects of different doses of paeoniflorin against PM2.5-induced BEAS-2B cell growth inhibition and the effects of paeoniflorin on the contents of malondialdehyde (MDA) and intracellular reactive oxygen species (ROS) in the cell cultures.
RESULTSExposure to increased PM2.5 concentrations caused significant decrease in the cell survival rate (P<0.05) with a clear dose-response relationship (r=-0.759, P<0.05). Treatment of the cells with paeoniflorin significantly attenuated PM2.5-induced inhibition of BEAS-2B cell survival (P<0.05), but the effect of paeoniflorin was not dose-dependent (P>0.05). PM2.5 exposure also significantly increased the contents of MDA and intracellular ROS (P<0.05), and paeoniflorin obviously antagonized these effects of PM2.5.
CONCLUSIONPaeoniflorin can protect BEAS-2B cells from PM2.5-induced growth inhibition, and the mechanism might be related to the anti-oxidant effects of paeoniflorin.

OBJECTIVE To analyze the preliminary regulation between the skin cell toxicity of EOs and drug property char-acteristics of Chinese materia medica(CMM)on the same platform.METHODS The physicochemical parameters of 33 kinds of EOs were measured.MTT assays were applied to evaluate the cell toxicity of EOs on human HaCaT keratinocytes. Finally, data mining technique was applied to analyse experimental data for the purpose to illustrate the relationship between skin cell toxicity of EOs and drug properties of CMMs.RESULTS By non-linear canonical correlation analysis, high correlation(P=0. 041 1<0.05)was found between skin cell toxicity of EOs and channel tropism property of CMMs.EOs with the properties of liver, lung, stomach, kidney and large intestine channel possess lower skin cell toxicity. However,EOs with the properties of heart,pericardium,spleen,bladder,san-jiao and gall bladder channel possess higher skin cell toxicity.A mathematical model between drug property including four mature, five flavor, channel tropism of CMMs and skin cell toxicity of EOs was estab-lished based on non-linear canonical correlation analysis. In the model, significant correlation was found(P=0.041 4<0.05). EOs from CMMs with the properties of cool nature, bitter flavor plus liver or lung or kidney or large intestine channel might be superior in safety towards the skin cells. On contrary,EOs from CMMs with the properties of hot nature, sweet flavor plus heart or pericardium or spleen or bladder or san-jiao or gall bladder channel might be superior in toxicity towards the skin cells. CONCLUSION The skin cell toxicity of EOs is affected by drug property characteristics of CMMs,especially channel tropism property.

Trichosporon species now ranks as the second most common cause of disseminated yeast infections with a high mortality rate. Breakthrough trichosporonosis in patients receiving echinocandins therapy is being recognized recently. We present a case of breakthrough trichosporonosis with acute viral myocarditis while receiving caspofungin therapy. Trichosporon infection should be considered in patients, who have risk factors for invasive fungal infection and develop unexplained clinical manifestations of infection despite treatment with echinocandins.
