A 56-year-old man presented to the emergency room with a sudden onset of drowsiness that began 2 hours before his arrival. Brain computed tomography revealed massive pneumocephalus. The patient was diagnosed with cerebrospinal fluid leakage accompanied by septal perforation. A planned skull base repair was performed using multilayer reconstruction via a trans-septal approach. Following surgery, the patient’s mental state recovered, and the skull base defect was successfully reconstructed. The patient disclosed that he had undergone faith healing sessions at a religious facility, where a metal rod-like instrument was repeatedly inserted into his nose.

Skull base defects often manifest as meningocele/meningoencephalocele or cerebrospinal fluid (CSF) leaks. Ventral and lateral skull base defects are effectively treated endoscopically and microscopically using various approaches. This case series study analyzes the utility and efficacy of post-aural soft tissue and temporalis fascia grafts in repairing small (<1 cm) skull base defects. Five out of six patients (83.33%) achieved successful defect closure. Patients were followed for donor site morbidity, postoperative CSF leak, and/or recurrent meningocele. One patient experienced reconstruction failure and developed a CSF leak eight months after surgery. The results suggest that TF grafts combined with post-aural soft tissue may be effectively used to repair small skull base defects without significant donor site morbidity.

Background and Objectives: Obstructive sleep apnea (OSA) has been associated with an increased risk of cancer in various organs. OSA is also linked to chronic inflammation in the biliary tract and pancreas, a well-established risk factor for carcinogenesis in these organs. However, its relationship with biliary tract and pancreatic cancers remains unclear and has been rarely investigated. Therefore, we aimed to evaluate whether OSA serves as an independent risk factor for these malignancies by analyzing a nationwide healthcare claims database in South Korea. Methods: A retrospective cohort study was conducted using the Korean National Health Insurance Service (KNHIS) database. Adults aged ≥20 years who were newly diagnosed with OSA (ICD-10: G47.30) between 2007 and 2014 were identified and propensity score-matched (1:5) with controls based on age, sex, and comorbidities. Individuals with pre-existing cancer diagnoses were excluded. The primary endpoints were the incidence of overall cancer, biliary tract cancer (C23–C24), and pancreatic cancer (C25). Cox proportional hazards regression models were used to calculate hazard ratios (HRs), adjusting for demographic and clinical factors. Results: A total of 1,191,444 individuals were included, comprising 198,574 patients diagnosed with OSA and 992,870 matched controls. OSA was associated with an increased overall cancer incidence (HR, 1.132; 95% confidence interval [CI], 1.097–1.169); however, it was not significantly associated with pancreatic cancer (HR, 0.941; 95% CI, 0.823–1.072) or biliary tract cancer (HR, 0.931; 95% CI, 0.751–1.142). Subgroup analyses stratified by sex and age revealed no statistically significant associations across these groups. Conclusion Our findings do not support OSA as an independent risk factor for biliary tract or pancreatic cancers.

Background and Objectives: Inconsistencies in nasal nitric oxide (nNO) values, due to anatomical variations and comorbidities, challenge the accurate assessment of upper airway inflammation severity. We hypothesized that changes in nNO levels following treatment for chronic rhinitis would be consistent and provide relative value. This study aimed to evaluate the correlation between changes in nNO levels and symptomatic improvements following treatment for chronic rhinitis. Methods: This prospective observational study included 46 participants diagnosed with chronic rhinitis between December 2021 and November 2023. nNO measurements, evaluations of four nasal and two ocular symptoms, and quality of life questionnaires were conducted at baseline and after one month of treatment. Baseline laboratory tests included serum total immunoglobulin E levels, blood eosinophil percentages, and skin prick tests. Results: The Total Nasal Symptom Score (TNSS), TNSS with ocular symptoms (TNSS eye), and Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) scores significantly decreased following treatment (all p<0.001). nNO levels also decreased significantly after treatment (p=0.036). Moreover, changes in nNO were significantly correlated with changes in TNSS, TNSS eye, and RQLQ scores (p=0.047, r=0.294; p=0.021, r=0.340; and p=0.004, r=0.419, respectively). Conclusion In patients with chronic rhinitis, changes in TNSS, TNSS eye, and RQLQ scores were correlated with changes in nNO levels after treatment. nNO may serve as a potential objective evaluation tool for chronic rhinitis, particularly in patients who have difficulty reporting symptoms.

Background and Objectives: YouTube has become a widely used educational platform for medical trainees in endoscopic surgery. However, the quality of surgical videos on this platform remains unregulated. This study evaluates the educational quality of YouTube videos on endoscopic choanal atresia repair using a validated assessment tool. Methods: In this descriptive cross-sectional study, 50 YouTube videos on endoscopic choanal atresia surgery were analyzed. Video quality was assessed using the LAParoscopic surgery Video Educational GuidelineS (LAP-VEGaS) checklist, which evaluates content structure, procedural clarity, and outcomes reporting. Results: Among 108 initially identified videos, 50 met the inclusion criteria. Video quality scores ranged from 1 to 16, with a median score of 7. The most frequently included elements were step-by-step approach (96%), patient anonymity (96%), and descriptive title (76%). Procedural clarity received moderate scores overall, with only the “step-by-step approach” achieving consistent quality. Outcomes reporting was notably deficient, with 90% of videos failing to address postoperative morbidity or complications. Conclusion Most YouTube videos on endoscopic choanal atresia surgery lack the quality required for effective surgical education. As digital platforms increasingly supplement traditional training, academic institutions and specialists should prioritize creating and sharing high-quality, standardized educational content on public platforms like YouTube.

Background and Objectives: Pediatric chronic rhinosinusitis (CRS) significantly affects children’s quality of life and learning abilities. This study aimed to evaluate the postoperative outcomes in pediatric patients who underwent functional endoscopic sinus surgery (FESS) for CRS. Methods: A retrospective review was conducted on pediatric patients who underwent FESS for CRS at 11 university hospitals. The inclusion criteria were patients under 20 years old with bilateral disease who were operated on between January 2005 and December 2021. The data collected included demographics, clinical history, blood tests, preoperative computed tomography, and preoperative and postoperative symptom control. The Kruskal-Wallis and Fisher exact tests were used to compare the quantitative and qualitative data, respectively. Results: In total, 213 patients were enrolled. The mean age was 13.4±3.0 years, and 145 (68.1%) were male. One hundred sixty-four patients (77.0%) had nasal polyps and 33 patients (15.5%) underwent revision FESS. The preoperative symptoms, in order of prevalence, included nasal obstruction (87.8%), rhinorrhea (71.8%), a sense of postnasal drip (58.2%), hyposmia (44.6%), cough (24.4%), and facial fullness (18.3%). These symptoms were significantly alleviated for up to 3 years after surgery (p<0.001). At the time of the last follow-up, 121 patients (56.8%) were controlled, 80 (37.6%) were partly controlled, and 12 (5.6%) were uncontrolled. Patients in the uncontrolled group had higher Lund-Mackay scores, longer follow-up durations, and more instances of revision surgery compared to those in the controlled and partly controlled groups. When age was categorized into three groups, those aged 16 years or older tended to have lower Lund-Mackay scores and better control. Conclusion FESS significantly improves both the postoperative symptoms and the long-term quality of life in pediatric CRS patients. Better symptom control is associated with older age and a lower disease burden.

The prelacrimal recess approach (PLRA) has advanced endoscopic sinonasal surgery by providing improved access to the maxillary sinus and adjacent anatomical regions while preserving critical structures such as the inferior turbinate (IT) and nasolacrimal duct (NLD). First introduced in 2013, the PLRA has become an important technique for addressing various sinonasal pathologies. This review comprehensively evaluates the advancements, applications, and outcomes associated with the PLRA. The PLRA enables superior visualization and access to regions that are traditionally difficult to reach with conventional techniques. Standardized surgical steps emphasize meticulous preservation of the NLD and IT, while technical modifications have broadened its feasibility in patients with narrow prelacrimal recesses. Applications of the PLRA span diverse pathologies, including sinonasal inverted papilloma, fungal infections, odontogenic cysts, and tumors of the lacrimal system, orbit, and skull base. Anatomical studies reveal significant variations in prelacrimal recess dimensions across populations, affecting surgical feasibility. Sex-specific differences, ethnic variations, and age-related factors are important in patient selection. Clinical outcomes from multiple investigations validate the PLRA’s efficacy in maintaining sinonasal function while achieving comprehensive lesion removal. Comparative analyses with traditional approaches underscore the PLRA’s advantages in reducing postoperative morbidity and recurrence rates. Integration of the PLRA with complementary surgical approaches further expands its therapeutic applications while maintaining favorable safety profiles. The PLRA is a safe and effective surgical method that offers favorable outcomes in disease management, symptom resolution, and anatomical preservation. With ongoing innovations and refinements, the PLRA is poised to remain a cornerstone of minimally invasive sinonasal surgery, enabling the precise and safe treatment of complex pathologies.

Background and Objectives: Malignant neoplasms of the nasal cavity and paranasal sinuses are an aggressive form of tumour that tends to be diagnosed at a locoregionally advanced stage. Among its various histological subtypes, squamous cell carcinomas (SCC) are the most common form of sinonasal cancer, with approximately 50%–60% of sinonasal malignancies being SCC. This review aims to investigate the impact of induction chemotherapy on survival outcomes for patients undergoing cancer care for sinonasal squamous cell carcinomas (SNSCC). Methods: Two reviewers independently assessed 2,118 studies pooled from four bibliographic databases, namely MEDLINE, Embase, Cochrane Central Register of Controlled Trials and Web of Science and hand-searched grey literature. This systematic review and meta-analysis was performed in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. As this was a time-to-event analysis, hazard ratios (HRs) and standard errors were derived from individual studies and pooled together for analysis. If these values were not available directly, statistical methods were used to indirectly extract the required data via Kaplan-Meier curves or via the p-value and the numbers of events. Results: Results suggest a notable improvement in overall survival (HR=0.56, 95% confidence interval [CI]=[0.36, 0.86], p<0.009) and a promising improvement in disease-free survival (HR=0.82, 95% CI=[0.62, 1.08], p=0.16) for patients who underwent induction chemotherapy in addition to definitive treatment for SNSCC. Conclusion Our findings add to existing literature by providing a precise pooled estimate of the beneficial effects, revealing that induction chemotherapy is not just a viable add-on to traditional treatment regimens but also improves survival outcomes when compared to definitive local therapy as a standalone treatment. Clinicians should consider incorporating induction chemotherapy as a treatment option for locoregionally advanced SNSCC.

Chronic rhinosinusitis with nasal polyps (CRSwNP) is a prevalent condition that significantly impacts quality of life and places a burden on healthcare systems. The advent of biologics targeting type 2 immune pathways offers new therapeutic options for severe and/or uncontrolled CRSwNP. Initially, biologic use was guided by the European Position Paper on Rhinosinusitis and Nasal Polyps (EPOS) 2020 and the European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA) guidelines, despite limited data on clinical indications, response measures, and treatment duration. Since then, numerous studies and the EPOS/EUFOREA 2023 update have refined these guidelines. The update defines clinical indications for biologics based on type 2 inflammation markers by lowering the blood eosinophil threshold from 250 to 150 cells/μL. The response to biologics is now more simply categorized into three levels based on reductions in nasal polyp size, improvements in quality of life, and enhancement of smell. Treatment evaluation is recommended at 6 months with annual follow-up. Longer administration intervals, such as every four weeks, have also proven effective in well-controlled patients. Although specific guidelines for discontinuation or switching biologics remain lacking, clinical judgment is essential in determining when treatment should be stopped or adjusted. Additionally, regulatory updates support the use of biologics for CRSwNP, and novel agents such as tezepelumab (an anti-thymic stromal lymphopoietin monoclonal antibody) continue to show promise. Finally, in Korea, biologics for CRSwNP are not covered by national health insurance, leading to extended dosing intervals due to high costs. Despite this limitation, studies have shown that adjusted dosing can maintain subjective quality of life. Recent studies by Korean authors have also explored practical considerations such as dosing intervals and comparisons to surgery. Further research is needed to optimize treatment strategies, particularly regarding cost-effectiveness and prospective studies tailored to the Korean healthcare system.
