1.Study on the assessment of real-time SWE and UFPWV techniques in carotid artery stiffness of patients with gouty arthritis

Objective:To explore the application value of real-time shear wave elastography(SWE)and ultrafast pulse wave velocity(UFPWV)techniques in assessing carotid artery stiffness of patients with gouty arthritis(GA).Methods:A total of 80 GA patients admitted to the First People's Hospital of Yibin from August 2022 to July 2023 were selected as GA group.Among of them,24 cases combined with bone erosion who were confirmed by imaging examination,and 56 cases did not combine with bone erosion.In addition,a total of 40 healthy individuals,who underwent physical examinations in our hospital during the same period,were selected as the healthy control group.SWE and UFPWV were used to detect the maximum,minimum and mean values of Young's modulus values of the vessel wall of common carotid artery,and the pulse wave velocity at the beginning of systole(PWVBS)and pulse wave velocity at the end of systole(PWVES)of the vessel wall of common carotid artery of all subjects,and to assess the hardness of circumferential and longitudinal vessel wall of common carotid artery of them.The differences in Young's modulus values,PWVBS and PWVES between two groups were compared.And then,the correlations between bone erosion and Young's modulus values,PWVBS,PWVES of GA patients were further analyzed.Results:The maximum,minimum,mean values,PWVBS and PWVES in the GA group were respectively(78.80±12.38)kPa,(57.83±7.42)kPa,(67.67±8.38)kPa,(7.06±0.59)m/s and(9.50±0.63)m/s,which were significantly higher than those in the healthy control group(t=11.066,8.216,11.751,14.159,19.118,P<0.05).The maximum,minimum,mean values,PWVBS and PWVES of GA patients with bone erosion were respectively(82.95±6.37)kPa,(59.59±5.29)kPa,(72.18±7.62)kPa,(7.37±0.43)m/s,(10.43±0.47)m/s,which were significantly higher than those[(77.01±5.32)kPa,(57.08±4.73)kPa,(65.73±4.54)kPa,(6.92±0.39)m/s and(9.10±0.41)m/s]of GA patients without bone erosion,the differences were significant(t=4.309,2.099,4.699,4.586,12.720,P<0.05).Spearman correlation analysis showed that there was significant correlations between maximum,minimum,mean values,PWVBS,PWVES and the combined bone erosion in GA patients(r=0.602,0.421,0.682,0.647,0.732,P<0.05).Conclusion:SWE and UFPEV techniques can evaluate the hardness of vascular wall of GA patients,and the hardness of circumferential and longitudinal vessel wall of common carotid artery of GA patient is higher than that of healthy person,and the hardness of circumferential and longitudinal vessel wall of common carotid artery of GA patient with bone erosion is higher.

Nanomaterials, with the advantages of unique microstructure, have been widely used in the fields of material manufacturing, microelectronics and computer technology, medicine and health, environment and energy. Compared with traditional hemostatic materials, nanomaterials can improve the bioavailability and stability of traditional hemostatic drugs to a certain extent, enhance the controlled and targeted release of drugs, which lay a good foundation for the development of new-style modern hemostatic nanomaterials. This paper reviews the advanced design and application progress of various nanomaterials in hemostasis, such as liposomes, nanoparticles, self-assembled nano peptides, nanofibers, etc. Finally, the challenges and prospects of hemostatic nanomaterials are briefly described.

Novel Coronavirus Pneumonia (NCP) is a class B infectious disease, which is prevented and controlled according to class A infectious diseases. Recently, children′s NCP cases have gradually increased, and children′s fever outpatient department has become the first strategic pass to stop the epidemic. Strengthening the management of the fever diagnosis process is very important for early detection of suspected children, early isolation, early treatment and prevention of cross-infection. This article proposes prevention and control strategies for fever diagnosis, optimizes processes, prevents cross-infection, health protection and disinfection of medical staff, based on the relevant diagnosis, treatment, prevention and control programs of the National Health and Health Commission and on the diagnosis and treatment experience of experts in various provinces and cities. The present guidance summarizes current strategies on pre-diagnosis; triage, diagnosis, treatment, and prevention of 2019-nCoV infection in common fever, suspected and confirmed children, which provide practical suggestions on strengthening the management processes of children′s fever in outpatient department during the novel coronavirus pneumonia epidemic period.

5.Risk factors for delirium among older hospital patients: a systematic review and meta-analysia

Objective To confirm the correlation between general information、physical functioning and delirium among elderly patients in medical units by systematic review and meta-analysis. Methods With strictly designed search strategies, electronic databases as follows: CBM, CNKI, VIP, WanFang, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, Embase, MEDLINE, MEDLINEIn-Process & Other Non-Indexed Citations(OVID), PsycInFo, CINAHL (EBSCO), SCI(Web of Science)were searched dated from the database building to October 2015.Two researchers confirmed the included studies according to the inclusion and exclusion criteria, evaluated the methodological quality of studies and extracted data independently. The Revman 5.3 was used for data analysis. Results (1) Age:results reported indichotomous,combined OR=2.57 (95%CI:1.69-3.89), P<0.00001;results reported in continuous,combined MD=2.67 (95%CI:1.57-3.77), P< 0.01.(2) Male:OR=1.34 (95%CI:0.91-1.97),P=0.14;inverse variance, OR=1.11(95%CI:0.95-1.30), P=0.20. (3)Activities of daily living: OR=2.66 (95%CI:1.90-3.70),P< 0.01. (4)Vision impairment: combined OR=2.31 (95%CI:1.24-4.32), P = 0.009.(5) Physical restraints: OR=6.60(95%CI:4.22-10.31), P < 0.01. Conclusions Physical restraintsis correlative with delirium among elderly patients in medical units. Age, vision impairment, activity of daily living may be correlative with delirium among elderly patients. We suggest to pay attention to those factors to implement delirium prevention, treatment and nursing to reduce the incidence and improve clinical outcomes for elderly patients.
6.Huaiqihuang granule in adjuvant treatment of childhood asthma:a randomized controlled,multicenter study

Objective To observe the clinical efficacy and safety of GINA regimen and GINA regimen combined with oral Huaiqihuang granule in the treatment of children with bronchial asthma.Methods A ran-domized,single blind,multicenter,parallel controlled clinical trial wascarried out.A total of 1 128 patients with bronchial asthma in children were randomized into two groups.The observation group were treated with GINA regimen combined with oral Huaiqihuang granule.The GINA regimen treatment group was treated by GINA reg-imen.Clinical assessment and C-ACT scores was observed in first month,third month,sixth month after treat-ment.Clinical assessment included the times of upper respiratory tract infection occurrence,bronchitis and pneu-monia,asthmatic attacks,application of emergency medicine,hospitalizations due to asthmatic.Drug adverse effect in the two groups was compared.Results The times of upper respiratory tract infection,bronchitis and pneumonia,asthmatic was significantly decreased(P <0.05 ),and C-ACTscores were significantly higher(P <0.05)in the first month,the third month,and the sixth month after treatment.There were 16 cases of drug relat-ed adverse reactions.Seven cases were in observation group(n ﹦7)(1.15%)and 9 cases in the GINA regimen treatment group(n ﹦9)(1.73%).There was no significant difference of adverse effect between the two groups (P >0.05).Conclusion The treatment of bronchial asthma in children with GINA regimen combined with oral Huaiqihuang granule can significantly reduce the incidence of respiratory infections and the number of asthmatic attacks dramatically and safely improve clinical curative effect,asthma control.
7.Assessment of the quality of randomized controlled trials of delirium prevention of elderly in China

Objective To assess the quality of randomized controlled trials (RCTs) of delirium prevention of elderly published in China.Methods The literatures from China National Knowledge Infrastructure(CNKI),WANFANG Data and VIP Database for Chinese Technical Periodicals were evaluated according to Cochrane collaboration's tool for assessing risk of bias and Jadad scale.Results A total of 53 RCTs were included,14 (26.4%,14/53) described radom number table used to generate the random allocation sequence,4 (7.5%,4/53) conducted experiments in a blinded manner,7 (13.2%,7/53) did not use intentionto-treat to analyse those who did not complete the study,9 (17.0%,9/53) had high risk of other bias,none described allocation concealment mechanism and blinding to participants and intercention implementers.Based on Jadad scales,the score was 1-4,average score was (2.3±0.8),19 (35.8%,19/53) were high-quality literatures.Conclusions The quality of present published literatures is not high,the further domestic studies should be designed high-quality to better improve clinical practice.

9.Silencing IL-23 expression by a small hairpin RNA protects against asthma in mice.

To determine the impact of IL-23 knockdown by RNA interference on the development and severity of ovalbumin (OVA)-induced asthmatic inflammation, and the potential mechanisms in mice, the IL-23-specific RNAi-expressing pSRZsi-IL-23p19 plasmid was constructed and inhaled into OVA-sensitized mice before each challenge, as compared with that of control mice treated with alum or budesonide. Inhalation of the pSRZsi-IL-23p19, significantly reduced the levels of OVA-challenge induced IL-23 in the lung tissues by nearly 75%, determined by RT-PCR. In addition, knockdown of IL-23 expression dramatically reduced the numbers of eosinophils and neutrophils in BALF and mitigated inflammation in the lungs of asthmatic mice. Furthermore, knockdown of IL-23 expression significantly decreased the levels of serum IgE, IL-23, IL-17, and IL-4, but not IFNgamma, and its anti-inflammatory effects were similar to or better than that of treatment with budesonide in asthmatic mice. Our data support the notion that IL-23 and associated Th17 responses contribute to the pathogenic process of bronchial asthma. Knockdown of IL-23 by RNAi effectively inhibits asthmatic inflammation, which is associated with mitigating the production of IL-17 and IL-4 in asthmatic mice.

10.Biological function of IL-23

Interleukin(IL),generated by immunocytes or non-immune cells,is a group of cell factors that holds many important biological effects. Up to now,with the rapid progress of molecular biology and cell biology technique,29 types(IL-1 ~ IL-29)of IL have been reported since its denomination in 1979. IL-23,one kind of interleukin found in 2000,is composed of p19 and p40 sub group. Recent studies showed that it has various biological functions,such as acting on T cell to make it produce various cell factors,acting on DC and MP to affect the secretion of the relevance cell factors. The biological functions of IL-23 have played an important role in various diseases. This review describes the molecular structure,biological function,and its effects on related diseases.
