1.Efficacy of capsaicin combined with repetitive transcranial magnetic stimulation in patients with dysphagia after stroke

Objective To investigate the efficacy of capsaicin combined with repetitive transcranial magnetic stimulation(rTMS)in the treatment of dysphagia after stroke.Methods A total of 85 stroke patients with dysphagia and no true bulbar palsy were included and divided into 3 groups by random number table method:the capsaicin group(28 cases),the rTMS group(29 cases)and the capsaicin combined with rTMS group(28 cases).On the basis of conventional rehabilitation treatment,patients of the capsaicin group were given capsaicin to stimulate oropharynx 3 times a day(30 min before breakfast,lunch and dinner)for 6 days/week.Patients of the rTMS group were given S1 hot spot magnetic stimulation of the primary somatosensory cortex on the healthy side,once a day,6 times a week,and patients of the capsaicin combined with rTMS group were treated with capsaicin combined with S1 hot spot magnetic stimulation.The treatment course of the 3 groups was 3 weeks.Before and after treatment,patients were evaluated by WST,SSA,EAT-10 and VFSS,respectively,to determine the degree of improvement in their swallowing function.Results There were no significant differences in WST,SSA,EAT-10 and VFSS scores before treatment between the 3 groups,and the above indexes were improved after treatment in the 3 groups.The improvement in the capsaicin combined with rTMS group was more obvious than that of the other 2 groups(P<0.05).Conclusion Capsaicin combined with rTMS can significantly improve swallowing function of patients with swallowing disorder after stroke compared with capsaicin or rTMS alone.

Self-organized blastoids from extended pluripotent stem (EPS) cells possess enormous potential for investigating postimplantation embryo development and related diseases. However, the limited ability of postimplantation development of EPS-blastoids hinders its further application. In this study, single-cell transcriptomic analysis indicated that the "trophectoderm (TE)-like structure" of EPS-blastoids was primarily composed of primitive endoderm (PrE)-related cells instead of TE-related cells. We further identified PrE-like cells in EPS cell culture that contribute to the blastoid formation with TE-like structure. Inhibition of PrE cell differentiation by inhibiting MEK signaling or knockout of Gata6 in EPS cells markedly suppressed EPS-blastoid formation. Furthermore, we demonstrated that blastocyst-like structures reconstituted by combining the EPS-derived bilineage embryo-like structure (BLES) with either tetraploid embryos or tetraploid TE cells could implant normally and develop into live fetuses. In summary, our study reveals that TE improvement is critical for constructing a functional embryo using stem cells in vitro.

3.Normal ranges and Z-scores analysis of the normal fetal umbilical vein diameter, blood volume flow and normalized volume blood flow in the second and third trimesters of pregnancy

Objective:To explore the method of ultrasonography for detecting the fetal umbilical vein diameter, blood flow volume and normalized volume blood flow and establish normal reference ranges with umbilical vein diameter, blood flow volume and normalized blood flow and Z-scores for umbilical vein diameter and blood volume flow.Methods:This was a prospective study on 907 normal fetuses in the Second Xiangya Hospital, Central South University and Women and Children Healthcare Hospital of Zhuzhou from March 2019 to December 2020. The umbilical vein diameter (Duv), umbilical vein blood flow volume (Quv) and normalized volume blood flow (nQ = Quv/estimated fetal weight) of the free loop of umbilical vein (FUV) and fetal intra-abdominal umbilical vein (IUV) were collected. And the mean values and 90% confidence intervals of Duv, Quv and nQ in two segments of umbilical veins at different gestational ages were calculated. Regression analysis of Duv, Quv and nQ were performed with gestational age (GA), and the parameters of umbilical vein in different segments were compared. Finally, with gestational age (GA) as the independent variable, Z-scores of the Duv and Quv were built.Results:The mean values and 90% confidence intervals of Duv, Quv, and nQ in 858 (94.6%) normal fetal umbilical veins were successfully obtained. The Duv, Quv of the FUV and IUV increased as pregnancy progressed. The Quadratic curve of Duv and Linear curve of Quv were of the highest fitnesses, respectively( r=0.951, 0.941, 0.986, 0.982; all P<0.001). While nQ increased with GA followed by a decreased trend, and the Quadratic curve was the highest fitting curve of nQ( r=0.610, 0.611; all P<0.001). Duv-FUV was greater than Duv-IUV( P<0.001), nQ-FUV was bigger than Quv-IUV( P=0.001), and he difference was not statistically significant between Quv-FUV and Quv-IUV( P=0.133). Z-scores models of Duv and Quv were successfully established, and all Z-scores were Gaussian distribution. Conclusions:The normal ranges and Z-scores of umbilical vein parameters are useful to improve the evaluation of placental circulation and provide a strong basis for the monitoring of fetus-related diseases and the evaluation of pregnancy prognosis. The choice of FUV or IUV umbilical vein to evaluate placental circulation may depend on the actual situation in clinical application.

5.Measures of Developing Pharmaceutical Care in PIVAS of Our Hospital

OBJECTIVE:To evaluate the effects of pharmaceutical care in PIVAS of our hospital,and to improve the quality and level of pharmaceutical care. METHODS:Combined with the actual situation of our hospital,various measures which had been carried out in PIVAS of our hospital were analyzed retrospectively,and the effects of pharmaceutical care were also analyzed. RESULTS & CONCLUSIONS:Through strengthening safety awareness,formulating effective management system,continuous op-timization process to guarantee the quality of drug dispensing,and focusing on the ability to improve the quality of the team,an ef-fective improvement has gained in the quality of pharmaceutical care. The proportion of irrational medical order of anti-tumor intra-venous drug use has decreased from 0.62% in 2008 to 0.09% in 2012. 1 363 irrational medical orders have been checked out dur-ing Jul.-Dec. of 2014,accounting for 0.39% in total. The satisfactory degree of clinical staff has increased from 80.0% to 98.5%. PIVAS has a positive role in promoting pharmaceutical care.
6.Small interfering RNA suppresses the increase of myocyte enhancer factor 2A expression in PC12 cells exposed to hypoxia

Purpose To investigate the effect of small interfering RNA on the increase of myocyte enhancer factor 2A(MEF2A) expression in PC12 cells exposed to hypoxia.Methods PC12 cells were cultured under normal conditions or were exposed to hypoxic conditions.Small interfering RNA targeted MEF2A gene (MEF2A-siRNA) was chemically synthesized. Eukaryocytic expression vector was built and transfected into PC12 cells with liposome. The expression of MEF2A mRNA was detected by real-time PCR. Western blot was used to detect the MEF2A protein.Results Compared with normal control(2~(-△△CT)=1.01±0.02), the mRNA level of MEF2A gene in PC12 cells with the treatment of MEF2A-siRNA was down-regulated significantly by 88%(2~(-△△CT)=0.12±0.03, P<0.01).The expression of MEF2A protein in hypoxia-treated PC12 cells was markedly higher than that of normal control(98.4±11.7 and 47.5±7.6,P<0.01).However, MEF2A-siRNA could significantly suppress the increase of MEF2A protein (P<0.01).Conclusion MEF2A gene silence induced by siRNA might inhibit the increase of MEF2A protein by hypoxia in PC12 cells.
7.Study of Screening Differential Genes of Malignant Melanoma Using Gene Chip Expression Profiles

ObjectiveTo identify differential genes of malignant melanoma using gene chip expression profiles.MethodsAgilent Human 1A OligoDNA was employed to find out difference in gene expression between malignant melanoma and nevus. The total RNA was isolated from two type tissues, labled the fluorescent to the probe, hybridized, washed and analyzed.ResultsAmong the 21073 target genes, 1596 genes were differentially expressed in malignant melanoma, including 733 genes up-regulated, and 863 down-regulated.ConclusionThe gene chip technique can screen genes that may be specifically expressed in malignant melanoma.
8.Analysis of the EEG information of rats epileptic model using unstable periodic orbits.

In order to further research into the EEG information of rats epileptic model, we applied different nonlinear dynamic methods. After having analyzed the EEG signal of rat falling sickness by means of approximate entropy and correlation dimension, we adopted a the new method, unstable periodic orbits, which was used to analyze complex activity of neurons system to look for the change regularity of change in the EEG signal in the whole course of rat's falling sickness. We found period 1 orbits and period 2 orbits to be statistically significant in the data of ictal time of epilepsy. At the same time, we found period 1 orbits to be statistically significant in the data of preictal time of epilepsy.

9.Changes in electroencephalogram in rat epilepsy model via nonlinear dynamical approach

BACKGROUND: The dynamic characteristics of electroencephalogram (EEG) include a decrease in the chaotic dimension, the correlation dimen sion, the Lyapunov exponent, the chaotic complexity, the freedom of EEG and an enhanced synchronization and periodicity of the EEG from several minutes to tens of minutes before epileptic seizures. All these characteristics prefigure the forthcoming seizures. Some studies have proven that the non linear dynamical system can be used as a feasible approach to explore the potential variables for describing the chaos portrait of EEG. OBJECTIVE: To analyze the electric characteristics of EEG signal in the epileptic seizures in rat model by investigating the nonlinear dynamical variables, such as the approximate entropy (ApEn) and correlation dimen sion. DESIGN: Observational and experimental study based on animals. SETTING: Department of Medical Engineering, Department of Gastroen terology, Second Artilleryman General Hospital of Chinese PLA; Department of Physics, Faulty of Biomedical Engineering, Fourth Military Medical Uni versity of Chinese PLA. MATERIALS: From September 2001 to January 2002, this study was conducted at the Complexity Laboratory of the Biomedical Department of the Fourth Military Medical University of Chinese PLA. Six male SD rats,weighing 150- 200 g, were selected.INTERVENTIONS: After intraperitoneal injection of chloral hydrate (0. 5 mL), the male SD rats were deeply anesthetized. When their EEG signal became stable, bemegride injection was diluted at 1:1 with saline and was given on a volume of 0.5 mL to the rats intraperitoneally. After a while,the epileptic seizures started marked by a spasm with a deep roar. The entire epileptic seizures were recorded. According to the shape of EEG waves and the corresponding symptoms of the rats during their seizures, data of the four phases, referring to normal condition, preictal phase, ictal phase and postictal phases of epileptic seizures, were selected for nonlinear analysis. The variations of the ApEn and the correlation dimension were calculated.MAIN OUTCOME MEASURES: In the four phases of the seizures, before seizures, preictal phase, ictal phase and postictal phases, the changes in the ApEn and correlation dimension were observed.RESULTS: All the 6 rats entered the statistical procedure. During epilepsy, the ApEn and correlation dimension of the EEG signal in ictal phases (0. 447 ±0. 126, 2. 166 ±0. 377) decreased significantly while those in preictal phases(0. 807 ±0. 182, 4. 773 ±0. 319) and postictal phases (1. 241 ±0. 125, 6. 042 ±0. 373) (t = -3. 984to 17. 902, P <0. 01). The ApEn and the correlation dimension of the EEG signal in preictal and ictal phases had significant difference with those observed under normal conditions (1.313 ± 0. 090, 6. 405 ± 0. 694) (t = -5. 228 to 12. 740, P < 0. 01 ).CONCLUSION: The changes in ApEn and correlation dimension showed by nonlinear dynamical approach in this study reflect the characteristics of EEG signals in preictal time, ictal time and postictal timeof the epileptic seizures and the differences among them. Additionally, they also reveal the laws in the changes of the complex ictal EEG signal.
10.Nonlinear analysis on the EEG information of rat epileptic model.

The aim of this study was to develop a new method of epileptic prediction using nonlinear dynamic theory. When rat was falling sickness, its EEG was researched by using approximate entropy and correlation dimension. The results showed the approximate entropy and correlation dimension during epileptic seizure are obviously lower than those before seizure and after seizure. The span of time before seizure is a special phase. Before the seizure symptom appeared, the complexity of EEG had begun declining. Thus, the outbreak of epilepsy could be predicted in short time using nonlinear dynamic methods.
