1.Value of 131I SPECT/CT on the re-staging and recurrence risk stratification and its impact on subsequent treatment strategy in post-surgical patients with differentiated thyroid carcinoma

Objective To evaluate the role of 131I SPECT/CT in post-surgical re-staging and recurrence risk stratification in patients with DTC and its impact on subsequent treatment strategy.Methods 131I-WBS and 131I SPECT/CT were performed at the same time 5 to 7 d after 131I treatment in 118 patients (33 males,85 females,average age 45 years) with DTC.Difference in the localization and qualitative diagnosis of 131I uptake lesions between 131I-WBS and 131I SPECT/CT were compared.Value of 131I SPECT/CT in the diagnosis of TNM staging,risk stratification and impact on the treatment strategy was evaluated.Paired χ2 test was used for data analysis.Results A total of 509 foci with 131I uptake were detected.131I-WBS found 449 foci with 131I uptake,354 of which (78.84%) were correctly diagnosed.131I SPECT/CT found 509 foci with 131I uptake,and 504(99.02%) were correctly diagnosed.The difference was statistically significant (χ2=21.51,P<0.01).131I-WBS changed the clinical staging in 13 cases with diagnostic accuracy of 5/13.131I SPECT/CT changed the clinical staging in 19 cases and with diagnostic accuracy of 19/19 (χ2=74.41,P<0.01).131I-WBS changed the risk stratification of 13 patients after operation and the accuracy was 5/13,the corresponding data were 22 and 100%(22/22) for 131I SPECT/CT (χ2=74.41,P<0.01).The treatment strategy was changed in 50 patients with 131I SPECT/CT.Conclusions Compared with 131I-WBS,131I SPECT/CT could provide more accurate positioning and qualitative information for 131I treatment and is more accurate in re-staging and risk stratification.
2.Effect of Preoperative Acupuncture on Hypercoagulability of Elderly Patients with Intertrochanteric Fracture

Objective To observe the clinical effect of preoperative acupuncture on hypercoagulability of elderly patients with intertrochanteric fracture. Methods Sixty elderly patients with intertrochanteric fracture were evenly randomized into control group and acupuncture group. Both groups were treated with external application of Shangke Huangshui gauze on the affected region, and additionally the acupuncture group was treated with acupuncture on bilateral acupoints of Huantiao, Xuehai, Liangqiu, Zusanli, Sanyinjiao, and Taichong. The treatment for the two groups was performed from the hospitalized day to the day before operation. The parameters of thrombelastograph (TEG) before and after treatment and the occurrence of deep venous thrombosis (DVT) were observed . Results After treatment, the difference of TEG parameters of the control group was insignificant as compared with those before treatment (P>0.05). In the acupuncture group, the coagulation reaction time(R) and clot formation time(K) were significantly prolonged(P < 0.05) , and coagulation angle alpha (α), maximal amplitude (MA)of thrombus and clot strength (G) were significantly decreased (P <0.05), the effect being superior to that in the control group(P<0.05). Till the day before operation , 3 cases from the control group suffered from DVT, but none of acupuncture group suffered from DVT, the difference between the two groups being insignificant(P > 0.05). Conclusion Preoperative application of acupuncture can improve the hypercoagulability of elderly patients after hip fracture.
3.Construction of lentiviral vector of mFVII/Fc fusion gene and the research of its expression in human bone mesenchymal stem cells

Objective To construct a recombinant Ientiviral vector of mFVII/Fc and investigate its transfective efficiency into human bone mesenchymal stem cells (hBMSCs),and to detect the expression of mFVII/Fc fusion gene in vitro. Methods Coagulation factor VII (FVII) was cloned in vitro,with a point mutation from Lys to Ala in the position of 341 in the gene level.The cDNA fragments of mutational FVII (mFVII) and those of IgG1Fc were fused together with DNA ligase.After digestion,integration and sequencing,the fusion DNA was identified and transfected human embryonic kidney 293T cell packaging for re-mFVII/Fc lentiviral vector.After successful identification of vectors,detect the Ientiviral titer determination,bulk transfer after the determination of best MOI value of the third generation of hBMSCs,obseve the GFP expression with fluorescence microscope,have relative quantitative analyse of mRNA and protein expression of mFVII/Fc with RT-PCR and ELISA at different time points. Results In contrast with GenBank ID: AF 272774,the fusion gene matches exactly except the synonymous mutation,and the titer of packaging lentivirus was 2×108 TU/ml.Analyzed by Flow cytometry, indentification results of hBMSCs were as follows,CD+29(98.08%),CD+44 (97.63%),CD+34(0.31%) and CD+45(0.58%),respectively.The transfection efficiency of hBMSCs after 72 hours was (84±3)%,and the hBMSCs with mFVII/FC transfcetion have a large number of mRNA transcription and protein expression levels. Conclusions In this experiment we obtained a stable genetic vector with hBMSCs fusion gene expression successfully,which lay a foundation for the tissue factor study of prostate cancer targeting therapy and cancer gene therapy research.
4.Modified-coblation assisted UPPP in treatment of severe obstructive sleep apnea-hypopnea syndrome.

OBJECTIVE:
To explore a new way to modify the UPPP and to assess the operative efficacy of modified-coblation assisted UPPP (M-CAUP) on patients with severe obstructive sleep apnea-hypopnea syndrome (OSAHS).
METHOD:
A retrospective analysis was made on surgical therapeutic effect of M-CAUP performed in our hospital.
INCLUSION CRITERIA:
87 adult inpatients with severe OSAHS were involved in the study whose preoperative AHI were beyond 30/h and LSaO2 were under or equal to 85%. All those had the primary level obstruction within the retropalatal region as determined by PSG and airway pressure fluctuation monitoring (ApneaGraph 200, MRA-Medical Ltd, Gloucestershire UK) . The surgical approach of M-CAUP: (1) bilateral tonsillectomy by using Arthrocare EVac 70 T&A Wand. (2) dissection of velum palatinum space by using Arthrocare EVac 70 T&A Wand, ablation of fat tissue to removed it from the space, avoiding injure levator palatini muscle and tensor palatini muscle and preserving musculus uvula, maintaining the normal structure of oropharynx. (3) ablation of channel of soft palate by using Arthrocare EVac 55 T&A Wand to reduce the volume of soft palate moderately. (4) plasty: suturing the tonsilar fossa to close the space, interrupted suture two sides of palatine arch and uvula. All patients were followed up for six to eighteen months postoperatively and received PSG.
RESULT:
After M-CAUP, the pharyngeal cavity was enlarged while the basic structure of oropharynx was maintained and no nasopharyngeal refluxing occurred. The subjective symptoms were also improved evidently after operation. After operation, the AHI and Epworth sleepiness scale (ESS) decreased while LSaO2 increased. The reduction of AHI and ESS had statistical significance (P < 0.01), and the rise of LSaO2 also had statistical significance (P < 0.01). The total effective ratio of the operation was 89.7%.
CONCLUSION
Despite of various treatment, the operative efficacy of conventional UPPP was almost not ideal on severe OSAHS due to complex pathogeny. It was proved that M-CAUP was an effective surgical treatment of severe OSAHS with less blood loss. The operation was minimally invasive and maintained the normal functions of palatopharynx in principle. It could be applied in clinical practice.

5.~(99m)Tc-octreotide scintigraphy and planar X-ray mammography diagnosis of breast cancer

Objective To compare the value of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography in the diagnosis of breast cancer. Methods A total of 32 patients with suspected breast cancer were enrolled into this study. All patients underwent (~99m) Tc-octreotide scintigraphy and planar X-ray mammography. The diagnostic efficiency of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography were compared based on the post-surgery pathology. Results The sensitivity of (~99m) Tc-octreotide scintigraphy and planar X-ray mammography in the diagnosis of breast cancer was 91.67% and 66.67%, the specificity was 100% and 37.50%, the accuracy was 93.75% and 59.38%, respectively. Combining (~99m) Tc-octreotide scintigraphy with planar X-ray mammography, the sensitivity, specificity, accuracy, positive predicting values and negative predicting values for breast cancer was 95.83%, 100%, 96.88%, 82.14% and 47.06%, respectively. Conclusion The diagnostic efficiency of (~99m) Tc-octreotide scintigraphy for breast cancer is superior to planar X-ray mammography. (~99m) Tc-octreotide scintigraphy combined with planar X-ray mammography, the diagnostic efficiency for breast cancer will be further improved.
6.Plasma radiofrequency ablation in treatment of recurrence of adenoidal hypertrophy.

OBJECTIVE:
To investigate the long-term outcome and clinical value of utilizing plasma radio frequency ablation under indirect laryngoscope in treatment of recurrence of adenoidal hypertrophy.
METHOD:
Fifty patients with recurrence of adenoidal hypertrophy were subjected to the operation designed by our group.
RESULT:
The period of follow-up was more then 2 years. All patients were free of snore postoperatively; conductive hearing loss was improved as well.
CONCLUSION
Operation with plasma radiofrequency ablation had advantages of direct and clear view of surgical area, accurate remove of adenoids, non-bleeding, avoiding damage of normal nasopharyngeal structure structures, minimal invasion, high safety, and little complication, in treatment of recurrence of adenoidal hypertrophy.

7.Effect of plasma radiofrequency ablation on obstructive sleep apnea hypopnea syndrome.

OBJECTIVE:
It was aimed to investigate the effect of plasma radiofrequency ablation on obstructive sleep apnea hypopnea syndrome.
METHOD:
Fifty-four patients with OSAHS were treated with plasma radiofrequency ablation on soft palate, tonsil, uvula and root of tongue in light of needs.
RESULT:
All these cases were reevaluated with PSG by the end of postoperative period lasted for three month. Among them, 6 were cured, 24 had significant therapeutic effect, 19 had effective outcome, and 5 had no effective, with a total effective rate of 90.74%. No serious complications occurred.
CONCLUSION
This method was safe and effective, and patients had mild lesion, less bleeding, less painful and the advantages of simple operation.

With HL7 and DICOM applied, this paper presents a tele-medical information-sharing platform based on PACS and HIS. Then the general scheme is put forward from such aspects as medical image storing center, copy import system for medical image data, tele-consultation center, disparate data source middle-ware transponder based on HL7 and clinical rounds system based on NC wireless network.
9.Design and Implement of Data Collection and Management System for Water Meters, Electric Meters and Oxygen Gauges in Hospital

Objective To research and develop a data collection and management system for water meters, electric meters and oxygen gauges within the hospital intranet and perform an interface to the reckon of No.1 Military Project to make sure the accruracy of reckon entry. Methods Terminal equipment is water meters, electric meters and oxygen gauges with RS485 long-distance transmission interface. All the long-distance transmission meters for each independent reckon unit are connected to a single or a group of Comport-Network transformers. The data collected by data collection software will then periodically transmit and save the equipment data in a single or multiple special datatables built on No.1 Military database platform. Results This technology has been applied to the newly built in -patient building. Conclusion The interface designed to integrate decentralized equipment and data into the hospital's local area network using Comport- Network will have extensive application and promising marketing prospect.
10.Determination of Eugenol in Dikou Lizhong Wan by HPLC Combined with Solid-Phase Extraction

Objective: To establish a method for the determination of eugenol in Dikou Lizhong Wan. Methods: The sample was first purified on Sep-Pak C18 microcolumn. The chromatographic conditions were as follow: Kromasil C18 chromatographic column (200mm′4.6mm , i.d. 5mm), methanol-water mixed solution (60 ∶40) as mobile phase , the detection wavelength at 270 nm and the flow rate being 1.0 mL?min-1. Results: The average recovery of eugenol was 99.0 %and RSD= 1.8 %(N=6). Conclusion: This method was simple, convenient, rapid,accurate and reliable and it can be used to control the quality of Dikou Lizhong Wan.
