1.Application of "balance-shaped sternal elevation device" in the subxiphoid uniportal video-assisted thoracoscopic surgery for anterior mediastinal masses resection

Objective To introduce an innovative technique, the "balance-shaped sternal elevation device" and its application in the subxiphoid uniportal video-assisted thoracoscopic surgery (VATS) for anterior mediastinal masses resection. Methods Patients who underwent single-port thoracoscopic assisted anterior mediastinal tumor resection through the xiphoid process at the Department of Thoracic Surgery, West China Hospital, Sichuan University from May to June 2024 were included, and their clinical data were analyzed. Results A total of 7 patients were included, with 3 males and 4 females, aged 28-72 years. The diameter of the tumor was 1.9-17.0 cm. The operation time was 62-308 min, intraoperative blood loss was 5-100 mL, postoperative chest drainage tube retention time was 0-9 days, pain score on the 7th day after surgery was 0-2 points, and postoperative hospital stay was 3-12 days. All patients underwent successful and complete resection of the masses and thymus, with favorable postoperative recovery. Conclusion The "balance-shaped sternal elevation device" effectively expands the retrosternal space, providing surgeons with satisfactory surgical views and operating space. This technique significantly enhances the efficacy and safety of minimally invasive surgery for anterior mediastinal masses, reduces trauma and postoperative pain, and accelerates patient recovery, demonstrating important clinical significance and application value.
2.Subxiphoid uniportal approach using double sternum retractors versus subxiphoid and subcostal arch three-portal approach of video-assisted thoracoscopic surgery thymectomy for thymoma treatment: A retrospective cohort study

Objective To compare the efficacy and safety of video-assisted thoracoscopic surgery (VATS) thymectomy for the treatment of thymoma through subxiphoid uniportal approach using double sternum retractors, and subxiphoid and subcostal arch approach. Methods We retrospectively analyzed the clinical data of the patients diagnosed with thymoma who underwent VATS thymectomy from June 2023 to June 2024 in West China Hospital. Patients were categorized based on the surgical approach into two groups: a subxiphoid uniportal VATS thymectomy (SUVT) group and a subxiphoid and subcostal arch VATS thymectomy (SASAT) group. Comparisons were made between the two groups regarding surgical duration, intraoperative blood loss, postoperative drainage, thymoma size and location, and postoperative pain assessed using the visual analogue scale (VAS). Results The SUVT group consisted of 20 patients, including 11 males and 9 females, with an average age of (51.5±14.3) years. The SASAT group comprised 40 patients, including 26 males and 14 females, with an average age of (50.0±13.0) years. Compared to the SASAT group, the SUVT group had significantly larger thymomas [ (5.9±2.7) cm vs. (4.2±2.1) cm, P=0.010] and a higher proportion of neoplasms located in the superior mediastinum (30.0% vs. 2.5%, P=0.007). Additionally, the VAS pain scores on postoperative days 3, 7, and 30 were significantly lower in the SUVT group compared to the SASAT group (P<0.05). There were no statistical differences between the two groups in demographic characteristics, operative time, intraoperative blood loss, duration and volume of postoperative drainage, length of postoperative hospital stay, or the VAS pain score on the first postoperative day. Conclusion SUVT using double sternum retractors significantly reduces postoperative pain and provides superior efficacy in the resection of larger thymomas or those situated in the superior mediastinum.
3.Meta analysis of the effects of sports participation on adolescent aggressive behavior

Objective:
To explore the relationship between adolescent sports participation and aggression, so as to provide a reference basis for sports interventions aimed at reducing adolescent aggression.
Methods:
The search databases used in the study included China National Knowledge Infrastructure, Vipo, Wanfang, Web of Science, Ebsco, Pro Quest, and PubMed, and the search period was from the construction of the database to October 22, 2023. The search terms for sports participation were "sport" "exercise" "exercise" "physical activity" "physical activity"; the search terms for aggressive behavior were "assault" "aggressive behavior" "aggression"; the search terms for aggressive behavior were "assault" "aggressive behavior" "aggression" "bullying", and "violence", and the above keywords were jointly searched. Foreign language data were searched using Web of Science, Ebsco, Pro Quest, PubMed and supplemented by Google Scholar. The search terms for sports participation were sport, athletics, exercise, train, fitness, physical exercise, physical activity, physical education; and the search terms for aggressive behavior were aggression, bullying, violence, atrocity, fighting, aggressive behavior, physical assault; the above keywords were jointly searched. Statistical analysis was performed using CMA 3.0 software.
Results:
A total of 20 studies with 80 effect sizes and 9 308 subjects were included. The Metaanalysis showed that adolescent sports participation was moderately negatively correlated with indicators of aggression (r=-0.11), physical aggression (r=-0.14), verbal aggression (r=-0.14), and hostility (r=-0.16), and weakly negatively correlated with indicators of anger (r=-0.08) (P<0.05). Aggressive behavior was influenced by gender, school stage, mode of exercise, and the sports participants region (P<0.05).
Conclusions
Exercise participation has a positive impact on reducing aggressive behavior in adolescents, and mode of exercise, age, gender, and region are moderating variables in the relationship. Schools can reduce adolescent aggression by developing exercise and physical education interventions.
4.Study on protective effects of twin drugs of tetramethylpyrazine-scutellarein on cerebral ischemia-reperfusion injury model rats and its mechanism

OBJECTIVE To study the protective effects of twin drugs of tetramethylpyrazine-scutellarein (TMSC4) on cerebral ischemia-reperfusion injury (CIRI) model rats and its mechanism. METHODS One hundred and five SD rats were randomly divided into sham operation group, model group, scutellarein group (0.7 mmol/kg), tetramethylpyrazine group (0.7 mmol/kg), and TMSC4 low-dose, medium-dose and high-dose groups (0.35, 0.7, 1.4 mmol/kg), with 15 rats in each group. Sham operation group and model group were given constant volume of normal saline intragastrically, and other groups were given relevant drug intragastrically, once a day, for consecutive 14 d. Except for sham operation group, all other groups were treated to establish the CIRI model using the thread occlusion method. After 2 hours of ischemia and 22 hours of reperfusion, the brain index and brain water content of the rats were measured. Serum levels of interleukin 1β (IL-1β), IL-6 and tumor necrosis factor α (TNF-α), the levels of superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px) and catalase (CAT) in brain tissues, the situation of neuronal cell apoptosis, and the protein expressions of B-cell lymphoma 2 (Bcl-2), Bcl-2-associated X protein (Bax) and cleaved-caspase-3 were evaluated. RESULTS Compared with sham operation group, the brain index, brain water content, the serum levels of IL-1β, IL-6 and TNF-α, the levels of MDA in brain tissues, the brain cell apoptosis and the protein expressions of Bax and cleaved-caspase-3 in model group were significantly increased (P<0.05); the levels of SOD, GSH- Px and CAT and the protein expression of Bcl-2 in brain tissues were significantly decreased (P<0.05). Compared with model group, the above indexes of rats were reversed significantly in administration groups (P<0.05), while the reverse effects of TMSC4 medium-dose and high-dose groups were significantly better than those of scutellarein group and tetramethylpyrazine group (P<0.05). CONCLUSIONS TMSC4 has a certain protective effect in CIRI model rats, the mechanism of which may be related to relieving inflammatory reaction and oxidative stress, inhibiting cell apoptosis.
5.Effects of auranofin on the activity of ovarian cancer cells and its molecular mechanisms

Objective:
To explore the activity of auranofin against ovarian cancer cells and its possible molecular mechanism.
Methods :
The dose-response survival curve and IC50 of auranofin on ovarian cancer cell lines ,SKOV3,Caov3 and SW626 cells and immortalized normal human embryonic kidney HEK-293T cells were determined by CCK-8 method.Cell cycle was determined by flow cytometry.The levels of total glutathione ( GSH) ,reduced GSH and glutathione disulfide ( GSSG) ,thioredoxin reductase (TrxR) and reactive oxygen species (ROS) in cells were determined by microplate reader,and the reduced GSH / GSSG ratio was calculated.Western blot was used to determine the expression of cyclin dependent kinases( CDK) 4,CDK6,Cyclin D1,P53,p-P53 and MDM2 in SKOV3 and Caov3 ovarian cancer cells.
Results :
Compared with HEK-293T cells,the dose-response survival curves and IC50 values of SKOV3,Caov3 and SW626 cells showed that ovarian cancer cells were more sensitive to auranofin (P<0. 05) .After SKOV3 and Caov3 cells were treated with the dose of respective IC50 concentrations of auranofin,compared with the untreated cells group,the Auranofin IC50 group cells' intracellular levels of GSH,the ratio of reduced GSH / GSSG and the activity of TrxR decreased (t = 25. 11 /31. 18,14. 72 /19. 92,43. 30 /10. 74, all P<0. 05) ,and the levels of ROS increased (t = 23. 82 /27. 71,P<0. 05) ; cells number at G0 / G1 phases increased,with cells number at S and G2 phases decreased (P<0. 05) ; and the expression levels of cell cycle-related proteins CDK4,CDK6,Cyclin D1 and the P53-specific E3 ubiquitin ligase MDM2 were down-regulated (t = 7. 51 /15. 59,17. 32 /11. 26,20. 78 /20. 78,24. 25 /17. 32,all P<0. 05) ,while the expression levels of P53 and p-P53 were up-regulated (t = 17. 32 /24. 25,12. 12 /10. 39,all P <0. 05) .
Conclusion
Auranofin causes oxidative stress in ovarian cancer cells by inhibiting TrxR activity,and by partially degrading MDM2 to stabilize and acti- vate P53,so as to block the cancer cells in G0 / G1 phase,and exert anti-ovarian cancer activities.
6.Clinical evaluation of the anti-gingivitis efficacy of a novel stabilized stannous-containing sodium fluoride dentifrice

Objective: To evaluate the anti-gingivitis efficacy of a novel stabilized stannous-containing sodium fluoride dentifrice. Methods: A randomized, controlled and double blind clinical study was conducted. 156 adults with gingivitis were enrolled and randomly assigned to experimental group(group of novel stabilized stannous-containing sodium fluoride dentifrice, n = 51), positive control group (group of Yunnan Baiyao dentifrice, n = 54) and negative control group (group of Crest dentifrice, n = 51). Gingival health was assessed using Mazza Modification of the Papillary Bleeding Index(Mazza GI) at Baseline, day 3 and week 4 and pocket depth was evaluated at baseline and week 4, respectively. Results: At day 3 and week 4, the experimental and positive control groups exhibited lower clinical parameters than the negative control group(P< 0. 000 1). At week 4, the mean Mazza GI scores and PD of the experimental group were significantly lower than those of the positive control group(P< 0. 05). Conclusion: The novel stannous-containing sodium fluoride dentifrice has anti-gingivitis efficacy.
7.Study on chemical constituents of Scutelliaria regeliana.

OBJECTIVETo study the chemical constituents in ethyl acetate fraction from the root of Scutelliaria regeliana.
METHODThe compounds were isolated by silica gel column chromatography and HPLC, and their structures were elucidated by means of spectral analyses.
RESULT23 compounds were isolated and identified.
CONCLUSIONCompound 1 is new, named as scutellariae flavonol, and the others were isolated from S. regeliana for the first time.

8.Evaluation of minimally invasive peratrial device closure of secundum atrial septal defects in children.

OBJECTIVE:
To introduce peratrial device closure of secundum atrial septal defects (ASD) under the guidance of transesophageal echocardiography (TEE) without cardiopulmonary bypass (CPB) in children, and to summarize the clinical experiences.
METHODS:
A total of 115 children with secundum ASD (the occlusion group) underwent peratrial device closure of atrial septal defects through a small sternotomy under TEE guidance without cardiopulmonary bypass. Children were followed up closely for 1-13 months. Another 59 children (the bypass group) had closed atrial septal defects under cardiopulmonary bypass during the same period. The differences in the operation duration, convalescence and complication between the 2 groups were compared.
RESULTS:
Except 1 patient was operated under the CPB, the rest 114 patients in the occlusion group were successfully closed by the occluders. The duration of the operation, mechanical ventilation, intensive care and hospitalization, and the rate of blood-transfusion in the occlusion group were significantly lower than those in the bypass group (P<0.01), with no difference in complications in the 2 groups (P>0.05).
CONCLUSION
Minimally invasive peratrial device closure of ASD without CPB is a relatively simple, safe and effective operation under the guidance of TEE for children. The short and mid-term clinical outcomes are promising. Long-term follow-up is indispensable.

9.A New Phenolic Acid from Rhizomes of Abacopteris penangiana

Objective To study the chemical constituents of Abacopteris penangiana.Methods The compounds were separated and purified by various chromatographic techniques and their structures were elucidated on the basis of physiochemical properties and spectroscopic methods.Results Seven compounds were purified and their structures were identified as:(7'Z)-3-O-(3,4-dihydroxy phenylethenyl)-caffeic acid(1),caffeiein B(2),matteucinol(3),protocatechuic acid(4),p-methoxybenzoic acid(5),β-sitosterol(6),and daucosterol(7).Conclusion Compound 1 is a new phenolic acid compound named abacopteric acid,and the other compounds are isolated from the plant for the first time.
10.A 10-year survey on birth defects after In vitro fertilization-embryo transfer In Shanghai

Objective To survey birth defects of neonates conceived by using various types of in vitro fertilization and embryo transfer (IVF-ET) between 1998 and 2007 in Shanghai. Methods From 1998 to 2007, 8507 neonates from 6551 pregnancies conceived through assistant reproductive technology (ART) from 7 reproductive medicine center in Shanghai were enrolled in this retrospective study, including Shanghai Ji-Ai Genetics and IVF Institute, Shanghai Jiaotong University School of Medicine affiliated Renji Hospital,Buijin Hospital, China Welfare Institute International Maternal and Infant Health Hospital, Shanghai First Maternity and Infant Health Hospital, Shanghai the Ninth People's Hospital and the Second Military Medical University affiliated Changhal Hospital. The clinical data about the type and incidence of birth defect were analyzed. Meanwhile, the factors associated with birth defect were investigated, such as various ART,gender, mother age, numbers of gestational sac, the source and quality of sperm. Results The birth defect rate was 1.08% (92/8507) and the most remarkable malformation occurred in circulation system [34% (31/92)]. The rates of major congenital malformations were 1.21% (34/2799) in fresh conventional IVF-ET, 1.07% (20/1871) in IVF-frozen-thawed embryo transfer(IVF-FET), 1.04% (23/2212) in fresh intracyto plasmic sperm injection (ICSI)-ET and 0.92% (15/1625) in ICSI-FET, which did not show statistical difference (P>0.05). There was also no significantly different incidence of malformations between 1.12% (49/4371) in male neonates and 1.02% (42/4136) in female neonates (P >0.05).However, the occurrence of congenital malformation is positively related with maternal age, the rates were 0.84% (41/4884) in mother's age less than 30 years and 1.77% (16/902) in mother' age more than 35 years, which reached statistical difference (P < 0.05). It also showed remarkable different incidence among 0.53% (25/4679) in singletons, 1.59% (57/3576) in twins and 3.97% (10/252) in triplets (P< 0.05). No remarkable difference of malformation rate among sperm sources used in fertilization were observed, including 1.09% (81/7419)in ejaculated sperm, 1.21% (7/579)in percutaneous epididymal aspiration (PESA) and 0. 79% (4/509) in donor sperm (P>0.05). Conclusions The overall incidence of birth defect in neonates from ART is similar to those conceived naturally and is not associated with different types of IVF-ET, embryo cryopreservation, sperm sources. However, maternal age and multiple pregnancies coder the higher possibility of birth defect.
