1.The effect of topical oxygen therapy on the intervention of patients with diabetic foot ulcers:a Meta-analysis

Objective To systematically analyze and compare studies related to the intervention effect of topical oxygen therapy on patients with diabetic foot ulcers,and provide references for nursing practice.Methods We searched PubMed,Embase,Web of Science,CENTRAL,CINAHL,Clinical Trial,China Biomedical Literature Database,CNKI,Wanfang Database,and VIP from inception to February 1,2023,to collect studies on the effect of topical oxygen therapy interventions on patients with diabetic foot ulcers.2 researchers independently screened the litera-ture,and extracted the information,and a meta-analysis of the included literature was performed by RevMan 5.4 software.Results 8 studies with 622 patients were included.Meta-analysis results showed that compared with con ventional care,topical oxygen therapy improved the response rate[RR=1.59,95%CI(1.16,2.17),P=0.004]and the reduction of diabetic foot ulcer area[MD=28.78,95%CI(14.83,42.73),P<0.001],and the method did not increase the incidence of adverse events[RR=0.83,95%CI(0.63,1.10),P=0.190],but the difference was not statistically significant in terms of healing time[MD=9.86,95%CI(-15.39,35.11),P=0.440].Conclusion Topical oxygen therapy helps to im-prove the response rate and reduce the ulcer size in patients with diabetic foot ulcers with a better safety profile,but the effect of the intervention on healing time is unclear.Further high-quality randomized controlled trials should be conducted in the future to validate the efficacy of topical oxygen therapy in patients with diabetic foot ulcers.

3.Advances in endoscopic therapy for benign distal biliary strictures

Benign distal biliary strictures (BDBS) are fibrous tissue proliferation and biliary stricture caused by long-term stimulation of the affected bile ducts due to non-neoplastic factors such as iatrogenic injury, chronic inflammation, and bile duct stones, which further leads to recurrent cholangitis, obstructive jaundice, and liver impairment. Relieving distal biliary obstruction and maintaining bile duct patency for a long time are the core of the treatment of BDBS. With the continuous innovation of endoscopic retrograde cholangiopancreatography techniques, new techniques such as endoscopic stenosis dilatation, stent implantation, and magnetic compression anastomosis are gradually becoming effective treatment methods for BDBS. This article elaborates on the advances in endoscopic therapy for BDBS, so as to provide a reference for clinical research.
4.Effects of osteocalcin on bone growth and development and expression levels of related hormones in offspring rats under the intervention of sodium fluoride

Objective:To investigate the effects of osteocalcin (BGP) on bone growth and development and the expression levels of related hormones in offspring rats under the intervention of sodium fluoride.Methods:Twenty-four clean female SD rats and 24 clean male SD rats were selected, weighing 180-220 g, and the rats were mated in a 1∶1 cage for 10 d. The fluorosis rat model was established by drinking the fluorosis water method, female rats were divided into 3 groups according to body weight by random number table method, each group of 8 rats, including the high-dose, low-dose and control groups, with sodium fluoride of 200, 100, 0 mg/L in drinking water. The female rats were exposed to fluoride from the 0th day of pregnancy to the 3rd week after the offspring rats were born (before weaning). After weaning, 10 male offspring rats were selected from each group and continued to be exposed to fluoride in the same amount and manner until the 12th week after birth. The body weight and length of the offspring rats were measured every week before weaning and every two weeks after weaning. Enzyme-linked immunosorbent assay (ELISA) was used to detect the serum BGP, parathyroid hormone (PTH), calcitonin (CT), and alkaline phosphatase (ALP) contents in the offspring rats of each group exposed to fluoride for 12 weeks.Results:In the 2nd week of fluoride exposure, the body weights [(27.25 ± 3.57), (26.27 ± 4.48) g] and body lengths [(6.92 ± 0.46), (6.50 ± 0.54) cm] of the low-dose and high-dose groups were lower than those of the control group [(31.32 ± 3.62) g, (7.19 ± 0.26) cm, P < 0.05], but there were no significant differences in body weights and lengths between the high-dose group and the low-dose group ( P > 0.05). From the 3rd week of fluoride exposure, the body weight and length of the high-dose group were lower than those of the low-dose group and the control group ( P < 0.05). Serum BGP, PTH and ALP contents [(5.42 ± 0.26) mg/L, (157.53 ± 32.21) ng/L, (36.62 ± 6.01) U/L] in the control group were lower than those of the low-dose group [(6.15 ± 0.29) mg/L, (212.26 ± 51.97) ng/L, (50.68 ± 6.11) U/L] and high-dose group [(7.31 ± 0.77) mg/L, (274.21 ± 60.32) ng/L, (74.99 ± 9.08) U/L], and CT content [(182.40 ± 17.39) ng/L] was higher than those of the low-dose and high-dose groups [(135.77 ± 14.06), (70.09 ± 13.49) ng/L], and the differences were statistically significant ( P < 0.05); serum BGP, PTH and ALP contents in the high-dose group were higher than those in the low-dose group, and the CT content was lower than that in the low-dose group ( P < 0.05). Conclusion:Sodium fluoride may be involved in regulating the expression of related hormones by promoting the secretion of BGP, thereby affecting the bone growth and development of offspring rats.
5.The role of the hamstrings in stabilizing a knee with ACL deficit

Objective:To investigate the effect of enhancing the strength of the hamstring on the stability of the knee joint.Methods:Thirty patients with anterior cruciate ligament (ACL) tears were randomly divided into a training group ( n=15) and a control group ( n=15). After the injury′s edema stage, all of the subjects received the standard 6-stage rehabilitation training for ACL injury, including isokinetic exercise, isometric tension and contraction exercise, single or bipedal jumping, proprioception exercises and cardiovascular exercise. On the basis of that standard training, additional hamstring strengthening training was given to the training group. It involved three sessions of weight-bearing flexion of the knee joint six to eight times, at least five times a week for three months. All of the subjects underwent the passive relaxation test (PRT), knee function scoring (Lysholm scores) and weight-bearing MRI before and within 1 month after the training. Anterior shift of the tibia (TAS) was measured using weight-bearing magnetic resonance imaging (MRI). Results:Before the training there were no significant differences between the groups in terms of average PRT or Lysholm scores. After the training, the average PRT score in neither group had improved significantly. The average Lysholm scores of the training and control groups were not significantly different either, though both groups′ averages had improved significantly compared with before the training. The average tibial shifts were also significantly smaller than before the training, with the training group′s average significantly smaller than that of the control group.Conclusion:Increasing hamstring muscle strength can reduce tibial anteversion in the weight-bearing upright position and improve the stability of the knee joint after ACL injury.
6.Research progress of hepatitis C virus nucleic acid detection technology

Hepatitis C virus (HCV) infects more than 200 million people globally and its increasing incidence is placing a heavy economic burden on resource-poor countries.Effective HCV diagnosis is of guiding significance for treatment selection and monitoring.Techniques for detection of HCV infection include serological and nucleic acid detection.Serological assay is simple and easy to operate for initial HCV diagnosis.However,the window phase is long,so the active phase of the virus or previous infection cannot be determined.Nucleic acid detection has high sensitivity and wide linear range,which is the direct evidence to determine virus infection.The traditional thermal cycle-based nucleic acid amplification technology has been widely used in clinical laboratories.The development of isothermal nucleic acid amplification technology overcomes the limitations of traditional amplification technology,and it can be used as a tool for Point-of-care Testing (POCT),which has great potential and particularly suitable to be used in low-resource areas with high HCV prevalence,such as Africa and Southeast Asia.In this paper,we outline the current advance and development trends in the HCV molecular diagnosis.
7.Simultaneous Determination of 6 Components in Chaihuang Tablets by HPLC-DAD

OBJECTIVE: To establish a method for simultaneous determination of 6 components in Chaihuang tablets, such as baicalin, wogonoside, baicalein, wogonin, saikosaponin a and saikosaponin d in Chaihuang tablets. METHODS: HPLC-DAD method was used to detect 3 batches of Chaihuang tablets from same manufacturers. The determination was performed on Agilent Eclipse XDB-C18 column with mobile phase consisted of acetonitrile-triethylamine phosphate aqueous solution (pH adjusted to 7.0, gradient elution) at flow rate of 1.0 mL/min. The detection wavelengths were set at 210 nm (saikosaponin a, saikosaponin d) and 277 nm (baicalin, wogonoside, baicalein, wogonin). The column temperature was 30 ℃, and sample size was 5 μL. RESULTS: The linear ranges of baicalin, wogonoside, baicalein, wogonin, saikosaponin a and saikosaponin d were 0.379 5-7.590 4 μg, 0.082 96-1.659 2 μg, 0.039 39-0.787 8 μg, 0.040 72-0.814 4 μg, 0.040 45-0.809 0 μg, 0.038 63-0.772 6 μg (all r≥0.999 3), respectively. The limits of detection were 0.008, 0.007, 0.005, 0.005, 0.020 and 0.018 μg/mL. The limits of quantitation were 0.025, 0.022, 0.015, 0.015, 0.060, 0.054 μg/mL. RSDs of precision, reproducibility and stability tests (48 h) were all lower than 1.5% (n=6). Average recoveries were 98.46%, 97.06%, 100.90%, 96.13%, 96.91%, 96.57% (RSD<2.0%, n=6). CONCLUSIONS: Established method is simple, accurate and reproducible for 6 components in Chaihuang tablets, and can be used for quality control of the tablet.
8. Understanding of the lower mediastinal lymph node dissection in the adenocarcinoma of the esophagogastric junction through abdomino-transhiatal approach

In recent years, the incidence of adenocarcinoma of esophagogastric junction (AEG) keeps increasing. Siewert type II and type III AEG invades at 2-4 cm in the lower esophagus, and it has a higher rate of lower mediastinal lymph node metastasis. Lower mediastinal lymph node clearing through the abdomino-transhiatal (TH) approach is preferred, which can be accomplished by entering the lower mediastinum through the hiatus and mobilize the esophagus upward and the surrounding lymph and connective tissue for approximately 6.5 cm. Using the infracardiac bursa (IBC) as an anatomical landmark improves the safety and operability of the thorough dissection of the lower mediastinum. Total resection of the mesenterium at the esophagogastric junction can entirely dissect the lower mediastinal lymph nodes, which conforms to the safety principles in oncology.
9.Analysis of prognosis-relating factors in the elderly patients after the initial onset of delirium

Objective To investigate the correlations of risk factors of age,delirium-inducing factors,dementia,delirium severity,and disease severity with the prognosis of elderly patients with delirium.Methods A total of 112 patients aged 70 years and older with initial onset of delirium during hospital stay were enrolled in this study.They were hospitalized in Dalian Municipal Central Hospital from Jan.2013 to Dec.2015.The severity of delirium was evaluated by delirium rating scalerevised-98(DRS-R-98).The acute physiology and chronic health enquiry(APACHE-Ⅱ)score and the sequential organ failure assessment(SOFA)score were recorded within 48 h after delirium onset.After periods of hospitalization of Jan.2013 to Dec.2015,patients were followed up until 31 Dec 2017.Correlations of age,delirium-inducing factors and dementia with survival time and long-term viability after delirium onset were statistically analyzed.Results Age,delirium-inducing factors,cognitive impairment,duration of cognitive impairment,type of dementia,and delirium severity had no correlations with the lifetime and long-term viability in patients with delirium.While,APACHE Ⅱ score for assessment of severity degree of delirium(P <0.001,r =-0.390) and SOFA score(P <0.001,r=-0.638)were negatively correlated with the lifetime in patients with delirium.SOFA score (P =0.004) and delirium recurrence (P < 0.001) were significantly correlated with the long-term viability in patients with delirium.Conclusions The severity and recurrence of delirium are strongly correlated with the lifetime and quality of life in patients with delirium.Compared with APACHE Ⅱ score,SOFA score may have a more important clinical application value in predicting the prognosis of patients with delirium.
10.Evaluation of coronary artery hemodynamics and its relationship with atherosclerosis in west diet fed LDL‐R knockout mice by ultrasound bio‐microscopy

To evaluate the relationship between atherosclerosis and hemodynamic of coronary artery in mice detecting by ultrasound bio‐microscopy flow imaging . Methods Double 14 20‐week‐old LDL‐R‐/‐and C57BL/6 male mice were selected ,and randomly divided into two groups in each genotype according to weight . Each two groups were fed to 28 weeks or 36 weeks age respectively with west diet . Coronary artery hemodynamics in these mice were assessed in vivo by Vevo ?2100 ultrasound imaging system ,then the intima‐media thickness( IM T ) of aorta in histopathology were analyzed . T he differences of coronary artery hemodynamic parameters such as maximum velocity ( Vmax ) ,mean velocity ( Vmean) and velocity time integral ( V T I) were compared between mice of different genotypes of the same week and mice of different weeks of the same genotype . And the relationship between coronary artery hemodynamic in ultrasound and aortic IM T in histopathology were analyzed . Results ① All coronary hemodynamic parameters in LDL‐R‐/‐ mice were significantly lower than those of wild‐type mice except the Vmax between two 28‐week‐old genotypes group at the same weeks of age of different genotypes ( all P <0 .05) . But there was no significant difference in coronary artery hemodynamic parameters between mice of the same genotype at different weeks of age( P >0 .05) . ②T he histopathological measurements of aortic IM T in LDL‐R‐/‐mice were significantly higher than those of wild type mice ( all P < 0 .05 ) ,and those of 36‐week‐old mice were significantly higher than those of 28‐week‐old mice ( all P < 0 .05 ) . ③ All coronary hemodynamic parameters such as Vmax ,Vmean and V TI were negatively correlated with pathological measurements of aortic IM T ( r = -0 .532 , -0 .423 , -0 .524 ; all P < 0 .05 ) . Conclusions The parameters of coronary artery hemodynamics obtained by ultrasound bio‐microscopy are well correlated with the pathological results of atherosclerosis . Ultrasound bio‐microscopic flow imaging can be used as a new method to evaluate the degree of atherosclerosis in mice by detecting the hemodynamic parameters of coronary artery .
