1.Establishment of HPLC fingerprints for Ardisia crenata,Sophora tonkinensis and their couplet medicines and content determination of 5 chemical components

OBJECTIVE To establish the fingerprints of Ardisia crenata, Sophora tonkinensis and their couplet medicines, and to determine the contents of five components in them. METHODS Using water as solvent, single lyophilized powder of A. crenata and S. tonkinensis and combined lyophilized powder of their couplet medicines were prepared by combining lyophilization technology. The fingerprints of three lyophilized powder samples were established by using high-performance liquid chromatography (HPLC), and the contents of 5 kinds of components such as gallic acid were determined simultaneously. RESULTS There were 5, 10 and 14 common peaks in the fingerprints for single lyophilized powder of A. crenata and S. tonkinensis and combined lyophilized powder of their couplet medicines; the similarities of them with the control fingerprints were all greater than 0.90. In single lyophilized powder of A. crenata, the average contents of gallic acid and bergenin were 0.499 3 and 4.962 6 mg/g, respectively. In single lyophilized powder of S.tonkinensis, the average contents of matrine, oxymatrine and trifolirhizin were 3.046 0, 2.336 6 and 0.278 6 mg/g, respectively. In combined lyophilized powder of couplet medicines, the average contents of gallic acid, matrine, oxymatrine, bergenin and trifolirhizin were 0.560 6, 2.548 7, 1.382 2, 5.960 7 and 0.279 1 mg/g, respectively. The transfer rates were 8.87%-513.19%. CONCLUSIONS The established fingerprint and content determination methods are stable and feasible, and can be used for the quality control of A. crenata and S. tonkinensis and their couplet medicines. The average contents of matrine and oxymatrine in combined lyophilized powder of A. crenata-S. tonkinensis couplet medicines are decreased.
2.Randomized, Double-blind, Single-simulation, Placebo-controlled, Multicenter Clinical Trial of An'erning Granules in Reducing Use of Antibiotics for Children's Community-acquired Pneumonia

ObjectiveTo evaluate the efficacy and safety of An'erning granules in the treatment of community-acquired pneumonia in children. MethodA randomized, double-blind, single-simulation, placebo-controlled trial was designed in this study. The children were randomly assigned into an observation group (An'erning granules combined with ceftriaxone sodium) and a control group (An'erning granules placebo combined with ceftriaxone sodium) according to the ratio of 2∶1. The disease cure rate was taken as the main indicator of efficacy, and the safety of An'erning granules was observed. ResultA total of 206 children (137 in the observation group and 69 in the control group) were included in this study. Before treatment, the age, sex, body height, body weight, diagnosis time of pneumonia, and symptom and sign scores had no significant differences between the two groups. After 8 days of continuous medication, the observation group[70.80%(97/137)] had higher cure rate than the control group[56.52%(39/69)](χ2=4.17,P<0.05) and total effective rate of chest X-ray [97.98%(97/99)] than the control group[86.27%(44/51)] (χ2=12.98,P<0.01). The observation group was superior to the control group in the alleviation of TCM syndrome under the condition of 0-3 g dose stratification on day 3 of medication (P<0.01). The recovery time, time to complete fever abatement, time to fever abatement and expectoration alleviation, rate of conversion to severe case, and reduction in the frequency of antibiotic use showed no significant differences between the two groups. In terms of safety, 13 and 7 adverse events occurred in the observation group and control group, respectively, which were relieved or disappeared after drug withdrawal or symptomatic treatment and showed no significant difference between the two groups. ConclusionIntravenous drip of ceftriaxone sodium combined with An'erning granules is effective in the treatment of community-acquired pneumonia in children. It can accelerate the absorption of pulmonary inflammation, alleviate the clinical symptoms in a short time for young children or the children with mild symptoms, and is safe in clinical application.
3.Connectivity between visual brain regions and whole brain among children diagnosed with autism spectrum disorder

Objective:
To explore the functional connectivity between the visual brain regions and whole brain in children with autism spectrum disorder (ASD) at resting state, and to further analyze the correlation with their clinical manifestations.
Methods:
The functional magnetic resonance imaging (fMRI) data of 34 boys with ASD enrolled from ASD designated rehabilitation institutions and 29 healthy boys enrolled from several kindergartens in Heilongjiang were collected. Based on the resting-state functional connectivity magnetic resonance imaging (rs-fc MRI) analysis, the BA17 of the primary visual brain region and the BA18/19 of the higher visual brain region were taken as the regions of interest (ROI) to calculate the functional connectivity level between the visual brain regions and whole brain, and the differences between the two groups were compared. Multiple developmental scales were used to evaluate the behavior of ASD children, and Pearson correlation analysis was used to explore the relationship between functional connection strength and autistic behavior.
Results:
The ASD group had decreased positive connectivity between BA17 and the right fusiform gyrus (FFG), and was negatively correlated with social interaction of ADI-R and the total scores of CARS (r=-0.41, -0.48, P<0.05); ASD group had decreased positive connectivity between BA17 and the left FFG, there was a negative correlation with social motivation of SRS (r=-0.43, P<0.05); ASD group had decreased positive connectivity between BA17 and the left posterior cingulate gyrus (PCG). Children with ASD had decreased positive connectivity between BA18/19 and left calcarine fissure and surrounding cortex (CAL), which was positively correlated with attention conversion of AQ, total scores of CARS (r=0.43, 0.40, P<0.05), and the children with ASD had deceased positive connectivity between BA18/19 and right precuneus (PCUN).
Conclusion
In resting state, the functional connectivity of primary and higher visual brain regions and whole brain of ASD children is different from that in healthy children, and there is a significant correlation between abnormal level and autistic behaviors.
4.Operation procedure for 125I seed brachytherapy in malignant solid tumor of the abdomen

The apphcation of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen has been 18 years in China.The teatment is applied by more and more hospitals,however,the operation procedure is not the same in each hospital,which affected the therapeutic effect of radioactive seeds implantation,and means more safety risks.This paper summarizes the operation procedures of radioactive seeds implantation in treatment of malignant solid tumor of the abdomen,and minimizes the loopholes in clinical operation,in order to improve the therapeutic effect and reduce the safety risks.
5.Clinical application of fully-covered segmented esophageal internal irradiation stent: preliminary results

Objective To investigate the differences in the survival time and the occurrence of complications between esophageal cancer patients treated with fully-covered segmented esophageal internal irradiation stent and esophageal cancer patients treated with conventional esophageal internal irradiation stent.Methods The clinical data of 66 esophageal cancer patients,who had received esophageal internal irradiation stents placement,were retrospectively analyzed.The patients were divided into the study group (using fullycovered segmented esophageal internal irradiation stent,n=30) and the control group (using conventional esophageal internal irradiation stent,n=36).The postoperative complications,including restenosis,stent migration,chest pain,etc.,and the survival time of the two groups were recorded.The results were analyzed,and P<0.05 was considered to be statistically significant.Results No statistically significant difference in the restenosis rate existed between the study group and the control group (20.0% vs.30.6%,P=0.403);although the median time of restenosis in the study group was longer than that in the control group (161.5 d vs.138 d,P=0.025).The stent migration rate in the study group was higher than that in the control group (33.3% vs.8.3%,P=0.014).The difference in the median time of stent migration between the two groups was not statistically significant (91.5 d vs.166 d,P=0.236).No statistically significant difference in the median survival time existed between the two groups (186 d vs.178 d,P=0.486).No statistically significantly differences in the incidence of other stent-related complications existed between the two groups.Conclusion Fully-covered segmented esophageal internal irradiation stent can delay the occurrence of restenosis,although it can increase the stent migration rate to a certain degree.
6.Distribution of age at onset and its influence on clinical characteristics of 164 patients with synovitis, acne, pustulosis, hyperostosis, and osteitis syndrome

Objective To investigate the distribution of age at onset and its influence on clinical characteristics in synovitis,acne,pustulosis,hyperostosis,and osteitis (SAPHO) syndrome.Methods We recruited 164 patients with SAPHO syndrome who presented to Peking Union Medical College Hospital from Jan 2004 to Mar 2015.All the patients were assessed for medical history,laboratory tests and imaging presentations.The distribution of age at onset was analyzed using Shapiro-Wilknormality test and Kolmogorov-Smimov test for mixed normal distribution.The influence of age at onset on clinical features was analyzed using Mann-Whitney U test and x2 test.Results A double-peak mixed normal distribution of age at onset of skin lesions was found in female patients with SAPHO syndrome,with means and standard deviations of (30±6) years (early-onset) and (51 ±7) years (late-onset) for each mixed normal distribution.The cut-off point was determined to be 42 years old.Nonetheless,a typical single-peak normal distribution of age at onset of skin lesions was observed in male patients.A significantly higher frequency of thoracic region pain [14/36 (38.9%) vs 6/70 (8.6%),x2=14.28,P<0.01,spinal lesions revealed by bone scintigraphy [23/35 (65.7%) vs 23/66(34.8%),x2=8.79,P=0.003],and peripheral skeletal lesions revealed by bone scintigraphy [17/35 (48.6%) vs 17/66(25.8%),x2=5.33,P=0.021] were found in late-onset female patients compared with early-onset ones.Moreover,female patients with late onset had significantly higher hs-CRP level [(12±12) mg/L vs (9±11) mg/L;U=911.5,P=-0.042)],pain VAS (4.8±1.8 vs 4.0±2.1;U=948,P=0.036),and BASFI (3.0±2.2 vs 1.8±2.0;U=822.5,P=0.003) at baseline than those with early onset.Conclusion Female patients with SAPHO syndrome have a double-peak distribution of age at onset of skin lesions.Female patients with early and late onset of skin lesions exhibit distinct clinical characteristics.
7.Clinical analysis of the initial symptoms among 164 synovitis, acne, pustulosis, hyperostosis, osteitis syndrome patients

Objective To explore the initial symptoms of synovitis,acne,pustulosis,hyperostosis,osteitis (SAPHO) syndrome,and to analyze the clinical and laboratory characteristics of these patients.Methods We retrospectively analyzed the initial symptoms of 164 patients diagnosed with SAPHO syndrome presented at Peking Union Medical College Hospital from 2004 to 2015,and their clinical,laboratory,and radiological data were collected.The t test,Mann-Whitney U test and chi-square test were used to compare the clinical differences between the SAPHO patients with different initial symptoms.Results Among the 164 patients recruited,84(51.2%) had skin lesions before osteoarticular symptoms,whereas 29(17.7%) after and 42(25.6%) simultaneously.Nine (5.5%) patients had no skin lesions.The time interval between onset of skin and osteoarticular lesions was less than 2 years in 133 (81.1%) patients,but up to 35 years at most.Interestingly,a significantly higher age at onset was observed in patients with osteoarticular symptoms prior to skin lesions than those after [(41 ±10) years vs (36±11) years,t=-2.174,P=0.032].Moreover,positive HLA-B27 was more frequently detected in patients having osteoarticular symptoms as the initial presentations (10.3% vs 0,P=0.016).Although treated more aggressively before baseline,patients presented with osteoarticular symptoms prior to skin lesions had significantly higher level of hs-CRP at baseline compared with those after [5.42 (1.88,12.70) mg/L vs 11.60 (3.76,22.08) mg/L,Z=-2.096,P=0.036].Conclusion Skin lesions tend to appear prior to osteoarticular symptoms in most SAPHO syndrome patients.The percentage of patients who developed skin lesions after osteoarticular symptoms increase with age at onset.
8.Assessment of the brain function with 18F-FDG PET/CT in patients with disorders of consciousness

Objective To evaluate the changes of brain metabolism with 18F-fluorodeoxyglucose (FDG) PET/CT in patients with disorders of consciousness (DOC).Methods This retrospective study,from January 2007 to October 2016,included 40 patients (26 males,14 females,age range:17-73 years)in the vegetative state (VS),12 patients (11 males,1 female,age range:25-53 years) in the minimally conscious state (MCS),and 11 patients (10 males,1 female,age range:12-68 years) in the state of recovering from DOC.All patients underwent 18F-FDG PET/CT imaging.The standardized uptake value (SUV) of multiple brain areas among 3 groups of patients was calculated and compared.One-way analysis of variance was used for data analysis.Results The SUV in different encephalic regions among 3 groups were significantly different (F values:6.214-13.642,all P<0.01) except for mesencephalon.Compared with MCS group,the SUV of cerebral cortex of VS group was lower (t values:2.263-3.548,all P<0.05).Compared with the recovered group,the SUV of cerebral cortex and cerebellum of VS group was lower (t values:1.299-5.136,all P<0.05).Compared with the recovered group,the SUV of parietal lobe,temporal lobe,occipital lobe,thalamus and cerebellum of MCS group was lower (t values:1.962-2.841,all P<0.05).Conclusion 18F-FDG PET/CT may be significant in evaluating brain function of DOC patients.
9.Cluster analysis of clinical phenotypes and its implication in 164 patients with Synovitis-Acne-Pustulosis-Hyperostosis-Osteitis syndrome

Objective To use cluster analysis to explore the clinical phenotypes of Synovitis-Acne-Pustulosis-Hyperostosis-Osteitis (SAPHO) syndrome.Methods One hundred and sixty-four patients fulfilled the Kahn and Khan's criteria for SAPHO syndrome were recruited in Peking Union Medical College Hospital from 2004 to 2015.For all patients,demographics,clinical,laboratory and imaging data were collected.Cluster analysis was performed using an iterative partitioning K-means method including 11 variables that was most characteristic in patients with SAPHO syndrome.The optimal number of clusters was determined by the elbow method and Silhouettes coefficient in combination with clinical significance.Results An optimal of two phenotypes with distinct clinical features were identified.Cluster 1 was characterized by axial skeletal involvement with older age at onset [(38±11) years] and lower prevalence of severe acne (11.2%);Cluster 2 had no axial involvement with younger age at onset [(33±8) years;U=1 800,P=0.010] and higher prevalence of severe acne (26.8%;x2=4.567,P=0.033).Cluster l patients had been treated more aggressively by baseline compared with Cluster 2 patients;and were more frequently prescribed TNF-α inhibitors (32.8% vs 2.4%;x2=1 672.5,P<0.01) and bisphosphonates (39.7% vs 19.5%;x2=1962,P=0.032).Nonetheless,the disease activity indices were significantly higher at baseline in Cluster 1 than Cluster 2 patients [Bath ankylosing spondylitis disease activity index (BASDAI) (3.5±1.8) vs (2.8±2.0);U=1 800,P=0.010] [Bath ankylosing spondylitis functional Index(BASDFI) (2.4±2.3) vs (1.5±1.7);U=1 791,P=0.009).Moreover,Cluster 1 patients had significantly increased inflammatory markers at baseline compared with Cluster 2 patients [erythrocyte sedi-ment-ation rate(ESR) (34.9±2.9) mm/1 h vs (19.0±14.6) mm/1 h;U=1 204.5,P<0.01] [high-sensitivity C-reactive protein (hs-CRP) (16±19) mg/L vs (8±11) mg/L;U=1 628,P=0.01].Conclusion Char-acterized by the presence or absence of axial skeletal involvement,two disease subtypes exist in SAPHO syndrome,which exhibit distinct features in age at onset,the prevalence of severe acne,and disease severity.
10.Effects of salvia tetramethylpyrazine on hemorheology in vertebrobasilar transient ischemia attack

Objective To analyze the effect of salvia tetramethylpyrazine on hemorheology in patients with vertebrobasilar transient ischemia attack (VB-TIA). Methods 84 patients with VB-TIA were divided into observation group and control group, 42 cases in each group. They both received the treatment of Ginkgo-diyidamolum injection. The observation group were treated by salvia tetramethylpyrazine, and the control group were treated by compound Danshen injection. The therapeutic effect of the two groups and the changes of blood rheology were compared. Results The clinical remission rate of the observation group was significantly higher than that of the control group, and the time of onset and symptoms of the drug were shorter than those of the control group (P < 0.05). The blood flow velocity of vertebral basilar artery in the two groups were significantly higher than those in the observation group (P < 0.05). After treatment, the hemorheology of the two groups were significantly improved, the high shear viscosity and low shear viscosity, platelet aggregation rate, erythrocyte deformation index, erythrocyte aggregation index and blood haematocrit of observation group improved more obviously (P < 0.05). The incidence of adverse reactions in the two groups was not statistically significant. Conclusion Salvia tetramethylpyrazine could effectively shorten the drug onset time, improve the blood velocity of vertebrobasilar artery, recover blood rheology and stability on the condition of not increasing the adverse reactions, which has a positive effect on the clinical remission rate.
