1.Correlations of brain network functional connectivity alterations with cerebrospinal fluid pathological markers in patients with Alzheimer's disease

Objective:To explore the correlations of brain network functional connectivity (FC) alterations with cerebrospinal fluid (CSF) pathological biomarkers in patients with Alzheimer's disease (AD).Methods:A total of 39 patients with cognitive impairment, admitted to Department of Neurology, Drum Tower Hospital, Affiliated Hospital of Medical School, Nanjing University from January 2020 to December 2022 were recruited; 23 patients were with AD and 16 with non-AD. Clinical data were compared between the 2 groups. Resting-state functional MRI (rs-fMRI) data were collected, and FC differences between brain networks and FC differences within brain networks were compared by independent component analysis. Correlations of FC differences between brain networks and FC differences within brain networks with concentrations of β-amyloid protein 1-42 (Aβ 1-42) and Tau protein in CSF were analyzed. Results:Compared with the non-AD group, AD group had significantly lower Aβ 1-42 in CSF ( P<0.05). Compared with those in the non-AD group, FC alterations between the left frontoparietal network (lFPN) and anterior default mode network (aDMN) and between the visual network (VN) and posterior cingulate cortex (PCC), as well as FC alterations in lFPN, were significantly increased in AD group ( P<0.05). Compared with those in the non-AD group, FC alterations between lFPN and cerebellar network (CEN), and FC alterations in aDMN, sensorimotor network (SMN) and VN were significantly decreased in AD group ( P<0.05). In AD group, FC in SMN was positively correlated with total Tau and phosphorylated-Tau181 in CSF ( P<0.05); FC between VN and PCC was positively correlated with total Tau in CSF ( P<0.05). CSF Aβ 1-42 was positively correlated with FC alterations in aDMN and VN, but negatively correlated with FC in FPN ( P<0.05). Conclusion:In AD patients, characteristic changes in FC within and between multiple brain networks are noted, which are related to changes of Tau protein and Aβ 1-42 in CSF.
2.Clinical Analysis of 311 Cases with Giant Ovarian Mass

Objective:To analyze the clinicopathological features of giant ovarian masses(mean diameter≥10 cm)and analyze the clinical characteristics of patients in different age groups.Methods:The clinicopathological characteristics of 311 patients diagnosed with giant ovarian masses by surgery at Nanjing Drum Tower Hospital,Affiliated Hospital of Medical School,Nanjing University from January 2014 to December 2022 were retrospectively analyzed.Patients were further stratified by age and mass size to compare the differences of clinical and patho-logical features among different age groups and different mass diameter groups.Results:①The median age of thepatients were 44(24,60)years old.The first symptoms were as follows:ovarian mass discovered during physi-cal examination,abdominal pain,bloating,conscious abdominal distension,and symptoms of compression.②The surgical methods were as follows:unilateral oophorectomy(30.5%,95/311),ovarian cystectomy(28.9%,90/311),tumor staging or cytoreductive surgery(28.0%,87/311),total hysterectomy with bilateral adnexectomy(12.5%,39/311).③The pathological types were benign(49.5%,154/311),malignant(31.8%,99/311)and borderline(18.7%,58/311).④ Patients complained abdominal distension in<20 years old group were signifi-cantly higher than the other two groups(P<0.05).The ovarian resection rate in the>50-year-old group was higher than that of the other two groups(P<0.05),and the rate of unilateral ovarian resection in the<20-year-old group was still as high as 30.1%(15/49).⑤ The size of the mass correlated with the duration of the disease.When the disease course was between 1 to 6 months,the mass diameter line>30 cm was the most common(P<0.05).The incidence of borderline tumors in the>30 cm group was significantly higher than that in the other two groups,and the difference was statistically significant(P<0.05).Conclusions:Ovarian mucinous and mucinous borderline tumors are the most common types of giant adnexal masses.The size of the mass tends to increase with the prolongation of the disease course.The incidence of borderline tumors increases with the in-crease of mass.Health education for young people should be strengthened.When abdominal pain,abdominal bloating,especially lower abdominal distension occurs,they should seek medical treatment in time to avoid adnex-ectomy due to borderline tumors.
3.A survey on nutritional intake and analysis of factors influencing malnutrition among the elderly population in Jiangning District, Nanjing City

Objective To investigate the nutritional status of the elderly population in an elderly care institution in Jiangning District of Nanjing City, and analyze the relevant influencing factors, and to provide theoretical guidance for the prevention and control of malnutrition in the elderly population. Methods A total of 1 040 elderly individuals who were admitted to the Nanjing Jiangning Muchun Garden Nursing Home from September 2019 to December 2023 were included as the study subjects. According to the evaluation scores using the Mini Nutrition Assessment Short Form (MNA-SF), the elderly people were divided into a normal nutrition group, a malnutrition risk group , and a malnutrition group . The differences in general condition, dietary intake or elderly syndromes such as anxiety, depression , and activity ability among the three groups of study subjects were compared and analyzed . Results The incidence of malnutrition in the elderly was 15.87%. There were differences in the incidence of malnutrition among the elderly population with different educational levels, marital status, and pre-retirement occupations. The frequency of intake of fresh fruits, fresh vegetables, and dairy products in the malnutrition group of the elderly population was lower than that in the normal nutrition group. The proportion of elderly people with malnutrition and poor sleep quality was higher than that of those with normal nutrition. In addition, the proportion of elderly people with malnutrition, moderate or above depression, and moderate action dependence was also higher than that of those with normal nutrition (P<0.05). The logistic regression results showed that an increase in educational level, a decrease in dairy product intake, and an exacerbation of depression and disability levels were risk factors for malnutrition in the elderly population (OR>1), while good sleep quality was protective factor for malnutrition in the elderly population (OR<1). Conclusion The incidence of malnutrition among elderly people in a nursing home in Nanjing is relatively high. Low frequency of dairy intake, depression, and disability are independent influencing factors for malnutrition among elderly people in this nursing home.
4.GPNMB regulates M2 polarization of microglia thereby alleviating nerve injury after cerebral ischemic stroke through PI3K/Akt pathway

Objective:To investigate the mechanism by which glycoprotein non-transferable melanin B(GPNMB)regulates microglia M2 polarization to reduce nerve damage after cerebral ischemic stroke(CIS).Methods:SD rats were used for establishment of middle cerebral artery occlusion(MCAO)model.Neurons,astrocytes and microglia were cultured under oxygen and glucose depri-vation(OGD)conditions,and the expression of GPNMB in tissues and cells were measured by Western blot.Gpnmb wrapped with adeno-associated virus(AAV)or shRNA-Gpnmb were injected into rat brain tissues for overexpression or inhibition of GPNMB,modified neurological deficit score(mNSS),Rotarod fatigue test and tape removal test were used to evaluate rat nerve function,the proportion of cerebral infarction was determined by TTC staining,microglia M1/M2 polarization markers were detected by immunofluorescence and RT-PCR,and the expression of Phosphatidylinositol 3-kinase(PI3K)/Serine/threonine kinase(Akt)pathway was determined by Western blot.Microglia was cultured under OGD conditions in vitro,Gpnmb was overexpressed and PI3K expression were inhibited by LY294002,and M1/M2 polarization markers were measured.Results:Compared with normal rats or normal cultured cells,the expres-sion of GPNMB in MCAO model or OGD-intervened microglia was up-regulated(P<0.05);when Gpnmb was overexpressed in the brain tissue of MCAO rats,the mNSS score decreased,the Rotarod time of latency to fall lengthened,the contact time and removal time shortened in the tape removal test,the proportion of cerebral infarction decreased,the M1 polarization level of microglia decreased while the M2 polarization level increased,PI3K/Akt pathway activated,and these difference were statistically significant(P<0.05);inhibition of PI3K reversed the effect of overexpression of Gpnmb on promoting M2 polarization of microglia in vitro(P<0.05).Conclusion:GPNMB promotes M2 polarization of microglia by activating the PI3K/Akt pathway,thereby reducing nerve damage after CIS.
5.Epidemiological characteristics of varicella in Songjiang District, Shanghai from 2013 to 2021

ObjectiveTo evaluate the effectiveness of the varicella vaccine immunization strategy in Shanghai, and to provide evidence for enhancing prevention and control measures by analyzing the incidence and trends of varicella in Songjiang District, Shanghai from 2013 to 2021. MethodsA descriptive analysis was conducted on 12 417 varicella cases reported in Songjiang District between 2013 and 2021. The epidemiological distribution of varicella cases was described, and the incidence rates were compared among different demographic variables, including registered residence, gender, age, and occupation. ResultsA total of 12 417 varicella cases were reported in Songjiang District from 2013 to 2021,with an average annual incidence of 77.03/105. The incidence of varicella showed an increasing trend from 2013 to 2017 (χ2trend=19.39, P<0.001), followed by a subsequent decrease from 2017 to 2021(χ2trend=758.62, P<0.001). Varicella cases demonstrated significant seasonal variation, with peak incidences occurring in April to May and November to January of the following year. The incidence rate among local residents was higher than that among non-local residents (χ2=2 935.58, P<0.001). Most (38.59%) of the cases were students aged between 5 and 9 years old, accounting for 23.45% of the total cases. However, the age distribution of cases varied over the years (χ2=615.57, P<0.001), and the incidence among children aged 5 to 9 years showed a declining trend since 2017. ConclusionThe overall incidence of varicella in Songjiang District exhibites a declining trend in recent years. Although varicella primarily affects children, there have notable changes in the age distribution. Adjustment of the immunization strategy for varicella vaccines in Shanghai effectively reduces the proportion of school-age subjects.
6.Analysis of the genome and transmission characteristics of an aggregated novel coronavirus infection based on metagenomic next-generation sequencing

Objective:To analyze the viral genome sequence of novel coronavirus infected persons in Baotou City, understand the mutation characteristics of novel coronavirus genome in the process of transmission among cases, and explore the transmission rule of novel coronavirus in the clustered populations.Methods:Nine throat swabs samples (No. 1 - 7, No. 9, and No. 10), two sputum samples (No. 8, No. 11, and No. 11 sample was from No. 10 case), and one surface smear sample (No.12, and No. 12 sample was from No. 10 case) were collected from 10 confirmed cases of novel coronavirus infection in Baotou City from January 25 to February 21, 2020. Samples 1 and 3 were from single cases, and the rest were from clustered cases. The virus genome was sequenced by metagenomic next-generation sequencing (mNGS), and single nucleotide polymorphism (SNP) mutation sites were screened by comparing with NC_045512, a reference strain of novel coronavirus. Combined with relevant epidemiological information, gene mutation, virus typing, and evolutionary traceability analysis were carried out.Results:The results of viral genome mNGS showed that 76 SNP mutation sites were detected in 12 samples compared with the reference strain NC_045512, including 3 (3.95%) transitions and 73 (96.05%) reversals. There were 19 (25.00%) synonymous mutations and 57 (75.00%) non-synonymous mutations. The analysis of nucleotide and amino acid variation sites showed that mutations were found at five sites (T2821C, C6548T, T16464C, G16858A and T251C) in all the clustered cases (cases 2, 4 - 10). In the single cases, sample 1 had mutations at C9245T and A15340T, and sample 3 had mutation at C13T. The virus typing analysis showed that the samples 1 and 3 belonged to the L type of novel coronavirus, while the rest belonged to the S type of novel coronavirus. The results of genomic evolutionary relationship analysis showed that all the samples could be divided into two branches. The branches of sample 1 and 3 belonged to single cases, and the rest belonged to family clustered cases.Conclusion:The genomic characteristics of the clustered cases of novel coronavirus infection in Baotou City are basically consistent with the epidemiological investigation results, and the transmission of the virus is mainly related to close contact and family gathering.
7.Study on the mechanism of Poria cocos polysaccharides on the regulation of gluconeogenesis in liver of type 2 diabetic mellitus model rats

OBJECTIVE To investigate the effect and mechanism of Poria cocos polysaccharides on the regulation of blood glucose in type 2 diabetes mellitus (T2DM)model rats by phosphatidylinositol 3-kinase(PI3K)/protein kinase B (Akt)/forked box transcription factor O 1(FoxO1)pathway. METHODS SD rats were randomly divided into blank control group (no modeling ,no administration),model group (modeling,no administration ),metformin group (modeling,200 mg/kg)and P. cocos polysaccharide low-dose,medium-dose and high-dose groups (modeling,100,200,400 mg/kg),8 in each group. Except for blank control group , other groups were given high fat diet combined with streptozotocin to construct the model of T 2DM rats. At the same time , administration groups were given relevant dose of medicine intragastrically ,and blank control group and model group were given constant volume of water intragastrically ,once a day ,for consecutive 42 days. During the experiment ,general condition and bodyweight of rats were observed every day ;fasting blood glucose (FBG)of rats were collected ,and oral glucose tolerance test were conducted and area under curve (AUC)was calculated the day before last administration. After last medication ,the heart ,liver, kidney organ index were calculated ;the levels of HbA 1c,TC,TG,MDA,SOD,GSH-Px and hepatic glycogen content were detected. HE staining was used to observe the pathological changes of liver and pancreatic tissue ,and the pathological grade score was calculated. Western blot assay was used to detect the protein expressions of p-PI 3K,p-Akt,p-FoxO1, PEPCK and G 6Pase in liver tissues. RESULTS Compared with blank control group ,the rats of model group suffered cc1965@163.com from polydipsia ,polyphagia and polyuria ;the body weight , the levels of SOD and GSH-Px ,the protein expressions of p-PI 3K,p-Akt and p-FoxO 1 were significantly decreased (P<0.05);liver and kidney organ index ,blood glucose level at 0,0.5 and 2 hours after intragastric administration of glucose solution ,AUC, FBG,HbA1c,serum levels of MDA ,TC,TG and hepatic glycogen content ,liver and pancreatic pathological grade score ,the protein expressions of PEPCK and G 6Pase were all increased significantly (P<0.05). Compared with model group ,the general condition of rats in P. cocos polysaccharide groups were all improved ,and all of above indicators had been reversed to varying degrees. CONCLUSIONS P. cocos polysaccharide can downregulate protein expressions of PEPCK and G 6Pase which are key enzymes of gluconeogenesis ,inhibit hepatic gluconeogenesis ,effectively decrease blood glucose levels and regulate glucolipid metabolism in T 2DM model rats by weakening oxidative stress and upregulating PI 3K/Akt/FoxO1 pathway.
8.Development of symptom assessment questionnaire for embolization syndrome after transcatheter hepatic arterial chemoembolization in patients with liver cancer and the test of its reliability and validity

Objective:To establish a questionnaire to assess symptom of embolization syndrome after transcatheter hepatic arterial chemoembolization (TACE) in patients with liver cancer, so as to provide a tool of assessing and managing symptom management after TACE.Methods:From March 2020 to June 2021, through literature review, qualitative interview and Delphi expert consultation, the first draft of symptom assessment questionnaire for TACE post-operative embolism syndrome was prepared. The reliability and validity of the questionnaire were tested in 200 patients with liver cancer treated by TACE in department of Liver Oncology, Zhongshan Hospital affiliated to Fudan University.Results:According to the feedback from Delphi expert consultation a draft questionnaire with 9 items of physiological symptoms and 6 items of psychological and social symptoms was formed. Item analysis showed that each item in the questionnaire had a good degree of differentiation. There was significant correlation between each item and the total score of the questionnaire. Three factors were extracted by exploratory factor analysis, naming psychosocial symptom group as factor 1, somatic discomfort symptom group as factor 2 and gastrointestinal reaction symptom group as factor 3, and the cumulative variance contribution rate was 62.592%. Spearman correlation coefficient between this questionnaire and the Anderson Symptom Assessment Scale was 0.855( P<0.05). The Cronbach α of the total questionnaire was 0.898, and. The Cronbach α of the three factors were 0.885, 0.771 and 0.870 respectively. Conclusions:The symptom assessment questionnaire of embolization syndrome after TACE in liver cancer patients prepared in this study has good reliability and validity, which can provide an evaluation basis for the symptom management of TACE.
9.Qualitative screening of 52 illegally adulterated weight-losing compounds in foods by ultra high-performance liquid chromatography-quadrupole time-of-flight mass spectrometry

ObjectiveAn analytical method was developed for the qualitative screening of 52 illegally adulterated weight-losing compounds in foods by ultra high-performance liquid chromatography-quadrupole time-of-flight mass spectrometry. MethodsThe samples were extracted by methanol. Waters CORTECS T3 column (100 mm×2.1 mm, 2.7 μm) was used with acetonitrile -0.1% formic acid aqueous solution as mobile phases by gradient elution. The compounds were detected by electrospray ionization mass spectrometry and Q-TOF-MS detector in positive and negative ion mode. A standard spectrum library was established by reference standards, and the qualitative analysis was finished by the comparison of the retention time, parent ion and fragment ion accurate molecular mass of each compound in the sample and the library. ResultsThe method was specific without interference of blank matrix, and repeatable in sextuplicate. The detection limits of 52 compounds in 5 matrix were 1‒100 mg‧kg-1. The method was successfully applied to the analysis of actual samples,and 16 compounds were checked out in 246 samples. ConclusionThe method is accurate, specific and sensitive, which can be used to combat the illegal adulteration behavior effectively.
10.Evaluation of high-throughput methods for the detection of neutralizing antibodies in serum samples from individuals infected with 2019-nCoV based on a microneutralization test

Objective:To evaluate the detection ability of two kinds of high-throughput method to determine neutralizing antibodies based on a microneutralization test (MNT).Methods:Serum samples were collected from the early phase and follow-up period (117 samples in total) for neutralizing antibody testing. They were tested using MNT, pseudovirus neutralization assay (PBNA), competitive inhibition assay (including enzyme-linked immunosorbent assay (ELISA) and chemiluminescence immunoassay (CLIA)) to evaluate the correlation coefficients and threshold values for the effectiveness of these high-throughput neutralizing antibody assays.Results:The correlation coefficients for PBNA, ELISA, and CLIA relative to MNT were 0.760, 0.778, and 0.725, respectively, for individuals infected with 2019-nCoV. The area under the ROC curve was 0.901 for a cutoff value of 50 for the PBNA assay, 0.934 for a cutoff value of 1∶8 for the ELISA assay and 0.838 for a cutoff value of 1.28AU/ml for the CLIA assay when the threshold value for the microneutralization test was taken as 1: 10 (less than 1: 10 is considered negative).Conclusions:The high-throughput method for the detection of COVID-19 neutralizing antibodies are scientific and feasible, and provide an important technical tool for the regular prevention and control of the epidemic.
