Objective:To evaluate the clinical outcomes of combined complete preservation of chordal structure mitral valve replacement (C-MVR) with total anatomical arterial myocardial revascularization (TACR) in coronary patients with moderate-to-severe or severe ischemic mitral regurgitation (IMR).Methods:This is a retrospective multi-center case series study. Data were retrospectively collected from 127 patients with coronary artery disease with moderate to severe or severe IMR who received TACR with C-MVR from July 2015 to April 2024 in 13 hospitals in China. There were 90 males and 37 females, aged (56.5±10.7) years (range: 33 to 74 years). Perioperative data and follow-up data including left ventricular ejection fraction, left ventricular end-diastolic diameter, and patency rate of arterial grafts of patients were collected. Comparisons were made using paired sample t-test or χ2 test. Results:In this cohort of 127 patients, 67 underwent concurrent tricuspid valve repair. During surgery, 113 grafts of the left internal mammary artery (LIMA), 127 grafts of the left radial artery, 80 grafts of the right radial artery, and 110 grafts of the right internal mammary artery (RIMA) were harvested. The number of the distal anastomosis was 4.2±0.4 (range: 3 to 5). The aortic cross-clamp time and cardiopulmonary bypass time were (97.5±23.4) minutes (range: 90 to 161 minutes) and (145.4±19.2) minutes (range: 101 to 210 minutes), respectively. There was one operative death. Intraoperative placement of an intra-aortic balloon pump was performed in 21 patients to improve the left ventricular ejection. No sternal ischemic occurred. All patients completed follow-up, with a mean follow-up period of (64.3±7.5) months (range: 4 to 110 months). No major cerebrovascular events occurred during the follow-up period, and all patients survived. Left ventricular ejection fraction improved postoperatively (55.0%±5.3% vs. 41.0%±15.3%, t=17.23, P<0.01). The proportion of patients with New York Heart Association functional class ≤2 increased postoperatively (23.6% (30/127) vs. 87.3% (110/126), χ2=103.77, P<0.01). The proportion of patients with Canadian Cardiovascular Society Angina Classification ≤3 decreased postoperatively (4.8% (6/126) vs. 78.7% (100/127), χ2=142.19, P<0.01). The left ventricular end-diastolic diameter decreased postoperatively ((5.70±4.50) cm vs. (6.10±0.23) cm, t=12.15, P<0.01). Coronary multi-detector computed tomography angiography (MDCTA) follow-up was conducted for (60.5±11.7) months (range: 6 to 109 months) postoperatively. MDCTA confirmed the patency rates of the grafts: 96.4% (108/112) for the LIMA grafts, 88.9% (112/126) for the left radial artery grafts, 93.7% (74/79) for the right radial artery grafts, and 90.9% (100/110) for the free RIMA grafts. No significant differences in graft patency rates were observed between the arterial grafts ( χ2=5.24, P=0.155). Conclusion:The results of this multi-centre study demonstrate satisfactory mid-term results of C-MVR with TACR for the treatment of coronary artery disease with moderate to severe or severe IMR.

BACKGROUND:Superovulation is a common therapy in assisted reproductive technology. In clinical practice, some patients experience repeated superovulation to get pregnant. OBJECTIVE:To explore the effect of repeated superovulation on the developmental potential of oocytes in mice and humans. METHODS:Both animal experiments and retrospective clinical research were conducted. The animal study involved 90 SPF grade ICR 8-week-old female mice, who were randomly divided into three groups for 1,3,and 5 superovulations, respectively. The clinical study involved 306 patients who had undergone three consecutive in vitro fertilization cycles. The number of ovules obtained and embryonic development in different cycles were compared. RESULTS AND CONCLUSION:(1)The animal study indicated that repeated superovulation did not affect the embryonic development or developmental speed of mouse embryos.Similarly, there was no significant difference in the mouse blastocyst apoptosis,DNA damage,or the formation of inner cell mass and trophectoderm(P>0.05).(2)The clinical study also revealed no significant differences in the number of retrieved oocytes(8.60±5.04,8.58±4.87,and 8.38±4.63,P=0.81)and transferable embryos(2.42±1.99,2.40±1.92,and 2.64±2.00,P=0.26)over the three cycles.(3)In both the young group(<35 years)and the old group(≥35 years),the embryo quality was not affected by repeated superovulation(P>0.05).(4)These findings show that repeated superovulation does not affect the developmental potential of oocytes in mice and humans.

Objective:To explore the characteristics of early postural control disorders in elderly patients with Parkinson's disease(PD). Method:The sensory organization test(SOT),limits of stability(LOS),and adaptation test(ADT)were performed on 15 patients with early-stage PD and 10 healthy elderly patients using a dynamic and static balance training system. Result:Compared with the healthy group,in the SOT,early PD patients showed no significant differences in the comprehensive score of sensory organization, proprioceptive, visual, and visual dependence(P>0.05).How-ever,there was a significant difference in vestibular balance control(P<0.05).In the LOS,early PD patients exhibited decreased forward and backward max excursions(MXE)(P<0.05),increased reaction time(RT)(P<0.05),and decreased backward directional contro1(DCL)(P<0.05).There were no significant differences in for-ward DCL,left and right MXE,RT,DCL(P>0.05).In the ADT,there were no significant differences in the incidence of the first fall and the incidence of the last four falls in PD patients and healthy groups in the for-ward and backward tilt tests(P>0.05). Conclusion:Early PD elderly patients already have postural control disorders, particularly with decreased limits of stability in forward and backward directions and reduced ability to maintain balance using vestibular input.

Objective:To explore the effect of endometriosis (EM) on reproductive outcomes of young patient with EM after laparoscopic treatment in the first in vitro fertilization-embryo transfer (IVF-ET) cycle. Methods:The clinical data and reproductive outcomes of 394 infertile patients with EM after laparoscopic treatment (EM group) and 3 242 infertile patients caused by gamete transport disorder (control group) in the first IVF-ET cycle were collected in Chongqing Health Center for Women and Children from January 2016 to June 2021. The information included baseline characteristics, oocyte retrieval, embryo development, clinical pregnancy, miscarriage, and live birth. Propensity score matching (PSM) method was used to perform 1∶2 matching between EM group and control group. The impact of EM on reproductive outcomes was analyzed in the retrospective observational study. Results:In the initial data, compared with control group, the number of two pronucleus (2PN) zygotes (9.7±4.8 vs 9.0±4.4), the number of transferable embryos (6.2±3.6 vs 5.5±3.4) and the rate of transferable embryos (64.0% vs 60.8%) on the third day were significantly lower in EM group, and the differences were statistically significant (all P<0.05). After PSM was performed, there were 394 and 787 cases in EM group and control group, respectively. Compared with control group, the number of 2PN zygotes (9.7±4.9 vs 9.0±4.4), the 2PN fertility rate (77.1% vs 75.3%), the number of transferable embryos on the third day (6.2±3.6 vs 5.5±3.4), the transferable embryos rate on the third day (63.8% vs 60.8%) were significantly lower in EM group, and the differences were statically significant (all P<0.05). The study did not find the effect of EM on embryo implantation rate, pregnancy rate, early miscarriage rate, live birth rate and preterm birth rate (all P>0.05). Conclusions:EM might interfere with the development of oocytes and embryos. Obtaining top-quality embryos may be an effective way to improve the prognosis of patients with EM after laparoscopic treatment.

Objective:To compare the clinical efficacy between radiofrequency ablation(RFA)and pulmonary metastasectomy in the colorectal cancer(CRC)patients with lung metastases after radical resection. Methods:The clinical data of 80 CRC patients with lung metastases after radical resection were analyzed retrospectively, and were divided into the surgery group(33 cases)and the RFA group(47 cases)according to the local treatment. The overall survival(OS)and progression-free survival(PFS)of the two groups were compared,as well as the prognostic factors of patients were analyzed. Results:The 3-year PFS and OS rates were 42.4%vs 31.9%and 75.8%vs 72.3%in the surgery group and the RFA group, respectively. There was no significant difference in PFS and OS between the two groups(P>0.05).In multivariate analysis, maximum lung metastasis diameter, preoperative serum carcinoembryonic antigen(CEA)level and history of extrapulmonary metastasis were independent factors influencing OS in the CRC patients with lung metastases after radical resection(P<0.05).In addition, preoperative serum carcinoembryonic antigen(CEA)level and history of extrapulmonary metastasis were also independent factors influencing PFS in the CRC patients with lung metastases after radical resection(P<0.05). Conclusion:The short-term efficacy of RFA is comparable to that of pulmonary metastasectomy in the CRC patients with lung metastases after radical resection, and long-term follow-up studies are needed.

ObjectiveTo investigate the characteristics of surface electromyography (sEMG) of muscles in stroke patients with lower limb spasticity. MethodsFrom October, 2016 to July, 2020, a total of 32 stroke patients with hemiplegia and eleven healthy subjects (control group) were selected. The patients were divided into modified Ashworth Scale (MAS) 0 group (n = 13), MAS 1 group (n = 10) and MAS 1+ group (n = 9). All the groups were subjected to isokinetic passive movement at baseline, 90°/s, 150°/s, 210°/s and 270°/s, respectively. sEMG signals of rectus femoris muscle and lateral head of quadriceps femoris muscle including root mean square (RMS) and integrated electromyography (iEMG) were collected synchronically during isokinetic passive movement. ResultsThere was no significant difference in RMS and iEMG among baseline and the four angular velocities in the control group and MAS 0 group (P > 0.05). The RMS and iEMG of MAS 1 group at 270°/s were significantly different from baseline and the other angular velocities (P < 0.05), and no significant difference was found among the other velocities (P > 0.05). The RMS and iEMG of MAS 1+ group were significantly different among baseline and the four angular velocities (P < 0.05). ConclusionRMS and iEMG varies with angular velocity in hemiplegic patients with different muscle tension levels. In patients with the same muscle tension level, the changes of RMS and iEMG with angular velocity are consistent. RMS and iEMG can reflect the degree of spasm. The quantitative evaluation of spasm by sEMG in isokinetic passive movement mode is highly feasible and worthy of further clinical research and promotion.

ObjectiveTo study the effect on quality of life and the bone turnover markers of Buzhong Yiqitang in the treatment of senile osteoporotic vertebral compression fracture (OVCF, syndrome of Qi deficiency in spleen and stomach) after operation based on ''spleen governing muscle''. MethodA total of 135 senile patients with OVCF treated by percutaneous kyphoplasty in Rizhao Hospital of Traditional Chinese Medicine from January 2020 to January 2021 were enrolled in this study. They were randomly assigned to two groups on the basis of block randomization at a ratio of 2∶1 (90 cases in the observation group and 45 cases in the control group). Both groups were administrated with calcitriol capsules (0.5 μg·d-1) and caltrate D (1 200 mg·d-1) for basic treatment of osteoporosis. The observation group was additionally treated with Buzhong Yiqitang. Bone mineral density (BMD), procollagen type Ⅰ N-terminal propeptide (PINP), osteocalcin (OST), β cross-linked C-telopeptide of type 1 collagen (β-CTx), appendicular skeletal muscle mass index (ASMI), and quadriceps muscle strength were compared between the two groups before and 6, 12 months after treatment. Additionally, traditional Chinese medicine (TCM) symptom score and visual analogue score (VAS) before and 3, 6 months after treatment, as well as quality of life questionnaire of the European Foundation for osteoporosis (QUALEFFO) score before and 3, 6, 12 months after treatment, were compared between the two groups. ResultA total of 85 patients in the observation group and 41 patients in the control group were followed up. The general curative effect of the observation group was better than that of the control group (χ2=10.503, P<0.05). Specifically, the observation group had higher PINP, BMD, ASMI, and quadriceps muscle strength but lower β-CTx, TCM symptom score, VAS, and QUALEFFO score than the control group (P<0.05, P<0.01). No adverse reactions related to Buzhong Yiqitang were observed. ConclusionBuzhong Yiqitang can regulate bone metabolism indexes, promote osteogenesis, increase bone density, enhance skeleton appendiculare and quadriceps muscle strength, relieve clinical symptoms, and improve quality of life in patients with senile OVCF (syndrome of Qi deficiency in spleen and stomach), being worthy of promotion in clinical application.

Objective:To explore one-stage total knee arthroplasty (TKA) combined with open reduction and internal fixation (ORIF) for knee osteoarthritis complicated with tibial stress fracture. Methods:The 3 patients were retrospectively analyzed who had been treated for knee osteoarthritis complicated with tibial stress fracture at Department of Orthopedics, Ganzhou District People's Hospital from March 2018 to March 2020. They were all female, aged from 54 to 76 years (average, 66 years). There were 2 transverse fractures and one short oblique fracture; all of them had knee varus deformity. The Hospital for Special Surgery (HSS) scores averaged 37.6 (from 28 to 50) for the left knee and 28.3 (from 22 to 39) for the right knee. One-stage TKA was performed for the articular surface while ORIF for the right tibial stress fracture for all patients. Recorded were fracture union time, HSS knee score and range of articular motion. Results:The 3 patients were followed up for 25 to 44 months (average, 32 months). The fracture union time ranged from 4 to 7 months (average, 5 months). The last follow-ups revealed no such complications as prosthesis loosening, peri-prosthesis osteolysis or joint instability. Knee varus deformity was corrected in all patients. The HSS knee scores at the last follow-up averaged 89.6 (from 88 to 91) for the left knee and 88.3 (from 85 to 90) for the right knee. Conclusion:In the treatment of knee osteoarthritis complicated with tibial stress fracture, one-stage TKA combined with ORIF can restore the function of knee joint, leading to fine curative effects.

Background: High-dose dual therapy (HDDT) is a novel regimen for the eradication of Helicobacter pylori (Hp) infection, however, its efficacy and safety remain unclear. Aims: To evaluate the efficacy, safety and compliance of HDDT for Hp eradication. Methods: Randomized controlled trials (RCTs) on HDDT for eradication of Hp infection were retrieved from PubMed, Embase, The Cochrane Library, Web of Science from the date of database establishment to October 2020. Literatures were enrolled according to the inclusion and exclusion criteria, and the data were extracted. RevMan 5.2 software was used for performing meta-analysis. Results: Nine RCTs including 2 627 patients were included. Meta-analysis results showed that no significant differences in ITT eradication (85.4% vs. 79.8%, RR=1.03, 95% CI: 0.96-1.10, P=0.40), PP eradication (88.7% vs. 83.4%, RR=1.01, 95% CI: 0.95-1.08, P=0.68), and compliance (96.5% vs. 95.9%, RR=1.01, 95% CI: 0.99-1.02, P=0.37) were found between HDDT and the guideline-recommended regimens, however, the incidence of adverse events was significantly decreased in HDDT (15.3% vs. 27.0%, RR=0.57, 95% CI: 0.42-0.76, P=0.000 2). Conclusions: There are no significant differences in eradication rates and compliance between HDDT and the guideline-recommended regimens, however, HDDT is much safer.
