1.Effect and Mechanism of Shuangshenling Granules on Chronic Renal Failure Rat Model

ObjectiveTo investigate the pharmacological action and mechanism of Shuangshenling granules in treating chronic renal failure in rats,providing laboratory data to support clinical application of Shuangshenling granules. MethodSD rats (150-180 g),half males and half females in number,were used,with ten rats designated as the normal group,ten as the sham operation group,and the remaining rats undergoing chronic renal failure modeling induced by 5/6 nephrectomy. Two weeks after operation,serum creatinine (SCr) and blood urea nitrogen (BUN) levels were measured via orbital blood sampling to select successful model rats. Based on SCr values,the rats were evenly divided into the model group,Shenshuaining positive group (0.84 g·kg-1·d-1),and high,medium,and low dose groups of Shuangshenling granules (4.8,2.4,1.2 g·kg-1·d-1),with ten animals in each group. Each treatment group received drugs at 10 mL·kg-1 via intragastric administration once daily for six weeks. At 2,4,6 weeks after administration,SCr,BUN,24-hour urine volume,total urinary protein (UTP),urinary creatinine (UCr),creatinine clearance rate (CCr),serum albumin (SAlb),and total serum protein (STP) were measured. Following the experiment,kidney tissues were dissected for pathological examination. The expression levels of autophagy-related proteins,including PTEN-induced kinase 1 (PINK1),E3 ubiquitin-protein ligase parkin (Parkin),and microtubule-associated protein 1 light chain 3B (LC3B),were detected by immunofluorescence. ResultCompared with the normal group,the model group exhibited significantly increased levels of SCr,BUN,24-hour urine volume,UTP,and UCr (P<0.01),and decreased levels of SAlb and STP (P<0.01). CCr showed an initial increase followed by a decrease. Histopathological results revealed glomerular hyperplasia and atrophy,with varying degrees of mesangial cell reduction,blood stasis in the glomeruli,and significant widening of Bowman's capsule. Visceral parietal layer cells were displaced or absent,leading to incomplete and damaged glomeruli. A large number of protein casts were present in the proximal and distal convoluted tubules,with reduced and displaced cells,swelling in some tubules,and interstitial inflammatory exudation predominantly comprising lymphocytes and a small number of neutrophils. Compared with the model group,all dose groups of Shuangshenling granules significantly reduced levels of SCr,BUN,24-hour urine volume,UTP,and UCr (P<0.05,P<0.01) and increased SAlb and STP levels (P<0.01) at 2,4,and 6 weeks after administration. The three dose groups also improved CCr and alleviated renal pathological injury in varying degrees at 2-6 weeks after administration. Immunofluorescence results showed that the expression levels of PINK1,Parkin,and LC3B were significantly reduced in the model group compared with the normal group,whereas all dose groups of Shuangshenling granules significantly upregulated the expression levels of PINK1,Parkin,and LC3B compared with the model group. ConclusionShuangshenling granules significantly improved renal function and pathological injury in rats with chronic renal failure,likely through the upregulation of PINK1-mediated autophagy.
2.Efficacy and Mechanism of Lutongning Granules in Treatment of Trigeminal Neuralgia Induced by Injection of Talc into Infraorbital Foramen of Model Rats Based on P2X7R-mediated Neuroinflammation

ObjectiveTo evaluate the effectiveness of Lutongning granules in the treatment of trigeminal neuralgia in animal models and study its mechanism of action, so as to provide laboratory data support for the clinical application of Lutongning granules and precise treatment. MethodMale SD rats were randomly divided into normal group, model group, carbamazepine group (0.06 g·kg-1·d-1), high-dose Lutongning group (2.70 g·kg-1·d-1), and low-dose Lutongning group (1.35 g·kg-1·d-1) according to the stratified basic mechanical pain thresholds, with 10 rats in each group. A trigeminal neuralgia model of rats was prepared by injecting 30% talc suspension into the infraorbital foramen area of the rat. The drug groups were administered 10 mL·kg-1 of drugs by gavage after 2 h of modeling. The normal group and the model group were administered distilled water by gavage under the same conditions once a day for 10 consecutive days. Von Frey brushes were used to determine the mechanical pain threshold of rats. A fully automated blood and body fluid analyzer was employed to detect the blood routine of rats. Hematoxylin and eosin (HE) staining was utilized to detect the pathological changes in the trigeminal ganglion and medulla oblongata tissue. Transmission electron microscopy was used to scan the ultrastructure of the medulla oblongata tissue. Enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of inflammatory factors interleukin (IL)-1, IL-6, IL-8, tumor necrosis factor (TNF)-α, neuropeptide substance P, and β-endorphins (β-EP) in the serum of rats, and Western blot was used to detect the protein expression levels of IL-1β, purinergic receptor P2X7 (P2X7R), and phosphorylated p38 mitogen-activated protein kinase (p-p38 MAPK). ResultCompared with that in the normal group, the pain threshold of rats in the model group was significantly lower (P<0.01). The absolute value of neutrophils (NEUT#) and the percentage of neutrophils (NEUT) were significantly improved, and the percentage of lymphocytes (LYMPH) was significantly reduced (P<0.01). The serum levels of IL-1, IL-6, IL-8, and TNF-α were significantly increased (P<0.01). SP content in brain tissue was significantly increased, and β-EP content was significantly decreased (P<0.01). The relative protein expression of IL-1β, P2X7R, and p-p38 MAPK was significantly increased (P<0.05). HE staining and transmission electron microscopy results of medulla oblongata tissue revealed neuronal degeneration, mild proliferation of microglial cells, reduction in the number of myelinated nerves, and obvious demyelination. The trigeminal nerve fibers of rats were disarranged, and some nerve fibers showed vacuolization. Axons were swollen, and Schwann cells proliferated. Demyelination was observed. Compared with the model group, each administration group significantly increased the pain threshold of rats (P<0.05, P<0.01), reduced NEUT# and NEUT, and elevated LYMPH (P<0.05, P<0.01). The administration group significantly decreased the levels of IL-1, IL-6, IL-8, and TNF-α in serum and SP in brain tissue (P<0.01) and increased the level of β-EP (P<0.01). They significantly down-regulated the protein expression of IL-1β, P2X7R, and p-p38 MAPK(P<0.05, P<0.01) and significantly ameliorated the pathological changes in medulla oblongata tissue and trigeminal nerves of rats. ConclusionLutongning Granules had significant therapeutic effects on trigeminal neuralgia induced by injection of talc into the infraorbital foramen of model rats, and the mechanism may be related to amelioration of P2X7R-mediated neuroinflammation and inhibition of demyelination of myelinated nerves.
3.The effect of hemodialysis and conventional treatment on the heart function of elderly patients with chronic renal-cardiac syndrome

Objective:To explore the effect of hemodialysis and conventional treatment on the heart function of elderly patients with chronic renal heart syndrome(CRCS).Methods:Sixty elderly patients with CRCS who were admitted to Qinhuangdao Port Hospital from July 2017 to July 2018 were selected and divided into the observation group (received hemodialysis and conventional treatment) and the control group (received conventional treatment) according to the random number table method, each group with 30 patients. The levels of left ventricular end-systolic dimension (LVESD) and left ventricular end-diastolic dimension (LVEDD), left ventricular ejection fraction (LVEF) and left atrium anterior and posterior diameter (LAD) before and after treatment of two weeks were compared between two groups, the levels of N-terminal pro-B-type natriuretic peptide (NT-proBNP), blood urea nitrogen (BUN), cystatin C (CysC), creatinine (Cr) and tumor necrosis factor (TNF)-α, high sensitivity C-reactive protein (hs-CPR), interleukin 6 (IL-6) before and after treatment for two week were compared between two groups, and the safety of the two treatment methods was evaluated.Results:After treatment, the levels of LVESD, LVEDD, LAD in the observation group were lower than those in the control group: (26.13 ± 1.28) mm vs. (29.09 ± 1.26) mm, (47.58 ± 1.22) mm vs. (51.34 ± 1.49) mm, (30.02 ± 0.83) mm vs. (33.15 ± 1.02) mm, the level of LVEF in the observation group was higher than that in the control group: (39.63 ± 5.21)% vs. (35.77 ± 4.93)%, the differences were statistically significant ( P<0.05). After treatment, the levels of NT-proBNP, BUN, CysC and Cr in the observation group were lower than those in the control group: (1 089.31 ± 243.29) ng/L vs. (2 990.56 ± 531.22) ng/L, (16.08 ± 3.35) mmol/L vs.(21.93 ± 4.21) mmol/L, (0.35 ± 0.11) mg/L vs. (0.57 ± 0.18 mg/L, (251.62 ± 29.28) μmol/L vs. (290.66 ± 38.12) μmol/L, the differences were statistically significant ( P<0.05). After treatment, the levels of TNF-α, hs-CPR and IL-6 in the observation group were lower than those in the control group: (149.26 ± 19.53) μg/L vs. (182.33 ± 20.25) μg/L, (4.02 ± 1.33) mg/L vs. (5.38 ± 1.57) mg/L, (18.91 ± 3.11) μg/L vs. (22.45 ± 3.82) μg/L, the differences were statistically significant ( P<0.05). The incidence of adverse reactions during the treatment in the two groups had no significant difference ( P>0.05). Conclusions:The use of hemodialysis on the basis of conventional treatment can improve the heart and kidney function of elderly patients with CRCS, and this method is beneficial to alleviate left heart dilation and inflammation. It is a safe and effective treatment method.
4.Preliminary clinical study on treatment of prostate cancer with Cyber-Knife

Objective To evaluate the safety and effectiveness of ultra-hypofractionated stereotactic body radiotherapy in the treatment of prostate cancer.Methods A total of 26 patients with prostate cancer treated with Cyber-Knife from May 2010 to May 2018 were analyzed retrospectively.The median age of the patients was 69 years old (range,57 to 87).Ultra-hypofractionated radiotherapy was delivered in five fractions of 7.0-7.5 Gy for a total dose of 35.0-37.5 Gy.Androgen deprivation therapy (ADT) was administered in combination with the Cyber-Knife.The primary endpoints were radiation toxicity,PSA-response,local control and symptom alleviation,while the secondary endpoints were progression-free survival and overall survival.Results No graded ≥ 3 acute and late radiation toxicities occurred during follow-up.The acute toxicity of Grades 1 and 2 was 38.4% and 19.2%,while the late toxicity of Grades 1 and 2 was 30.8% and 3.8%,respectively.At a median follow-up of 22.44 months,for patients with localized stage,PSA level was decreased significantly after radiotherapy (Z =2.900,2.794,2.510,2.090,P<0.05).However,there was no statistically significant difference for the metastatic group (P> 0.05).Conclusions Ultra-hypofractionated stereotactic body radiotherapy is a safe and effective treatment for patients with prostate cancer.

Objective The aim of this study was to investigate the effect of flow cytometry on peripheral blood T cell subsets in patients with malignant lymphoma and its relationship with clinicopathological and tumor types.Methods Ninety-eight patients with malignant lymphoma treated in our hospital from August 2014 to September 2016 were selected as the study group.Ninety-eight healthy subjects were selected as the control group.The peripheral blood T cell subsets(CD3 +,CD4 +,CD8 +,CD4 +/CD8 +)were detected in patients and healthy controls by flow cytometry.Results The levels of CD3 +and CD4 +/CD8 +in the study group were (55.63±11.25)and(1.32±0.62),respectively,which were significantly lower than those(68.96±12.63)and (1.59±0.59)of the control group(P<0.05).The levels of CD4 +and CD8 +were(33.67±8.14)and(26.02±4.67),respectively in the study group,were no difference from the control group(34.12±8.33)and(25.67±4.53)(P>0.05).The levels of CD3+and CD4 +/CD8 +in patients with Hodgkin's lymphoma were(54.63±11.36),(1.22±0.65),respectively,and(55.52±12.02),(1.34±0.71)for non-hodgkin lymphoma.They were significantly decreoseg in the control group(68.96±12.63 for CD3 +and 1.59±0.59 for CD4 +/CD8 +) (P<0.05).The level of CD4 +and CD8 +were no difference amoupst Hodgkin's lymphoma(33.78±8.23 for CD4 +and 25.74±4.88 for CD8 +),non-Hodgkin's lymphoma(25.74±4.88 for CD4 +and 33.62±8.74 for CD8 +)and control group(34.12±8.33 for CD4 +and 25.67±4.53 for CD8 +)(P>0.05).The levels of CD3 +,CD4+and CD4 +/CD8 +in patients with Ⅲ ~Ⅳ stage malignant lymphoma were(52.66±12.47), (28.25±6.32)and(1.30±0.62),respectively,which were significantly lower than those(68.96±12.63), (34.12±8.33)and(1.59±0.59)in the control group(P<0.05).The level of CD3 +in patients with phase Ⅰ-Ⅱ malignant lymphoma(58.63±11.85)was significantly lower than that in the control group(68.96±12.63)(P<0.05).The level of CD8 +in patients with phase Ⅰ-Ⅱ malignant lymphoma(29.63±3.57)was significantly higher than that in the control group(25.67±4.53)(P<0.05).Conclusion The detection of peripheral blood T cell subsets by flow cytometry can be used as an important methods to diagnose the disease,staging and immune status of patients with malignant lymphoma,which has high application value.
7.Effect of comprehensive intervention for patients with hypertension in Kailuan community

Objective To investigate the effect of comprehensive intervention including education, lifestyle intervention and drug intervention for patients with hypertension in Kailuan community. Methods Employees who took part in the health checkup of Kailuan Group, Ltd. from June 2008 to June 2009 were enrolled in this study. The changes of hypertension awareness, treatment and control rates and cardiovascular health behaviors and factors have been observed in the follow-up. Results A total of 5 361 patients with hypertension were enrolled. After a period of 2 years of follow-up, the treatment rate and up to standard rate hypertension were significantly increased compared to pre-intervention (11.90%vs. 100.00%;0.82% vs. 36.93%. P<0.01). After comprehensive intervention, the average score of ideal cardiovascular health behaviors and factors increased from (6.85 ± 1.90) to (7.52 ± 1.92) (P<0.01). Conclusion Comprehensive intervention is an effective means for the prevention and control of hypertension in the community.
8.Risk Factors Analysis for Prevalence of Acute Myocardial Infarction in Young and Middle-aged Population

Objective: To study the risk factors for prevalence of acute myocardial infarction (AMI) in young and middle-aged population. Methods: A prospective cohort study was conducted in 110100 subjects at the age of (18-98) years who received physical examination in Kailuan Group from 2012-06 to 2014-10. Based on the limitations of male≤53 years and female≤63 years, a total of 62367 subjects were enrolled in our study. The subjects were followed-up for 2 years by the end point event of AMI to analyze the risk factors ofAMI occurrence. Results: According to AMI occurrence at the follow-up period, the subjects were divided into 2 groups: AMI group, n=56 and Control group, n=62152. Compared with Control group, AMI group had increased BMI, SBP, DBP and elevated blood levels of LDL-C, TG; AMI group also showed the higher ratios of subjects with the history of diabetes and taking anti-hypertension medication. Cox proportional hazard regression analysis indicated that age (RR=1.37), male (RR=60.54), LDL-C (RR=1.12), and TG (RR=5.93) were the risk factors forAMI occurrence in young and middle-aged population, allP<0.05. Conclusion: Age, male gender, blood levels of LDL-C, and TG were the risk factors for AMI occurrence in young and middle-aged population.
9.Inflammatory Factors in Hypertensive Patients Complicated with Acute Myocardial Infarction

Objective To investigate the alteration and significance of inflammatory factors in hypertension patients complicated with acute myocardial infarction(AMI).Methods Eighty seven AMI patients were grouped as normotensive group(n=43) or hypertensive group(n=44),25 healthy subjects served as control.Plasma level of IL-1?,IL-6,IL-8,TNF-? and IFN-? were determined at admission of hospitalization,24 h,48 h,5 d,7 d and 14 d after myocardial infarction using ELISA method.Results 1)Compared with the control group,the level of all inflammatory markers were elevated significantly in 2 weeks after myocardial infarction and showed a pattern of dynamic changes.2)Compared with normotensive group,IL-6 in hypertensive group was increased more significantly on 5th day(P=0.019) to 7th day(P=0.005)after AMI.Conclusion Inflammation was involved in the course of AMI since the early stage.Hypertension seems to exaggerated the inflammatory reaction after myocardial infarction.

Objective To study the effects of hypoxia on repair of radiation damage and the possible mechanism. Methods Rat jejunal epithelium cell was taken as study object and divided into control group, irradiation group and irradiation-hypoxia group. The irradiation group was irradiated only once by 60 Co-? ray at the dose of 12 Gy and the irradiation-hypoxia group was treated with hypoxia (5% oxygen) for 6 h after irradiation. MTT was used to detect the survival rate of the irradiated cells, 3H-TdR incorporation was used to detect cell proliferation, and unscheduled DNA synthesis and single cell gel electrophoresis were used to detect DNA repair and flow cytometry was used to detect cell cycle.
