1.Prevalence and risk factors of hyperuricemia among workers in an electronics factory in Wuhan

Objective To understand the prevalence and risk factors of hyperuricemia in electronics factory workers in Wuhan, and to provide evidence for the health protection of electronics factory workers. Methods A total of 1 415 employees in an electronics factory in Wuhan were selected as the research subjects, and the physical examination and determination of various biochemical indicators, as well as questionnaire survey were carried out. Results The detection rate of hyperuricemia among workers in the electronics factory in Wuhan was 32.43%, with 36.33% for men and 14.11% for women, and the difference was statistically significant ( χ2=46.077,P<0.001). The detection rate of hyperuricemia was the highest (33.77%) among those with university or college education, followed by graduate students and above (31.50%). Compared with subjects with good lifestyle habits, people with drinking habits had higher hyperuricemia detection rate (49.38%), and the difference was statistically significant (P =0.001). The detection rates of hyperuricemia in those with central obesity and elevated alanine aminotransferase were 48.23% and 61.29%, respectively, which were significantly higher than those in the subjects without the above diseases (26.91% and 27.21%, respectively), and the differences were statistically significant (P <0.001). Obese people had the highest detection rate of hyperuricemia (66.95%), followed by overweight people (43.75%), and the difference was statistically significant (P <0.001). Multivariate logistic analysis showed that alcohol drinking (OR=1.836, 95% CI=1.139-2.961, P =0.013) and body mass index ≥ 24 kg/m2 (OR=2.175, 95% CI=1.686 -2.806, P <0.001) were risk factors for hyperuricemia in electronic factory workers. Elevated alanine aminotransferase (ALT) was significantly correlated with hyperuricemia (OR=2.964, 95%CI=2.146-4.095 , P <0.001). Female gender was a protective factor for hyperuricemia in workers in the electronics factory (OR=0.441, 95%CI=0.297-0.653 , P <0.001). Conclusion The detection rate of hyperuricemia among workers in an electronics factory in Wuhan is high, and the detection rate of hyperuricemia in men is higher than that in women. Alcohol consumption, overweight and obesity will increase the risk of hyperuricemia. Elevated ALT is associated with hyperuricemia. Maintaining an ideal body mass index and establishing a good lifestyle play an important role in preventing hyperuricemia.
2.Factors affecting nosocomial death in elderly patients with COVID-19 and construction of a risk predictive model

Objective To study the factors affecting hospital death in elderly patients with novel coronavirus infec-tion/disease 2019(COVID-19),and to build a risk prediction model.Methods According to the diagnostic criteria of Diagnosis and Treatment Protocol for COVID-19 Infection(Trial 10th Edition).Totally 775 elderly patients(≥60 years old)diagnosed as COVID-19 infection in the emergency department and fever clinic of the First Hospital of Changsha were selected as the research objects.General data and serum biomarkers of patients were collected.After treatment,the patients'data were divided into survival group and hospital death group.Binary Logistic regres-sion was used to screen the independent influencing factors of death,and ROC curve was used to analyze the pre-dictive value of related indicators on hospital death.Results After treatment,712 patients(91.9%)survived and 63 patients(8.3%)died in hospital.Binary Logistic regression analysis showed that:≥90 years old[OR=5.065,95%CI(1.427,17.974)],type 2 diabetes mellitus[OR= 3.757,95%CI(1.649,8.559)],COPD[OR= 5.625,95%CI(2.357,13.421)],monocyte ratio[OR=0.908,95%CI(0.857,0.963)],plasma fibringen[OR=1.376,95%CI(1.053,1.800)]and lactate dehydrogenase[OR=1.005,95%CI(1.001,o1.008)]were independent factors of in-hospital death(P<0.05).The predictive value of diabetes mellitus+COPD+age+monocyte ratio+plasma fibrinogen+lactate dehydrogenase was proved in hospital death from COVID-19 infected patients:the area under the curve(AUC)was 0.883(95%CI:0.827,0.940,P<0.001),the critical value≥0.710 suggested the risk of death in hospital,the specificity was 0.851,the sensitivity was 0.857.Conclusions The hospital mortality of the elderly after COVID-19 infection is higher and closely related to type 2 diabetes,COPD,monocyte ratio,plasma fibrinogen and lactate dehydrogenase.
3.Research progress of Alzheimer's disease and intestinal microecology

Alzheimer's disease(AD)is one of the most common types of dementia in clinical practice.The increased permeability of the gut and blood-brain barrier caused by intestinal microecological dysbiosis may be an important incentive to affect the incidence of AD.Gut microbiota regulates the central nervous system through the gut microbiota-gut-brain axis(gut-brain axis),and plays an important role in the cognitive func-tion,occurrence and development of clinical symptoms in AD patients.This review comprehensively reviewed the current literature on the relationship between gut microbiota dysregulation and inflammatory cytokine changes in AD,and the treatment of AD targeting with gut microbiota,so as to clarify the new progress in the influence of intestinal microecology changes on the cognitive function of AD patients,the potential diagnosis in the early onset of AD,and the potential mechanism of gut microbiota participating in the regulation of AD.It provides a new idea for the treatment strategy of AD.
4.ZHANG Zhiyuan's Experience in Syndrome Differentiation and Treatment of Leukorrheal Diseases

To summarize Professor ZHANG Zhiyuan's experience in treating leukorrheal diseases based on syndrome differentiation. It is believed that the main pathogenesis of leukorrheal diseases includes mainly depression, dampness and deficiency. Leukorrheal diseases can be classified into four syndrome types: syndrome of stagnation of liver qi and spleen deficiency, syndrome of internal retention of damp-heat, syndrome of cold-dampness invading downward, and syndrome of kidney yang deficiency. The treatment approach focuses on restoring the ascending and descending opening and closing of qi, regulating the liver and spleen to promote yang qi respectively. The four types of syndromes correspond to the application of four different herbal formulas: By using the combination of Danggui Shaoyao Powder (当归芍药散) from the Synopsis of Golden Chamber (《金匮要略》) with wind-dispelling herbs, adjustments are made to harmonize the liver and spleen; By applying modifications to the Baxian Drink (八仙饮) from the Obstetrics Enlightenment (《产科发蒙》) to clear heat and promote diuresis; A self-designed Shiwei Wendai Decoction (十味温带汤) is adjusted to warm the yang and dispel cold; The Baji Tusi Decoction (巴戟菟丝汤) from the New Compilation of Effective Recipes (《验方新编》) is adjusted to tonify and warm the kidney yang.
5.Study on the correlation among nurses′ adversity quotient, self-efficacy and positive coping style and work engagement

Objective:To explore the chain mediating effect of self-efficacy and positive coping styles between nurses′ adversity quotient on work engagement, so as to provide a theoretical reference for nursing managers to promote work engagement.Methods:A convenience sampling method was used to select 512 nurses from four tertiary hospitals in Shaanxi Province, and a cross-sectional study was conducted on them using the general information questionnaire, Adversity Response Profile, General Self-Efficacy Scale, Simplified Coping Style Questionnaire and Utrecht Work Engagement Scale-9.Results:The total scores of nurses' adversity response profile, self-efficacy, positive coping style and work engagement were 132.00(120.00, 139.00), 27.00(23.00, 29.00), 24.00(20.00, 25.00), 27.00(25.00, 35.00) points. Nurses' work engagement was significantly positively correlated with adversity quotient, self-efficacy and positive coping styles( r values were 0.523-0.614, all P<0.01). The independent mediation of self-efficacy or positive coping styles and the chain mediation of both accounted for 23.6%,40.9% and 15.9% of the total effect respectively. Conclusions:Nurses′ adversity quotient not only directly affected work engagemnet, but also indirectly affected work engagement through self-efficacy and positive coping style. The research suggested that managers should pay attention to the improvement of nurses′ self-efficacy and positive coping style, so as to promote nurses′ work engagement.
6.Effect of thymopentin combined with levofloxacin for retreatment smear positive pulmonary tuberculosis

Abstract: Objective To analyze the effect of adjuvant to levofloxacin in the treatment of retreatment smear positive pulmonary tuberculosis, as well as its effect on respiratory function, immune function and inflammatory factors. Methods One hundred cases of retreatment smear positive pulmonary tuberculosis patients admitted to Rudong County People's Hospital in Nantong city in Jiangsu province from 2017 to 2021 were randomly divided into a control group (n=50) and an observation group (n=50) according to random number table method. Both groups received conventional treatment (3 months of isoniazid, rifampicin, ethambutol, pyrazinamide / 6 months of isoniazid, rifampicin, ethambutol), with levofloxacin added to the control group, and thymopentin added to the observation group for the first three months in addition to routine treatment. The treatment effect of the two groups were compared. Results The sputum smear conversion rate of the observation group was significantly higher than that of the control group after 3 months and 5 months of treatment (χ2=7.142, P<0.05; χ2=6.250, P<0.05). The cavity absorption time and lesion absorption time in the observation group were significantly lower than those in the control group (t=4.006, P<0.05; t=5.165, P<0.05). The turning time of bacteriological culture in the observation group was significantly lower than that in the control group (t=4.220,P<0.05). After 3 months of treatment, CD4+, CD3+, CD4+/CD8+ of the observation group were higher than those of the control group, the differences were statistically significant (t=8.885, P<0.05; t=6.274, P<0.05; t=4.357, P<0.05). After 3 months of treatment, the IFN-γ (interferon-γ) of the observation group was higher than that of the control group (t=8.892, P<0.05), whereas the , IL-10 (interleukin-10) was significantly lower than that in the control group (t=5.986, P<0.05). After 3 months of treatment, forced vital capacity (FVC), forced expiratory volume in one second (forced expiratory volume in one second, FEV 1) and the one-second rate (forced expiratory volume in one second / forced vital capacity, FEV1/FVC) in the observation group were significantly higher than those in the control group (t=11.223, P<0.05; t=10.128, P<0.05; t=4.464, P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (χ2=0.378, P>0.05). Conclusions Thymopentin combined with levofloxacin had a significant application effect in the treatment of retreatment smear positive pulmonary tuberculosis, s, which led to improved inflammatory reaction, respiratory function and immune function. Additionally, it can increase sputum smear conversion rate and accelerate patient recovery, improving overall treatment efficacy, with a relatively high clinical application value.
7.A comparative study of semi-quantitative and quantitative models in risk assessment of optical fiber manufacturing enterprises

Objective To explore the results, feasibility and existing problems of semi-quantitative and quantitative risk assessment models in the assessment of chemical hazard exposure in optical fiber manufacturing enterprises. Methods The chemical hazard factors of an optical fiber enterprise in Wuhan were investigated, detected and evaluated, and the semi-quantitative and quantitative risk assessment models of occupational health of chemical hazard factors in the workplace were used for risk assessment. Results In the semi-quantitative risk assessment model, the consistency between the contact index method and the comprehensive index method was good (Kappa=0.820, P<0.01), and the result was grade 2-3. The evaluation results of the contact ratio method were significantly correlated with those of E/OEL (r2=0.539,P<0.05), and the results were grade 1, 2, 3 and 5. The non-carcinogenic risk hazard quotient of the quantitative risk assessment model was not consistent with the three semi-quantitative methods (Kappa=0), and the results were grade 1, 2, 3, 4 and 5. Conclusion The semi-quantitative risk assessment model is more suitable for the risk assessment of the optical fiber industry than the quantitative risk assessment model.
8.Compliance of oral anticoagulant medication and influencing factors among nonvalvular atrial fibrillation patients with new-onset acute ischemic stroke

Objective:To investigate the compliance of oral anticoagulant(OAC) medication and influencing factors among nonvalvular atrial fibrillation(NVAF) patients with new-onset acute ischemic stroke (AIS).Methods:A total of 396 NVAF patients, who initiated OAC therapy after a new-onset AIS from August 2011 to December 2020 were enrolled from China Atrial Fibrillation Registry (China-AF). The demographic characteristics, medical history, comorbid diseases and medication of patients were collected before and after the index stroke, and the influencing factors of compliance of OAC medication were analyzed.Results:Patients were followed up for a mean of 26.9 months. Among 396 patients, 228 (57.6%) had continuous anticoagulant medication (persistent OAC group);while 168 (42.4%) discontinued OAC therapy within 2 years after the index stroke (non-persistent OAC group). Patients on persistence OAC had a higher proportion of atrial fibrillation episodes than patients on non-persistent OAC [83.3% (190/228) vs. 73.8% (126/168); χ 2=5.34, P=0.021], while lower proportion of radiofrequency ablation(RFA)[18.9% (43/228) vs. 32.1% (43/228); χ 2=9.22, P=0.002]. Multivariate Cox regression modelshowed that history of RFA ( HR=1.77, 95% CI: 1.25-2.50; P=0.001) was positively associated with non-persistence of OAC. Conclusion:The study indicates that quite large proportion of NVAD patients with a new-onset of AIS discontinued OAC therapy during 2 years of follow up, and a history of RFA procedure might be an independent factor associated with discontinuing of anticoagulant therapy.
9.Study on the secondary metabolites from a polar marine sponge associated actinomyces Streptomyces sp. LHW11-07

Objective In order to obtain small molecule compounds with novel structure and good biological activity, the secondary metabolites of polar sponge-symbiotic Streptomyces sp. LHW11-07 were studied. Methods The fermentation product of Streptomyces sp. LHW11-07 was isolated and purified by gel column chromatography, silica gel column chromatography, reversed-phase medium pressure column chromatography and high performance liquid chromatography. The structures of the monomeric compound were identified by modern spectroscopic methods such as mass spectrometry, nuclear magnetic resonance and related literature reports. Results A total of nine compounds were isolated from the fermentation of this strain, which were cyclo-(L-Tyr-L-Trp) (1), cyclo-(L-Trp-L-Ser) (2), cyclo-(D-Tyr-D-Pro) (3), cyclo-(L-Tyr-L-Phe) (4), cyclo-(L-Tyr-L-Leu) (5), albaflavenol B (6), β-adenosine (7), N-formylantimyic acid methyl ester (8) and conglobatin A (9). Conclusion Compounds 1 and 2 were isolated from Streptomyces sp. for the first time.
10.Status and influencing factors of positive feelings among caregivers of patients with enterostomy

Objective:To explore the status and influencing factors of positive feelings of caregivers of patients with enterostomy, so as to provide a reference for targeted interventions.Methods:From May to October 2021, convenience sampling was used to select 130 caregivers of patients with enterostomy who received follow-up visits in the Stoma Clinic of the First Affiliated Hospital of Xi'an Jiaotong University as the research subject. The survey was carried out using the General Information Questionnaire, the Positive Aspects of Caregiving (PAC) , and the Care Preparedness Scale (CPS) . A total of 130 questionnaires were distributed, and 127 valid questionnaires were recovered, with a valid recovery rate of 97.69%.Results:Among 127 caregivers of patients with enterostomy, the total score of PAC was (32.69±4.99) , and the self-affirmation dimension score was (18.39±2.84) , and the life outlook dimension score was (14.36±2.53) , and the total score of CPS was (17.76±4.36) . Multiple linear regression analysis showed that caregiver gender, education level, work status, and readiness were the influencing factors of positive feelings of caregivers of patients with enterostomy ( P<0.05) , which explained 43.7% of the total variance. Conclusions:The positive perception of caregivers of patients with enterostomy is at an upper-middle level. Nurses should formulate targeted humanistic care strategies according to the influencing factors, increase the positive feelings of caregivers, help patients adapt to the stoma as soon as possible, and then improve the quality of life of patients.
