1.Exploration of the regulatory mechanism of Danshen decoction on dyslipidemia in hyperlipidemia model rats based on proteomics

		                        			OBJECTIVE To explore the regulatory mechanism of Danshen decoction on dyslipidemia in hyperlipidemia model rats. METHODS The experimental rats were divided into blank group (n=9, no modeling), model group (n=8, modeling), and Danshen decoction group (n=9, modeling). Starting from the 9th week of feeding with the high-fat diet, rats in the Danshen decoction group were given the corresponding medication solution (3.6 g/kg) intragastrically, while blank group and model group were given equal volume of normal saline, once a day, for 4 consecutive weeks. After 4 weeks of administration, the plasma levels of triglycerides (TG), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) were measured in each group of rats; the pathological and morphological changes of liver tissue were observed; the differential proteins between samples were screened out by TMT quantitative proteomic analysis; the expression levels of the key differentially expressed proteins in the liver, including epoxide hydrolase 2 (EPHX2), perilipin 2 (PLIN2), peroxisome proliferator activated receptor γ (PPAR γ) and glycogen synthase kinase-3β (GSK-3β)were detected. RESULTS Compared with the model group, the plasma levels of TC, TG and LDL-C in the Danshen decoction group were significantly reduced (P<0.05), while the level of HDL-C was significantly increased (P<0.05). The liver tissue of rats inmodel group showed uneven staining, disordered arrangement of liver plates, disappearance of liver sinusoids, nuclearcondensation or disappearance of some cells, swelling and fusion of cytoplasm, proliferation of connective tissue, and diffuse vacuolar-like fat droplet changes. The liver tissue of Danshen decoction group showed varying degrees of improvement in the above pathological and morphological. The results of differential protein analysis showed that the total number of differential proteins was 298 between the model group and the blank group; the total number of differential proteins was 139 between the model group and Danshen decoction group. Compared with the model group, the expression levels of EPHX2 and PLIN2 proteins in the liver tissue of rats in the Danshen decoction group were significantly reduced (P<0.01), while the expression levels of GSK-3β and PPARγ were significantly increased (P<0.01). CONCLUSIONS Danshen decoction has a significant improvement effect on the plasma lipid levels and the pathological and morphological of the liver tissue in hyperlipidemia model rats. Its regulatory mechanism may be related to the up-regulation of PPARγ and GSK-3β expression and down-regulation of EPHX2 and PLIN2 expression, and the signaling pathways involved may include PPAR-γ signal pathway.
		                        		
		                        		
		                        		
		                        	
2.Mechanism of Danshenyin in Inhibiting Platelet Activation in Rat Model of Hyperlipidemia Based on Proteomics

		                        			ObjectiveTo decipher the mechanism of Danshenyin in regulating platelet activation in the rat model of hyperlipidemia by means of proteomics and molecular biology. MethodWistar rats were randomized into blank, model, and Danshenyin groups (n=10) according to the blood lipid level. The rats in the blank group were fed with a basic diet, and those in the model and Danshenyin groups with a high-fat diet. All the rats had free access to water and food. The treatment began at the 9th week. The rats in the Danshenyin group were administrated with Danshenyin by gavage at a crude drug dose of 3.6 g·kg-1. The rats in the model and blank groups were administrated with an equal volume of normal saline according to body weight. At the 12th week, the tissue samples were collected for the measurement of related indicators, and the blood lipid level was measured by an automatic biochemical analyzer. The whole blood viscosity and plasma viscosity were measured by an automatic hemorheometer. The platelet proteome was determined by liquid chromatography-mass spectrometry. Western blotting was employed to determine the protein levels of platelet membrane glycoprotein 4 (CD36), focal adhesion kinase (FAK), phosphatidylinositol 4-phosphate 5-kinase (PIP5K), phosphorylated phosphatidylinositol 3-kinase (p-PI3K), protein kinase B (Akt), and phosphorylated protein kinase B (p-Akt). ResultCompared with the model group, Danshenyin lowered the levels of total cholesterol (TC), triglyceride (TG), and low-density lipoprotein cholesterol (LDL-C) in the plasma (P<0.05), elevated the level of high-density lipoprotein cholesterol (HDL-C) (P<0.05), and reduced the platelet aggregation rate (P<0.05). Compared with the blank group, the modeling up-regulated the expression of 44 proteins and down-regulated the expression of 12 proteins. Compared with the model group, Danshenyin up-regulated the expression of 21 proteins and down-regulated the expression of 22 proteins. Compared with the blank group, Danshenyin up-regulated the expression of 31 proteins and down-regulated the expression of 49 proteins. The gene ontology (GO) functional enrichment showed that the differentially expressed proteins were mainly involved in cholesterol transport and efflux, production of cytokines, dyslipidemia, and platelet activation. The Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment showed that the differentially expressed proteins were mainly involved in ECM-receptor interaction, peroxisome proliferators-activated receptors (PPAR), focal adhesion, and PI3K/Akt signaling pathways. Danshenyin can significantly down-regulate the expression of CD36, FAK, PIP5K, PI3K, p-Akt (Ser473), and p-Akt1/2/3 (Thr308). ConclusionDanshenyin can restore the blood lipid level of hyperlipidemia rats and inhibit the platelet activation caused by abnormal lipid levels by down-regulating the CD36/PI3K/Akt signal cascade. 
		                        		
		                        		
		                        		
		                        	
3.Effect of aluminum-fluoride interactions on overall cognitive function of aluminum plant workers

		                        			Background Aluminum and fluoride are neurotoxic, and aluminum exposure alone is closely related to the overall cognitive function of operational workers. It is unclear about the effect of aluminum and fluoride interactions on cognitive function. Objective To evaluate a potential interaction effect of blood aluminum and urinary fluoride on the overall cognitive function of workers working in an aluminum plant. Methods Using cluster sampling, 230 workers in the electrolysis workshop of an aluminum group company in Shanxi Province were selected, and plasma aluminum concentrations were determined by inductively coupled plasma mass spectrometry (ICP-MS) and urinary fluoride by ion-selective electrode. The study participants were divided into a low blood aluminum group and a high blood aluminum group according to the median (M) of blood aluminum concentration, and a low urinary fluoride group and a high urinary fluoride group by a predetermined cutoff point (2.160 mg·L−1). The Montreal Cognitive Assessment-Beijing (MoCA-BJ) was used to assess overall cognitive function of the workers. Logistic regression model was used to analyze the relationship between blood aluminum, urinary fluoride, and mild cognitive impairment (MCI), including multiplicative interaction analysis and correlation analysis; R language was used to fit an additive interaction model of blood aluminum and urinary fluoride on MCI and to calculate synergy index (S), relative excess risk due to interaction (RERI), and attributable proportion due to interaction (API). Results Among the 230 operational workers, the median blood aluminum concentration (P25, P75) was 40.11 (25.16, 58.89) µg·L−1, and there were 104 cases of abnormal urinary fluoride, with an abnormality rate of 45.2%. There was a multiplicative interaction (OR=7.783, 95%CI: 1.377, 43.991) and no additive interaction (RERI=0.030, 95%CI: −0.498, 0.559; API=0.018, 95%CI: −0.279, 0.316; S=1.049, 95%CI: 0.519, 2.118) for the effect between blood aluminum and urinary fluoride on overall cognitive function of the workers. The logistic regression analysis showed that the risk of MCI was 12.105 (95%CI: 2.802, 52.287) times higher in workers with both high blood aluminum and high urinary fluoride than in those with low blood aluminum and low urinary fluoride, after adjusting for selected influencing factors. Conclusion Occupational exposure related high blood aluminum and high urinary fluoride are risk factors for cognitive dysfunction, and the coexistence of both indicators increases the risk of MCI in workers with occupational aluminum exposure, with a multiplicative interaction.
		                        		
		                        		
		                        		
		                        	
4.Effects and mechanism of Danshen decoction on physiological characteristics and function of platelet in hyperli-pidemia model rats

		                        			OBJECTIVE To investigate the effects and mechanism of Danshen decoction influencing platelet physiological characteristics and function in hyperlipidemia model rats based on platelet membrane glycoprotein 4 (CD36)/phosphatidylinositol 3- kinase (PI3K)/protein kinase B (Akt) signaling pathway. METHODS The hyperlipidemia model rats were induced by feeding high- fat diet, and then randomly divided into model group, simvastatin group (0.004 g/kg), Danshen decoction high-dose and low-dose groups (3.6, 0.9 g/kg), and blank group (fed with basic feed), with 10 rats in each group. Rats in each administration group were intragastrically administered with corresponding drugs every day, and the other groups were intragastrically administered with equal volume of normal saline for 4 weeks. After the last administration, the contents of blood lipid biochemical indexes [total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C)], whole blood viscosity, plasma viscosity and fibrinogen (FIB)content in plasma, platelet-related parameters [platelet count (PLT), platelet distribution width (PDW) and mean plateletvolume (MPV)] were detected. The levels of plateletphysiological characteristics and function-related factors [von Willebrand factor (vWF), fibronectin (Fn), phospholipase A2(PLA2), thromboxane B2 (TXB2), 6-keto-prostaglandin F1α (6-keto-PGF1α), thromboxane A2 (TXA2), prostaglandin I2 (PGI2), cyclic adenosine monophosphate (cAMP), cyclic guanosine monophosphate (cGMP), β-thromboglobulin (β-TG)], platelet aggregation rate (maximum aggregation rate, 60 s aggregation rate, 180 s aggregation rate) and fibrinolytic system-related factors [tissue plasminogen activator (t-PA), plasminogen activator inhibitor 1 (PAI-1)] and the expressions of CD36/PI3K/Akt signaling pathway-related proteins [CD36, focal adhesion kinase (FAK), phosphatidylinositol 4-phosphate 5-kinase (PIP5K), PI3K, phosphorylated Akt (p-Akt), p-Akt1/2/3] in platelet were all determined. RESULTS Compared with blank group, the contents of TC, TG and LDL-C in plasma, whole blood viscosity, plasma viscosity, the plasma levels of FIB, PLT, MPV, vWF, Fn, PLA2, TXB2, TXA2, cGMP and β-TG, maximum platelet aggregation rate, 60 s aggregation rate, the expressions of PAI-1 in plasma, protein expressions of CD36, FAK, PIP5K, PI3K, p-Akt and p-Akt1/2/3 were significantly increased in the model group (P<0.05). The content of HDL-C and the levels of 6-keto-PGF1α, PGI2 and t-PA were significantly decreased (P<0.05). After the intervention of Danshen decoction, most of the above indexes were significantly reversed (P<0.05). CONCLUSIONS Danshen decoction can improve the physiological characteristics and function of platelets in hyperlipidemia model rats, and its mechanism may be related to the down-regulation of CD36/PI3K/Akt signaling pathway activity and the reduction of platelet activation.
		                        		
		                        		
		                        		
		                        	
5.Autosomal dominant neurodevelopmental disorders associated with KIF1A gene variants in 6 pediatric patients.

		                        			OBJECTIVES:
		                        			To analyze the clinical and genetic characteristics of children with autosomal dominant neurodevelopmental disorders caused by kinesin family member 1A (KIF1A) gene variation.
		                        		
		                        			METHODS:
		                        			Clinical and genetic testing data of 6 children with KIF1A gene de novo heterozygous variation diagnosed in Shanghai Children's Medical Center Affiliated to Shanghai Jiao Tong University School of Medicine from the year 2018 to 2020 were retrospectively analyzed. Pathogenic variants were identified based on whole exome sequencing, and verified by Sanger sequencing. Moreover, the effect of variants on three-dimensional structure and stability of protein was analyzed by bioinformatics.
		                        		
		                        			RESULTS:
		                        			Among 6 patients there were 4 males and 2 females, and the age of consultation varied from 7 months to 18 years. All cases had varying degrees of motor developmental delay since childhood, and 4 of them had gait abnormalities or fell easily. In addition, 2 children were accompanied by delayed mental development, epilepsy and abnormal eye development. Genetic tests showed that all 6 cases had heterozygous de novo variations of KIF1A gene, including 4 missense mutations c.296C>T (p.T99M), c.761G>A (p.R254Q), c.326G>T (p.G109V), c.745C>G (p.L249V) and one splicing mutation c.798+1G>A, among which the last three variants have not been previously reported. Bioinformatics analysis showed that G109V and L249V may impair their interaction with the neighboring amino acid residues, thereby impacting protein function and reducing protein stability, and were assessed as "likely pathogenic". Meanwhile, c.798+1G>A may damage an alpha helix in the motor domain of the KIF1A protein, and was assessed as "likely pathogenic".
		                        		
		                        			CONCLUSIONS
		                        			KIF1A-associated neurological diseases are clinically heterogeneous, with motor developmental delay and abnormal gait often being the most common clinical features. 6.Preventive treatment of nitroglycerin-induced chronic migraine by peripheral single injection of Botulinum Neurotoxin A in mice

		                        			Objective:To evaluate the effect of botulinum neurotoxin A (BoNT/A) on prevention of chronic migraine (CM) in mice and explore the potential mechanism.Methods:Twenty-four male C57BL/6 mice were randomly divided into control group, nitroglycerin (NTG) group, and BoNT/A+NTG group ( n=8). Mice in the latter two groups were intraperitoneally injected with 10 mg/kg NTG on the 1 st, 3 rd, 5 th, 7 th and 9 th d of experiments to establish CM models. Mice in the BoNT/A+NTG group were injected with 0.18 U/100 μL BoNT/A one h before the first injection of NTG. Mice in the control group were injected with the same dose of normal saline. Basal mechanical withdrawal threshold (MWT) and evoked MWT 2 h after NTG in the facial and hindpaw regions on the 1 st, 3 rd, 5 th, 7 th and 9 th d of experiments were evaluated by von Frey filament test. The motor function of mice 2 h after NTG injection was tested by rotarod test on the 1 st, 3 rd, 5 th, 7 th and 9 th d of experiments. On 9 th d of experiments, the mice were sacrified; the calcitonin gene-related peptide (CGRP), synaptosomal-associated protein 25 (SNAP25), glial fibrillary acidic protein (GFAP) and TRP channel protein expressions in the trigeminal ganglia (TG) and trigeminal nucleus caudalis (TNC), and NOD-like receptor protein 3 (NLRP3) inflammatory factor pathway-related protein expressions in TNC were detected by Western blotting; real-time quantitative PCR (RT-qPCR) was used to detect the NLRP3 inflammatory factor pathway-related mRNA expressions in TNC. The CGRP expression in TNC was detected by immunofluorescent staining. Results:(1) As compared with the control group, the NTG group had significantly decreased basal facial MWT on the 7 th and 9 th d of experiments ( P<0.05); as compared with the NTG group, the BoNT/A+NTG group had significantly increased basal facial MWT on the 7 th and 9 th d of experiments ( P<0.05). As compared with the control group, the NTG group had significantly decreased evoked facial MWT on the 5 th and 9 th d of experiments ( P<0.05); as compared with the NTG group, the BoNT/A+NTG group had significantly increased evoked facial MWT on the 5 th and 9 th d of experiments ( P<0.05). As compared with the control group, the NTG group had significantly decreased basal and evoked MWT in the hindpaw regions on the 3 rd, 5 th, 7 th and 9 th d of experiments ( P<0.05); as compared with the NTG group, the BoNT/A+NTG group had significantly increased basal and evoked MWT in the hindpaw regions on the 3 rd, 5 th, 7 th and 9 th d of experiments ( P<0.05). (2) There was no significant difference in running time on rotarod among the three groups ( P>0.05). (3)Western blotting results showed that as compared with those in the control group, the CGRP and SNAP25 protein expressions were significantly increased in TG of the NTG group ( P<0.05); and those in the BoNT/A+NTG group were significantly decreased as compared with those in the NTG group ( P<0.05). As compared with those in the control group, the CGRP and NLRP3 protein expressions were significantly increased in TNC of NTG group ( P<0.05); and those in the BoNT/A+NTG group were significantly decreased as compared with those in the NTG group ( P<0.05). (4)RT-qPCR results showed that as compared with that in the control group, the IL-1β mRNA expression in TNC of the NTG group was significantly increased ( P<0.05), and that in the BoNT/A prevention group was statistically decreased as compared with that in the NTG group ( P<0.05). (5) Immunofluorescent staining results showed that as compared with that in the control group, the CGRP expression in TNC of the NTG group was significantly increased, and that in the BoNT/A+NTG group was significantly decreased as compared with that in the NTG group ( P<0.05). 7.Distribution and drug resistance of wound pathogenic microorganisms in outpatients of wound healing center

		                        			Objective:To analyze the distribution and drug resistance of wound pathogenic microorganisms in outpatients of wound healing center so as to provide a basis for the standardized construction of wound healing centers.Methods:A retrospective case series study was used to analyzed the data of 365 outpatients treated at Ruijin Hospital, Shanghai Jiaotong University School of Medicine from December 2017 to October 2019. There were 220 males and 145 females, aged (58.8±18.9)years (range, 18-98 years). The patients included 92 first-visit patients and 273 re-visit patients. The culture results (positive rate of pathogenic microorganisms, bacterial species, bacterial distribution) and drug sensitivity results of the wound secretions were compared and analyzed.Results:(1) Among 365 samples of wound secretions, 198 patients were positive for pathogenic microorganisms with a positive rate of 54.3%. A total of 107 strains (51.0%) of Gram-positive bacteria were detected, mainly Staphylococcus aureus (70 strains, 33.3%); 95 strains (45.2%) of Gram-negative bacteria were detected, mainly Escherichia coli (20 strains, 9.5%), followed by Pseudomonas aeruginosa (17 strains, 8.1%); 8 strains (3.8%) of fungi were detected. (2) A total of 26 (28.3%) first-visit patients were positive for pathogenic microorganisms, and 172 (63.0%) re-visit patients were positive for pathogenic microorganisms. The rate of positive microorganism detection had significant differences between first-visit and re-visit patients ( P<0.05). (3) A total of 29 strains were detected in first-visit patients, including 16 strains (55.2%) of Gram-positive bacteria, 11 strains (37.9%) of Gram-negative bacteria and 2 strains (6.9%) of fungi. A total of 181 strains were detected in re-visit patients, including 91 strains (50.3%) of Gram-positive bacteria, 84 strains (46.4%) of Gram-negative bacteria and 6 strains (3.3%) of fungi. The microbial distribution was significantly different between first-visit and re-visit patients ( P<0.05). (4) Compared with first-visit patients, the resistance of Staphylococcus aureus isolated from the re-visit patients to spenicillin, oxacillin, ciprofloxacin, tetracycline, clindamycin, moxifloxacin, erythromycin, and levofloxacin were increased variably. No vancomycin-resistant Staphylococcus aureus was detected, indicating that the staphylococcus aureus presented in the wound was highly sensitive to vancomycin. Conclusions:Staphylococcus aureus is the most common microorganism in wound secretions in outpatients of wound healing center. The rate of positive pathogenic microorganisms in wound secretions of re-visit patients is significantly higher than that of first-visit patients, and the distribution of pathogenic microorganisms of first-visited and revisited patients differs significantly. The Staphylococcus aureus detected in re-visit patients has a higher resistance to common antibiotics compared with first-visit patients. It is suggested that timely detection of pathogenic microorganisms in outpatients and effective control and supervision of outpatient infections are important contents that cannot be ignored in the construction of wound healing center.
		                        		
		                        		
		                        		
		                        	
8.Effects of Buyang Huanwu Decoction on Hemorheology and Platelet Related Biological Indexes in Hyperlipidemia Model Rats

		                        			OBJECTIVE:To investigate the effe cts of Buyang huanwu decoction on hemorheology and platelet related biological indexes of hyperlipidemia model rats. METHODS :Male Wistar rats were randomly divided into blank control group (10 rats)and model group (40 rats). Blank control group was given normal diet ,and model group was given high-lipid diet for 6 weeks at least to induce hypelipidemia model. After modeling ,rats were randomly divided into model control group ,positive control group (simvastatin,0.004 g/kg),Buyang huanwu decoction low-dose and medium-dose groups (3.5,14.0 g/kg,by crude drug),with 10 rats in each group. Blank control group and model control group were given normal saline intragastrically , administration groups were given relevant drug intragastrically ,0.01 mL/g,once a day ,for consecutive 4 weeks. Thirty min after last medication ,hemorheological indexes (whole blood viscosity ,plasma viscosity ),platelet adhesion related indexes [adhesion rate,von Willebrand factor (vWF),fibronectin(FN)],platelet release related indexes [ β-thromboglobulin(β-TG),platelet factor 4 (PF4)] and platelet fibrinolytic system related indexes [tissue plasminogen activator (t-PA),plasminogen activator inhibitor (PAI-1)],platelet parameters (PLT,PDW,MPV,PCT,PLCR),4 kinds of coagulation parameters (APTT,TT,PT,FIB)were detected. RESULTS :Compared with blank control group ,the whole blood viscosity (low,medium and high shear rate ),plasma viscosity,platelet adhesion rate ,the contents or levels of vWF ,FN,β-TG,PAI-1,PLT,MPV,PCT,PLCR and FIB in model control group were increased significantly (P<0.05 or P<0.01),and t-PA content was significantly decreased (P<0.05). Compared with model control group ,the whole blood viscosity (except for whole blood viscosity of high shear rate in Buyang huanwu decoction high-dose group ),plasma viscosity ,platelet adhesion rate ,the contents or levels of vWF ,FN,β-TG,PAI-1, PLT,PDW(except for Buyang huanwu decoction low-dose and high-dose groups ),MPV,PCT,PLCR(except for Byang huanwu decoction low-dose group )and FIB were decreased significantly (P<0.05 or P<0.01),while t-PA content (except for positive control group ) was increased significantly (P<0.05). CONCLUSIONS :Buyang huanwu decoction can significantly improve the pathological state in hyperlipidemia model rats by reducing blood viscosity and FN content ,improving platelet adhesion,enhancing fibrinolytic activity ,improving platelet aggregation ,inhibiting hypercoagulability and hyperplatelet release.
		                        		
		                        		
		                        		
		                        	
9.Effects of occupational exposure to aluminum on verbal function of workers

		                        			Objective:To explore the effect of occupational aluminum (Al) exposure on workers' overall cognitive level and speech function.Methods:In July 2019, by using cluster sampling method, the group of 232 exposed to Al from an Al plant in Shanxi Province were selected as the exposed group, and the group of 228 not exposed to Al were selected as the control group. The blood Al concentration was determined by inductively coupled plasma mass spectrometry (ICP-MAS) . The exposed group was divided into high exposure group and low exposure group according to the median ( M) concentration of Al in serum. The general cognitive function and speech function were evaluated with the Montreal Cognitive Assessment Scale (MoCA) , and the differences in the general cognitive function and speech function between the groups were compared, and non-conditional logistic regression was used to analyze the possible influencing factors of mild cognitive impairment (MCI) . Results:There were significant differences in age, working age, education and drinking status between Al exposed group and control group ( P<0.05) . The total MoCA score, animal naming tese (ANT) score and category fluency repetition (CFT) score in Al exposure group were lower than control group and the differences were statistically significant ( P<0.05) . The full rate of ANT was lower than that of CFT in Al exposure group ( P<0.05) . The total MoCA score, ANT score and CFT score in the high exposure group were significantly lower than those in the control group ( P<0.05) . The score of MoCA, ANT and CFT in high exposure group were lower than those in low exposure group between the level of serum aluminum>60 μg/L group and ≤60 μg/L group ( P<0.05) . Logistic regression analysis showed that working age, serun Al concentration, ANT score, CFT score and SR score were the possible influencing factors of MCI in occupational Al exposure workers ( P<0.05) . Conclusion:Occupational Al exposure can lead to impaired speech function of workers, the higher the ANT score, CFT score and SR score, the lower working age and serum Al concentration, the lower risk of MCI.
		                        		
		                        		
		                        		
		                        	
10.Advice on the rationalized layout of outpatient clinics in a wound repair department

		                        			According to a document issued by the General Office of National Health Commission, "one person, one diagnosis, and one room" is required in the process of outpatient consultation. However, the patient will need to go to another room for dressing change after the doctor checks the wound if sticking to the conventional layout of current wound repair specialist outpatient clinic in hospitals and following the regulation of "separation of diagnosis and treatment". To allow a patient walking back and forth with the exposed wounds to different clinics or going to another clinic for dressing change with the original dressing reapplied to the wound is against the regulation of nosocomial infection control and the principle of sterility. To ensure that the layout of the outpatient clinic in the wound repair outpatient department not only conforms to the principle of "one person, one diagnosis, and one room", but also meets the characteristics of the diagnosis and treatment process of chronic wounds, this paper proposes the layout of "large space and small partition" in the wound repair clinic.
