1.Effects of tele-rehabilitation program on ataxic dysarthria following stroke

Objective:To investigate the effect of tele-rehabilitation program on ataxic dysarthria following stroke.Methods:A total of 101 patients with stroke and dysarthria who received treatment at The First People's Hospital of Huzhou between January 2021 and October 2023 were included in this study. They were divided into an observation group ( n = 51) and a control group ( n = 50) using the random number table method. Patients in the control group received conventional family rehabilitation guidance, while those in the observation group participated in a tele-rehabilitation program. All patients were treated for 12 weeks. Before and 12 weeks after the intervention, the Frenchay Dysarthria Assessment (FDA) and speech clarity tests were used to assess speech function. Changes in FDA scores and speech clarity scores post-intervention were compared with pre-intervention scores. The observation group was further divided into mild, moderate, and severe groups based on FDA scores, and the differences in effect of tele-rehabilitation program on ataxic dysarthria among the three groups after the intervention were compared. Results:At 12 weeks after intervention, both the observation and control groups showed improvements in FDA scores and speech clarity [observation group: (43.88 ± 16.13) points, (68.65 ± 16.99)%; control group: (49.68 ± 12.09) points, (62.36 ± 14.20)%] compared with their baseline measurements [observation group: (55.12 ± 12.63) points, (57.96 ± 13.48)%; control group: (54.16 ± 10.69) points, (57.84 ± 12.78)%] ( t = 12.23, -14.01, 12.42, -13.86, all P < 0.001). Moreover, the observation group showed significant improvements compared with the control group ( t = 2.04, -2.01, both P < 0.05). At 12 weeks after intervention, both the mild and moderate groups showed improvements in FDA scores and speech clarity score [mild group: (31.41 ± 3.08) points, (82.94 ± 6.97)%, moderate group: (40.08 ± 4.67) points, (70.92 ± 6.26)%] compared with their baseline measurements [mild group: (42.24 ± 6.78) points, (70.53 ± 7.98)%, moderate group: (56.17 ± 4.65) points, (57.58 ± 6.54)%, t = 10.06, -20.25, 45.21, -21.34, all P < 0.001]. There were no significant improvements in the severe group for FDA scores and speech clarity score [(74.20 ± 4.29) points, (38.90 ± 5.80)%] compared with their baseline measurements [(74.50 ± 4.01) points, (37.50 ± 4.74)%, t = 1.96, -1.44, both P > 0.05]. Conclusion:The tele-rehabilitation program can improve post-stroke ataxic dysarthria, particularly showing significant effectiveness for patients with mild to moderate ataxic dysarthria.
2.Porphyromonas gingivalis bacteremia increases the permeability of the blood-brain barrier via the Mfsd2a/Caveolin-1 mediated transcytosis pathway.

Bacteremia induced by periodontal infection is an important factor for periodontitis to threaten general health. P. gingivalis DNA/virulence factors have been found in the brain tissues from patients with Alzheimer's disease (AD). The blood-brain barrier (BBB) is essential for keeping toxic substances from entering brain tissues. However, the effect of P. gingivalis bacteremia on BBB permeability and its underlying mechanism remains unclear. In the present study, rats were injected by tail vein with P. gingivalis three times a week for eight weeks to induce bacteremia. An in vitro BBB model infected with P. gingivalis was also established. We found that the infiltration of Evans blue dye and Albumin protein deposition in the rat brain tissues were increased in the rat brain tissues with P. gingivalis bacteremia and P. gingivalis could pass through the in vitro BBB model. Caveolae were detected after P. gingivalis infection in BMECs both in vivo and in vitro. Caveolin-1 (Cav-1) expression was enhanced after P. gingivalis infection. Downregulation of Cav-1 rescued P. gingivalis-enhanced BMECs permeability. We further found P. gingivalis-gingipain could be colocalized with Cav-1 and the strong hydrogen bonding between Cav-1 and arg-specific-gingipain (RgpA) were detected. Moreover, P. gingivalis significantly inhibited the major facilitator superfamily domain containing 2a (Mfsd2a) expression. Mfsd2a overexpression reversed P. gingivalis-increased BMECs permeability and Cav-1 expression. These results revealed that Mfsd2a/Cav-1 mediated transcytosis is a key pathway governing BBB BMECs permeability induced by P. gingivalis, which may contribute to P. gingivalis/virulence factors entrance and the subsequent neurological impairments.

3.Comparative analysis of C-TIRADS and ACR-TIRADS in the diagnostic efficiency of thyroid nodules

Objective:To investigate the diagnostic efficiency of the 2020 Chinese Thyroid Imaging Reporting and Data System (C-TIRADS) and the 2017 American College of Radiology Thyroid Imaging Reporting and Data System (ACR-TIRADS) in the diagnosis of benign and malignant thyroid nodules.Methods:A retrospective analysis of the two-dimensional ultrasound image results of 324 thyroid nodules in 289 patients with thyroid nodules and thyroid nodules were performed in the physical examination of the Health Management Department of the Guangdong Second Provincial General Hospital from January 2018 to January 2019. A superficial professional doctor with a senior professional title simultaneously uses the C-TIRADS and ACR-TIRADS methods to evaluate the above nodules. The results are all pathologically referenced for the χ2 test and the receiver operating characteristic curve is drawn. Results:The sensitivity of C-TIRADS in diagnosing benign and malignant thyroid nodules was 81.90%, specificity was 97.72%, accuracy was 92.59%, negative predictive value was 91.85%, positive predictive value was 84.51%; ACR-TIRADS diagnosis The sensitivity of benign and malignant thyroid nodules was 59.05%, specificity was 99.54%, accuracy was 86.42%, negative predictive value was 83.52%, and positive predictive value was 98.41%. The area under the ROC curve was 0.958 and 0.935( Z=2.31 P=0.021). Conclusion:C-TIRADS classification based on counting method is better than ACR-TIRADS classification based on sub-method in the diagnosis of thyroid nodules. It has better efficacy and is more suitable for the current status of diagnosis and treatment of thyroid nodules in China.
4.Construction of health education scheme for discharge planning of adolescent idiopathic scoliosis surgery patients

Objective:To build a health education scheme related to discharge planning for adolescent idiopathic scoliosis surgery patients, and provide systematic and standardized health education related to discharge planning for adolescent idiopathic scoliosis surgery patients.Methods:From January to June 2021, through literature review and semi-structured interviews, the first draft of health education scheme for discharge planning of adolescent idiopathic scoliosis surgery patients was formulated. The Delphi method was used to conduct 2 rounds of consultation with 15 experts, and the final version of the health education scheme for the discharge plan was determined according to the expert consultation.Results:The effective recovery rates of the 2 rounds of expert consultation questionnaires were both 100.0%, the coefficients of expert authority were 0.93 and 0.94, and the Kendall's harmony coefficients of expert opinions were 0.251 and 0.273 ( P<0.01) . Finally, the health education program was constructed according to the health needs of patients and caregivers at 7 time periods including admission day, 1 day before surgery, operation day, postoperative day 1, 1 day before discharge, discharge day and 1 week after discharge, including 7 first-level items, 24 second-level items, and 78 third-level items. Conclusions:The health education scheme related to the discharge plan for adolescent idiopathic scoliosis surgery patients constructed in this study is scientific and reasonable in design, comprehensive, reliable and clinically practical. It is of great significance for the effective implementation of the discharge plan for adolescent idiopathic scoliosis surgery patients .
5.Effect of health management under the transtheoretical model and stages of change on diabetes mellitus patients with obesity

Objective:To study the effect of health management based on the transtheoretical model and stages of change on patients with diabetes mellitus complicated by obesity.Methods:A total of 122 patients with type 2 diabetes mellitus (T2DM) complicated by obesity and admitted to Guangdong Second Provincial General Hospital between January 2019 and June 2020 were recruited to participate in the study. They were divided into two equal groups at random, the observation group and the control group —with 61 patients in each group. Both groups were subjected to individualized health management based on conventional drug treatment. The control group received routine health education and psychological support, while the observation group was subjected to the transtheoretical model and stages of change interventions for six months. The independent-samples t-test was used to compare blood glucose, body weight, and self-efficacy in the two groups after interventions, and the χ 2 test or Fisher′s precise test was used to compare behavior habit compliance rates in the two groups after interventions. Results:FBG with (6.27±0.92) mmol/L and (6.58±0.91) mmol/L, 2hPG with (8.53±1.39) mmol/L and (8.53±1.39) mmol/L, and HbA1c with (6.49±0.91)% and (6.78±0.96)% in the two groups were significantly lower at six months of interventions than those before ( P<0.05), but the differences between the two groups were not statistically significant ( P>0.05). BMI, waist circumference, and body fat rate with (30.46±2.51) kg/m 2, (91.72±10.45) cm, and (34.09±3.84)% of the observation group at six months of interventions were significantly lower than those before interventions ( P<0.05), whereas there were no significant changes in the control group before and after interventions ( P>0.05). The observation group′s BMI and body fat rate were lower than those of the control group ( P<0.05). The General Self-Efficacy Scale (GSES) scores in the two groups were (31.62±2.59) points and (28.75±2.48) points, which were significantly higher than those before interventions ( P<0.05). The observation group′s GSES score was higher than that of the control group ( P<0.05). At six months of interventions, the compliance rates of diet control (92.98% and 77.97%), reasonable exercise (87.72% and 71.19%), adherence to medication (98.25% and 91.53%), quitting smoking and alcohol (94.74% and 81.36%), blood glucose monitoring (91.23% and 94.92%), and regular review (100.00% and 98.31%) were all significantly higher in the two groups than those before interventions ( P<0.05). Additionally, the compliance rates of diet control, reasonable exercise, and quitting smoking and alcohol in the observation group were higher than those of the control group ( P<0.05). Conclusion:The use of the transtheoretical model and stages of change for the self-management intervention of diabetes mellitus patients with obesity is beneficial to improve self-efficacy, behavioral habits, and weight control effectiveness.
6.Efficacy and safety of anti-PD-1 monoclonal antibody in advanced hepatocellular carcinoma after TACE combined with TKI therapy

Objective:To evaluate the efficacy and safety of anti-programmed cell death 1 (PD-1) receptor monoclonal antibody (MoAb) in patients with advanced hepatocellular carcinoma (HCC) after treatment of transcatheter arterial chemoembolization (TACE) combined with tyrosine kinase inhibitor (TKI).Methods:From February 2019 to February 2020, 56 HCC patients who relapsed after TACE-TKI treatment in Department of Interventional Radiology, The Second Affiliated Hospital of Guangzhou Medical University were enrolled. All patients received anti-PD-1 MoAb (sintilimab injection) and followed up every 6 weeks. According to mRECIST, the curative effect was evaluated as complete response (CR), partial response (PR), stable disease (SD) or progressive disease (PD). Objective response rate (ORR) and disease control rate (DCR), progression-free survival (PFS) and treatment-related adverse events (TRAEs) were recorded. Univariate analysis by Chi-square test and binary logistic regression model was used to determine the influencing factors of DCR. The Kaplan-Meier method and Cox proportional hazard regression model were used to analyze the survival data.Results:A total of 48 patients were enrolled in this study including 42 males and 6 females, with a median age of 55 years (29-71 years). ECOG scores comprised of 0 in 24 cases, 1-2 in 24 cases. Thirty-six patients were in Child-Pugh grade A of liver function and 12 cases were grade B. The median follow-up time was 4.5 months. There were 2 patients achieved CR, 12 patients with PR and 16 with SD. ORR was 29.2%, DCR was 62.5%. The independent influencing factors of DCR was ECOG score and AFP level ( P=0.031, P=0.012). Median PFS was 4.1 months (95% CI 2.7-5.4 months), and ECOG score was the independent influencing factor of PFS ( P=0.042). Treatment-related adverse events were reported in 70.8% (34/48) patients. Incidence of grade Ⅲ-Ⅳ TRAEs was 22.9% (11/48). Conclusion:In patients with HCC who relapse from TACE and TKI treatment, anti-PD-1 monoclonal antibody is efficacious safe especially in those with ECOG 0 score.
7.Differential expression of cyclooxygenase gene in familial aggregation and sporadic large-artery atherosclerosis cerebral infarction patients

Objective:To investigate the differential expression of cyclooxygenase (COX) gene between familial aggregated atherosclerotic large-artery atherosclerosis(LAA) cerebral infarction patients and sporadic LAA cerebral infarction patients.Methods:Twenty-five patients with familial aggregation LAA cerebral infarction (familial aggregation LAA group) and 25 sporadic LAA cerebral infarction patients (sporadic LAA group) were collected.Another 25 patients with non-cardiovascular and cerebrovascular diseases were selected as control group.Real-time quantitative PCR and ELISA were used to detect COX-1 and COX-2 protein in the supernatant of samples.Results:The expression level of COX-1 gene was 0.5436±0.2737, 0.2400±0.1656 and 0.8340±0.3799 in the familial aggregation, sporadic LAA cerebral infarction and control groups.Compared with control group, COX-1 gene expression in sporadic LAA cerebral infarction group and familial aggregation LAA cerebral infarction group was significantly down-regulated, the differences were significant( P<0.05). The expression level of COX-2 gene was 1.3672±0.8249, 1.3932±0.7158 and 0.7212±0.9623 in the familial aggregation, sporadic LAA cerebral infarction and control groups.Compared with the normal control group, the expression of COX-2 gene in familial aggregation LAA cerebral infarction group and sporadic LAA cerebral infarction group increased significantly( P<0.05). The expression of COX-1 protein was 2.9956±0.5672, 2.5572±1.0289 and 2.6721±0.7944 in the familial aggregation, sporadic cerebral infarction and control groups.The expression of COX-2 protein was 16.63±4.06, 16.86±7.93 and 15.94±5.94 in the familial aggregation, sporadic cerebral infarction and control groups.There was no significant difference in the relative expression of COX-1 and COX-2 proteins among familial aggregation LAA cerebral infarction group, sporadic LAA cerebral infarction group and control group(all P>0.05). Conclusion:There is significant difference in COX-1 gene expression between patients with familial LAA and sporadic LAA cerebral infarction.
8.Long-term effects of kidney transplantation in children

Objective To explore the long-term clinical effect of kidney transplantation in children.Methods The clinical data of 53 children with kidney transplantation from March 2008 to September 2014 were retrospectively analyzed.The influence of the dependent factors on the estimated glomerular filtration rate (eGFR) (greater than 90 mL/min/1.73 m2 or <90 mL/min/1.73 m2) was estimated in the three years after the operation,and the influencing factors were analyzed by the dual logistic regression equation.Results There were 19 cases of living donors,17 cases of organ donors after death,and 6 others.The 53 patients were followed up for 3-9 years.The level of blood creatinine was decreased from the preoperative (820.1 ± 323.1) μmol/L to (51.6 ± 24.9) μmol/L 3 years after the operation (P<0.05).eGFR was increased to (103.5 ± 11.4) mL/min/1.73 m2at 3rd year after the operation from the preoperative (17.1 ± 7.8) mL/min/1.73 m2 (P<0.05).The age of recipients,preoperative dialysis time,number of HLA mismatching and postoperative delayed graft function healing (DGF),rejection and infection were the influencing factors of eGFR at 3rd year postoperation (P<0.05).The multi-factor binary logistic regression equation analysis showed that only rejection was the risk factor for eGFR at 3rd year p0ostoperation.Eight cases of DGF (8/53,15.1%) recovered rapidly.There were 6 cases of acute rejection (6/47,12.8 %) and 1 case of chronic rejection (1/47,2.1%).There were 9 cases of infection (9/47,19.1%).There were 6 cases of recurrence after surgery.The 3-year recipient and kidney survival rate was 94.3% (50/53) and 88.7% (47/53) respectively.The average height of the patients in the first,second and third year after the surgery was increased by (4.6 ± 1.9) cm (0.5-19.1 cm),(3.7 ± 1.8) cm (0.7-14.3 cm) and (2.8± 1.2) cm (0.3-8.7 cm) respectively.Conclusion The long-term effect of children kidney transplantation is satisfactory.
9.Epidemiology of fungal infections in China.

With the increasing number of immunocompromised hosts, the epidemiological characteristics of fungal infections have undergone enormous changes worldwide, including in China. In this paper, we reviewed the existing data on mycosis across China to summarize available epidemiological profiles. We found that the general incidence of superficial fungal infections in China has been stable, but the incidence of tinea capitis has decreased and the transmission route has changed. By contrast, the overall incidence of invasive fungal infections has continued to rise. The occurrence of candidemia caused by Candida species other than C. albicans and including some uncommon Candida species has increased recently in China. Infections caused by Aspergillus have also propagated in recent years, particularly with the emergence of azole-resistant Aspergillus fumigatus. An increasing trend of cryptococcosis has been noted in China, with Cryptococcus neoformans var. grubii ST 5 genotype isolates as the predominant pathogen. Retrospective studies have suggested that the epidemiological characteristics of Pneumocystis pneumonia in China may be similar to those in other developing countries. Endemic fungal infections, such as sporotrichosis in Northeastern China, must arouse research, diagnostic, and treatment vigilance. Currently, the epidemiological data on mycosis in China are variable and fragmentary. Thus, a nationwide epidemiological research on fungal infections in China is an important need for improving the country's health.

10.Clinical efficacy of en-bloc kidney transplantation from pediatric donor kidneys

Objective To evaluate the clinical efficacy of en-bloc kidney transplantation from pediatric organ donation after death. Methods Clinical data of donors and recipients undergoing en-bloc kidney transplantation from pediatric donor kidneys were retrospectively analyzed. The 1-year survival rates of the recipient and grafted kidney were calculated. The recovery of renal function at postoperative 1 year was observed. The changes in the length of grafted kidney and incidence of postoperative adverse events were monitored. Results The 1-year survival rate of the recipients was 8/9, and 72% for the grafted kidney. During 1-year follow-up, the serum creatinine (Scr) level was down-regulated from (747± 170) μmol/L before transplantation to (83±27) μmol/L post-transplantation, the blood urea nitrogen concentration was decreased from (24.5±4.9) mmol/L to (6.8±2.0) mmol/L, and the length of transplanted kidney was increased from (61.1±9.8) mm to (100.3±1.7) mm. Two recipients suffered from delayed graft function(DGF) and restored after hemodialysis. Two cases developed acute rejection and healed after methylprednisolone shock therapy. One recipient presented with lung fungal infection at postoperative 2 weeks after transplantation, and was treated by the withdrawal of immunosuppressive agents and antibacterial treatment with poor clinical efficacy. Then the recipient died at 3rd month. One case had renal arterial thrombosis at postoperative 7 d, underwent nephrectomy at postoperative 10 d and returned to hemodialysis. At postoperative 1st month, one recipient suffered from thrombosis of unilateral renal artery. The grafted kidney in other side normally functioned and significantly grew in size at postoperative 6 months. In addition, two cases had ureterostenosis of the transplanted kidney, albuminuria in 2, abdominal aortic stenosis in 1 and urinary fistula in 1. All these symptoms were cured or alleviated after corresponding treatment. Conclusions The incidence of perioperative complications is relatively high in en-bloc kidney transplantation from pediatric organ donation after death, whereas the clinical efficacy of such kidney transplantation can be gradually increased along with the accumulation of clinical experience.
