1.Comparison of different atomizing inhalation times on noninvasive ventilation in patients with acute exacerbation of chronic obstructive pulmonary disease

		                        			Objective:To compare the dynamic changes of transcutaneous partial pressure of carbon dioxide (PtCO 2) and treatment effect of non-invasive intermittent nebulization and non-invasive simultaneous nebulization in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods:This was a randomized parallel controlled trial study. A total of 70 patients with acute exacerbation of COPD in Changzhou First People′s Hospital from October 2021 to September 2022 were selected by convenience sampling method, and divided into control group and experimental group by randomized digits table method with 35 cases in each group. The control group was given non-invasive intermittent oxygen-driven nebulization, and the experimental group was given non-invasive simultaneous oxygen-driven nebulization. The PtCO 2 values at 0, 5, 10, 15 min (the end point of atomization) of the 2 groups were observed, the daily arterial blood gas analysis indexes (mainly including PaCO 2, PaO 2 and pH) were recorded, and the clinical pulmonary infection score and the self-assessment score of COPD patients were recorded before treatment, on the 4th and 7th day of treatment. Results:Finally, 33 patients were included in both the control group and the experimental group. There were 25 males and 8 females in the control group, aged (75.33 ± 8.24) years old. There were 25 males and 8 females in the experimental group, aged (72.39 ± 8.56) years old. The PtCO 2 values at 0, 5, 10, 15 min in the control group were (63.83 ± 12.47), (64.40 ± 12.57), (65.42 ± 13.77), (66.62 ± 14.59) mmHg (1 mmHg=0.133 kPa). There were statistically significant differences in PtCO 2 at all time points ( F=8.05, P<0.01). Further pairwise comparison by Sidak method showed that there were statistically significant differences in PtCO 2 at 15 min compared with 0, 5, 10 min (all P<0.05). The PtCO 2 values at 0, 5, 10, 15 min in the experimental group were (67.62 ± 11.89), (67.15 ± 12.12), (67.82 ± 12.22), (68.15 ± 12.09) mmHg. There was no statistically significant difference in PtCO 2 at all time points ( F=2.00, P>0.05). The PaCO 2 and pH value of the two groups were improved with the treatment time, the control group had a statistically significant difference on the 4th day of treatment compared with before treatment ( P<0.05), while the experimental group on the second day of treatment compared with before treatment ( P<0.05). Conclusions:Both kinds of nebulization have achieved good therapeutic effects, but non-invasive simultaneous nebulization can better maintain the stability of PtCO 2 in the process of nebulization with higher safety, and can improve the arterial blood gas index PaCO 2 and pH value of patients earlier, which is a more suitable nebulization method for the combination of non-invasive ventilation and nebulization, especially for patients with hypercapnia.
		                        		
		                        		
		                        		
		                        	
2.Research progresses of PET molecular imaging for Parkinson disease complicated with levodopa-induced dyskinesia

3.Correlation between brachial and ankle pulse wave velocity and cardiac structure and function in type 2 diabetes mellitus patients

		                        			Objective To explore the correlation between brachial and ankle pulse wave conduction velocity(baPWV)and cardiac structure and function in patients with type 2 diabetes mellitus(T2DM).Methods A total of 443 patients with T2DM who were treated in The Endocrinology Department of Gansu Provincial People's Hospital were enrolled in this study from June 2022 to October 2023.All the patients were divided into simple T2DM group(T2DM,baPWV<1400 cm/s,n=221)and atherosclerosis group(AS,baPWV≥1400 cm/s,n=222)based on baPWV.The cardiac ultrasound diagnostic instrument was used to record left ventricular ejection fraction(LVEF),end diastolic left ventricular diameter(LVd),end diastolic interventricular septal thickness(IVSd),end diastolic left ventricular posterior wall thickness(LVPWd),ratio of early to late diastolic peak velocity of mitral valve annulus(E/A),and ratio of early diastolic peak velocity of mitral valve orifice to early diastolic peak velocity of mitral valve annulus(E/E').Results Age,DM duration,SBP,DBP,HbA1c,TG,ABI,LVEF,IVSd,LVPWd,E/A<1,E/E'>14,were higher in AS group than in T2DM group(P<0.05 or P<0.01).Spearman correlation analysis shows that E/A<1 was positively correlated with age,DM duration,BMI,SBP,DBP,and ABI(P<0.05 or P<0.01),while negatively correlated with baPWV(P<0.01).Pearson correlation analysis shows that LVPWd was positively correlated with age,DM duration,BMI,SBP,DBP,baPWV,and Scr(P<0.05 or P<0.01).Logistic regression analysis shows that baPWV,ABI,and SBP were the influencing factors for left ventricular diastolic dysfunction.The analysis of the working characteristic curve of the subjects shows that the area under the curve of baPWV,SBP and ABI for predicting left ventricular diastolic dysfunction is 0.647,0.643,and 0.624,with the best cutoff points being 1398.5 cm/s,125.5 mmHg,and 1.107.Conclusions baPWV is closely related to cardiac structure and function in T2DM patients.As baPWV increases,the risk of left ventricular diastolic insufficiency and hypertrophy rises.
		                        		
		                        		
		                        		
		                        	
4.Correlation between frailty and symptom clusters during first oral chemotherapy in postoperative patients with gastric cancer

		                        			Objective:To understand the status of frailty in postoperative gastric cancer patients and to explore its influence on symptom groups during the initial oral chemotherapy. Provide theoretical guidance for symptom management of cancer patients.Methods:A total of 133 postoperative gastric cancer patients who received initial oral chemotherapy were selected as subjects by cross section survey from Octomber 2021 to Octomber 2022 in Department of Oncology, First People′s Hospital of Changzhou by couvenient sampling method. Baseline data of patients before chemotherapy were investigated by general data questionnaire and Frailty Phenotype. Symptoms on the 2nd and 14th day of chemotherapy were investigated by M. D. Anderson Symptom Inventory-Gastrointestinal Cancer Module.Results:The incidence of frailty in postoperative gastric cancer patients was 48.9% (65/133). On the 2nd day of chemotherapy, there were four symptom clusters, namely emotional symptom cluster, gastrointestinal symptom cluster, disease related symptom cluster and neurological symptom cluster, and the cumulative variance contribution rate was 61.20%. On the 14th day of chemotherapy, there were five symptom clusters after gastric cancer specific symptom cluster appeared. and the cumulative variance contribution rate was 63.90%. On the 2nd day of chemotherapy, frailty had statistical significance on emotional symptom cluster, gastrointestinal symptom cluster andneurological symptom cluster ( t = 3.52, 2.66, 5.48, all P<0.05), which explained 7.2%, 4.1% and 15.1% of symptom group severity, respectively. On the 14th day of chemotherapy, frailty still had statistical significance on emotional symptom cluster, gastrointestinal symptom cluster and neurological symptom cluster ( t = 3.41, 5.47, 4.06, all P<0.05), which explained 7.1%, 13.2% and 9.6% of symptom group severity, respectively. Conclusions:The incidence of frailty in patients with postoperative gastric cancer was at a relatively high level. The emotional symptom cluster, gastrointestinal symptom cluster, and neurological system related symptom cluster in patients with frailty were more serious at different time points.
		                        		
		                        		
		                        		
		                        	
5.Analysis of risk factors of death after cardiopulmonary bypass in children with congenital heart disease less than 5 kg

		                        			Objective:To investigate the risk factors for surgical death in children with congenital heart disease (CHD) less than 5 kg undergoing cardiopulmonary bypass.Methods:The clinical data of 225 children with CHD who underwent open cardiopulmonary bypass in the First Affiliated Hospital of Air Force Military Medical University from February 2017 to February 2020 were collected for a retrospective case-control study. The independent sample T test was used for inter-group comparison of measurement data, Chi square test or chi squre correction test was used for the comparision between count data. Mann-Whitney rank-sum test was used for rank distribution data and multiple Logistic regression analysis was performed for factors affecting surgical death of children with CHD.Results:Among the 225 cases, 204 (90.67%, 204/225) survived surgery and 21 (9.33%, 21/225) died. Univariate analysis showed that age (2.48±0.68) months and body weight (2.28±0.56) kg in the death group were significantly lower than that in the survival group (4.92±0.65) months ( t=16.33, P<0.001) and body weight (4.26±0.52) kg ( t=16.38, P<0.001), while the proportion of female (66.67% (14/21)), malnutrition (none: 14.28% (3/21); Mild: 42.86% (9/21). 6.Establishment and Implementation of Central Venous Line Management Information System in Cancer Special Hospital

		                        			Objective To establish and implement a central venous linemanagement information system based on the hospital information system (HIS). Methods The central venous line information management system was developed by the infusion therapygroup and the information technology department according to the requirement of completed central venous line management and the Chinese intravenous nursing practice standard. Results ① A complete information management system was constructed for central venous line from placement, maintenance and management of complications to drawing tube, and it operated smoothly. ②Totally, 4304 cases of PICC, 14,330 cases of CVC and 123 cases of PORT were collected from January 2016 to December 2017. The information of the maintenance number, catheter retention time, complications can be counted. 7.Screening of mitochondrial DNA damage repair genes in rats with septic acute kidney injury.

		                        			OBJECTIVE:
		                        			: To screen genes involved in mitochondrial DNA (mtDNA) damage repair in rats with septic acute kidney injury (SAKI).
		                        		
		                        			METHODS:
		                        			: Forty male C57BL/6J mice were randomly divided into SAKI group (=28) and sham operation group (=12). The SAKI mouse model was established by cecal ligation and puncture. Blood and kidney samples were collected at 8, 24, and 48 h after surgery. Serum creatinine and urea nitrogen were measured by a dry biochemical analyzer. Serum inflammatory cytokines were detected by ELISA. Histopathological changes were observed with HE staining. The mtDNA damage repair related genes were screened by RNA sequencing and bioinformatics analysis; the mRNA and protein expression levels of related genes were detected by real-time quantitative RT-PCR and immunohistochemisry, respectively.
		                        		
		                        			RESULTS:
		                        			: Symptoms of sepsis were observed in SAKI group, and 16 out of 28 mice were died in the SAKI group; serum TNF-α, IL-6, creatinine and urea nitrogen levels were higher than those in the sham group (<0.05 or <0.01). Histopathological examination in SAKI group showed that renal tubular epithelial cells were swollen, inflammatory cells infiltrated, and a large number of cell vacuoles were seen, suggesting successful modeling. Mitochondrial DNA damage repair related genes and were screened out. The expression of these genes was detected by real-time RT-PCR, and the results were consistent with RNA sequencing trends. Immunohistochemical staining showed that Gadd45α was mainly expressed in the nucleus of renal tubular epithelial cells, and the positive rate of Gadd45α in SAKI group was higher than that in the sham operation group (<0.05).
		                        		
		                        			CONCLUSIONS

8.Status and risk factors of postoperative hemorrhage in patients undergoing surgery

		                        			Objective To investigate the incidence and related factors of postoperative hemorrhage, so as to provide data support for preventing its occurrence and developing reasonable risk assessment tools. Methods The retrospective analysis of clinical data of 50 patients with postoperative hemorrhage after surgical operation in a 3A hospital in Shanghai from January to December 2016 was conducted. A 1:2 matching case-control study was conducted to explore the risk factors of postoperative bleeding in surgical patients by univariate and multivariate regression analysis.Results The total number of surgical operations (excluding obstetrics and gynecology, five senses) was 18942. Postoperative hemorrhage occurred in 50 cases, including 6 deaths. The incidence of postoperative hemorrhage was 0.26% and the mortality rate was 12%. The incidence of postoperative bleeding in the top four departments were gastrointestinal surgery (13 cases), hepatobiliary and pancreatic surgery (11 cases), cardiac surgery (10 cases), neurosurgery (9 cases). The statistical analysis of binary Logistic in the case group and the control group showed that postoperative albumin<35g/L, postoperative AST abnormality, postoperative ALT abnormality, postoperative hypertension were independent risk factors of postoperative bleeding.Conclusions The incidence of postoperative bleeding in this hospital is relativelylow, but the mortality is relatively high. Medical staff should pay more attention to the patients undergoing gastrointestinal, liver, pancreas, heart and brain surgery. It is also necessary to pay more attention to the blood pressure, albumin and liver function of the patients after operation,and establish the evaluation of prevention of hemorrhage, which has a certain early warning effect on the prevention of postoperative hemorrhage.
		                        		
		                        		
		                        		
		                        	
9.Enteric dysbiosis-linked gut barrier disruption triggers early renal injury induced by chronic high salt feeding in mice.

		                        			Chronic high-salt diet-associated renal injury is a key risk factor for the development of hypertension. However, the mechanism by which salt triggers kidney damage is poorly understood. Our study investigated how high salt (HS) intake triggers early renal injury by considering the ‘gut-kidney axis’. We fed mice 2% NaCl in drinking water continuously for 8 weeks to induce early renal injury. We found that the ‘quantitative’ and ‘qualitative’ levels of the intestinal microflora were significantly altered after chronic HS feeding, which indicated the occurrence of enteric dysbiosis. In addition, intestinal immunological gene expression was impaired in mice with HS intake. Gut permeability elevation and enteric bacterial translocation into the kidney were detected after chronic HS feeding. Gut bacteria depletion by non-absorbable antibiotic administration restored HS loading-induced gut leakiness, renal injury and systolic blood pressure elevation. The fecal microbiota from mice fed chronic HS could independently cause gut leakiness and renal injury. Our current work provides a novel insight into the mechanism of HS-induced renal injury by investigating the role of the intestine with enteric bacteria and gut permeability and clearly illustrates that chronic HS loading elicited renal injury and dysfunction that was dependent on the intestine.

10.Standards on Research of Healthcare Eye Pad Based on Traditional Chinese Medicine Theories

		                        			With the rapid development of information technologies and change of production method and life style,the visual health crisis has become a public health endangering the sustainable development of the economic society and national security,which shall be concerned and tackled by all levels of the society.Under the background of fully playing the role of traditional Chinese medicine (TCM) in preventive treatment,coping with the visual health crisis and satisfying the social needs for vision care,healthcare eye pad manufacturers with fairly scale have been emerged in China.However,there are no technical standards for healthcare eye pad products that are applicable for the whole country to standardize the production behaviors of the enterprises and protect the interests of the consumers.This research proposed the design and description for two technical standards of general requirements and inspection methods,as well as the vision on standards to be concerned and prepared by the eye healthcare industry in the future on the basis of analyses made for the contents and key indicators of the self-declared standards of the healthcare eye pad manufacturers.
