1.Mitochondrial Quality Control Regulating Pathogenesis of Sarcopenia and Its Intervention by Traditional Chinese Medicine: A Review

Sarcopenia is a clinical syndrome characterized by a decrease in skeletal muscle strength and quality, often accompanied by adverse outcomes such as falls, loss of function and weakness. The pathogenesis of sarcopenia is complex, and studies have shown that dysfunction due to impaired mitochondrial quality control is an important pathological factor in the occurrence and development. Traditional Chinese medicine(TCM) has been widely favoured for regulating mitochondrial homeostasis and preventing sarcopenia by virtue of its multi-target and multi-pathway advantages. They can play a role in the prevention and treatment of sarcopenia by regulating the mitochondrial quality control system to inhibit the occurrence of mitochondrial oxidative stress, regulate the balance of mitochondrial dynamics, inhibit mitochondrial autophagy, promote mitochondrial biosynthesis, resist the occurrence of mitochondrial apoptosis, and maintain the mitochondrial calcium and protein homeostasis. Based on this, the paper reviewed the relationship between mitochondrial quality control and sarcopenia, as well as the mechanism of TCM in intervening the mitochondrial quality control system to treat sarcopenia, in order to provide a new idea for the prevention and treatment of sarcopenia by TCM and to a theoretical basis for the clinical research on TCM intervention in sarcopenia.
2.The Latest Research Progress on Cell-Free DNA and Prospects of Its Forensic Application

In recent years,with the continuous progress of DNA extraction and detection technology,cell-free DNA(cfDNA)has been widely used in the life science field,and its potential application value in forensic identification is becoming more and more obvious.This paper reviews the concept,formation mechanism,and classification of cfDNA,etc.,and describes the latest research progress of cfDNA in personal identification of crime scene touch DNA samples and non-invasive prenatal pater-nity testing(NIPPT).Meanwhile,this paper summarizes the potential application of cfDNA in injury inference,and discusses the advantages and disadvantages of common cfDNA analysis methods and techniques,and its application prospects,to provide a new idea for the wide application of cfDNA in the field of forensic science.
3.Application value of 100 kVp low tube voltage combined with ASIR-V in Adamkiewicz artery imaging of spinal cord

Objective To investigate the effect of 100 kVp low tube voltage combined with adaptive statistical iterative reconstruc-tion-Veo(ASIR-V)on the imaging display,image quality and radiation dose of Adamkiewicz artery(AKA).Methods Ninety patients with computed tomography angiography(CTA)of aorta were randomly divided into two groups.In group A,the tube voltage was 100 kVp,and ASIR-V was 80%reconstructed.The tube voltage in group B was 120 kVp,and adaptive statistical iterative recon-struction(ASIR)was 40%reconstructed.The CT value and the standard deviation(SD)value of descending aorta were measured at the 12th thoracic vertebra level,and the CT value and the SD value of the spinal cord were measured at the same level to calculate the signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of AKA.According to the continuity between AKA and intercostal artery or lumbar artery,the visualization of AKA was evaluated by two doctors with a double-blinded method and a five-point method,and the radiation doses of group A and group B were compared.Results Compared with group B,the CT value of aorta in group A was sig-nificantly increased by about 22.83%(P<0.001).The background noise of AKA in group A was significantly lower than that in group B(P<0.001),and the SNR and CNR were significantly higher than those in group B(P<0.001).The visualization score of AKA in group A was higher than that in group B(P<0.05).The effective dose(ED)in group A was significantly lower than that in group B by about 41.28%(P<0.001).Conclusion 100 kVp combined with ASIR-V technique for spinal cord AKA imaging can obtain better image quality,improve the sensitivity of AKA display and reduce the radiation dose.
4.Correlation between Combined Urinary Metal Exposure and Grip Strength under Three Statistical Models: A Cross-sectional Study in Rural Guangxi

Objective This study aimed to investigate the potential relationship between urinary metals copper (Cu), arsenic (As), strontium (Sr), barium (Ba), iron (Fe), lead (Pb) and manganese (Mn) and grip strength. Methods We used linear regression models, quantile g-computation and Bayesian kernel machine regression (BKMR) to assess the relationship between metals and grip strength.Results In the multimetal linear regression, Cu (β=-2.119), As (β=-1.318), Sr (β=-2.480), Ba (β=0.781), Fe (β= 1.130) and Mn (β=-0.404) were significantly correlated with grip strength (P < 0.05). The results of the quantile g-computation showed that the risk of occurrence of grip strength reduction was -1.007 (95% confidence interval:-1.362, -0.652; P < 0.001) when each quartile of the mixture of the seven metals was increased. Bayesian kernel function regression model analysis showed that mixtures of the seven metals had a negative overall effect on grip strength, with Cu, As and Sr being negatively associated with grip strength levels. In the total population, potential interactions were observed between As and Mn and between Cu and Mn (Pinteractions of 0.003 and 0.018, respectively).Conclusion In summary, this study suggests that combined exposure to metal mixtures is negatively associated with grip strength. Cu, Sr and As were negatively correlated with grip strength levels, and there were potential interactions between As and Mn and between Cu and Mn.
5.Association between age of life adversity and insulin resistance in girls with precocious puberty

Objective:
To investigate the association between early life adversity(ELA) and the triglycerideglucose (TyG) index for an indicator of insulin resistance among girls with precocious puberty, so as to provide scientific basis for effective prevention and intervention measures.
Methods:
From July 2020 to September 2021, girls with precocious puberty were recruited from the childrens health clinic of Anhui Provincial Childrens Hospital. Among them, 150 girls with complete blood indicators and questionnaire information were included. Both parental reports and child selfreports were combined to assess ELA exposure. Fasting blood samples were collected to evaluate thetyg index. According to the ELA score classification, girls were classified into 3 groups for 0, 1 and >2, multiple linear regression analysis were conducted to examine the association between ELA exposure and TyG index in girls with precocious puberty.
Results:
Precocious pubertal girls subjectively reported high rates of ELA exposure, with an average ELA score of (1.07±1.17) and an average TyG level of (7.99±0.49). A single adverse association was found that the exposure of girls with precocious puberty to a lack of warm nurturing was significantly positively correlated with the TyG index (β=0.26, 95%CI=0.03-0.50, P<0.05). Multiple linear regression analysis showed that girls in the ELA≥2 group had a 0.24 increase in TyG levels compared to girls who did not experience ELA (β=0.24, 95%CI=0.04-0.43). After controlling for covariates such as child age, mothers age, fathers age, physical activity, screen time, birth weight, birth method (including natural and cesarean sections), perceived stress, BMI standardized Zscore, and parental assessment SDQ score, The association remained significant after controlling for covariates and was independent of BMI (β=0.25, 95%CI=0.04-0.46)(P<0.05).
Conclusions
Cumulative early life adversity in girls with precocious puberty is significantly positively correlated with the TyG index. It should early identify the girls exposed to high ELA for precocious puberty and timely intervent to improve their glucose metabolism function.
6.Research progress on protein engineering technology and its application in the synthesis biology of medicinal natural products

Natural products are important sources of drug discovery. However, the traditional methods of extraction and isolation, as well as chemical synthesis for obtaining natural products are associated with issues such as operational complexity, high costs, low efficiency, and environmental pollution. Constructing microbial cell factories through synthetic biology methods to produce medicinal natural products has the advantages of high efficiency, low cost, and environmental protection. Nevertheless, the scope and yield improvement of the products are limited by the limitations of enzymes in microbial cell factories. Protein engineering is considered one of the most effective approaches to overcome these limitations. This article introduces commonly used methods of protein engineering technology and summarizes its specific applications in improving enzyme performance, modifying the enzymatic environment, and promoting the development of synthetic biology tools in the field of pharmaceutical natural product synthesis. Furthermore, it analyzes the current bottlenecks and challenges in protein engineering and looks forward to its future application prospects, offering insights for the development and practical use of protein engineering technology.
7.Discrete element modeling and breakage behavior analysis of oral solid dosage form particles

The breakage pattern of unit particles during the production of oral solid dosage forms (OSD) is closely related to the quality of intermediate or final products. To accurately characterize the particles and study the evolution law of particle breakage, the Bonding model of the discrete element method (DEM) was used to investigate the breakage patterns of model parameters, particle shape and process conditions (loading mode and loading rate) on the dynamic breakage, force-time curve, breakage rate, maximum breakage size ratio and fracture strength of particles. The results showed that the particle breakage force was positively correlated with normal strength and bonded disk scale, negatively correlated with normal stiffness per unit area and tangential stiffness per unit area, and weakly correlated with tangential strength. The particle breakage rate was negatively correlated with the aspect ratio of the particles, and the maximum breakage size ratio was positively correlated with the aspect ratio of the particles; among the three loading modes, the breakage rate of compression breakage model was the largest, the breakage rate of shear breakage model was the second largest, and the breakage rate of wear breakage model was the smallest; the maximum breakage size ratio was positively correlated with the loading rate, the loading mode and the loading rate had no mutual influence on particle breakage rate, but had mutual influence on the maximum breakage size ratio. The research results will provide a theoretical basis for the shift of OSD from batch manufacturing to advanced manufacturing.
8.Effects of Danggui Buxue Decoction on myocardial ferroptosis in a rat model of heart failure with preserved ejection fraction

AIM To observe the effects of Danggui Buxue Decoction on myocardial ferroptosis in a rat model of heart failure with preserved ejection fraction(HFpEF).METHODS The rats were randomly divided into the control group,the model group and the Danggui Buxue Decoction group.After 8 weeks feeding of 8%high-salt diet in the model and trial groups to induce the rats into HFpEFd models,rats of the Danggui Buxue Decoction group were given 4 g/kg Danggui Buxue Decoction once daily for 4 weeks,in contrast to those of the control group given 12 weeks feeding of 0.3%low-salt diet.The rats had their left ventricular ejection fraction(LVEF),left ventricular fraction shortening(LVFS),left ventricular end-diastolic diameter(LVIDD),and end-diastolic left ventricular posterior wall thickness(LVPWd)detected by echocardiography;their pathological changes of myocardial tissue by HE and Masson staining;their myocardial mitochondrial morphology observed by transmission electron microscopy;their serum BNP,NT-proBNP levels,myocardial tissue Fe3+levels,and their levels of ROS,MDA,LPO and GSH,and SOD activity in serum and myocardium detected by ELISA method;and their myocardial expressions of ferroptosis marker proteins GPX4,FTH1 and xCT detected by immunofluorescence staining and Western blot method.RESULTS Compared with the model group,the Danggui Buxuetang group displayed increased LVEF,LVFS(P<0.01);decreased LVIDD,LVPWd(P<0.05,P<0.01);decreased serum BNP,NT-proBNP levels(P<0.01);decreased myocardial Fe3+level(P<0.01);decreased MDA,ROS and LPO levels in serum and myocardium(P<0.01);increased GSH level and SOD activity(P<0.01);and increased expressions of myocardial ferroptosis related protein GPX4,FTH1 and xCT(P<0.05,P<0.01).CONCLUSION Danggui Buxue Decoction protects the cardiac function of the rat model of HFpEF through inhibiting the occurrence of myocardial ferroptosis and reducing the myocardial oxidative stress level as well.
9.Cohort study on the incidence,mortality and risk factors of malignant tumors in the middle-aged and elderly population in Chengguan district of Lanzhou

Objective To analyze the incidence and mortality of malignant tumors among the middle-aged and elderly population in Chengguan district of Lanzhou from 2011 to 2021,and to discuss the related risk factors.Methods Using the research data of REACTION in Lanzhou,an epidemiological survey was conducted through cluster sampling in three communities in Chengguan district of Lanzhou since April 2011.The target population was middle-aged and elderly residents over 40 years old.Two follow-up surveys were carried out in 2014-2016 and 2021 successively,and 6543 people with complete follow-up data were finally included.The incidence and mortality rates of malignant tumors were calculated,as well as their age standardized rates with reference to the age composition of Segi's world standard population.Multivariate Cox regression analysis was used to screen the risk factors affecting the incidence and mortality of malignant tumors.Results After an average follow-up of 10.6 years,314 new cases of malignant tumors were found in middle-aged and elderly residents in Chengguan district of Lanzhou,with an incidence rate and age standardized incidence rate of 454.30/100 000 and 128.93/100 000,respectively.A total of 158 deaths were attributed to malignant tumors,with a mortality rate and age standardized rate of 228.41/100 000 and 607.9/100 000,respectively;The age standardized incidence rate and mortality rate of malignant tumors males were both higher than those females(P<0.05).During the follow-up period,the age standardized incidence rate of malignant tumors in the general population showed an significant upward trend(P<0.05),whereas the age standardized mortality rate gradually decreased after a brief increase(P<0.05).Lung cancer,colorectal cancer,gastric cancer,and liver cancer were the main types of malignant tumors ranking in the top five in terms of incidence and mortality by gender.Multivariate Cox regression results indicate that male,age≥60 years old,college education level or above,smoking history,drinking history,having been hit by major stressful events,central obesity,hypertension,and coronary heart disease are risk factors for the onset or death of malignant tumors(HR>1).Married,with family size≥4,frequent consumption of fresh fruit,frequent consumption of fresh vegetables,frequent consumption of grains and tubers are protective factors for the onset or death of malignant tumors(HR<1).Conclusion The incidence of malignant tumors among middle-aged and elderly people in Chengguan district of Lanzhou from 2011 to 2021 showed an increasing trend,while the overall mortality was decreasing.Our study indicates early cancer screening in elderly populations,maintaining a healthy lifestyle and strengthening the management of chronic diseases are crucial for the prevention and treatment of malignant tumors.
10.Development and application of review indicators for best evidence application in the management of high-risk diabetic foot in community

Objective:To carry out evidence transformation for the management of high-risk diabetic foot in communities, construct review indicators to evaluate the status quo of community application, analyze the promoting and obstacle factors in evidence transformation and formulate reform strategies and optimization measures.Methods:Based on the Canadian Knowledge to Action Framework as the theoretical framework, the review indicators and review methods were developed on the basis of identifying clinical nursing problems, summarizing evidence and introducing community scenarios. Based on the results of baseline review, obstacle factors were analyzed and countermeasures were developed.Results:A total of 31 best evidences were included in this study, and 25 review indicators were developed. The implementation rates of the 2, 4 to 7, 16, 20, 21 and 23 review indicators were 8%, 6%, 8%, 8%, 8%, 16%, 17%, 4% and 4%, respectively, and the implementation rates of the remaining indicators were 0. Through focus interview analysis, the main obstacle factors included that the management mode of diabetic high-risk foot in the community was still exploring, the construction of multidisciplinary nursing outpatient platform needed strong leadership and resource support, the post setting of multidisciplinary nursing team needed to be improved, the management of diabetic high-risk foot lacked reasonable medical procedures, the outpatient department lacked diversified publicity and education materials for diabetic high-risk foot management and community nurses lacked evidence-based preparation. The promotion factors included the high level of attention paid by the management of the hospital and nursing department, the significance of the management of high-risk diabetic foot paid by specialist nurses and patient expectations of good outcomes as motivation for specialist nurses to work. The reform strategy included building an outpatient nursing platform, clarifying the responsibilities of outpatient nursing posts, optimizing the outpatient treatment process, configuring hardware facilities (various screening and examination instruments and propaganda and education sports room), completing the supporting materials for high-risk foot health education (including courseware, practice manual, videos, posters and cards), and carrying out the management of diabetic high-risk foot in the community. After in-hospital and out-of-hospital training, the implementation rate of indicators 1 to 17, 20 to 23 was 100.00%, the implementation rate of indicator 18 was 61.11%, the implementation rate of indicator 19 was 79.63%, the implementation rate of indicator 24 was 66.67% and the implementation rate of indicator 25 was 82.41%.Conclusions:There is still a great gap between the foot management of high-risk diabetic foot and community practice. A full and comprehensive analysis of the obstacles and promoting factors in community practice and the implementation of corresponding reform strategies can promote the effective application of evidence in community situations.
