Based on the strategy of metabolomics combined with bioinformatics, this study analyzed the potential allergens and mechanism of pseudo-allergic reactions (PARs) induced by the combined use of Reduning injection and penicillin G injection. All animal experiments and welfare are in accordance with the requirements of the First Affiliated Experimental Animal Ethics and Animal Welfare Committee of Henan University of Chinese Medicine (approval number: YFYDW2020002). Based on UPLC-Q-TOF/MS technology combined with UNIFI software, a total of 21 compounds were identified in Reduning and penicillin G mixed injection. Based on molecular docking technology, 10 potential allergens with strong binding activity to MrgprX2 agonist sites were further screened. Metabolomics analysis using UPLC-Q-TOF/MS technology revealed that 34 differential metabolites such as arachidonic acid, phosphatidylcholine, phosphatidylserine, prostaglandins, and leukotrienes were endogenous differential metabolites of PARs caused by combined use of Reduning injection and penicillin G injection. 2.Application of artificial bone material in percutaneous vertebroplasty treatment for osteoporotic vertebral compression fractures

Objective:To explore the clinical efficacy of combined use of artificial bone materials in percutaneous vertebroplasty (PVP) for osteoporotic vertebral compression fractures (OVCF).Methods:One hundred and eighty-four consecutive patients with OVCF admitted to Beijing Friendship Hospital, Capital Medical University from June 2020 to June 2021 were retrospectively analyzed. All patients had single-level fracture and treated with PVP. According to whether artificial bone materials were used, the patients were divided into experimental group ( n=95) and control group ( n=89). The experimental group was treated with bone cement mixed with artificial bone materials, and the control group was treated with bone cement. The following indices were observed in the two groups before surgery and at 3 days, 3 months, 12 months (final follow-up) after surgery: visual analogue scale (VAS) score, Oswestry disability index (ODI), Cobb angle of kyphosis, and the percentage of anterior vertebral height, the amount of bone cement injected, postoperative complications and adjacent vertebral fractures were recorded. Measurement data were expressed as mean±standard deviation ( ± s), and t-test was used for comparison between groups; Chi- test was used for comparison between groups for count data. Results:All patients successfully completed the operation and were followed up for 12-20 months, with a mean follow-up of (14.24±2.51) months. The VAS score at 3 days, 3 months after operation and final follow-up (experimental group: 2.00±0.71, 1.89±0.71, 1.41±0.49; control group: 2.13±0.73, 1.81±0.60, 1.44±0.50) and ODI index at 3 months after operation and the final follow-up [experimental group: (21.56±4.68)%, (23.22±4.11)%; control group: (22.46±3.74)%, (22.39±4.05)%] were significantly improved compared with those before operation [VAS, experimental group: 7.66±0.86, control group: 7.81±0.89; ODI, experimental group: (70.11±8.24)%, control group: (68.97±8.59)%], and the differences were statistically significant ( P<0.05). There were no significant differences in the amount of bone cement injected between the two groups ( P>0.05). There was no significant difference in the Cobb angle of kyphosis and the percentage of anterior vertebral height at each time point ( P>0.05). The incidence of bone cement leakage in the experimental group was 15.8% (15/95), slightly lower than that in the control group [22.5% (20/89)], but the difference was not statistically significant ( P>0.05). As of the final follow-up, the incidence of adjacent vertebral fracture in the experimental group was 8.4% (8/95), which was lower than that in the control group (19.1%, 17/89), and the difference was statistically significant ( P< 0.05). Conclusion:The application of bone cement mixed with artificial bone materials in PVP for OVCF, can achieve good clinical efficacy, and reduce the incidence of adjacent vertebral fractures.

Most chemical medicines have polymorphs. The difference of medicine polymorphs in physicochemical properties directly affects the stability, efficacy, and safety of solid medicine products. Polymorphs is incomparably important to pharmaceutical chemistry, manufacturing, and control. Meantime polymorphs is a key factor for the quality of high-end drug and formulations. Polymorph prediction technology can effectively guide screening of trial experiments, and reduce the risk of missing stable crystal form in the traditional experiment. Polymorph prediction technology was firstly based on theoretical calculations such as quantum mechanics and computational chemistry, and then was developed by the key technology of machine learning using the artificial intelligence. Nowadays, the popular trend is to combine the advantages of theoretical calculation and machine learning to jointly predict crystal structure. Recently, predicting medicine polymorphs has still been a challenging problem. It is expected to learn from and integrate existing technologies to predict medicine polymorphs more accurately and efficiently.
4.Clinical safety and validity analysis of retrograde new endo-scopic visual field in miniature pigs

Objective:To study the clinical safety and validity of retrograde new endoscopic field of vision in miniature pigs.Methods:6 live miniature pigs were selected as study subjects,En-doscopic Retrograde New View(ERNV)was selected.The performance,image quality and intraoper-ative and postoperative complications were evaluated.To evaluate whether all the experimental ani-mals could complete the relevant endoscopy.Verify ERNV's operating performance,including whether the duodenoscope can enter the biliary tract smoothly,and made sure whether the injection,suction,and instrument channels were unobstructed.Choledochoscope image clarity,color resolu-tion,image deformation and distortion,accurate evaluation of lumen conditions and clear observation of mucosal surface conditions were analyzed.Whether there were operant injuries such as bleeding and perforation,as well as adverse events such as respiratory depression and cardiac arrest.The sur-vival status and adverse reactions of all pigs were observed.Results:The choledochoscope was successfully inserted into the bile duct of 6 miniature pigs.The product had good operation perfor-mance and could enter the bile duct through the duodenoscope smoothly.The injection,suction and instrument channels were relatively smooth.In addition,the endoscopic images are clear,with better color resolution,and without image deformation and distortion,which can realize accurate evaluation of the conditions in the lumen and observe the mucosal surface conditions more clearly.No bile duct stenosis or dilatation occurred in all miniature pigs,and the bile duct mucosa was smooth,without hyperemia and edema,and no abnormal thickening or bending of mucous vessels.During the exami-nation,there were no operational injuries such as bleeding and perforation,and no adverse events such as respiratory depression and cardiac arrest occurred.The vital signs of all miniature pigs tended to be stable after operation,and the survival state was good,and there were no complications such as cholangitis,bleeding and perforation.Conclusion:ERNV has good clinical safety and efficacy,ex-cellent operation performance and excellent image quality,and is worthy of clinical application.

Exploring the risk "time interval window" of sequential medication of Reduning injection (RDN) and penicillin G injection (PG) by detecting the correlation between serum biochemical indexes and plasma metabonomic characteristics, in order to reduce the risk of adverse reactions caused by the combination of RDN and PG. All animal experiments and welfare are in accordance with the requirements of the First Affiliated Experimental Animal Ethics and Animal Welfare Committee of Henan University of Chinese Medicine (approval number: YFYDW2020002). The changes of biochemical indexes in serum of rats were detected by enzyme-linked immunosorbent assay. It was determined that RDN combined with PG could cause pseudo-allergic reactions (PARs) activated by complement pathway. Further investigation was carried out at different time intervals (1.5, 2, 3.5, 4, 6, and 8 h PG+RDN). It was found that sequential administration within 3.5 h could cause significant PARs. However, PARs were significantly reduced after administration interval of more than 4 h. LC-MS was used for plasma metabolomics analysis, and the levels of serum biochemical indicators and plasma metabolic profile characteristics were compared in parallel. 22 differential metabolites showed similar or opposite trends to biochemical indicators before and after 3.5 h. And enriched to 10 PARs-related pathways such as arachidonic acid metabolism, steroid hormone biosynthesis, linoleic acid metabolism, glycerophospholipid metabolism, and tryptophan metabolism. In conclusion, there is a risk "time interval window" phenomenon in the adverse drug reactions caused by the sequential use of RDN and PG, and the interval medication after the "time interval window" can significantly reduce the risk of adverse reactions.

Jumonji domain-containing protein D3(JMJD3)is a 2-oxoglutarate-dependent dioxygenase that specif-ically removes transcriptional repression marks di-and tri-methylated groups from lysine 27 on histone 3(H3K27me2/3).The erasure of these marks leads to the activation of some associated genes,thereby influencing various biological processes,such as development,differentiation,and immune response.However,comprehensive descriptions regarding the relationship between JMJD3 and inflammation are lacking.Here,we provide a comprehensive overview of JMJD3,including its structure,functions,and involvement in inflammatory pathways.In addition,we summarize the evidence supporting JMJD3's role in several inflammatory diseases,as well as the potential therapeutic applications of JMJD3 inhibitors.Additionally,we also discuss the challenges and opportunities associated with investigating the functions of JMJD3 and developing targeted inhibitors and propose feasible solutions to provide valuable insights into the functional exploration and discovery of potential drugs targeting JMJD3 for inflammatory diseases.
8.Application of Ropivacaine combined with Lidocaine for incision infiltration anesthesia in lumbar fusion surgery

Objective:To investigate the efficacy and safety of Ropivacaine combined with Lidocaine for incision infiltration anesthesia in lumbar fusion surgery.Methods:The case data of 154 patients with lumbar degenerative diseases who underwent lumbar fusion surgery at the Department of Orthopedics, Beijing Friendship Hospital, Capital Medical University from March 2021 to September 2023 were retrospectively analyzed, and the patients were divided into the experimental group ( n=72) and the control group ( n=82) according to whether or not they underwent Ropivacaine combined with Lidocaine incisional infiltration anesthesia. The experimental group was anesthetized with Ropivacaine combined with Lidocaine incisional infiltration anesthesia, and the control group was not anesthetized with incisional infiltration. The static and dynamic pain visual analog score (VAS) at six postoperative time points (2, 4, 6, 12, 24, 48 h after surgery), the application of postoperative analgesic medications, and related complications were compared between the two groups. The measurement data of normal distribution were expressed as mean±standard deviation( ± s), and t-test was used for comparison between groups, the measurement data of non-normal distribution were expressed as median (interquartile distance) [ M( Q1, Q3)], and non-parametric test was used for comparison between groups; the count data were expressed as the number of cases and percentage, and the Chi-square test was used for comparison between groups. Results:All patients underwent successful surgery, and the static [(4.40±1.67), (3.86±1.22), (3.58±1.15), (3.43±1.11) points] and dynamic [(4.56±1.69), (4.03±1.21), (3.79±1.16), (3.65±1.13) points] VAS scores of the patients in the experimental group were lower than those in the control group [static: (5.38±1.73), (5.06±1.58), (4.68±1.37), (3.82±1.22) points; dynamic: (5.55±1.62), (5.29±1.50), (4.89±1.41), (4.12±1.29) points] at 2, 4, 6, 12 h after surgery, and the differences were statistically significant ( P<0.05); at 24, 48 h after surgery, there was no significant difference in the static and dynamic VAS scores between the two groups ( P>0.05). The dosage of oral Tramadol [100(0, 100) mg] and subcutaneous injection of Morphine [0(0, 0) mg] in the experimental group at 48 h after surgery were significantly lower than those in the control group [100(100, 100), 0(0, 10) mg], and the differences were statistically significant ( P<0.05). There was no significant difference in the incidence of postoperative incision complications and cerebrospinal fluid leakage between the two groups ( P>0.05). Conclusion:Ropivacaine combined with Lidocaine for incision infiltration anesthesia in lumbar fusion surgery can effectively relieve early pain in the surgical area, reduce the use of postoperative analgesic medications, and will not increase related complications.
9.Clinical value of manual massage in treatment of grade Ⅰ internal hemorrhoids under endoscopic foam sclerotherapy

Objective To evaluate the effect of manual massage on complications after endoscopic foam sclerotherapy injection for the treatment of internal hemorrhoids.Methods Consecutive 113 patients with grade Ⅰinternal hemorrhoids were prospectively enrolled and completed endoscopic foam sclerotherapy.The patients were randomly divided into a massage group(n=65)and a control group(n=68).Massage group performed manual perianal massage,Visual analogue scale(VAS)was used to evaluate perianal pain.The postoperative bleeding,short-term and long-term efficacy were also compared.Results The median VAS of 24 h postoperation was 1.0(0.0,3.0)in massage group,which was significantly lower than 2.0(1.0,4.0)in control group,the difference was statistically significant(P=0.014).The no bleeding rate of one week postoperation was 84.6%in massage group,which was significantly higher than 64.7%in control group,the difference was statistically significant(P=0.009).After 12 weeks,6 months and 12 months of follow-up,there were no significant differences in cure rate and remission rate between the two groups(P>0.05).Conclusion Manual massage after endoscopic sclerosing agent injection is beneficial to relieve postoperative pain of grade Ⅰ internal hemorrhoids and reduce bleeding.
10.Evaluation of the Promoter Activity of the CHO Cell Expression System Based on Site-specific Integration

In industrial production,the expression level of drug proteins in Chinese hamster ovary cells(CHO)is influenced by many factors:the regulatory elements on transcription and translation,the ge-nomic integration sites,and the expression system.Transcription,as the first step of gene expression,largely affects protein expression,and the promoter plays a crucial role in the initiation of transcription.Most of the promoters were screened through transient transfection or random integration,but the presence of unclear copy number or random integration sites makes it difficult to accurately evaluate the promoter activity.To some extent,site-specific integration can reduce the impact of positional effects on exogenous genes and may potentially increase the expression level of exogenous genes.In the early stage of our re-search,multiple sites that can stably express exogenous proteins were identified and verified in the CHO cell genome.In this study,one of these sites(2c6)was selected for the evaluation of promoter activity.The CRISPR/Cas9 gene editing technique was used to site-specifically integrate the reporter gene(EG-FP)regulated by the simian virus early promoter(SV40),mouse elongation factor-1α(mEF-1α),chicken β-actin(cACTB)promoter,and human phosphoglycerate kinase promoter(hPGK)into the 2c6 site,respectively.The mean fluorescence intensity of the cells was analyzed by flow cytometry,and the mRNA level of EGFP was detected by qPCR to comprehensively evaluate the activity of the promoter.The results showed that the activities of the mEF-1α and mACTB promoters were better than those of SV40 and hPGK.The results of the secondary flow cytometry sorting showed that site-specific integration can more accurately evaluate the activity of the promoter in the CHO cell expression system.
