1.Mechanism of Action of Huangqi Guizhi Wuwutang Against Cerebral Ischemia-reperfusion Injury Based on Bioinformatics and Experimental Validation

ObjectiveTo investigate the mechanism of action of Huangqi Guizhi Wuwutang (HGWT) against cerebral ischemia-reperfusion injury (CIRI) based on bioinformatics and experimental validation. MethodsBiological informatics methods were used to screen for active components of HGWT and their targets. The GEO database was utilized to obtain CIRI-related differentially expressed genes (DEGs), and platforms such as GeneCards were used to identify disease targets. Venn diagram analysis was conducted to identify overlapping targets, followed by protein-protein interaction (PPI), gene ontology (GO), and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis, as well as immune infiltration and immune cell differential analysis. Core genes (Hub genes) were screened using LASSO regression and ROC curves, and molecular docking was used to validate the binding efficiency between the active components of the drug and the core targets. A rat CIRI model was established, with rats randomly divided into five groups (n=10): Sham surgery group (Sham), model group (MG), and low-dose (LD,5.3 g·kg-1), medium-dose (MD,10.6 g·kg-1), and high-dose (HD,21.2 g·kg-1) HGWT groups. From 3 days before modeling to 7 days after surgery, oral administration was performed daily: Sham and MG groups received physiological saline, while each drug group received the corresponding dose of HGWT. Hematoxylin-eosin (HE) staining, Nissl staining, and terminal deoxynucleotidyl transferase-mediated dUTP nick-end labeling (TUNEL staining) were used to assess the repair effects of HGWT on neural damage. Western blot analysis was used to detect B-cell lymphoma-2 protein (Bcl-2), Bcl-2-associated X protein (Bax), signal transducer and activator of transcription 3 (STAT3), phosphorylated STAT3 [p-STAT3 (Tyr705)], protein kinase B1 (Akt1), and phosphorylated Akt1 [p-Akt1 (Ser473)], among other target proteins. ResultsAfter screening, 56 common target points of DEGs-disease-drug were obtained. GO and KEGG analyses indicated that HGWT primarily functions in pathways such as apoptosis, oxidative stress, and inflammatory responses. Immune infiltration analysis revealed a significant association between HGWT's anti-CIRI activity and immune cells such as Th17 cells and myeloid-derived suppressor cells (MDSCs) (P0.01). LASSO-ROC analysis identified Akt1, Caspase-3, glycogen synthase kinase-3β (GSK-3β), and STAT3 as core genes. Molecular docking confirmed that Hub genes exhibit significant binding affinity with the active components of HGWT (binding energy ≤ -5 kJ·mol-1)(1 cal≈4.186 J). Animal experiment results showed that compared with the sham group, the MG group exhibited significant neuronal necrosis, nuclear condensation, and vacuolar degeneration in rat brains, with a significant decrease in Nissl body density (P0.01) and increased neuronal apoptosis in rat brains as indicated by TUNEL staining (P0.01). Compared with the MG, the LD, MD, and HD groups showed reduced neuronal necrosis, nuclear condensation, and vacuolar degeneration in rat brain neurons, increased Nissl body density, and reduced apoptosis (P0.01), with significant differences among the drug groups (P0.01). Western blot results showed that compared with the sham group, the MG group had reduced Bcl-2 and p-Akt1 (P0.01) and increased Bax and p-STAT3 (P0.01). Compared with the MG group, the drug groups showed increased Bcl-2 and p-Akt1 (P0.01) and decreased Bax and p-STAT3 (P0.01). There were no significant changes in total Akt1 and STAT3 protein levels among the groups. ConclusionBased on network pharmacology and experimental verification, HGWT may exert its neuroprotective effects by regulating the phosphorylation levels of Akt1 and STAT3, thereby alleviating cell apoptosis, inflammatory responses, and oxidative stress in rat brain tissue following CIRI. This provides theoretical support for the clinical treatment of CIRI.

Analgesia is an important link in the treatment of severe patients after neurosurgery and plays a vital role in improving the prognosis of the patients.Understanding the status quo and influencing fac-tors of pain in severe patients after neurosurgery helps to predict the occurrence of pain,which is crucial for determining the new pain assessment methods and auxiliary analgesic methods and developing novel analgesic drugs.This paper reviews the pain status,pain evaluation and analgesic methods of severe patients after neuro-surgery in recent years so as to understand the pain management current status of the patients with severe neurological conditions and provide reference for the medical staff to implement the analgesic programs.
3.Progress in prevention and treatment of hypoxemia during painless gastrointestinal endoscopy

The goal of gastrointestinal endoscopy anesthesia management is to effectively calm and re-lieve pain while minimizing related adverse reactions and ensuring patient safety.Hypoxemia is the most com-mon adverse event during painless gastrointestinalendoscopy,and severe hypoxemia can cause cardiac and brain accidents.Therefore,how to prevent and reduce the occurrence of hypoxemia isa hot topic in clinical re-search.This article reviews the methods of preventing and reducing hypoxemia in general painless gastrointes-tinal endoscopy,and provides a reference for the selection of appropriate sedation and ventilation strategies for general painless gastrointestinal endoscopy anesthesia.
4.Clinical value of abdominal adipose volume in predicting early tumor recurrence after resec-tion of hepatocellular carcinoma

Objective:To investigate the clinical value of abdominal adipose volume in predicting early tumor recurrence after resection of hepatocellular carcinoma (HCC).Methods:The retrospective case-control study was conducted. The clinicopathological data of 132 HCC patients with tumor diameter ≤5 cm who were admitted to The First Affiliated Hospital of Army Medical University from December 2017 to October 2019 were collected. There were 110 males and 22 females, aged (51±4)years. All patients underwent resection of HCC. Preoperative computer tomography scanning was performed and the visceral and subcutaneous fats of patients were quantified using the Mimics Research 21.0 software. Based on time to postoperative tumor recurrence patients were divided to two categories: early recurrence and non-early recurrence. Observation indicators: (1) consistency analy-sis; (2) analysis of factors influencing early tumor recurrence after resection of HCC and construction of prediction model. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the t test. Measurement data with skewed distribu-tion were represented as M( Q1,Q3) or M(range), and comparison between groups was conducted using the Mann-Whitney U test. Count data were expressed as absolute numbers, and comparison between groups was conducted using the chi-square test or Fisher exact probability. Consistency analysis was conducted using the intragroup correlation coefficient (ICC) test. Multivariate analysis was performed using the binary Logistic regression model forward method. Independent risk factors influencing early tumor recurrence after resection of HCC were screened. The area under curve (AUC) of receiver operating characteristic (ROC) curve was applied to select the optimal cut-off value to classify high and low risks of recurrence. The Kaplan-Meier method was used to draw survival curve and calculate survival time. The Log-Rank test was used for survival analysis. Results:(1) Consistency analysis. The consistency ICC of abdominal fat parameters of visceral fat volume (VFV), subcutaneous fat volume, visceral fat area, and subcutaneous fat area measured by 2 radiologists were 0.84, 1.00, 0.86, and 0.94, respectively. (2) Analysis of factors influencing early tumor recurr-ence after resection of HCC and construction of prediction model. All 132 patients were followed up after surgery for 662(range, 292-1 111)days. During the follow-up, there were 52 patients with non-early recurrence and 80 patients with early recurrence. Results of multivariate analysis showed that VFV was an independent factor influencing early tumor recurrence after resection of HCC ( odds ratio=4.07, 95% confidence interval as 2.27-7.27, P<0.05). The AUC of ROC curve based on VFV was 0.78 (95% confidence interval as 0.70-0.85), and the sensitivity and specificity were 72.2 % and 77.4 %, respectively. The optimal cut-off value of VFV was 1.255 dm 3, and all 132 patients were divided into the high-risk early postoperative recurrence group of 69 cases with VFV >1.255 dm 3, and the low-risk early postoperative recurrence group of 63 cases with VFV ≤1.255 dm 3. The disease-free survival time of the high-risk early postoperative recurrence group and the low-risk early post-operative recurrence group were 414(193,702)days and 1 047(620,1 219)days, showing a significant difference between them ( χ2=31.17, P<0.05). Conclusions:VFV is an independent factor influen-cing early tumor recurrence of HCC after resection. As a quantitative indicator of abdominal fat, it can predict the prognosis of HCC patients.
5.Misdiagnosis of intrapancreatic accessory spleen: A report of two cases

Accessory spleen refers to the spleen tissue that exists outside of the normal spleen, with a similar structure to the main spleen and certain functions. Intrapancreatic accessory spleen (IPAS) completely enveloped by the pancreas has an incidence rate of only 2%, and it is easily misdiagnosed in clinical practice due to its atypical clinical symptoms and similar radiological features to pancreatic neuroendocrine tumor, pancreatic solid pseudopapillary tumor, and other pancreatic space-occupying lesions. This article reports the clinical data of two patients with IPAS who were misdiagnosed as pancreatic neuroendocrine tumor and pancreatic solid pseudopapillary tumor, respectively, analyzes the reasons for misdiagnosis, and summarizes the experience in diagnosis and treatment, in order to improve the ability for the differential diagnosis of IPAS in clinical practice.
6.Pathological diagnosis of thyroid cancer histopathological image from intraoperative frozen sections based on deep transfer learning

Purpose To explore the artificial intelligence(AI)-assisted diagnosis system of thyroid cancer based on deep transfer learning and evaluate its clinical application value.Methods The HE sections of 682 cases thyroid disease patients(including benign lesions,papillary carcinoma,follicular carci-noma,medullary carcinoma and undifferentiated carcinoma)in the Pathology Department of the Renmin Hospital of Wuhan Uni-versity were collected,scanned into digital sections,divided into training sets and internal test sets according to the ratio of 8 ∶ 2,and the training sets were labeled at the pixel level by patholo-gists.The thyroid cancer classification model was established u-sing VGG image classification algorithm model.In the process of model training,the parameters of the breast cancer region recog-nition model were taken as the initial values,and the parameters of the thyroid cancer region recognition model were optimized through the transfer learning strategy.Then the test set and 633 intraoperative frozen HE section images of thyroid disease in Jianli County People's Hospital,Jingzhou City,Hubei Province wereused as the external test set to evaluate the performance of the established AI-assisted diagnostic model.Results In the internal test set,without the use of the breast cancer region rec-ognition model transfer learning,the accuracy of the AI-assisted diagnosis model was 0.882,and the area under the Receiver op-erating characteristic(AUC)valuewas0.938;However,inthe use of the Transfer learning model,the accuracy of the AI-assis-ted diagnosis model for was 0.926,and the AUC value was 0.956.In the external test set,the accuracy of the zero learning model was 0.872,the AUC value was 0.915,and the accuracy of the Transfer learning model was 0.905,the AUC value was 0.930.Conclusion The AI-assisted diagnosis method for thy-roid cancer established in this study has good accuracy and gen-eralization.With the continuous development of pathological AI research,transfer learning can help improve the performance and generalization ability of the model,and improve the accura-cy of the diagnostic model.
7.Analysis of typical experiences of public hospital salary system reform in Sichuan province

Objective:To analyze the experiences and practice in the reform of public hospital salary system in Sichuan province, summarize the typical modes of such reform in the province, and provide references for further reform.Methods:As of October 29, 2021, the research group received 77 sets of typical experience materials submitted by the health commissions and public hospitals in Sichuan province on enforcing the reform of the public hospital salary system. The analysis framework was based on the five main elements proposed in the Guidance to Deepening the Reform of the Salary System of Public Hospitals for the purpose of furthering the reform. These five elements refer to " reasonably determining the level of salary in public hospitals" " fully implementing the autonomy of internal distribution in public hospitals " " establishing and improving the incentive and restraint mechanism for the remuneration of public hospital leaders" " improving the assessment and evaluation mechanism oriented to public welfare" and " funding sources ". A quantitative analysis was made on the typical experience materials using the social network analysis method, while a qualitative analysis was made on the typical experience materials using the content analysis method. Results:The results of social network analysis showed that the network density was 0.272; the highest point centrality was " fully implement the autonomy of internal distribution in public hospitals" (0.935), and the highest intermediary centrality was " improving the assessment and evaluation mechanism oriented to public welfare" (0.870), while the closeness to centrality of " establishing and improving the incentive and constraint mechanism for the salary of public hospital leaders" (0.434) and " funding sources" (0.421) were relatively low. The results of content analysis showed that the ones with higher frequency among all the typical experience materials were " fully implementing the autonomy of internal distribution of hospitals" (72 times) and " improving the assessment and evaluation mechanism oriented to public welfare" (67 times), while the ones with lower frequency were " establishing and improving the salary incentive and constraint mechanism for public hospital leaders" (17 times) and " funding sources" (14 times). In terms of unity and synergy, the typical models of public hospital salary system reform in the province could be categorized as the fine standard mode, the fair value mode, the autonomous synergy mode and the circular symbiosis mode.Conclusions:Deepening the reform of the salary system of public hospitals should unify the standards and improve the fair and refined assessment and evaluation mechanism; explore various forms of distribution and build an internal autonomous and synergistic incentive mechanism; pay attention to the weak remuneration incentive mechanism for hospital leaders and the problem of a relatively single source of funding.

OBJECTIVES:
To investigate the clinical treatment outcomes and the changes of the outcomes over time in extremely preterm twins in Guangdong Province, China.
METHODS:
A retrospective analysis was performed for 269 pairs of extremely preterm twins with a gestational age of <28 weeks who were admitted to the department of neonatology in 26 grade A tertiary hospitals in Guangdong Province from January 2008 to December 2017. According to the admission time, they were divided into two groups: 2008-2012 and 2013-2017. Besides, each pair of twins was divided into the heavier infant and the lighter infant subgroups according to birth weight. The perinatal data of mothers and hospitalization data of neonates were collected. The survival rate of twins and the incidence rate of complications were compared between the 2008-2012 and 2013-2017 groups.
RESULTS:
Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of severe asphyxia and smaller head circumference at birth (P<0.05). The mortality rates of both of the twins, the heavier infant of the twins, and the lighter infant of the twins were lower in the 2013-2017 group compared with the 2008-2012 group (P<0.05). Compared with the 2008-2012 group, the 2013-2017 group (both the heavier infant and lighter infant subgroups) had lower incidence rates of pulmonary hemorrhage, patent ductus arteriosus (PDA), periventricular-intraventricular hemorrhage (P-IVH), and neonatal respiratory distress syndrome (NRDS) and a higher incidence rate of bronchopulmonary dysplasia (P<0.05).
CONCLUSIONS
There is a significant increase in the survival rate over time in extremely preterm twins with a gestational age of <28 weeks in the 26 grade A tertiary hospitals in Guangdong Province. The incidences of severe asphyxia, pulmonary hemorrhage, PDA, P-IVH, and NRDS decrease in both the heavier and lighter infants of the twins, but the incidence of bronchopulmonary dysplasia increases. With the improvement of diagnosis and treatment, the multidisciplinary collaboration between different fields of fetal medicine including prenatal diagnosis, obstetrics, and neonatology is needed in the future to jointly develop management strategies for twin pregnancy.

9.Bone Marrow Osteoblasts Promotes the Proliferation Leukemia Stem Cell by Up-regulating Interleukin-1.

OBJECTIVE:
To explore the extrinsic regulation mechanism of bone marrow microenvironment in leukemia cells, and investigate the promoting effect of osteoblast niche on the proliferation and self-renewal of leukemia stem cell by up-regulating the expression of interleukin-1 (IL-1) in leukemia cell.
METHODS:
The gene expression profiles on leukemia cells derived from AE9a mouse bone marrow endosteum and central bone marrow were determined by RNA sequencing and gene set enrichment analysis (GSEA). Quantitative real-time PCR (qRT-PCR) was used to detect the expression of IL-1 in AE9a mouse leukemia cells co-cultured with or without osteoblasts in vitro. In addition, qRT-PCR was also used to determine the expression of IL-1 in bone marrow mononuclear cell (BMMNC) from 43 patients with acute myeloid leukemia (AML). For leukemia cells co-cultured with osteoblasts or treated with IL-1β, colony forming ability of AE9a leukemia cells was determined by colony formation assay.
RESULTS:
In AE9a leukemia mouse, RNA-seq data and GSEA showed that the enrichment of the upregulated genes in leukemia cells located in endosteum fell into inflammatory response gene set, among them, IL-1α and IL-1β were significantly higher expressed in AE9a leukemia cells that located osteoblast niche (IL-1α: P<0.001, IL-1β:P<0.001). After AE9a leukemia cells were co-cultured with osteoblasts in vitro, the expression of IL-1α and IL-1β in leukemia cells were increased by 2.5 and 3.5 times respectively. In colony formation assay, the number of colonies was increased significantly after leukemia cells were co-cultured with osteoblasts (P<0.001). In addition, when AE9a leukemia cells were treated with IL-1β, the number of colonies was also increased significantly (P<0.01). In AML patients, BMMNC with high percentage of CD34 positive cells exhibited higher level of IL-1 expression.
CONCLUSION
Osteoblast niche can promote leukemia cell proliferation and self-renewal through up-regulating the expression of IL-1 in leukemia cells. In AML patients, the expression level of IL-1 was correlated to the percentage of CD34 positive cells in BMMNC.

10.Preparation of Rehmanniae Radix Juice for Baihe Dihuangtang Based on UPLC-Q-TOF-MS

ObjectiveTo compare the four preparation methods of Rehmanniae Radix juice described in ancient literature and find the method that is most suitable for the preparation of Rehmanniae Radix juice used in Baihe Dihuangtang. MethodThe ancient medical books record four methods for preparing Rehmanniae Radix juice: crushing fresh Rehmanniae Radix for juice, steaming fresh Rehmanniae Radix for juice, boiling fresh Rehmanniae Radix for juice, and boiling dry Rehmanniae Radix for juice. Ultra performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF-MS) was employed to detect the compounds in the four juice samples, followed by principal component analysis (PCA). Result① Totally 27 compounds were identified in the juice samples, including 10 iridoid glycosides, 14 phenylethanoid glycosides, 2 phenolic acids, and 1 irisone. Among them, 15 common compounds were shared by the four juice samples, including 7 iridoid glycosides, 7 phenylethanoid glycosides, and 1 phenolic acid. ② Five common compounds in the four juice samples can be matched with the reference standards, which were catalpol, aucubin, rehmannioside D, ajugol, and purpureaside C. ③ Verbascoside and isoacteoside were not detected in the juice prepared by crushing fresh Rehmanniae Radix, while it was detected in the other three juice samples, which indicated that the two components were produced after heating rather than being the original components in fresh Rehmanniae Radix. ④ The comparison of the ion fragments demonstrated that verbascoside was produced from purpureaside C after the cleavage of the glycosidic bond and removal of a molecule of mannose. ⑤ Isoacteoside could be isomerized from verbascoside, and its relative content increased with the extension of heating time. However, the relative content of verbascoside and purpureaside C did not decrease significantly. Therefore, it was hypothesized that purpureaside C was produced from its upstream component. ConclusionThe juice prepared by crushing fresh Rehmanniae Radix has the chemical composition significantly different from the juice samples prepared with the other 3 methods, while the latter 3 juice samples had similar chemical composition. Although all the four methods can be used, it is more suitable to prepare Rehmanniae Radix juice by steaming fresh Rehmanniae Radix, boiling fresh Rehmanniae Radix, and boiling dry Rehmanniae Radix.
