1.Effects of isocaloric high-fat diet on energy metabolism and endurance exercise capacity in SD rats

		                        			Objective To investigate the effects of a 45%high-fat diet(HFD)with isocaloric intake on energy metabolism and endurance exercise capacity in SD rats.Methods Twenty-four male SD rats were randomly divided into normal chow diet group(CON),HFD group,normal chow diet+exercise training group(CONT),and HFD+exercise training group(HFDT).The CON and CONT groups received normal chow diet,while the HFD and HFDT groups received a 45%high-fat diet with isocaloric intake.The HFDT and CONT groups underwent an endurance training of moderate-intensity running for 6 weeks.Body weight,fat mass,and lean mass were measured weekly.Energy expenditure and basal metabolic rate during rest and exercise states were measured using Pheno Master/Calo Treadmill system.Blood glucose,lipids,and creatine kinase levels were detected after the exhaustion test.Results In 6 weeks after intervention,the endurance exercise capacity was significantly enhanced in the HFDT group than the CONT group(P<0.05).There were no obvious differences in body weight and body composition among the groups under isoenergetic feeding conditions.At rest,no statistical differences were observed in total energy expenditure and basal metabolic rate among the groups.However,prior to the 4th week,the CON group primarily metabolized carbohydrates while the HFD group primarily metabolized fats.But the carbohydrate metabolism was decreased and then increased,and the substrate metabolism rates eventually reached similar levels between the 2 groups on the 5th to 6th week.The HFDT group primarily metabolized fats while the CONT group primarily metabolized carbohydrates,with significant differences persisting after 6 weeks of training(P<0.05).HFD led to elevated levels of serum cholesterol,triglycerides(TG),and high-density lipoprotein cholesterol(HDL-C),but,endurance training resulted in decreased lipid levels in the HFDT group,accompanied by an increase inβ-hydroxybutyrate(βHB)level(P<0.05).Isoenergetic diets had no significant differences in their effects on liver and kidney function or muscle damage indicators.Conclusion An isoenergetic HFD can improve fat utilization ability and extend endurance exercise time in rats without altering body composition or affecting liver and kidney function.
		                        		
		                        		
		                        		
		                        	
2.Efficient production of L-asparaginase in Bacillus licheniformis by optimizing expression elements and host.

		                        			L-asparaginase (L-ASN) is widely applied in the treatment of malignant tumor and low-acrylamide food production, however, the low expression level hampers its application. Heterologous expression is an effective strategy to increase the expression level of target enzymes, and Bacillus is generally used as the host for efficient production of enzymes. In this study, the expression level of L-asparaginase in Bacillus was enhanced through optimization of expression element and host. Firstly, five signal peptides (SPSacC, SPAmyL, SPAprE, SPYwbN and SPWapA) were screened, among which SPSacC showed the best performance, reaching an activity of 157.61 U/mL. Subsequently, four strong promoters (P43, PykzA-P43, PUbay and PbacA) from Bacillus were screened, and tandem promoter PykzA-P43 showed the highest yield of L-asparaginase, which was 52.94% higher than that of control strain. Finally, three Bacillus expression hosts (B. licheniformis Δ0F3 and BL10, B. subtilis WB800) were investigated, and the maximum L-asparaginase activity, 438.3 U/mL, was reached by B. licheniformis BL10, which was an 81.83% increase compared with that of the control. This is also the highest level of L-asparaginase in shake flask reported to date. Taken together, this study constructed a B. licheniformis strain BL10/PykzA-P43-SPSacC-ansZ capable of efficiently producing L-asparaginase, which laid the foundation for industrial production of L-asparaginase.

		                        			Pulmonary inflammatory myofibroblastic tumor (PIMT) is a rare lung tumor that is poorly understood by clinicians. Based on the data of patients with PIMT in our center, a comprehensive search and a brief summary analysis of previous reports of the disease were carried out in this article. PIMTs were more likely to be present in male patients and in the right lung, and their clinical and radiographic findings were heterogeneous. Surgery is the most important treatment method, and complete resection is of great significance to the prognosis of patients. 4.Peripheral Blood Motilin,Neuropeptide Y and Leptin Levels in Patients with Functional Dyspepsia

		                        			Background:The pathophysiology of functional dyspepsia(FD)is complicated and unclarified yet. Gastrointestinal hormone dysfunction may contribute to the development of FD. Aims:To investigate the correlation of different subtypes of FD with gastrointestinal hormone motilin(MTL),neuropeptide Y(NPY)and leptin(LEP). Methods:A total of 57 FD patients fulfilling Rome Ⅲ criteria were recruited and divided into epigastric pain syndrome(EPS)group(n = 24)and postprandial distress syndrome( PDS) group( n = 33 ). Ten healthy volunteers were served as controls. Fast and postprandial levels of MTL,NPY and LEP in peripheral blood were detected by radioimmunoassay. Results:Peripheral levels of fast MTL in EPS and PDS groups[(182. 90 ± 108. 57)pg/ mL and(145. 21 ± 67. 18)pg/ mL vs.(224. 47 ± 64. 55)pg/ mL,P < 0. 05],fast NPY in EPS group[(57. 40 ± 28. 75)pg/ mL vs.(90. 75 ± 49. 57)pg/ mL,P < 0. 01], and fast and postprandial NPY in PDS group[fast level:(38. 25 ± 20. 66)pg/ mL vs. (90. 75 ± 49. 57)pg/ mL,P <0. 01;postprandial level:(30. 26 ± 15. 12)pg/ mL vs.(65. 23 ± 54. 42)pg/ mL,P < 0. 01]were significantly lower than those in control group at same time points,especially the PDS group. Fast and postprandial levels of NPY were significantly lower in PDS group than in EPS group at same time points( P < 0. 05). No significant differences were observed in peripheral LEP levels among the three groups at any time points(P > 0. 05). Conclusions:The pathogenic mechanism of FD is related to the level of gastrointestinal hormones,and is different in EPS and PDS subtypes. 5.Diagnostic values of salivary versus and plasma microRNA-21 for early esophageal cancer.

OBJECTIVETo evaluate the diagnostic value of salivary and plasma miR-21 in patients with esophageal cancer (EC).
METHODSTotal RNA was extracted from saliva and plasma samples from 50 stage I and 50 stage II patients with EC and 50 healthy controls for measurement of miR-21 levels using qPCR. The diagnostic values of salivary and plasma miR-21 levels were assessed for stage I, stage II, and stage I+II EC.
RESULTSSalivary and plasma miR-21 were significantly higher in the EC patients than in the control group. The diagnostic sensitivities of plasma miR-21 for stage I, stage II, and stage I+II EC were 96%, 64% and 97%, with specificities of 44%, 84%, and 56%, respectively; the sensitivities of salivary miR-21 were 90%, 88%, and 89%, respectively, with the same specificities of 64%. Regardless of EC staging, the expression of plasma miR-21 showed a significant positive correlation with that of salivary miR-21, and their diagnostic values were comparable.
CONCLUSIONBoth salivary and plasmatic miR-21 can be sensitive biomarkers for EC, and salivary miR-21 detection has the potential to replace plasma detection for EC diagnosis.

		                        			Objectives To study the effect of Lactulose oral solution combined with polyethylene glycol electrolyte powder on the intestinal cleanness.Methods One hundred and twenty patients undergoing electronic colonoscopy were divided into the experiment group and the control group,60 cases in each group.The experiment group orally took Lactulose oral solution and polyethylene glycol electrolyte powder 1d before the check,while the control group polyethylene glycol electrolyte powder 1d before the check.The two groups were compared in terms of intestinal cleanness and adverse reactions.Results The experiment group was significantly better than the control group in intestinal cleanness(P>0.05)and there were no significant difference has in adverse reactions(P>0.05).Conclusion The combined use of polyethylene glycol electrolyte powder and Lactulose Oral Solution may improve the intestinal cleanness and reduce the incidence of adverse reactions.
		                        		
		                        		
		                        		
		                        	
9.Effect of improved posterolateral minimally invasive approach combined with modular femoral head prosthesis in treatment of femoral neck fractures in elderly patients

		                        			Objective To compare the clinical effects of improved posterolateral minimally invasive approach and traditional posterolateral approach combined with modular femoral head prosthesis in the treatment of femoral neck fractures in the elderly. Methods The study involved 70 patients with femoral neck fractures (26 males and 44 females; at age range of 67-95 years,mean 78.3 years) treated from October 2008 to June 2010.There were 62 patients with fresh femoral neck fractures and eight with old femoral neck fractures (2-4 months post-injury),all of whom were type Ⅲ or Ⅳ fractures according to the Garden' s classification.All surgeries were completed by the same operation group.The improved posterolateral minimally invasive approach group involved 32 patients including 12 males and 20 females and the traditional posterolateral approach group involved 38 patients including 14 males and 24 females.The two groups were compared in aspects of incision size,operative time,perioperative complications,prosthesis stability and hip functional recovery. Results All patients were followed up for an average of 16 weeks (range,12-18 weeks).The differences between the two groups were significant regarding the incision length,perioperative blood loss,and blood product transfusio amount (P <0.01 ),but insignificant in operative time ( P > 0.05 ).All patients were able to take weight-bearing walk six weeks postoperatively.The two groups showed significant difference in the hip joint function by Chamley's criteria six weeks postoperatively ( P < 0.05 ),but insignificant difference in the hip joint function by Harris' s criteria at the last follow-up ( P < 0.05). Conclusion The modified posterolateral minimally invasive approach combined with modular femoral head prosthesis is a satisfactory method in treatment of femoral neck fractures in senile patients,for it can result in less intra-operative soft tissue damage and blood loss,as well as shorter rehabilitation time.
		                        		
		                        		
		                        		
		                        	
10.Application of new proximal femoral nail antirotation in treatment of subtrochanteric fractures

		                        			ObjectiveTo evaluate the clinical outcome of new proximal femoral nail antirotation (PFNA) in treatment of subtrochanteric fractures. MethodsThere were 31 patients with traumatic subtrochanteric fractures intramedullarily fixed with PFNA. The patients included 14 males and 17 fe-males at a mean age of 54.6 years (range 34-90 years). According to Seinsheimer classification of sub-trochanteric fractures, 14 patients were with type Ⅱ fractures (including four patients with type ⅡA,seven with type ⅡB and three with type ⅡC), 12 with type Ⅲ fractures (including eight with type ⅢA and four with type ⅢB), two with type Ⅳ fractures and three with type Ⅴ fractures. Close reduction was performed under traction and C-arm fluoroscopy in 26 patients and open reduction through a mini-incision before inserting the nail in five patients due to difficult close reduction. ResultsThe operation lasted for a mean time of 72.5 minutes (range 45-120 minutes), with mean blood loss of 127.5 ml (range 100-350 ml). Of all, 27 patients (87.1%, 27/31) were followed up for a mean duration of 14.3 months (range 8-24 months), which showed fracture union in all patients, with a mean union period of 17.4 weeks (range 10-21 weeks). According to Harris Hip Rate Scale, the results were. excellent in 21 pa-tients, good in two and fair in four, with excellence rate of 85.2% (23/27). ConclusionsPFNA is an effective device for treatment of subtrochanteric fracture, with a high union rate and a low complication rate, for it has advantages of easy use and minor trauma, low bleeding and stable fixation in operation.
