1.Clinical and pathological characteristics analysis of benign pulmonary nodules clinically highly suspected as malignant: A retrospective cohort study

Objective To discuss the main pathological types and imaging characteristics of pulmonary nodules that are highly suspected to be malignant in clinical practice but are pathologically confirmed to be benign. Methods A retrospective analysis was performed on the clinical data of patients with pulmonary nodules who were initially highly suspected of malignancy but were subsequently pathologically confirmed to be benign. These patients were treated at the First Affiliated Hospital of Xiamen University from December 2020 to April 2023. Based on the outcomes of preoperative discussions, the patients were categorized into a benign group and a suspicious malignancy group. The clinical data and imaging characteristics of both groups were compared. Results A total of 232 patients were included in the study, comprising 112 males and 120 females, with a mean age of (50.7±12.0) years. Among these, 127 patients were classified into the benign group, while 105 patients were categorized into the suspicious malignancy group. No statistically significant differences were observed between the two groups regarding age, gender, symptoms, smoking history, or tumor history (P>0.05). However, significant differences were noted in nodule density, CT values, margins, shapes, and malignant signs (P<0.05). Further analysis revealed that in the suspicious malignancy group, solid nodules were predominantly characterized by collagen nodules and fibrous tissue hyperplasia (33.3%), followed by tuberculosis (20.4%) and fungal infections (18.5%). In contrast, non-solid nodules were primarily composed of collagen nodules and fibrous tissue hyperplasia (41.2%) and atypical adenomatous hyperplasia (17.7%). Conclusion Benign pulmonary nodules that are suspected to be malignant are pathologically characterized by the presence of collagen nodules, fibrous tissue hyperplasia, tuberculosis, atypical adenomatous hyperplasia, and fungal infections. Radiologically, these nodules typically present as non-solid lesions and may exhibit features suggestive of malignancy, including spiculation, lobulation, cavitation, and pleural retraction.
2.Construction of evaluation index system of infectious disease prevention and control ability in colleges and universities

Objective:
To construct a scientific and perfect evaluation index system of infectious disease prevention and control ability in colleges and universities, so as to provide reference tools for colleges and universities to effectively respond to infectious disease.
Methods:
The initial framework of the evaluation index system of infectious disease prevention and control ability in colleges and universities was constructed by using literature analysis method. Experts familiar with infectious disease prevention and control or school health work were selected to conduct two rounds( n =16,18) of Delphi expert consultation for determining the evaluation index system. Analytical hierarchy process was used to calculate the index weights and combined weights. About 198 prevention and control personnel were conveniently selected from 3 universities in Inner Mongolia Autonomous Region to comprehensively evaluate the evaluation indicators by using fuzzy comprehensive evaluation method.
Results:
After two rounds of Delphi consultation questionnaire, the effective recovery rates were 80.0% and 90.0%, the expert authority levels were 0.89 and 0.86, the expert harmony coefficients for Kendall W were 0.166 and 0.310, and the variation coefficient of each index was <0.25. Finally, the evaluation index system of infectious disease prevention and control ability of colleges and universities included 4 first level indicators, 14 second level indicators and 75 third level indicators. The weights of prevention and monitoring and early warning, organizational system guarantee, emergency management, rehabilitation and summary were 0.176, 0.476, 0.268 and 0.080, respectively. The top 3 weights of the secondary indexes were 0.623 for infectious disease surveillance and early warning, 0.595 for loss assessment and 0.370 for emergency response. The score of fuzzy comprehensive evaluation of the index system of infectious disease prevention and control ability in colleges and universities was 79.148, suggesting a high level.
Conclusion
The established evaluation index system of infectious disease prevention and control ability in colleges and universities is scientific and reasonable, which is conducive to provide tool reference for the evaluation of infectious disease prevention and control ability in colleges and universities.
3.Efficacy and safety of CM310 in moderate-to-severe atopic dermatitis: A multicenter, randomized, double-blind, placebo-controlled phase 2b trial

Background::Atopic dermatitis (AD) affects approximately 10% of adults worldwide. CM310 is a humanized monoclonal antibody targeting interleukin-4 receptor alpha that blocks interleukin-4 and interleukin-13 signaling. This trial aimed to evaluate the efficacy and safety of CM310 in Chinese adults with moderate-to-severe AD.Methods::This multicenter, randomized, double-blind, placebo-controlled, phase 2b trial was conducted in 21 medical institutions in China from February to November 2021. Totally 120 eligible patients were enrolled and randomized (1:1:1) to receive subcutaneous injections of 300 mg CM310, 150 mg CM310, or placebo every 2 weeks for 16 weeks, followed by an 8-week follow-up period. The primary endpoint was the proportion of patients achieving ≥75% improvement in the Eczema Area and Severity Index (EASI-75) score from baseline at week 16. Safety and pharmacodynamics were also studied.Results::At week 16, the proportion of EASI-75 responders from baseline was significantly higher in the CM310 groups (70% [28/40] for high-dose and 65% [26/40] for low-dose) than that in the placebo group (20%[8/40]). The differences in EASI-75 response rate were 50% (high vs. placebo, 95% CI 31%–69%) and 45% (low vs. placebo, 95% CI 26%–64%), with both P values <0.0001. CM310 at both doses also significantly improved the EASI score, Investigator’s Global Assessment score, daily peak pruritus Numerical Rating Scale, AD-affected body surface area, and Dermatology Life Quality Index compared with placebo. CM310 treatment reduced levels of thymus and activation-regulated chemokine, total immunoglobulin E, lactate dehydrogenase, and blood eosinophils. The incidence of treatment-emergent adverse events (TEAEs) was similar among all three groups, with the most common TEAEs reported being upper respiratory tract infection, atopic dermatitis, hyperlipidemia, and hyperuricemia. No severe adverse events were deemed to be attributed to CM310. Conclusion::CM310 at 150 mg and 300 mg every 2 weeks demonstrated significant efficacy and was well-tolerated in adults with moderate-to-severe AD.Trial Registration::ClinicalTrials.gov, NCT04805411.
4.Clinical Characterization of Neurobrucellosis in Kashi Region

[Objective]To explore the clinical characteristics of neurobrucellosis in Kashi,Xinjiang Uygur Autono-mous Region,thus improve the diagnosis and treatment.[Methods]A retrospective analysis was conducted on the clinical data of 18 cases of neurobrucellosis who were admitted to the First People's Hospital of Kashi Prefecture between Decem-ber 2019 and January 2024.[Results]The study included 9 males and 9 females,with a median age of 36 years(range:17-54.5).A clear epidemiological history was found in all the 18 brucllosis patients,12 of whom presented with meningo-encephalitis,5 meningitis,and 1 encephalitis.Two comorbided with spinal meningitis,2 osteoarthritis and 1 epididymitis.Most frequently reported clinical symptoms were headache,fever and fatigue.The prevalence rates of brucellosis by rose bengal plate agglutination test(RBPT)and serum agglutination test(SAT)were 11/12 and 8/9,respectively.Two of 10 patients had positive blood cultures,four of 16 had positive cerebrospinal fluid(CSF)cultures and five of five were detect-ed to be positive by next-generation sequencing(NGS)for pathogens in CSF.CSF showed exudative changes and elevated number of leukocytes,with predominance of single nucleated cells.All patients were treated with the combined use of two to four from the drugs like doxycycline,rifampicin,ceftriaxone,cefixime,minocycline,levofloxacin and sulfanilamide.Most patients had a favorable prognosis.[Conclusions]Neurobrucellosis should be considered in all patients with central nervous system manifestations from endemic areas.If there are exudative changes in CSF,differential diagnoses can be made by serological testing,blood culture,CSF culture and NGS.NGS could significantly increase the accuracy for neuro-brucellosis diagnosis.
5.Analysis of human papillomavirus infection status and related factors among men who have sex with men in Tianjin

Objective:The status quo and related factors of human papillomavirus (HPV) infection in multiple parts of men who have sex with men (MSM) in Tianjin were analyzed.Methods:Using the cross-sectional survey method, a total of 600 MSM were recruited in Tianjin from September to December 2023, including 200 (18-26 years old), 300 (27-45 years old), and 100 (≥46 years old) stratificaed by age group. Questionnaire survey was used to collect knowledge and behavioral information related to demography and STD prevention and treatment. Exfoliated cells from genital, perianal and oral cavity were collected for HPV typing detection. Logistic regression model was used to analyze the related factors of HPV infection.Results:The infection rate of HPV was 53.67% (322/600) in MSM, with high-risk type as 41.17% (247/600), low-risk type as 27.17% (163/600). The infection rate of HPV in both multiple and single types was 26.83% (161/600).The infection rates of genotypes covered by nine-valent HPV vaccine, quadrivalent HPV vaccine and bivalent HPV vaccine were 36.83% (221/600), 25.17% (151/600) and 9.50% (57/600), respectively.The infection rates of genital HPV, high-risk or low-risk types were 24.67% (148/600), 16.67% (100/600) and 11.00% (66/600), respectively. The infection rates of perianal HPV, high-risk or low-risk types were 42.00% (252/600), 30.17% (181/600) and 20.50% (123/600), respectively. The infection rates of oral HPV high-risk and low-risk types were 3.17% (19/600), 2.33% (14/600) and 1.17% (7/600), respectively. Divorce or widowhood (a OR=2.38, 95% CI: 1.26-4.50), history of homosexual anal intercourse in the past 6 months (a OR=2.28, 95% CI: 1.00-5.50), and use of dependent drugs (including new psychoactive substances) (a OR=1.62, 95% CI: 1.02-2.58) were risk factors for genital HPV infection. College degree/university degree or above (a OR=0.36, 95% CI: 0.16-0.81) was a protective factor for perianal HPV infection, and a history of drug dependence (a OR=1.85, 95% CI: 1.19-2.88) was a risk factor. Conclusions:MSM in Tianjin was a high-risk group for HPV infection, and the HPV infection in the perianal area was more severe than in the genital area and oral area.Marital status, education level, history of homosexual anal sex in the past 6 months, and history of drug dependence were related factors of HPV infection among MSM.
6.The effects of lncRNA GAS5 on the proliferation,invasion ability and PTEN/Akt/mTOR signaling pathway of cervical canc-er cells

Objective This objective of this study was to investigate the effects of long chain non-coding RNA GAS5(ln-cRNA GAS5)on the proliferation,invasion ability and PTEN/Akt/mTOR signaling pathway of cervical cancer cells,and to elucidate the possible mechanism of lncRNA GAS5 in cervical cancer.Methods Real-time quantitative reverse transcription polymerase chain reaction(qRT-PCR)was used to detect the levels of lncRNA GAS5 in human cervical cancer HeLa cells,C33A cells,Caski cells,Siha cells and normal cervical epithelial End1/E6E7 cells.HeLa cells were divided into the blank control group(BC group),negative con-trol group(NC group)and overexpressing lncRNA GAS5 group(GAS5 group);Siha cells were divided into the blank control group(BC group),negative control siRNA group(si-NC group)and siRNA-lncRNA GAS5 group(si-GAS5).The CCK8 assay was used to measure proliferation of cervical cancer cells.The Transwell was used to measure the invasion and migration capabilities of cervical cancer cells.Western blot was used to determine the expression of proliferating cell nuclear antigen(PCNA),matrix metalloproteinase(MMP)-2,MMP-9,PTEN,Akt,phosphorylated Akt(p-Akt),mTOR and p-mTOR proteins in cervical cancer cells.Results The results showed that the levels of lncRNA GAS5 in HeLa cells,C33A cells,Caski cells and Siha cells were lower than that in End1/E6E7 cells(P<0.001).Compared with the NC group,overexpression of lncRNA GAS5 in HeLa cells resulted in a decrease in the cell proliferation,invasion,and migration abilities,as well as a decrease in the levels of PCNA,MMP-2,MMP-9,p-Akt,and p-mTOR proteins(P<0.001),while an increase in the level of PTEN protein(P<0.001).Compared with the si-NC group,inhibition of ln-cRNA GAS5 expression in Siha cells resulted in increased the cell proliferation,invasion,and migration abilities,as well as increased the levels of PCNA,MMP-2,MMP-9,p-Akt,and p-mTOR proteins(P<0.001),while decreased the level of PTEN protein(P<0.001).Conclusion The level of lncRNA GAS5 in cervical cancer cells is reduced,and lncRNA GAS5 can inhibit the proliferation,invasion and migration abilities of cervical cancer cells,as well as activate the PTEN/Akt/mTOR signaling pathway.
7.Analysis of the etiology and clinical features of prolonged and chronic diarrhea in children

Objective:To investigate the etiology, incidence and clinical characteristics of pediatric prolonged and chronic diarrhea.Methods:The clinical data of children with prolonged and chronic diarrhea were collected and analyzed, which were hospitalized in the Department of Gastroenterology of the Children′s Hospital Affiliated to the Capital Institute of Pediatrics from January 2017 to June 2020.Results:A total of 190 children with prolonged and chronic diarrhea were collected, with a male-to-female ratio of 1.64∶1(118/72) and a median age of 11.2(5.0, 48.0)months.Among them, 74.3%(141/190) were infants aged 0-3 years, and 54.3%(103/190) were infants aged 0-1 years.The overall cure and improvement rate was 83.7%(159/190). Gastrointestinal concomitant symptoms were dominated by abdominal pain, and vomiting, bloating, and extraintestinal concomitant symptoms were mainly fever, weight loss, and growth and development disorders.Common comorbidities included malnutrition (46.3%), anemia (35.3%), and electrolyte abnormalities (20.5%). The lesion detection rate of electronic gastrointestinal endoscopy and capsule endoscopy reached 93.1%(122/131). The detection rate of genetic testing was 60.0%(9/15). In this group of studies, 169 patients had a clear cause, and the confirmed diagnosis rate was 88.9%.The main causes of childhood prolonged and chronic diarrhea were food allergy(36.8%), inflammatory bowel disease(12.6%), and irritable bowel syndrome(9.5%), but 11.1% of the children did not identify the cause after comprehensive examination.Conclusion:Children with prolonged and chronic diarrhea are mainly infants and young children, especially infants, with diverse etiology, mainly non-infectious factors, food allergy and inflammatory bowel disease are important causes.Malnutrition, anemia and other complications are easy to occur.Endoscopy is helpful in diagnosing and differentiating the cause, if necessary, genetic testing could help to determine the cause.
8.Diagnostic efficacy of alpha-fetoprotein and alpha-fetoprotein L3% in hepatitis B virus-related early-stage hepatocellular carcinoma

ObjectiveTo investigate the diagnostic efficacy and optimal cut-off values of alpha-fetoprotein (AFP) and alpha-fetoprotein variant L3 (AFP-L3) in hepatitis B virus (HBV)-related early-stage hepatocellular carcinoma (HCC). MethodsA total of 1 080 patients with HBV-related HCC (HBV-HCC) who were diagnosed for the first time and not yet treated in The Third Affiliated Hospital of Sun Yat-Sen University from January 2019 to July 2022 were enrolled as HCC group in the study, among whom there were 620 patients with CNLC Ⅰa-Ⅱa HCC, and in addition, 346 patients with HBV-related chronic hepatitis B (CHB group) and 293 patients with HBV-related liver cirrhosis (LC group) were enrolled as controls. The diagnostic efficacy of AFP and AFP-L3% in screening for HBV-related early-stage HCC was analyzed, including sensitivity, specificity, and the area under the ROC curve (AUC). The Mann-Whitney U test was used for comparison of continuous data with skewed distribution between two groups; the Kruskal-Wallis H test was used for comparison between multiple groups, and the Bonferroni method was used for further comparison between two groups. ResultsThe HCC group had significantly higher levels of AFP and AFP-L3% than the CHB group and the LC group (H=542.479 and 418.974, both P<0.001). In early-stage HCC, AFP and AFP-L3% had an optimal cut-off value of 8.7 ng/mL and 5%, respectively, and AFP alone had the largest AUC of 0.816, with a sensitivity of 66.9% and a specificity of 85.1%. There was no significant difference in AUC between AFP-L3%+AFP and AFP alone (Z=0.609, P=0.543), but both AFP-L3%+AFP and AFP alone had a significantly larger AUC than AFP-L3% alone (AFP vs AFP-L3%: Z=8.173, P<0.001; AFP+AFP-L3% vs AFP-L3%: Z=8.802, P<0.001). ConclusionAFP has a good value and is superior to AFP-L3% in the diagnosis of HBV-related early-stage HCC, and the screening cut-off value of AFP should be lowered in order to improve the detection rate of early-stage HCC.
9.Research progress on the application of artificial intelligence in the pathology and prognosis of non-small cell lung cancer

Non-small cell lung cancer is the main cause of cancer death in the world, and its incidence is increasing year by year, seriously endangering human health. Early non-small cell lung cancer is generally difficult to be detected based on symptoms and signs. Therefore, accurate pathological diagnosis and accurate prediction of prognosis are crucial for formulating the best treatment plan for non-small cell lung cancer patients and improving their survival. The application of artificial intelligence in the diagnosis and treatment of non-small cell lung cancer has shown good performance and great potential effect. This paper introduces the research progress of artificial intelligence in predicting the classification, staging, genomics and prognosis of non-small cell lung cancer.
10.Effects of Dendrobium officinale flowers on testivular tissue and sperm quality in parent and offspring rats

Objective:
To learn the effects of Dendrobium officinale flowers on testivular tissue morphology and sperm quality in parent and offspring rats,so as to provide reference for safety evaluation of Dendrobium officinale flowers.
Methods:
The 40 SD rats was randomly divided into the low-,middle-,high-dose and the control group,given 2.0,4.0,6.4 and 0 g/kgbw Dendrobium officinale flowers,respectively. After three months,the body weight,mass and organ/body coefficients of testis and epididymis of parent (P) and offspring (F1,F2) rats were measured;the number,activity and deformity of sperms were observed under microscope;the changes of testis and epididymis were observed by hematoxylin eosin staining.
Results:
There were no significantly statistical differences in the body weight,mass and organ/body coefficients of testis and epididymis,sperm quantity,sperm motility rate among four groups of P、F1、F2 male rats (P>0.05). There were no significantly statistical differences in sperm malformation rate between the high-dose group and the control group (P>0.05). There was no significant change in testis and epididymis of P,F1 and F2 male rats.
Conclusion
Dendrobium officinale flowers did not show obviously adverse effects on testivular tissue morphology and sperm quality in parent and offsping rats.
