1.Analysis of the quality of life in patients with early esophageal cancer after endoscopic submucosal dissection

Objective:To explore the quality of life (QOL) and the related influencing factors of patients with early esophageal cancer after endoscopic submucosal dissection (ESD).Methods:A questionnaire survey was conducted in 167 early esophageal cancer patients who underwent ESD in Shanxi Province Cancer Hospital from January 2022 to July 2022. European Organization for Research and Treatment of Cancer Quality of Life Assessment Core Scale (EORTC QLQ-C30) and the Esophageal Cancer Supplementary Scale (EORTC QLQ-OES18) were used to compare QOL of patients with different clinical characteristics before surgery, 1 month after surgery and 6 months after surgery, And multiple logistic regression analysis was used to analyze the influencing factors of patients' QOL at 6 months after surgery.Results:EORTC QLQ-C30 showed that the scores of the patients' physical function, role function, and social function at 1 month and 6 months after surgery were lower than those before surgery, and the differences were statistically significant (all P < 0.05). The scores of dyspnea, constipation, nausea and vomiting, fatigue, and economic status in the symptom area were higher than those before surgery, and the differences were statistically significant (all P < 0.05). According to EORTC QLQ-OES18, the scores of difficulty in swallowing oral fluid, obstruction, poor eating initiative, dry mouth, and cough at 1 month and 6 months after surgery were higher than those before surgery, and the differences were statistically significant (all P < 0.05). The score of dysphagia at 1 month after surgery was higher than that before surgery, while the score at 6 months after surgery was lower than that before surgery, and the differences were statistically significant (all P < 0.05). The score of dyspepsia at 1 month and 6 months after surgery was lower than that before surgery, and the difference was statistically significant (all P < 0.05). Multivariate analysis showed that the lesion perimeter >1/2 perimeter (lesion perimeter >1/2 perimeter vs. lesion perimeter ≤ 1/2 perimeter: OR = 2.072, 95% CI 1.536-2.796, P < 0.05) and postoperative esophageal stricture dilatation (undergoing esophageal stricture dilatation or not: OR = 2.193, 95% CI 1.429-2.789, P < 0.05) were independent risk factors affecting the QOL of patients at 6 months after surgery. Conclusions:The QOL of early esophageal cancer patients after ESD is decreased compared with that before surgery, and the main manifestations include physical function, role function, social function, and symptom. The area of lesion and undergoing esophageal stricture dilatation or not are factors affecting the QOL of patients after surgery.
2.A study on diagnostic performance of 3.0 T non-contrast-enhanced Dixon water-fat separation compressed SENSE whole-heart coronary MR angiography

Objective:To evaluate the diagnostic performance of non-contrast-enhanced Dixon water-fat separation Compressed SENSE (CS-SENSE) whole-heart coronary magnetic resonance angiography (CMRA) at 3.0 T on patients with suspected coronary artery disease (CAD).Method:The study complied with the Declaration of Helsinki. Local ethics committee approved this study and written informed consent was obtained from each patient. In this prospective study, from March 2021 to September 2021, 53 consecutive participants with suspected CAD who were scheduled for X-ray coronary angiography (CAG) were prospectively recruited in Zhongshan Hospital. CMRA was performed with a 3.0 T scanner without contrast agent enhancement during free breathing with Dixon water-fat separation and CS-SENSE methods. The accuracy of CMRA for detecting a ≥ 50% reduction in diameter was determined using CAG as the reference method.Results:Acquisition of whole-heart CMRA images was successfully performed in 46 (86.8%) of 53 patients with an average imaging time of (7.8±1.8) min. The sensitivity, specificity, positive predictive values, negative predictive values, and accuracy of CMRA according to a patient-based analysis were 95.8%(95%CI 78.9%-99.9%), 81.8%(95%CI 59.7%-94.8%), 85.2%(95%CI 66.3%-95.8%), 94.7%(95%CI 74.0%-99.9%), 89.1%(95%CI 76.4%-96.4%), respectively. The areas under the receiver-operator characteristic curve (AUC) from CMRA images according to patient-, vessel-and segment-based analyses were 0.876(95%CI 0.745-0.955), 0.880(95%CI 0.814-0.929), 0.903(95%CI 0.877-0.926), respectively.Conclusion:3.0 T non-contrast-enhanced Dixon water-fat separation CS-SENSE whole-heart CMRA is a promising technique to detect clinically significant coronary stenosis on patients with suspected CAD.
3.Simultaneous determination of five components in Anemarrhenae Rhizoma by HPLC-ELSD

Objective To establish a method for the simultaneous determination of new mangiferin, mangiferin, artemisinin BⅡ, icariin and artemisinin A in Anemarrhenae Rhizoma by high performance liquid chromatography-evaporation light scattering detector (HPLC-ELSD). Methods The column was Agilent Poroshell 120 EC-C18. The mobile phase used acetonitrile-0.2% acetic acid water system with gradient elution. Column temperature was 30 ℃. Flow rate was 0.7 ml/min. Evaporative light scattering detector used nitrogen as atomizing gas. The atomizing gas temperature was 40 ℃ and the drift tube temperature was 90 ℃. The nitrogen volume flow rate was 2.00 L/min and the sample volume was 20 μl. Results The five components were able to achieve baseline separation. Neomangiferin, Mangiferin, Anemaponin BⅡ, Baohuoside I , Anemarrhena saponin AⅢwere determined as 24.1-386 μg/ml (r=0.999 3), 23.2-371 μg/ml (r=0.998 6), 54.2-867.2 μg/ml(r=0.995 6), 5.3-84.8 μg/ml (r=0.996 8), 10-160 μg/ml (r=0.998 9) respectively, which showed a good linear relationship within the concentration range. The average recovery rate of the five components was between 101.8% and 105.0%, and the repeatability RSD was less than 2.4%. The content of the above five components in Zhimu medicinal materials were 1.62%, 0.82%, 7.36%, 0.07%, 0.34%, respectively. Conclusion The method is simple, accurate, and highly sensitive, which could be used as the quantitative determination of multiple index components of Anemarrhenae Rhizoma.
4.Predictive Value of CD44v6 and EGFR Expression in Neoadjuvant Chemotherapy Sensitivity of Stage Ⅱ-Ⅲ Cervical Cancer

Objective To explore the predictive value of the expression of CD44v6 and EGFR on the efficacy of neoadjuvant chemotherapy (NACT) in stageⅡ-Ⅲ cervical cancer. Methods A total of 53 patients with stageⅡ-Ⅲ cervical cancer diagnosed by pathology were selected. All patients received two cycles of paclitaxel+platinum NACT. The pathological tissue samples of cervical tumors before NACT treatment were collected. The expression of CD44v6 and EGFR were detected by the immunohistochemical SP method, and we analyzed their predictive value of NACT in stageⅡ-Ⅲ cervical cancer. Results Among the 53 patients, 38 were in the NACT effective group (CR+PR), and 15 were in the NACT ineffective group (SD+PD). 6.Evidence-based nursing practice for prevention and management of enteral nutrition feeding intolerance in patients after upper gastrointestinal surgery

Objective:To apply the best evidence for prevention and management of enteral nutrition feeding intolerance in patients after upper gastrointestinal surgery to clinical practice and to evaluate its clinical effects.Methods:Guided by the clinical evidence practice application model of the Joanna Briggs Institute (JBI) Evidence-based Healthcare Centre in Australia, the best appropriate evidence was selected based on expert advice and evidence-based practice protocols were developed. The convenient sampling method was adopted to select 84 patients and 21 nurses who were admitted to the Department of General Surgery in a Class Ⅲ Grade A hospital in Taiyuan from September to December 2020 as the research objects. A baseline review of 42 patients and 21 nurses admitted from September to October 2020 was conducted, and education, training and quality supervision were conducted to promote practice change. A total of 42 patients and 21 nurses (same as before) admitted from November and December 2020 were reviewed. Incidence of enteral feeding intolerance, the time to recovery of intestinal function, the knowledge, attitude and practice of nurses towards prevention and management of enteral feeding intolerance and implementation of audit indicators among nurses were compared before and after the application of the program.Results:The incidence of abdominal distension, diarrhea and feeding intolerance in patients after the application of the evidence was lower than that in patients before the application of the evidence, and the difference was statistically significant ( P<0.05) . The first anal exhaust time and first anal defecation time of patients after the application of the evidence were (59.02±17.19) and (68.10±19.98) h, which were shorter than those before the application of evidence (78.79±22.34) and (82.76±22.35) h, and the differences were statistically significant ( P<0.01) . After the application of the evidence, the scores of all dimensions of nurses' knowledge, belief and practice questionnaire for the prevention and management of feeding intolerance were higher than those before the application of evidence, and the differences were statistically significant ( P<0.05) . The implementation rates of the 11 audit indicators were improved from 0-78.57% to 80.95%-100%. Conclusions:The best-evidence based prevention and management strategies of enteral nutrition intolerance in patients after upper gastrointestinal surgery can be applied to clinical practice to standardize the work behavior of nurses in implementing enteral nutrition and reduce the incidence of feeding intolerance in patients.
7. A multicenter retrospective study on clinical value of lymph node dissection in the radical resection of intrahepatic cholangiocarcinoma

Objective:
To investigate the clinical value of lymph node dissection (LND) in the radical resection of intrahepatic cholangiocarcinoma (ICC).
Methods:
The propensity score matching and retrospective cohort study was conducted. The clinicopathological data of 448 patients with ICC who were admitted to 12 medical centers from December 2011 to December 2017 were collected, including 279 in the Eastern Hepatobiliary Surgery Hospital of Navy Medical University, 32 in the Mengchao Hepatobiliary Hospital of Fujian Medical University, 21 in the First Hospital Affiliated to Army Medical University, 20 in the Cancer Hospital Chinese Academy of Medical Science and Peking Union Medical College, 19 in the West China Hospital of Sichuan University, 18 in the Second Hospital Affiliated to Zhejiang University School of Medicine, 18 in the Renji Hospital Affiliated to Shanghai Jiaotong University School of Medicine, 16 in the Beijing Friendship Hospital Affiliated to Capital Medical University, 10 in the Xuanwu Hospital Affiliated to Capital Medical University, 7 in the Tongji Hospital Affiliated to Tongji Medical College of Huazhong University of Science and Technology, 5 in the Beijing Tiantan Hospital Affiliated to Capital Medical University, and 3 in the Affiliated Hospital of North Sichuan Medical College. There were 281 males and 167 females, aged from 22 to 80 years, with a median age of 57 years. Of the 448 patients, 143 with routinely intraoperative LND were divided into LND group and 305 without routinely intraoperative LND were divided into control group, respectively. Observation indicators: (1) the propensity score matching conditions and comparison of general data between the two groups after matching; (2) intraoperative and postoperative situations; (3) follow-up; (4) survival analysis. Patients were followed up by outpatient examination, telephone interview and email to detect survival of patients and tumor recurrence up to October 31, 2018 or death. Follow-up was conducted once every 3 months within postoperative 1-2 years, once every 6 months within postoperative 2-5 years, and once a year after 5 years. 8.Application of mechanical ventilation in the treatment of central nervous hyperventilation

Objective To analyze the clinical effects of early and delayed mechanical ventilation in patients with central neurogenic hyperventilation. Methods Through retrospective analysis, we collected patients with central neurogenic hyperventilation who were treated with mechanical ventilation and screened them. Cases meeting the criteria were divided into early mechanical ventilation group and delayed mechanical ventilation group. The clinical indicators were observed and the test results were statistically analyzed. Results P02, LAC, and shallow breathing index in the early mechanical ventilation group within 72 hours The improvements were significantly better than the delayed mechanical ventilation group, and the differences were statistically significant (P<0. 05). The mechanical ventilation time and ICU treatment time in the early mechanical ventilation group were significantly shorter than those in the delayed mechanical ventilation group(P<0. 05). Conclusion Patients with central nervous system hyperventilation have a significant effect of early mechanical ventilation, can significantly improve the patient's hypoxia and respiratory status, and effectively improve the prognosis of patients. It is recommended that early mechanical ventilation be used.
9.Anxiety level of parents and the influencing factors in patients with PKU

Objective To analyze the anxiety level of parents and influencing factors in patients with phenylketonuria ( PKU) , in order to provide references for providing good psychological intervention methods for patients′parents.Methods Totally 122 parents of patients with PKU in China-Japan Frieadship hospital from March 2014 to March 2015 were selected and investigated. The anxiety level of patients and the proper influencing factors were surveyed by self-designed questionnaires and self-rating anxiety scale ( SAS ) . The influencing factors were analyzed by Logistic regression analysis. Results The results showed that the mean score of anxiety level of the PKU parents was (61.3±12.4) points, in which 66.4% were beyond 50 points, while 2. 5% was beyond 75 points. The proper influencing factors included the children′ recent clinical manifestation, parents′ understanding of disease and the social support degree. Conclusions The parents′anxiety level is significantly higher than normal people, and their anxiety level is associated with many influencing factors. So the health nursing workers should pay more attention to the parents′mental states and give necessary interventions to encourage their actively cooperation with doctors to achieve the common goals.
10.Effects of Huoxuexiaoying Tablet on the Goiter Model of Rats

Objective To investigate the effects and mechanism of huoxuexiaoying tablet on experimental goiter of rats. Methods The rats were randomly divided into six groups: the control, the model control group, huoxuexiaoying tablet at different doses,and the sodium levothyroxine group ( Euthyrox group) . Except for the rats in the control,the rats in other groups were given with propylthiouracil (20 mg·kg-1 ·d-1 ) by intragastric ( i. g. ) administration every day for 60 days. Meanwhile, some rats were treated with huoxuexiaoying tablet at low (4. 4 g·kg-1·d-1),middle (8. 8 g·kg-1·d-1) and high dose (17. 6 g·kg-1 ·d-1 ) orally,and those in the Euthyrox group were given with 7. 8μg·kg-1 ·d-1 Euthyrox by i. g. administration. The rats in the control group were administrated with the same volume of saline (N. S). After 60 days of treatment,the rats were sacrificed,the organ indexes of thyroid and pituitary and the levels of free triiodothyronine ( FT3 )、free thyroxin ( FT4 ) and thyroid stimulating hormone(TSH) in serum,were tested. The expression of basic fibroblast growth factor(bFGF),transforming growth factor-β( TGF-β) and B-cell lymphoma 2 gene ( Bcl-2 ) were examined by immunofluorescence. Results Compared with the model,organ indexes of thyroid were significantly reduced by huoxuexiaoying tablet at three doses (P<0. 05),but not for the pituitary (P>0. 05). The levels of FT3 and FT4 were in a elevating trend,but TSH decreased with no significance (P>0. 05). The morphological structure of thyroid was greatly improved by huoxuexiaoying tablet in comparison with the model. In which the gland dilated,thyroid follicular restored to moderate size,epithelia were cubic or flattened and follicular cavity filled with abundant glial. The expression of bFGF and Bcl-2 decreased significantly (P<0. 01) while TGF-β expression increased notably (P<0. 01). Conclusion Huoxuexiaoying tablet has a great anti-goiter effect,the mechanism of which may be related to promoting thyroid cells apoptosis and inhibiting thyroid cells proliferation.
