[摘 要] 目的：探讨橙花叔醇通过Wnt-β-catenin通路抑制黑色素瘤A-375和WM-115细胞恶性生物学行为的分子机制。方法: 体外培养黑色素瘤细胞A-375和WM-115，用不同浓度的橙花叔醇处理，采用SRB法和克隆形成实验、FCM术、Transwell实验和细胞划痕实验、DCFH-DA染色法、qPCR和WB法分别检测橙花叔醇对A-375和WM-115细胞的增殖能力、细胞周期和凋亡、迁移能力、活性氧（ROS）水平和Wnt-β-catenin通路及其下游相关基因和相关蛋白表达的影响。利用ULCAN和GEPIA2数据库分析黑色素瘤中Wnt-β-catenin通路的激活与患者预后的关系。结果：与对照组比较，橙花叔醇处理组A-375和WM-115细胞的增殖能力受明显抑制（均P<0.01）、细胞周期阻滞于G2/M期（P<0.05或P<0.01）、细胞凋亡率增加（均P<0.01）、迁移能力降低（P<0.05或P<0.01）、ROS水平升高（均P<0.01）、Wnt-β-catenin通路被抑制而其下游基因和蛋白表达明显上调（均P<0.01）。数据库数据分析显示，WNT1基因高表达患者OS低于低表达患者（P<0.01）。结论：橙花叔醇通过上调A-375和WM-115细胞中ROS水平影响Wnt-β-catenin通路，从而抑制其恶性生物学行为；Wnt-β-catenin通路可能是黑色素瘤治疗的潜在靶点。

Objective To investigate the stimulating effects of conjunctival sac implants following eye enucleation on orbital bone tissue in rabbits model.Methods Eighty 1-month-old New Zealand rabbits were randomly divided into normal control group,only eyeball enucleation group,2-week interval group and 4-week interval group.The right eyeballs were enucleated in the rabbits in the only eyeball enucleation group,2-week interval group and 4-week interval group,and conjunctival sac implants were placed intraoperatively.Then a larger implant was replaced in a 2-week interval or 4-week interval in different groups,respectively.Spiral CT scan was used to evaluate the orbital bone development with aging.The peripheral blood of 3 ml was collected in the rabbits for the detection of osteocalcin and alkaline phosphatase using osteocalcin test kit (N-MID Osteocalcin) and alkaline phosphatase assay kit (NPP substrate-AMP buffer method),respectively.The animals were sacrificed at 3 months after operation for the histopathological examination of orbital bone tissue.Results The operation was successful in all the rabbits and no infection occurred after operation.The variance of orbital bone CT value was obvious in all the rabbits.The intercalated osteocalcin concentration was (48.55 ± 7.99),(59.80 ± 2.96),(57.94 ± 5.20) and (55.96 ± 3.22) μg/L and the alkaline phosphatase concentration was (284.66± 132.69),(232.96±54.39),(232.40± 118.23),(284.20± 130.41) μg/L in the normal control group,only eyeball enucleation group,2-week interval group and 4-week interval group,respectively,showing insignificant differences among the four groups (F =2.710,0.281,both at P ＞ 0.05).Similar pathological findings were in the 2-week interval group and 4-week interval group,such as no obvious orbital bone malformations,weak bone absorption and few bone osteoclasts under the optical microscope.Varying degrees of orbital deformities and bone tissue absorption were found in the only eyeball enucleation group.No orbital bone developing abnormality was seen in the normal control group and the left eyes in various groups under the optical microscope.Conclusions Compared with the only eyeball enucleated eyes,the orbital bone tissue has a well developed process in conjunctiva sac implant eyes following eyeball enucleation.Conjunctiva sac implant is an effective method to stimulate orbital growth and orbital volume increasing.

Objective To observe the effect of early cognitive and speech intervention on children with developmental delay. Methods 58 inpatient or outpatient children with developmental delay from June, 2014 to June, 2015 were diveded into observation group (n=32) and control group (n=26). The observation group accepted early cognitive and speech therapy and routine rehabilitation training, while the con-trol group accepted the routine rehabilitation only. They were assessed with Gesell Development Schedule before and 3 months after treat-ment. Results The developmental quotient of the gross movement, fine movement, language and peasonal-social improved in both groups af-ter treatment (t>2.90, P<0.001), and improved more in the observation group than in the control group (t>2.84, P<0.05), especially in chil-dren of 1 year old than those of 1-2 or 2-3 years old (F>36.52, P<0.01). Conclusion Early cognitive and speech intervention may improve development of many dimensions in children with developmental delay. The earlier the intervention, the better the outcome.

Objective To investigate the effect of diabetic health education pictures on blood sugar control and self-care ability of type 2 diabetic patients. Methods Using the registration number and random digit table, 84 patients with type 2 diabetes mellitus were divided into the experiment group and the control group with 42 cases in each group. The control group received traditional diabetes knowledge education, and the experiment group was educated with the mode ofpicture and dialoguehealth education, 60 to 90min per week for 8 weeks. The patients in the two groups were observed before and after the intervention in terms of the fasting blood glucose, postprandial 2h blood glucose, glycatedhemoglobin (gycated, hemoglobin, HbAlc) and self-care ability. Result The differences in blood sugar value index and self-care behavior score after the intervention between the two groups were statistically significant (P<0.05). Conclusion Application of′picture and dialogue′education mode can enhance patients′ability of self-care behaviors the effect of health education and stabilize the level of blood glucose of patients.

BACKGROUND:Major complication after total hip replacement was instability in the form of dislocation. The probability of above complications has a great relationship with the mistakes of the angle of acetabular cup prosthesis. OBJECTIVE:To explore the influence of angle of acetabular cup in total hip replacement on the safety of joint flexion and extension. METHODS: A total of 60 patients, who underwent total hip replacement in the Wujin Hospital Affiliated to Jiangsu University from January 2012 to December 2014, were enroled in this study. Three dimensional directional monitoring device, CT scanning and image processing tools were used to preset the angle of the acetabular cup and to set abduction angle and anteversion angle. Test data were recorded and subjected to statistical analysis. RESULTS AND CONCLUSION:Acetabular cup was implanted under five different conditions: abduction angle 45°, anteversion angle 15°; abduction angle 60°, anteversion angle 15°; abduction angle 30°, anteversion angle 15°; abduction angle 45°, anteversion angle 5°; abduction angle 45°, anteversion angle 25°. According to statistics, in the patients with anteversion angle of 15°, when the acetabular cup was placed, the bigger the abduction angle, the bigger the range of abduction angle of the acetabular cup induced by pelvic tilt was. If the anteversion angle increased, the anteversion angle of the acetabular cup was reduced. With the increased pelvic tilt angle, the anteversion angle of the acetabular cup was big. Moreover, in patients with anteversion angle of 15°, the anteversion angle of the acetabular cup should keep consistent. When the abduction angle of the acetabular cup increased, the range of anteversion angle of the acetabular cup induced by pelvic tilt was diminished. These data showed that the abduction angle and anteversion angle of the acetabular cup have mutual restriction. When the acetabular cup was placed, big anteversion angle (15±10)° can wrap the femoral head, reduce the incidence of dislocation, and make flexion and extension safe.

Objective To evaluate the results of orthodontic micro- implant anchorage implantation by application of Spiral CT and three-dimensional reconstruction techniques,so as to explore its application value in assisting clinical diagnosis and treatment. Methods 20 orthodontic patients were implanted with 48 micro-planting screws. SOMATOM Sensation40/64 helical CT scan was performed . The axial unenhanced helical CT data were given multi-planar reconstruction (MPR),maximum intensity projection (MIP),volume rendering (VR) and surface multidimensional reconstruction (CPR) by using the Siemens 3D post-processing software. The axial scanning images and reconstructed images were compared, observed and analyzed . Results The planting results of 37 micro screws were satisfactory without injuring the vital tissues surrounding the root. The planting results of 11 micro-screw were not ideal, and these micro-screws were observed or reimplanted and got success. Spiral CT axial scan showed better the tooth root interval and the relationship between tooth root and the planting screws. VR can directly showed the overall relationship between the planting screws and teeth , jaw contour appearance. The relationship between the screw and tooth was showed satisfactorily by MIP which could rotate freely and observe from different angles. MPR could clearly show the structural relationships between micro-root planting screws and tooth root and surrounding tissues from the cross section, and could make up for the shortcomings of axial limitations, and was easy to measure the distance and tilt angle of the long axis . Conclusion Spiral CT and three-dimensional reconstruction can accurately show the jaw micro planting screw shape,location and spatial relationships with adjacent teeth , the image is clear,intuitive and definite,and spiral CT and three-dimensional reconstruction has high application value in orthodontics micro-implant anchorage clinic as a powerful auxiliary examination means.

Objective To observe the clinical effect oflamellar keratectomy+conjunctival flap+implanted artificial eyein stimulating the orbital and conjunctival sac growth. Methods A retrospective case study: 12 cases (12 eyes) with congenital microphthalmos in the Fourth Affiliated Hospital of Kunming Medical University in 2009-2013 were selected. In these cases, there were 11 cases of microphthalmos, and 1 patient due to congenital absence of the eye without surgery, were given direct implant of the artificial eye;7 patients without significant stenosis in conjunctival sac,received thelamellar keratectomy+conjunctival Flap+implantation of artificial eye, 4 patients with conjunctival sac stenosis recieved thelamellar keratectomy+conjunctival flap+implanted artificial eye+eyelid suture. Results For stunted children who couldn't wear a prosthetic eye, after treated withlamellar keratectomy + conjunctival flap + artificial eye implantation, the conjunctival sac developed well, cornea was covered with conjunctiva well and no exposure,the appearance and volume of orbit was also improved. ConclusionLamellar keratectomy+conjunctival flap+artificial eye implantsurgery is an effective way to promote orbital and conjunctival sac development of the children with congenital microphthalmos.

Objective To understand the difference between psychological characteristics and stress response as a result of military stress and to evaluate the rate of stress response among individuals with different psychological characteristics.Methods The State-Trait Anxiety Inventory,blood pressure,breath,pulse wave,and serum levels of lactate acid and cortisol were measured at 8 to 9 am for 372 servicemen following 15 days'training.ResultsIn 97 subjects ,the value of special characteristic anxiety Was 2-standard deviation higher than normal model(i.e.high special characteristic anxiety).and low special characteristic anxiety was found in 79.The condition anxiety of 2 standard deviation higher than normal model wag found in 172 participants.A total of 132 subjects showed significant changes in condition anxiety,serum cortisol and lactic acid,and systolic blood pressure(P＜0.01).Sixty-one or 71 subjects with high or low special characteristic anxiety experienced condition anxiety,changed serum cortisol and lactic acid,and systolic blood pressure,respectively.Conclusion Psychological characteristics may be considered as a predicting factor of stress response.

OBJECTIVE: To study some thermal problems in invasive microwave coagulation (IMC) therapy. METHODS: Temperature fields predicts, limitation and remedy methods of the Pennes bioheat transfer equation during the temperature distribution analyses and how to kill a large anomalistic tumor for only once were discussed. RESULTS: The coagulation area between the simulations and actual measured results showed good correspondence. Blood flow influenced the rise in temperature significantly. CONCLUSIONS: Thermal distribution with computer simulation in invasive microwave coagulation therapy is an accurate and reliable method.
