1.Multidimensional status of family nursing assistants for elderly people with chronic diseases and disability in Beijing communities

Objective:To conduct a survey on the current situation of family nursing assistants for elderly people with chronic diseases and disability in the community from multiple dimensions such as personal information, work status, professional quality, difficult problems encountered in the nursing process, and solutions.Methods:From April to August 2022, a questionnaire survey was conducted among family nursing assistants of elderly people with chronic diseases and disability in six communities in Beijing using cluster sampling. We conducted a survey and analysis on the current situation of family nursing assistants for elderly people with chronic diseases and disability in the community from multiple dimensions, based on the presence or absence of professional qualification certificates for nursing assistants.Results:The study included 611 nursing assistants, aged (49.99±6.82) years, mainly composed of rural registered residence and education level below junior high school. Only 43.04% (263/611) of nursing assistants had professional qualification certificates for nursing assistants. Compared with those without professional qualification certificate for nursing assistants, those with professional qualification certificate for nursing assistants had statistical differences in gender, registered residence, education level, daily working hours, specific work content of care, monthly income, solutions to reduce the mobility of elderly nursing assistants, working years, feelings of caring for the daily health of elderly people with chronic diseases, psychological status, self-awareness about job, training to improve work skills, basic medical knowledge training, and reasons for not participating in training ( P<0.05). Nursing assistants reported a lack of medical and nursing knowledge and an urgent need for professional training and guidance from medical and nursing staff during the investigation of difficult issues encountered in their work. Conclusions:It is necessary to strengthen and improve the training of nursing professionals and basic medical knowledge of family nursing assistants for elderly people with chronic diseases and disability before and during work, which can help improve the level and quality of care provided by family nursing assistants.
2.Research progress on the effect of single nucleotide polymorphism on drug efficacy of Alzheimer′s disease

Alzheimer′s disease is a progressive neurodegenerative disease that requires medication to improve patient symptoms, but there is an individual difference in the efficacy. In this paper, the correlation between single nucleotide polymorphism and the drug efficacy of Alzheimer′s disease (AD) in the past 20 years was searched through the databases of China National Knowledge Infrastructure, VIP Database, Wanfang Database, Pubmed, Springer Link and Cochrane Library with key words of Alzheimer′s disease, drug efficacy, single nucleotide polymorphism. The correlation between AD drug efficacy difference and gene single nucleotide polymorphism was reviewed, including ABCA1, ApoE, ChAT, CHRNA7, IL-6, A2M, CYP2D6, BChE, 5HT2a, PON-1 and ESR1 genes, so as to provide a reference basis for clinicians to select drugs in the treatment of AD.
3.Clinical analysis of 35 cases of drug-induced hypersensitivity syndrome

Objective:To investigate the clinical features of drug-induced hypersensitivity syndrome (DiHS) and enhance the recognition of its diagnosis and treatment.Methods:Drug-induced hypersensitivity syndrome diagnosed in the Second Xiangya Hospital of Central South University in recent 6 years were retrospectively analyzed and summarized.Results:Of 35 cases of drug-induced hypersensitivity syndrome there were ten types of suspected drugs. Antibiotics were the most common allergy-inducing drug, followed by allopurinol, antiepileptics, and non-steroidal anti-inflammatory drugs. The expected clinical findings of DiHS were characterized by skin rash, fever, enlarged lymph nodes, which may be accompanied by eosinophilia, liver function damaged. The lesions of DiHS were variable and the majority of cases 65.71%(23/35) were characterized by eruptive papules.The main therapeutic approach of DiHS was systemic application of glucocorticoids and intravenous immunoglobulin (IVIG). When necessary its combination with immunosuppressant or hemoperfusion (HP).Conclusions:DiHS should be on the high alert when a patient with medication history, rash, high fever and lymph nodes large, accompanied by eosinophilia, liver function damaged. Predictors of good prognosis are early diagnosis and treatment.
4.Evaluate the follow-up effect of drug treatment for middle cerebral artery atherosclerotic plaques using high resolution MRI

Objective:To explore the value of 3.0 T high resolution MRI (HR-MRI) in the follow-up of drug treatment in acute and non-acute ischemic stroke caused by middle cerebral artery (MCA) plaque.Methods:The perspective study enrolled patients with ischemic stroke caused by MCA stenosis from October 2012 to October 2015 in the department of Neurology and Neurosurgery of Changhai Hospital Affiliated to Naval Medical University. All the patients underwent HR-MRI and then were divided into acute and non-acute stroke groups according to the intervels of the last symptom onset to the time of HR-MRI examination. All patients were informed consent to receive antiplatelet drug and intensive lipid therapy and followed up with HR-MRI. The HR-MRI sequence including T 2WI, T 1WI and contrast-enhanced T 1WI of vessel wall, and T 2WI and DWI of brain were routinely performed. T-test of paired samples was used to evaluate the changes of stenosis rate of vascular lumen, plaque enhancement degree, plaque volume and plaque burden on HR-MRI, and the NIHSS score of nervous system and blood biochemical indicators of the patients before and after treatment. Chi square test was used to compare the difference in ischemic event recurrcence between the acute and the non-acute stroke group. Results:A total of 31 acute stroke patients and 20 non-acute stroke patients were enrolled in the study. The mean follow-up time of acute stroke group was (671.71±522.86) days. Compare with the baseline, the stenosis rate of vascular lumen ( P=0.039), plaque enhancement degree ( P<0.001), plaque volume ( P=0.024) and plaque burden ( P=0.031) were all improved after the drug treatment, the NIHSS score of nervous system was also significantly improved, and the levels of total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C) in 12 patients were significantly decreased. The mean follow-up time of patients with non-acute stroke was (695.35±555.90) days. The stenosis rate of vascular lumen, plaque enhancement degree, plaque volume and plaque burden were slightly improved, but without statistical significance ( P>0.05). There were no significant changes in NIHSS score of nervous system, TC, triglyceride (TG) and LDL-C ( P>0.05), however the high density lipoprotein cholesterol (HDL-C) was significantly increased than that in the baseline ( P=0.02). During the follow-up period, no new cerebral infarction was found in the DWI images of the two groups. Six patients had transient ischemic attack (TIA) recurrence in the acute stroke group and 5 patients in the non-acute stroke group, there was no significant difference between both groups(χ 2=0.229, P= 0.632). Conclusion:HR-MRI can be used as an important evaluation method for the follow-up of MCA atherosclerotic plaque therapy. After antiplatelet therapy and intensive lipid-lowering therapy, the plaque volume and burden of MCA offending plaque, and plaque enhancement decreased in acute stroke patients but there was no significant change in non-acute patients.
5.Clinical analysis for 108 cases of dermatomyositis.

To investigate the clinical characteristics of dermatomyositis, to investigate the types and clinical features of dermatomyositis complicated with malignant tumor, and to provide evidence for clinical diagnosis, treatment and prognostic evaluation.
Methods: The clinical manifestations and laboratory test results for 108 cases of dermatomyositis with complications in the Second Xiangya Hospital of Central South University were analyzed.
Results: Patients aged from 14 to 60 years accounted for 62.96%. The first symptom was single skin rash (54.63%), and the most characteristic cutaneous features were asymmetrical proximal myositis with various degrees (97.22%). The visceral involvement was as follows: the digestive tract (31.48%), the heart (19.44%), the lung (26.85%), and the thyroid damage (12.96%). Twelve (11.11%) patients were combined with malignant tumor. The positive rates for albumin (ALB), glutamic oxalacetic transaminase (AST), glutamic-pyruvic transaminase (ALT), lactate dehydrogenase (LDH), creatine kinase (CK), creatine kinase isoenzyme (CK-MB), erythrocyte sedimentation rate (ESR), anti Jo-1 antibody, anti ribonucleoprotein (RNP) antibody, and anti-topoisomerasel (Scl) antibody were 25.93%, 46.30%, 28.70%, 87.04%%, 51.85%, 26.85%, 55.56%, 2.27%, 8.99%, and 2.27%, respectively. The patients were divided into a tumor group and a non-tumor group. The chi-square test results from clinical symptoms and laboratory tests suggested that increase of ESR was a risk factor for dermatomyositis combining tumor. The main strategy of therapy was corticosteroids.
Conclusion: Dermatomyositis possesses typical skin lesions and dermatitis is the most common initial symptom of dermatomyositis. In clinic, diagnosis of dermatomyositis should be timely combined with muscle enzymes test, electromyography and muscle biopsy. Dermatomyositis can easily involve many organs. Thus relevant examinations (such as chest X-ray and CT) should be done preventively. Rapid ESR is a risk factor for dermatomyositis complicated with malignant tumor and it can be used as an index to guide clinical diagnosis.

Objective To explore the remodeling modes and the plaque distribution of atherosclerotic BA at 3.0T high resolution MRI.Methods 90 symptomatic patients with atherosclerotic stenosis of BA on digital subtraction angiography (DSA) (50 %-99 %) were recruited consecutively.Luminal area,vessel area of maximal narrow sites and the reference sites were measured.The differences of involved imaging parameters between negative group and positive group were analyzed.Results 51 patients with required imaging quality were enrolled finally.Among the 51 patients,the rate of positive remodeling cases was 72.5% (37/51) and negative remodeling took over 27.5% (14/51).Compared with the negative remodeling group,the positive remodeling group had greater plaque size,larger plaque burden percentage,and higher maximal wall thickness at maximal lumen narrowing sites.The plaques were mainly located at ventral wall of the vessel.Conclusion 3.0T high-resolution MR imaging could be applied in assessing the remodeling modes and plaque distribution of BA stenosis.
8.Screening for the Active Ingredients of Ziziphus jujuba with Neuroprotective Effects and Their Mechanisms of Action

OBJECTIVE:To conduct screening for the active ingredients of Ziziphus jujuba with neuroprotective effect and to il-luminate their mechanisms of action preliminarily. METHODS:After neuron-like cells (PC12 cells) were respectively cultured in the ingredient of Z. jujuba with polysaccharide enriched(1 mg/ml),that with polysaccharide removed(1 mg/ml),7 kinds of flavo-noid ingredients of Z. jujuba(catechin,procyanidine B2,epicatechin,hyperoside,rutin,quercetin-3-β-D-glucoside and kaempfer-ol-3-O-rutinoside,represented by A,B,C,D,E,F,G,all at the concentrations of 3,13,30 μmol/L)and 2 kinds of nucleoside ingredients of Z. jujuba (cyclic adenosine monophosphate and cyclic guanosine monophosphate,both at the concentrations of 3, 13,30 μmol/L)for 24 h,tert-butyl hydroperoxide(tBHP,150 μmol/L)was used to act on PC12 cells for 3 h to induce oxidative cellular damage,and MTT assay was employed to detect the survival rate of PC12 cells. The PC12 cells transfected with reporter gene plasmid(pARE-Luc)were cultured as above for 24 h,luciferase(Luc)assay was used to detect the transcriptional levels of the antioxidant response element(ARE)of all groups of cells(reflected as the activity of Luc)so as to investigate the anti-injury mechanism. RESULTS:The ingredient of Z. jujuba with polysaccharide enriched could significantly increase the survival rate of PC12 cells to which oxidative damage was caused and the transcriptional level of ARE in the transfected cells. Among the flavonoid ingredients of Z. jujuba, A(30 μmol/L),B(3-30 μmol/L)and C(10-30 μmol/L)could significantly increased the survival rate of the cells,and A(30 μmol/L),B(3-30 μmol/L),C(30 μmol/L),E(30 μmol/L)and F(3-30 μmol/L)could obviously in-creased the activation level of ARE in the transfected cells. However,the nucleoside ingredients of Z. jujuba including cyclic ade-nosine monophosphate and cyclic guanosine monophosphate had no obvious effect of increasing the survival rate of PC12 cells to which oxidative damage was caused or activating the transcription of ARE in the transfected cells. CONCLUSIONS:The polysac-charide and flavonoid ingredients of Z. jujuba may be the active ingredients which account for the neuroprotective effect against oxi-dative cellular damage,and their mechanisms of action may be related to the activation of ARE transcription.
9.Analysis of PDCD1 gene copy number differences between hepatocellular carcinoma tissues and paracancerous tissues of patients with HBV-related HCC

Objective To analyze the differences in the PDCD1 gene copy number of hepatocellular carcinoma and paracancerous tissues in patients with hepatocellular carcinoma (HCC) after HBV infection.Methods Analysis PDCD1 gene copy number of 27 paraffin embedded specimens of hepatocellular carcinoma tissues and para carcinoma tissues of patients with hepatocellular carcinoma after resection by Taqman real-time PCR.Results The gene copy number of PDCD1 of hepatocellular carcinoma tissue and paracancerous tissue had no significant difference (P > 0.05) ; the gene copy number of PDCD1 and preoperative AFP levels of hepatocellular carcinoma and paracancerous tissues had no significant correlation (P > 0.05).Gene copy number of PDCD1 of hepatocellular carcinoma and paracancerous tissues had no significant correlation with tissue tumor malignant degree (P > 0.05).Conclusions PD-1 plays a role in the escape response of tumor immunity,but the PDCD1 gene copy number variation of hepatocellular carcinoma and paracancerous tissues is not a factor in carcinogenesis and development of hepatocellular carcinoma.
10.Analysis of CT findings of benign and malignant pancreatic neuroendocrine tumors

Objective To explore the CT findings of benign and malignant pancreatic neuroendocrine tumors and improve its diagnostic accuracy.Methods The clinical information and enhanced CT findings of 96 cases with pathologically-proved pancreatic neuroendocrine tumors were retrospectively reviewed.The CT findings were evaluated by several factors,which included tumor size,morphology,location,internal composition,calcification,separation,bile duct and pancreatic duct dilation and CT value.Results All cases were divided into benign or malignant according to pathological grades,and benign group involved 40 cases with 41 lesions,while malignant group involved 56 cases with 59 lesions.The size of malignant lesions was significantly larger than that of benign lesions (median size 6.0 cm vs 2.2 cm),the shape of the lesions was irregular,and was mainly cystic solid,and mottling,curve shape,clumps calcification was present,then the bile duct and pancreatic duct was mild to moderately dilated,and the difference between the two groups was statistically significant (P <0.05).But the difference of tumor location,separation was not significant.45.76% (27/59) of the malignant lesions reached the peak value in arterial phase,and 44.07% (26/59) reached the peak value in venous phase;while 68.29% (28/41) of the benign lesions reached the peak value in arterial phase,and 31.71% (13/41) reached the peak value in venous phase.The CT values of malignant lesions in plain CT scanning,arterial phase,venous phase,balance phase were (39.02 ±7.53),(121.20 ± 54.73),(125.25 ± 40.77),(101.41 ± 28.68) Hu,while they were (41.49 ± 8.59),(144.73 ± 53.95),(157.05 ±44.72),(121.02 ±29.80) Hu in benign group.In plain CT scanning,the difference of CT value between malignant and benign lesions was not significant;but in the enhanced phase,the CT value of malignant lesions was significantly lower than that of benign lesions,and the difference was statistically significant (P < 0.05).Conclusions The lesion with its size ≥ 3.0 cm,irregnlar morphology,cystic necrosis,calcification,pancreatic and bile duct dilatation is suggestive of malignancy tumor.The average CT values of malignant group are lower than those of the benign group in arterial,venous and balance phases.
