1.Progress in the application of external therapies in traditional Chinese medicine for the management of gouty arthritis

		                        			Gouty arthritis is a condition resulting from the dysregulation of purine metabolism in the human body. It is characterized by redness, swelling, recurrent episodes of hot pain, and joint stiffness. Currently, Western medicine primarily relies on uric acid-lowering, anti-inflammatory, and analgesic drugs, but these medications often have significant side effects. In contrast, external therapies in traditional Chinese medicine are considered simpler, safer, and less likely to produce adverse reactions, thus making them more appealing to patients. 		                        			Patients with severe trauma require an extremely timely treatment and transfusion plays an irreplaceable role in the emergency treatment of such patients. An increasing number of evidence-based medicinal evidences and clinical practices suggest that patients with severe traumatic bleeding benefit from early transfusion of low-titer group O whole blood or hemostatic resuscitation with red blood cells, plasma and platelet of a balanced ratio. However, the current domestic mode of blood supply cannot fully meet the requirements of timely and effective blood transfusion for emergency treatment of patients with severe trauma in clinical practice. In order to solve the key problems in blood supply and blood transfusion strategies for emergency treatment of severe trauma, Branch of Clinical Transfusion Medicine of Chinese Medical Association, Group for Trauma Emergency Care and Multiple Injuries of Trauma Branch of Chinese Medical Association, Young Scholar Group of Disaster Medicine Branch of Chinese Medical Association organized domestic experts of blood transfusion medicine and trauma treatment to jointly formulate Chinese expert consensus on blood support mode and blood transfusion strategies for emergency treatment of severe trauma patients ( version 2024). Based on the evidence-based medical evidence and Delphi method of expert consultation and voting, 10 recommendations were put forward from two aspects of blood support mode and transfusion strategies, aiming to provide a reference for transfusion resuscitation in the emergency treatment of severe trauma and further improve the success rate of treatment of patients with severe trauma.
		                        		
		                        		
		                        		
		                        	
3.The effectiveness and safety of laparoscopic versus open hepatectomy for intrahepatic cholangiocarcinoma: a meta-analysis

		                        			Objective:To compare the effectiveness and safety of laparoscopic hepatectomy (LH) versus open hepatectomy (OH) for intrahepatic cholangiocarcinoma(ICC).Methods:PubMed, Web of Science, Embase, Cochrane Library, China Biology Medicine Disc, China National Knowledge Infrastructure, Wanfang Database and VIP Database were searched from inception of these databases to May 2022 to compare LH versus OH for ICC. The duration of operation, intraoperative bleeding, proportion of patients with intraoperative blood transfusion, number of resected lymph nodes, proportion of patients with R 0 resection margin, duration of hepatic occlusion, hospital stay, incidence of postoperative complication and postoperative mortality were compared between the two groups. Meta-analysis was carried out using the Review Manager 5.1 software, and the mean difference ( MD) or odds ratio ( OR) was used as the effect index. Results:This meta-analysis included twelve articles, all of which were retrospective cohort studies, with 3 189 patients. There were 667 patients in the LH group and 2 512 in the OH group. Meta-analysis showed that when compared to the OH group, the LH group had significantly less intraoperative bleeding ( MD=-116.06, 95% CI: -173.07--59.06, P<0.001), less proportion of patients receiving intraoperative blood transfusion ( OR=0.25, 95% CI: 0.10-0.62, P=0.003), less number of lymph nodes removed ( MD=-101.91, 95% CI: -124.78--79.03, P<0.001), less patients underwent portal occlusion ( OR=0.37, 95% CI: 0.14 - 0.99, P=0.050), shorter hospital stay ( MD=-2.43, 95% CI: -4.59--0.28, P=0.030) and less postoperative complications ( OR=0.41, 95% CI: 0.28-0.61, P<0.001). However, the proportion of patients with R 0 margin ( OR=1.49, 95% CI: 1.14-1.95, P=0.003) in the LH group was significantly higher than the OH group. There were no significant differences in operative time and postoperative mortality between the 2 groups. Conclusion:LH was more effective and safe than OH in the treatment of ICC. 4.Comparison of protein expressions of 8-oxodG versus 8-oxoG in the temporal lobe and frontal lobe in SAMP8 mice

		                        			Objective To study the changes in protein expression levels of 8 oxide of guanine in the temporal and frontal lobes in SAMP8 (senescence accelerated mice)mice versus SAMR1 (control mice).Methods In the study,we collected 12 SAMP8 mice and 12 SAMR1 mice at different month stage(1m,4m,8m,12m)(n=6,each group).After anesthesia,mice were sacrificed.The temporal and frontal lobe tissues of mice were labeled according to the stereotactic anatomical map of mouse brain and taken up for study.After paraffin sections were made,the expression level of 8 oxide of guanine in brain tissues was detected by immunohistochemical method.Results In the same age group at 1 month in the same temporal lobe,the average optical density of 8-oxodG was slightly higher in SAMP8 mice(0.086)than in SAMR1 mice(0.061,t =1.541,P>0.05).There were significant differences in the mean optical density of 8-oxodG in temporal lobe in 4,8 and 12 months groups between SAMP8 and SAMR1 group(0.101 vs.0.081,0.147 vs.0.109 and 0.176 vs.0.120,t =2.405,2.612 and 5.019,respectively,P <0.05).There was no significant difference in the average optical density levels of 8-oxodG in frontal lobe in 1 and 4 months groups between SAMP8 and SAMR1 mice (0.044 vs.0.030,0.062 vs.0.046,t =1.843 and 1.163,respectively,P > 0.05),while the average optical density levels of 8-oxodG in 8 and 12 months groups(0.090 and 0.138)were higher than those of SAMR1 mice in the same age group(0.049 and 0.063)(t =5.327 and 4.88,P<0.01).There was no significant difference in the average optical density of 8-oxoG in temporal lobe in 1 month group between SAMP8 and SAMR1 mice(0.099 vs.0.066,t =1.956,P >0.05).There were significant differences in the average optical density levels of 8-oxoG in temporal lobe in 4,8 and 12 months groups between SAMP8 and SAMR1 mice(0.119 vs.0.076,0.148 vs.0.094 and 0.173 vs.0.101,t =4.033,3.042 and 5.328,respectively,P<0.01).There was no significant difference in the average optical density levels of 8-oxoG in frontal lobe in 1 month and 4 months groups between SAMP8 and SAMR1 mice(0.049 vs.0.039,0.058 vs.0.056,t =0.713 and 0.172,respectively,P >0.05).The average optical density of 8-oxoG in frontal lobe in 8 months and 12 months groups had significant differences between SAMP8 and SAMR1 mice(0.087 vs.0.058,0.12 vs.0.063,t =2.261 and 4.185,P <0.05).In addition,we also found that the average optical density of 8 oxide of guanine in same SAMP8 mice at the same age was higher in temporal lobe than in frontal lobe.Conclusions The protein levels of 8-oxodG and 8-oxoG are increased with age,and the damage caused by nucleic acid oxidation is more severe in temporal lobe than in frontal lobe in SAMP8 mice.
		                        		
		                        		
		                        		
		                        	
5.Quality Analysis of 83 Batches of Compound Jiegeng Mahuangjian Syrup(Ⅱ)from 4 Manufacturers

		                        			OBJECTIVE:To analyze the results of national evaluation inspection for 83 batches of Compound jiegeng mahuangjian syrup(Ⅱ)from 4 manufacturers,and to evaluate their quality. METHODS:Legal inspection method and exploratory research were adopted to test,analyze and evaluate sample statistically. The contents of exploratory research included HPLC method was used to determine the contents of preservative benzoic acid and ephedrine hydrochloride;antibacterial efficacy examination of formulation was studied;pH value of solution was determined;the content of sucrose was determined by polarimetry photometry;the relationship of the content of ammonium chloride with microbial contamination was studied;headspace GC method was used to determine the contents of menthol and ethanol;Platycodon grandiflorum extractum was identified and studied by TLC. RESULTS:Results of legal inspection showed that among 83 batches of sample,80 batches were qualified and 3 batches were unqualified,with qualified rate of 96.4%. Unqualified items were loading capacity,microbial limit and content ofammonium chloride,content of ammonium chloride. The results of legal test combined with exploratory research showed thatantibacterial efficacy of formulation of one manufacturer was not in accordance with the regulations;Some raw materials were notfed according to formulation, and the quality of products was not even. It is recommended to revise the quality standard:identificatied the Campanulaceae by TLC; determination the pH, preservative content, menthol content; revision ephedrinehydrochloride assay method to HPLC. CONCLUSIONS:The overall quality of Compound jiegeng mahuangjian syrup(Ⅱ)is notsatisfactory;there is a big flaw in the production process and quality control of some manufacturers;quality standards needed to beimproved.
		                        		
		                        		
		                        		
		                        	
6.A study on the timing and modality of surgery for pancreatic sinistral portal hypertension

		                        			Objective To investigate the opportunity and skill of surgery for pancreatic sinistral portal hypertension.Methods Clinical data were retrospectively analyzed on 15 cases of pancreatic sinistral portal hypertension admired from Dec 2015 to Dec 2017.Results All fiften cases underwent surgical treatment,among them three cases were initially treated conservatively in the early stage and treated surgically for gastrointestinal bleeding,12 cases with definite pancreatic disease and pancreatic sinistral portal hypertension treated in the first stage.Three patients underwent second surgery for recurrent gastrointestinal bleeding.The patients were followed up for 6 to 18 months with symptoms significantly impioved without deaths.Conclusions Splenectomy combined with esophagogastric devascularization is the basic surgical treatment for pancreatic sinistral portal hypertension.
		                        		
		                        		
		                        		
		                        	
7.Association of interleukin-13RS20541 gene polymorphism with enterovirus 71 infection in children with hand-foot-mouth disease

		                        			Objective To explore the relationship between interleukin(IL)-13 RS20541 gene polymorphism and susceptibility of enterovirus 71 (EV71) infection in children's hand-foot-mouth disease.Methods Blood samples were collected from 123 children with EV71 infection from the Central Hospital of Xiangtan (experimental group),and they were divided into mild EV71 infection group (n =62) and severe EV71 infection group (n =61) according to their severity.And 52 healthy children without EV71 infection were selected as the controls,with age and sex matched.Two mL blood samples stored in the-80 ℃ freezer,were collected from all subjects,and DNA was extracted by Beijing ubiolab genetic technology company limited.The SNaPshot was used to determine genotype for G/A polymorphism at RS20541 position of IL-13 gene.SPSS 18.0 software was used to analyze the data.Results IL-13 RS20541 loci had 3 genotypes:AA,GA,GG;the frequency of AA,GA,GG in the experimental group was 4.07%,44.71%,51.22%,which was significantly lower than that in the healthy control group (the frequency of AA,GA,GG were 11.54%,32.69%,55.77%),there was no statistically significant difference in genotypes (x2 =4.676,P > 0.05);there was no statistically significant difference in allele frequency (the frequency of A,G in experimental group was 26.42 %,73.58 %,and that of the healthy control group was 27.88 %,72.12 %;x2 =0.080,P > 0.05).EV71 infection caused by mild group,severe group and healthy controls genotype frequencies between the 3 groups [(AA + GA) were 53.22%,44.26%,44.23%;GG were 46.78%,55.74%,55.77%;x2 =1.294,P > 0.05] and allele frequency (A were 30.65%,22.13%,27.88%;G were 69.35%,77.87%,72.12%;x2 =2.349,P > 0.05) among the mild group,severe group and healthy control group had no statistical significance.Conclusion There is no correlation between the IL-13RS20541 gene polymorphism and EV 71 infection in children with the hand-foot-mouth disease.
		                        		
		                        		
		                        		
		                        	
8.Study on the expression level of 8-Hydroxy-2'-deoxyguanine in rhesus monkeys of different ages

		                        			Objectives To investigate the expression of 8-Hydroxy-2'-deoxyguanine(8-oxodG)in white blood cell,plasma and urine of rhesus monkey of different age group.Methods 30 female rhesus macaques at different age(1y,5y,10y,15y,20y,25y)were selected and grouped(n=5,each).10 mL of morning urine and 5 mL of fasting venous blood were collected.The level of 8-oxodG expression in plasma,leukocyte and urine was measured by high-performance liquid chromatography-mass spectrometry(HPLC-MS) method.Results The level of 8-oxodG in leukocytes,plasma and urine was increased along with aging.The level of 8-oxodG was 1.8,1.6 and 1.4 times higher in 25 year group than in 1 year group in plasma,white blood cell and urine,respectively(P<0.05).The 8-oxodG level was more than 40 times higher in urine than in plasma.Conclusions The expression level of 8-oxodG is increased along with aging.It may be one of the experimental evidence of the aging markers.
		                        		
		                        		
		                        		
		                        	
9.Correlation of interleukin-6 RS1800796 gene polymorphism with enterovirus 71 infection

		                        			Objective To explore the relationship between the interleukin-6 (IL-6) RS1800796 gene polymorphism and susceptibility of Enterovirus 71 (EV71) infection.Methods One hundred and twenty-three children with EV71 infection were selected as infection experimental group from March 2012 to December 2014 in the Central Hospital of Xiangtan,and they were divided into mild EV71 infection group (62 cases) and severe EV71 infection group (61 cases).And 52 age-and gender-matched healthy children were selected as the healthy control group.Two mL blood samples were collected from all subjects,and DNA was extracted by Beijing Optimal Boland Gene Technology LTD.The SNaPshot was used to determine the genotype for G/C polymorphism at RS1800796 position of IL-6 gene.Results The genotype frequency of IL-6 RS1800796 GG in the infection experimental group [73.2% (90/123 cases)]was significantly higher than that in the healthy control group[48.1% (25/52 cases)],and the difference was statistically significant (x2 =10o 215,P =0.002,OR =2.945,95 % CI:1.500-5.782).No significant difference was found in the distribution of genotype frequency of the IL-6 RS1800796 GG between the mild EV71 infection group and the severe EV71 infection group[71.0% (44/62 cases)vs.75.4% (46/61 cases),x2 =0.309,P =0.685].The G allele in IL-6 RS1800796 G/C was more frequent in the infection group (85.0%)than that in the control group (70.2%),and the difference was statistically significant (x2 =10.183,P =0.002,OR =2.399,95% CI:1.389-4.143).No significant difference was found in allele frequency of the IL-6 RS1800796 G between the mild EV71 infection group and the severe EV71 infection group (83.1% vs.86.9%,x2 =0.703,P =0.477).Conclusion The G allele of IL-6 RS1800796 confers susceptibility to infection of EV71.But G allele carrier will not increase the risk of severity after infection.
		                        		
		                        		
		                        		
		                        	
10.Construction of Integrated Medication Decision-making System of Real-time Intervention in Advance,In-teractive Review in the Matter and Afterward Comment and Analysis in Our Hospital

		                        			OBJECTIVE:To construct integrated medication decision-making system,and to provide reference for safe,effec-tive,economical and reasonable drug use in clinic. METHODS:The construction of integrated medication decision-making system was introduced in our hospital. RESULTS&CONCLUSIONS:The integrated medication decision-making system included three as-pects as real-time intervention in advance,interactive review in the matter,afterward comment and analysis,as 3 steps before and after the implementation of inpatient medical order and outpatient prescription. The real-time intervention in advance was mainly to control the rationality of medical orders or prescriptions from their source,thus requiring the timely treatment. The interactive re-view in the matter required that pharmacists judged the medical order or prescription and made a decision of refusing prescribing within the fixed time. To make a decision accurately,the clinical information should be highly structured and correlated with drug signs. Afterward comment and analysis required that pharmacists evaluated the medical order or prescription regularly after the im-plementation. The three were not only different,but also coordinated,and they were an organic unity,especially interactive review in the matter needed to be coordinated with the other two processes. After the implementation of integrated medication decision-mak-ing system,the proportion of irrational medical orders or prescriptions in our hospital decreased from 6.07% in the first half of 2016 to 2.56% at corresponding period of 2017. Drug dose errors and incompatibility were greatly reduced,and medication against contraindications was more likely to be found in the review and analysis. The system can improve the efficiency of prescription re-view,improve rational drug use,and effectively ensure the safety,effectiveness,economy and rationality of clinical drug use.
