Background and Objectives:
The Fractional Flow Reserve and Intravascular UltrasoundGuided Intervention Strategy for Clinical Outcomes in Patients with Intermediate Stenosis (FLAVOUR) trial demonstrated non-inferiority of fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) compared with intravascular ultrasound (IVUS)-guided PCI. We sought to investigate the cost-effectiveness of FFR-guided PCI compared to IVUS-guided PCI in Korea.
Methods:
A 2-part cost-effectiveness model, composed of a short-term decision tree model and a long-term Markov model, was developed for patients who underwent PCI to treat intermediate stenosis (40% to 70% stenosis by visual estimation on coronary angiography).The lifetime healthcare costs and quality-adjusted life-years (QALYs) were estimated from the healthcare system perspective. Transition probabilities were mainly referred from the FLAVOUR trial, and healthcare costs were mainly obtained through analysis of Korean National Health Insurance claims data. Health utilities were mainly obtained from the Seattle Angina Questionnaire responses of FLAVOUR trial participants mapped to EQ-5D.
Results:
From the Korean healthcare system perspective, the base-case analysis showed that FFR-guided PCI was 2,451 U.S. dollar lower in lifetime healthcare costs and 0.178 higher in QALYs compared to IVUS-guided PCI. FFR-guided PCI remained more likely to be cost-effective over a wide range of willingness-to-pay thresholds in the probabilistic sensitivity analysis.
Conclusions
Based on the results from the FLAVOUR trial, FFR-guided PCI is projected to decrease lifetime healthcare costs and increase QALYs compared with IVUS-guided PCI in intermediate coronary lesion, and it is a dominant strategy in Korea.

5.Risk assessment of brain metastasis after prophylactic cranial irradiation for 550 limited stage small cell lung cancer patients with remission after radiochemotherapy

Objective:To evaluate the risk and prognostic factors of brain metastasis (BM) after prophylactic cranial irradiation (PCI) in limited stage small cell lung cancer (LS-SCLC) patients with complete and partial remission (CR/PR) after radiochemotherapy.Methods:Baseline data of 550 patients with LS-SCLC who obtained CR/PR after chemoradiotherapy and received PCI in Zhejiang Cancer Hospital between 2002 and 2017 were collected. The risk of BM and clinical prognosis were retrospectively analyzed. The survival analysis was performed by Kaplan-Meier method. Multivariate prognostic analysis was conducted byCox models.Results:The overall BM rate after PCI was 15.6%(86/550), with 9%(4/43), 13%(7/52), and 16.5%(75/455) for stage Ⅰ, Ⅱ and Ⅲ patients, respectively. The median overall survival (OS) for the entire cohort was 27.9 months, and the 5-year OS rate was 31.0%. The OS was 24.9 and 30.2 months for patients with or without BM, and the 5-year OS rates were 8.9% and 36.1%( P<0.001). BM was an independent factor of OS ( P<0.001). Clinical staging remained the influencing factor of OS and BM-free survival ( P<0.001, P=0.027). Having tumors of ≥5 cm in diameter significantly increased the risk of BM ( P=0.034) rather than the OS ( P=0.182). The median OS of patients aged<60 years was significantly longer than those aged ≥60 years (34.9 months vs. 24.6 months, P=0.001). The median OS of patients irradiated with 2 times/d was 29.8 months, significantly longer than 24.5 months of those irradiated with 1 time/d ( P=0.013). Age, sex, radiotherapy fraction and efficacy of radiochemotherapy (CR/PR) were not associated with the incidence rate of BM (all P>0.05). Conclusions:SCLC patients with tumors of ≥5 cm in diameter may have a higher risk of developing BM after PCI. Patients aged<60 years achieve better OS compared with their counterparts aged ≥60 years.
6.Effect of sevoflurane post-conditioning on oxygen-glucose deprivation/reoxygenation injury and autophagy of primary hippocampal neurons

Objective:
To explore the effect of sevoflurane post-conditioning on oxygen-glucose deprivation/reoxygenation(OGD/R) damage of primary hippocampal neurons, oxidative stress and autophagy.
Methods:
Hippocampal neurons were cultured in uterine endometrial rats for(18±0.5) days. After 7 days, the cultured hippocampal neurons were randomly divided into three groups: normal control group(CON group), oxygen-glucose deprivation/reoxygenation(OGD/R) group and sevoflurane post-conditioning group(OGD/R+SEVO group). Primary hippocampal neurons in the CON group were cultured normally; OGD/R group was treated with oxygen-glucose deprivation for 1.5 hours and reoxygenation for 24 hours to establish oxygen and glucose deprivation/reoxygenation injury model; OGD/R+SEVO group was treated with sevoflurane for 1 hour and then reoxygenated for 23 hours. After treatments, the activity of each group of LDH was detected by colorimetric method, TUNEL staining method detected the changes of hippocampal nerve cells in each group, immunofluorescence method detected mitochondrial ROS and the expression of autophagy proteins LC3 B,and Western blot analysis detected the expression of PINK1 and Parkin.
Results:
Compared with CON group, the release of lactate dehydrogenase increased(P<0.01); neuronal apoptosis increased; mitochondrial ROS and the expression of autophagy marker proteins LC3 B increased(P<0.01), PINK1 and Parkin also increased in OGD/R group(P<0.001,P<0.05). Compared with OGD/R group, the release of lactate dehydrogenase was reduced(P<0.05), the apoptosis was inhibited, mitochondrial ROS and the expression of autophagy marker proteins LC3 B were reduced(P<0.05), PINK1 and Parkin protein were reduced(P<0.01,P<0.05) in OGD/R+SEVO group.
Conclusion
Sevoflurane post-conditioning can protect primary hippocampal neurons by reducing OGD/R-induced excessive autophagy, reducing oxidative stress, and reducing apoptosis, and its mechanism of reducing autophagy may be related to the PINK1/Parkin pathway.

Objective:The investigation and research on the application status of Hepatic Venous Pressure Gradient (HVPG) is very important to understand the real situation and future development of this technology in China.Methods:This study comprehensively investigated the basic situation of HVPG technology in China, including hospital distribution, hospital level, annual number of cases, catheters used, average cost, indications and existing problems.Results:According to the survey, there were 70 hospitals in China carrying out HVPG technology in 2021, distributed in 28 provinces (autonomous regions and municipalities directly under the central Government). A total of 4 398 cases of HVPG were performed in all the surveyed hospitals in 2021, of which 2 291 cases (52.1%) were tested by HVPG alone. The average cost of HVPG detection was (5 617.2±2 079.4) yuan. 96.3% of the teams completed HVPG detection with balloon method, and most of the teams used thrombectomy balloon catheter (80.3%).Conclusion:Through this investigation, the status of domestic clinical application of HVPG has been clarified, and it has been confirmed that many domestic medical institutions have mastered this technology, but it still needs to continue to promote and popularize HVPG technology in the future.

The aim of the present study was to investigate the effects and mechanisms of lidocaine on lipopolysaccharide(LPS)-induced matrix metalloproteinase(MMP)-9 and MMP-2 activity in plasma, and the effects of lidocaine on LPS-induced acute lung injury(ALI). Mice were pretreated with lidocaine(2, 4, 8 mg/kg )for 30 minutes, and then treated with 10 mg/kg LPS(ip)for 12 h to induce ALI. The 7-day survival rate and lung wet/dry weight ratio of mice were monitored. Phosphorylation level of p38 was measured by western blot. The activity of MMP-9 and MMP-2 in plasma was evaluated by gelatin zymography. The results showed that pretreatment with lidocaine could significantly reduce the death rate of ALI mice as well as the lung wet/dry weight ratio in a dose-dependent manner and suppress the activity of MMP-9 and MMP-2 in plasma. Moreover, lidocaine also markedly inhibited LPS-induced upregulation of p-p38 in a dose-dependent manner. In conclusion, lidocaine alleviated LPS-induced acute lung injury by suppressing MMP-9 and MMP-2 activity.
10.Analysis on present situation of post-hospital care needs scale in patients with colostomy and validity of the research

Objective To build demand scale for Post-hospital intestinal colostomy patients and test the reliability and validity. Methods Through the investigation of relevant literature at home and abroad,Based on the Maslow's Need Hierarchy Theory, in combination with intestinal colostomy patients psychological scale, using qualitative research, a semi-structured interview was conducted in 12 patients with colostomy A thorough understanding of colostomy patients Post-hospital care needs content (interview time as follows:September-October 2015), to analysis the interview recording data, the item pool of the scale for post-hospital care needs in patients with colostomy was constructed. Choosing 200 patients from the department of Proctology of third-grade class-A hospitals of Jilin province, and carries on the scale of clinical testing, after collecting the relative data , and make use of Critical Ration、the correlation coefficient method and the internal consistency coefficient method、further concludes that the scale items should be added or not, and picking up the internal consistency reliability 1/2 reliability retest reliability content validity and structure validity to analysis each item. Results The demand scale for Post-hospital intestinal colostomy patients included 6 dimensions and 24 items. The total content validity index (S-CVI) of the scale was 0.968, and the content validity index (I-CVI) of each item was 0.78-1.00. By exploratory factor analysis, 6 principal components were extracted, and the cumulative explanation variation of 6 principal components was 91.753%. This indicated that the scale had good validity. The total Cronbach a coefficient of the scale was 0.751, and the Cronbach a coefficient of each dimension was 0.968-0.974, all above 0.7, which indicated that the internal consistency was good. The half-fold reliability and retest reliability of the scale were 0.794 and 0.827 respectively. Conclusion The scale designed in this study has good reliability and validity, and can be used as an effective assessment tool for nursing needs of patients after discharge from enterotomy.
