BACKGROUND:
This study aimed to determine the reasons for conversion and elucidate the safety and efficacy of transition to tenofovir alafenamide/emtricitabine/bictegravir sodium (TAF/FTC/BIC) in highly active antiretroviral therapy (HAART)-experienced HIV-infected patients in real-world settings.
METHODS:
We conducted a retrospective cohort study. The treatment conversion rationales, safety, and effectiveness in 1684 HIV-infected patients with previous HAART experience who switched to TAF/FTC/BIC were evaluated at Beijing Ditan Hospital from September 2021 to Auguest 2022.
RESULTS:
Regimen simplification (990/1684, 58.79%) was the most common reason for switching, followed by osteoporosis or osteopenia (375/1684, 22.27%), liver dysfunction (231/1684, 13.72%), decline in tenofovir alafenamide/emtricitabine/elvitegravir/cobicistat (TAF/FTC/EVG/c) with food restriction (215/1684, 12.77%), virological failure (116/1684, 6.89%), and renal dysfunction (90/1684, 5.34%). In patients receiving non-nucleotide reverse transcriptase inhibitors (NNRTI)-containing regimens, lipid panel changes 1 year after switching indicated a difference of 3.27 ± 1.10 mmol/L vs . 3.40 ± 1.59 mmol/L in triglyceride ( P = 0.014), 4.82 ± 0.74 mmol/L vs . 4.88 ± 0.72 mmol/L in total cholesterol ( P = 0.038), 3.09 ± 0.70 mmol/L vs . 3.18 ± 0.66 mmol/L in low-density lipoprotein ( P <0.001), and 0.99 ± 0.11 mmol/L vs . 0.95 ± 0.10 mmol/L in high-density lipoprotein ( P <0.001). Conversely, among patients receiving booster-containing regimens, including TAF/FTC/EVG/c and lopinavir/ritonavir (LPV/r), lipid panel changes presented decreased trends. We also observed an improved trend in viral load suppression, and alanine transaminase (ALT), aspartate transaminase (AST), estimated glomerular filtration rate (eGFR), and serum creatinine levels after the transition ( P <0.001).
CONCLUSION
The transition to TAF/FTC/BIC demonstrated good treatment potency. Furthermore, this study elucidates the motivations behind the adoption of TAF/FTC/BIC in real-world scenarios, providing clinical evidence supporting the stable conversion to TAF/FTC/BIC for HAART-experienced patients.

ObjectiveTo explore the relationship between the intestinal flora and the impairment of liver and kidney in HIV-infected men who have heterosexual sex with healthy women. MethodsFecal samples from 41 HIV-infected heterosexual men who have sex with women (PMSW) and 43 age- and BMI-matched healthy heterosexual men who have sex with women (NMSW) were collected and subjected to 16S rDNA sequencing. The blood levels of AST, ALT, TBIL, UREA, Cr, UA, β2-MG and other liver and kidney function indicators were measured. Bioinformatics methods were used to analyze the characteristics of the intestinal flora of the patients in these two groups, to compare the differential bacteria strains, and to analyze their correlation with liver and kidney function indicators. ResultsIn comparison with NMSW, the alpha diversity of intestinal flora was decreased in PMSW, and the beta diversity analysis showed significant differences in flora characteristics between the two groups (P<0.05). The abundance of Clostridium, Phylum thick-walled, Trichosporon, and Clostridium tumefaciens decreased but Fusobacteriota increased (LDA score >4). The comparison of liver and kidney function indexes revealed that AST, β2-MG levels were higher in PMSW than in NMSW, while TBIL was lower in PMSW than in NMSW. The number of patients with abnormal β2-MG was much higher in PMSW than in NMSW, and the difference was statistically significant (P<0.001). It was also found that AST was negatively correlated with Clostridium (P<0.05); TBIL was negatively correlated with Clostridium and positively correlated with Phylum thick-walled and Trichosporon (P<0.05). β2-MG was negatively correlated with Phylum thick-walled, Clostridium, Trichosporon and Rumenococcus (P<0.05) and positively correlated with Clostridium (P<0.05). ConclusionIn PMSW group, the alpha diversity of the flora is decreased. AST and β2-MG levels are increased, and TBIL level is decreased. These changes were significantly correlated with different strains of bacteria in the intestinal flora.

A new definition of metabolic dysfunction-associated fatty liver disease (MAFLD) has recently been proposed. We aim to examine the associations of MAFLD, particularly its discordance from non-alcoholic fatty liver disease (NAFLD), with the progression of elevated brachial-ankle pulse wave velocity (baPWV) and albuminuria in a community-based study sample in Shanghai, China. After 4.3 years of follow-up, 778 participants developed elevated baPWV and 499 developed albuminuria. In comparison with the non-MAFLD group, the multivariable adjusted odds ratio (OR) of MAFLD group for new-onset elevated baPWV was 1.25 (95% confidence interval (CI) 1.01-1.55) and 1.35 (95% CI 1.07-1.70) for albuminuria. Participants without NAFLD but diagnosed according to MAFLD definition were associated with higher risk of incident albuminuria (OR 1.77; 95% CI 1.07-2.94). Patients with MAFLD with high value of hepamet fibrosis score or poor-controlled diabetes had higher risk of elevated baPWV or albuminuria. In conclusion, MAFLD was associated with new-onset elevated baPWV and albuminuria independently of body mass index, waist circumference, and hip circumference. Individuals without NAFLD but diagnosed as MAFLD had high risk of albuminuria, supporting that MAFLD criteria would be practical for the evaluation of long-term risk of subclinical atherosclerosis among fatty liver patients.

Journal of Prevention and Treatment for Stomatological Diseases 2021;29(4):254-259
Objective:
To study the diagnostic accuracy and the distance between the root of maxillary posterior tooth and the maxillary sinus using panoramic radiography and cone beam computer tomography; to provide basic information for clinicians to treat diseases in the maxillary posterior region.
Methods:
Eighty patients were included in this study. A total of 671 specimens were measured for the distance between the root tip and the maxillary sinus floor in both imaging modalities.
Results :
The roots that did not contact the sinus floor or contacted but did not project into the sinus cavity showed an agreement of 82% and 70% when using panoramic radiography. Forty-eight percent of the roots that projected into the sinus cavity in panoramic radiography showed protrusion into the sinus with cone beam computer tomography (CBCT). For panoramic radiography and CBCT showing root projections into the sinus cavity, the average distances were 2.19 ± 1.82 mm and 1.47 ± 1.01 mm, respectively. There was a significant difference between the two values (P < 0.05).
Conclusion
Panoramic radiography is more accurate when roots of maxillary posterior teeth do not contact the sinus floor or contact it. However, it has a lower accuracy rate when the tooth roots protrude into the sinus.
10.Circ_0084927 regulates breast cancer immune escape via miR-623/EZH2 axis
Chinese Journal of Endocrine Surgery 2021;15(5):472-477
Objective:To investigate the effect of circ_0084927 as a molecular sponge adsorbing miR-623 to target and regulate EZH2 gene on immune escape in breast cancer (BC) cells. Methods:Bioinformatics websites were used to predict the circ_0084927/miR-623/EZH2 regulatory axis, which was then validated with the aid of dual luciferase reporter assay. BC cells were co-cultured with CD8+ T cells and theflow cytometry assays were performed to detect apoptotic rate and the proportion of T cells in the co-transfected microenvironment. Results:The expression level of EZH2 and circ_0084927 were significantly upregulated in BC patient tissue samples compared to paracancerous tissue, while miR-623 expression was dramatically diminished ( P<0.05) . circ_0084927 can be used as an effective diagnostic index of BC (AUC= 0.813 8, P<0.000 1) . Compared with apoptosis rate of CD8+ T cells (34.17±3.06) and percentage of T cells (26.55±2.83) , circ_0084927 inhibition caused a decrease in the apoptosis rate of CD8+ T cells (22.37±2.05) and an increase in the proportion of CD8+ T cells (31.88±3.03) in the co-transfected environment, but this effect was partiallycounteractedby overexpression of EZH2. miR-623 inhibited CD8+ T cell apoptosis. Circ_0084927 attenuated the inhibitory effect of miR-623 on immune escape (both P<0.05) . Conclusion:circ_0084927 regulates the miR-623/EZH2 axis to promote immune escape of BC cells, suggesting targeting circ_0084927 may be a potential way to improve the efficacy of immunotherapy for BC.
