Diabetic macular ischemia(DMI), playing key roles behind the progression of diabetic retinopathy(DR), is one of the causes of vision loss. Its pathophysiological process is complex, involving vascular structure changes, hemodynamic abnormalities, and many other aspects. Traditional inspection methods often make it difficult to accurately capture subtle changes in DMI. In turn, optical coherence tomography angiography(OCTA)has opened a pathway to the diagnosis and treatment of DMI, which not only has the advantages of non-invasive, safe and rapid imaging, but also furnishes high-resolution retinal vascular images to provide qualitative and quantitative descriptions of DMI. OCTA can visually show the morphological and structural changes of blood vessels in the macular region, as well as reveal the characteristics of pathological changes in the superficial and deep capillary plexus of DMI patients, which provides a deeper understanding of DMI and a new treatment option of diabetic eye disease.

Periodontitis is a chronic inflammatory and immune reactive disease induced by the subgingival biofilm. The therapeutic effect for susceptible patients is often unsatisfactory due to excessive inflammatory response and oxidative stress.Sinensetin(Sin)is a nature polymethoxylated flavonoid with anti-inflammatory and antioxidant activities. Our study aimed to explore the beneficial effect of Sin on periodontitis and the specific molecular mechanisms. We found that Sin attenuated oxidative stress and inflammatory levels of periodontal ligament cells(PDLCs)under inflammatory conditions.Administered Sin to rats with ligation-induced periodontitis models exhibited a protective effect against periodontitis in vivo.By molecular docking,we identified Bach1 as a strong binding target of Sin,and this binding was further verified by cellular thermal displacement assay and immunofluorescence assays.Chromatin immunoprecipitation-quantitative polymerase chain reaction results also revealed that Sin obstructed the binding of Bach1 to the HMOX1 promoter, subsequently upregulating the expression of the key antioxidant factor HO-1.Further functional experiments with Bach1 knocked down and overexpressed verified Bach1 as a key target for Sin to exert its antioxidant effects.Additionally,we demonstrated that Sin prompted the reduction of Bach1 by potentiating the ubiquitination degradation of Bach1,thereby inducing HO-1 expression and inhibiting oxidative stress.Overall,Sin could be a promising drug candidate for the treatment of periodontitis by targeting binding to Bach1.

By comparing the training status of clinical research methods between United States and China, this article introduces comprehensive training system in the United States in and after the university, and discusses the problems in clinical research methodology training in China. These issues include that when medical students at school do not receive the training of professional clinical research courses, and after they go to the work, they also lack professional and accurate clinical research methodology training, which make it difficult for doctors to independently conduct clinical research. Therefore, it is recommended that Chinese medical schools should systematically establish clinical research methodology courses for undergraduates and graduate students to cultivate the clinical research capabilities of medical students. Secondly, according to the clinical research levels of doctors, different training models are proposed in the study. The existing social resources should be scientifically guided. We hope our work would provide some references for the improvement of clinical research methodology in China, to promote clinicians to be independently responsible for conducting clinical research, and improve the national medical level.
5.Analysis on the psychological response of clinical front-line nurses in response to public health emergencies
Chinese Journal of Practical Nursing 2022;38(24):1879-1884
Objective:To investigate the psychological state and influencing factors of clinical front-line nurses in response to public health emergencies. Methods:We used stratified cluster random sampling method to survey 1187 front-line clinical nurses from February to March, 2020, using the Psychological Questionnaire for Emergent Events of Public Health. Results:The questionnaire survey showed that the scores for dimensions of depression, neurasthenia, fear, obsessive-anxiety and hypochondria were (0.52±0.02), (0.37±0.01), (0.87±0.02), (0.24±0.01) and (0.25±0.01), respectively. There were statistically significant differences in scores of hypochondriac dimension for clinical nurses at different hospital levels ( F=4.30, P<0.05), and there were statistically significant differences in scores of depression, neurasthenia, obsessive-anxiety and hypochondriac dimension for nurses in different nursing units ( F values were 4.34-11.11, all P<0.05). Multiple linear regression analysis showed that the nursing unit factors had influence on depression, neurasthenia, obsessive-anxiety and hypochondria. Conclusions:Clinical front-line nurses experienced negative psychology such as depression, neurasthenia and fear in public health emergencies. As a manager, it is necessary to strengthen preventive psychological counseling, do a good job in the theory, operation and protection knowledge training for public health emergencies, humanistic care, establish a good organizational atmosphere to maintain the physical and mental health of clinical nurses, and ensure the quality and safety of nursing.

Cardiovascular disease (CVD) has been an important public health issue in China. Recently, the prevalence of obesity, elevated blood pressure, hyperglycemia, dyslipidemia, and unhealthy lifestyles was high in Chinese children and adolescents, which might increase the risk of short-term target organ damage and adult CVD. Therefore, we selected one primary school in Huantai County, Zibo City between November 2017 and January 2018 to establish the Huantai Childhood Cardiovascular Health Cohort Study. The aim of this project was to investigate the effects of childhood exposures on short-term target organ damage and adult CVD, and to provide evidence to curb the epidemic of CVD in China from the source. A total of 1 515 children aged 6-11 years old were included at the baseline. In this paper, we will introduce the overall design of the survey, including participants, methods, quality control, basic characteristics of the participants, study limitations, and future directions, which might provide some useful implications for similar researches in the future.

Objective:To analyze the association between elevated blood pressure (BP) and carotid intima-media thickness (cIMT) in children according to four BP references. Methods:Study population came from"Huantai Childhood Cardiovascular Health Cohort Study"in Huantai County, Zibo City, Shandong Province. A convenient cluster sampling method was used to conduct a cross-sectional survey on 1 515 children from November 2017 to January 2018 in a primary school. A total of 1 431 children aged 6-11 years old with complete data were included in this study. Data on demographic characteristics, BP and cIMT were collected through questionnaire survey, physical examination and ultrasound examination. High cIMT was defined as the level of cIMT ≥ age-and sex-specific 90 th percentile of this study population. Based on the Chinese Guideline reference, the Health Industry reference, the International reference and the U.S. reference, all participants were divided into three subgroups: the normal BP, high normal BP and, elevated BP. The multivariate logistic regression models were used to examine the association between BP status and high cIMT in children. Results:The age of children was (8.9±1.5) years, and boys accounted for 53.4% ( n=682). The multivariate logistic regression models showed that after adjusting for relevant confounding factors, the risk of high cIMT in elevated BP group was increased compared with the normal BP group according to the four references (all P values<0.05) Conclusion:Elevated BP according to the four BP references is associated with high cIMT in children.
