2.A Case of Malignant Struma Ovarii with Cervical Papillary Thyroid Carcinoma

Struma ovarii (SO) is a rare ovarian teratoma, which is diagnosed when thyroid tissue accounts for more than 50% of the teratoma. The majority of SO are benign, but malignant tumors have been reported in a small percentage of cases. Five percent of SO cases have been proven to be malignant and, as in the thyroid gland, papillary carcinoma is the most common histotype arising in SO. Because this tumor is rare, there are no guidelines regarding the management of this cancer. Usually, total thyroidectomy followed by radioiodine treatment is the treatment of choice in metastatic malignant struma ovarii, but therapeutic decisions should be made individually based on clinical and pathological data. We recently experienced a case of a 45-year-old woman finally confirmed as malignant struma ovarii with cervical thyroid cancer. Therefore, we present this unique case with a review of the literature.
3.Comparison of Clinical Pregnancy Rates and Affecting Factors Between Elderly and Young Infertile Females After Intra-Uterine Insemination: Benefited by ‘National Medical-aid Program for ART (assisted reproductive technology) in 2016

Purpose:
The purpose of this study was to evaluate the intrauterine insemination (IUI) success rate and to define the variables for predicting success.
Methods:
The secondary data analysis was used with data collected from infertile females who underwent IUI in Fertility and IVF (In Vitro Fertilization) clinics, who benefited from the ‘National Medical-aid Program for ART (assisted reproductive technology) in 2016’, in which the data of 34,920 IUI cases were retrospectively reviewed. The primary outcome measure was the clinical pregnancy rate in elderly and young infertile females. Data were analyzed by descriptive statistics, χ2 test and logistic regression.
Results:
The pregnancy rate was 12.1% (2,095 cases) in elderly infertile females and 15.6% in young infertile females (2,758 cases) (χ2 = 87.90, p < .001). Using the logistic regression analysis, clinical pregnancy was positively associated with the ovulatory factor (OR= 1.48, p< .001) and male factor (OR= 1.19, p< .05) in elderly infertile females. It was positively associated with the ovulatory factor (OR= 1.30, p= .001) and the peritoneal cavity factor (OR= 0.58, p< .05) in young infertile females.
Conclusion
Our results indicate that the pregnancy rate in young infertile females was higher than that in old infertile females, and the IUI is the effective option in pregnancies in all ages with infertility due to the ovulatory factor. Additionally, further studies are necessary to fully describe pregnancy experiences for all the infertile females.
4.Primary Pelvic Peritoneal Yolk Sac Tumor in the Post-Pubertal Female: a Case Report with Literature Review

Yolk sac tumors are rare malignant germ cell neoplasms that usually arise from the gonads. Extragonadal yolk sac tumors (EGYSTs) frequently occur in the mediastinum in post-pubertal females. EGYSTs in the pelvis are extremely rare, and to date, only thirteen cases have been reported in the English literature. Among them, the primary EGYST of the pelvic peritoneum in post-pubertal females has only been reported in ten cases. The present case describes a 26-year-old female diagnosed with primary peritoneal yolk sac tumor located in the rectouterine pouch. We report clinical and tumor imaging features, including ultrasound, computed tomography (CT), magnetic resonance images (MRI), positron emission tomography-computed tomography (PET-CT), and present a review of the literature.

5.Research Trend about Complementary and Alternative Therapy in Korea using Text Network Analysis.

PURPOSE: This study was aimed to identify research trends related to complementary and alternative therapy (CAT) in Korea. METHODS: Data were collected from 2000 to 2018 articles in Korean database (KISS, RISS and DBPIA). 295 semantic morphemes were extracted from 123 articles by network analysis program. Co-occurrence matrixes of the morphemes were generated by weighting Jaccard-coefficient and then we did network analysis and visualization. RESULTS: Common morphemes with high centralities were ‘Complementary and alternative medicine (CAM)’, ‘CAT’, ‘Aged’, ‘Diet therapy’, ‘Herbal medicine’, ‘Acupuncture’, and ‘Oriental medicine’. Since 2000, studies on complementary and alternative therapies have been actively involved in biological and manipulative therapies, and it has been found that the main target of CAT is the elderly. CONCLUSION: Despite much attention of complementary alternative therapies, the scientific basis is insufficient. Through this study, we could find research trends in complementary alternative therapies and set the direction of future research. It is necessary to expand research on complementary replacement therapy for various diseases and age groups.

6.Prediction of Gestational Age at Birth using an Artificial Neural Networks in Singleton Preterm Birth

PURPOSE: The objective of the present study was to predict the gestational age at preterm birth using artificial neural networks for singleton pregnancy. METHODS: Artificial neural networks (ANNs) were used as a tool for the prediction of gestational age at birth. ANNs trained using obstetrical data of 125 cases, including 56 preterm and 69 non-preterm deliveries. Using a 36-variable obstetrical input set, gestational weeks at delivery were predicted by 89 cases of training sets, 18 cases of validating sets, and 18 cases of testing sets (total: 125 cases). After training, we validated the model by another 12 cases containing data of preterm deliveries. RESULTS: To define the accuracy of the developed model, we confirmed the correlation coefficient (R) and mean square error of the model. For validating sets, the correlation coefficient was 0.839, but R of testing sets was 0.892, and R of total 125 cases was 0.959. The neural networks were well trained, and the model predictions were relatively good. Furthermore, the model was validated with another dataset of 12 cases, and the correlation coefficient was 0.709. The error days were 11.58±13.73. CONCLUSION: In the present study, we trained the ANNs and developed the predictive model for gestational age at delivery. Although the prediction for gestational age at birth in singleton preterm birth was feasible, further studies with larger data, including detailed risk variables of preterm birth and other obstetrical outcomes, are needed.

7.Efficacy and safety of drospirenone 2 mg/17β-estradiol 1 mg hormone therapy in Korean postmenopausal women.

This regulatory post-marketing surveillance study aimed to evaluate the therapeutic efficacy and safety of drospirenone (DRSP) 2 mg/estradiol (E₂) 1 mg tablet in Korean postmenopausal women. A total of 4,149 patients were enrolled and the study was conducted at 207 clinical research centers. The patients' source data was collected between November 2006 and November 2012. More than 85% of patients experienced improvement of menopausal symptoms. The most frequently reported adverse events were vaginal bleeding and breast pain; most of the women suffering from these symptoms fully recovered. The incidence of adverse event was higher in patients of younger age (20 to 39 years), in patients with concomitant diseases, previous hormone replacement therapy in medical history, those treated with DRSP 2 mg/E₂ 1 mg for shorter duration (3 years or less) and in patients using concomitant medication. In conclusion, the results from this large post-marketing surveillance study confirm the efficacy and safety of DRSP 2 mg/E₂ 1 mg tablet in Korean postmenopausal women.

8.High-Grade Urothelial Carcinoma of the Bladder in a Child.

Bladder urothelial carcinoma typically occurs in individuals in their sixties or seventies: it rarely occurs in persons <20 years old. Moreover, in young patients, bladder urothelial carcinoma lesions are reported to be solitary and nonmuscle invasive, with low malignant potential. However, 13% of lesions in this age group are reported to be noninvasive high-grade papillary urothelial carcinoma but these are extremely rare in children <15 years. There is no specific consensus regarding treatment of bladder urothelial carcinoma in children and adolescent. In particular, method has been suggested for follow-up of high-grade bladder urothelial carcinoma. Therefore we report our experience of one case the, discuss surveillance methods, and provide a brief review of the literature.

9.Current Practice of Transradial Coronary Angiography and Intervention: Results from the Korean Transradial Intervention Prospective Registry.

BACKGROUND AND OBJECTIVES: Although increasing evidence has indicated that radial access is a beneficial technique, few studies have focused on Korean subjects. The aim of this study was to evaluate current practice of coronary angiography (CAG) and percutaneous coronary intervention (PCI) using radial access in South Korea. SUBJECTS AND METHODS: A total of 6338 subjects were analyzed from Korean Transradial Intervention prospective registry that was conducted at 20 centers in Korea. After evaluating the initial access, subjects intended for radial access were assessed for their baseline, procedure-related, and complication data. Subjects were categorized into three groups: group of overall subjects (n=5554); group of subjects who underwent PCI (n=1780); and group of subjects who underwent primary percutaneous coronary intervention (PPCI) (n=167). RESULTS: The rate of radial artery as an initial access and the rate of access site crossover was 87.6% and 4.4%, respectively, in overall subjects. Those rates were 82.4% and 8.1%, respectively, in subjects who underwent PCI, and 60.1% and 4.8%, respectively, in subjects who underwent PPCI. For subjects who underwent CAG, a 6-F introducer sheath and a 5-F angiographic catheter was the most commonly used. During PCI, a 6-F introducer sheath (90.6%) and a 6-F guiding catheter were standardly used. CONCLUSION: The large prospective registry allowed us to present the current practice of CAG and PCI using radial access. These data provides evidence to achieve consensus on radial access in CAG and PCI in the Korean population.

10.Black rice extract protected HepG2 cells from oxidative stress-induced cell death via ERK1/2 and Akt activation.

BACKGROUND/OBJECTIVES: The objective of this study was to evaluate the protective effect of black rice extract (BRE) on tert-butyl hydroperoxide (TBHP)-induced oxidative injury in HepG2 cells. MATERIALS/METHODS: Methanolic extract from black rice was evaluated for the protective effect on TBHP-induced oxidative injury in HepG2 cells. Several biomarkers that modulate cell survival and death including reactive oxygen species (ROS), caspase-3 activity, and related cellular kinases were determined. RESULTS: TBHP induced cell death and apoptosis by a rapid increase in ROS generation and caspase-3 activity. Moreover, TBHP-induced oxidative stress resulted in a transient ERK1/2 activation and a sustained increase of JNK1/2 activation. While, BRE pretreatment protects the cells against oxidative stress by reducing cell death, caspase-3 activity, and ROS generation and also by preventing ERKs deactivation and the prolonged JNKs activation. Moreover, pretreatment of BRE increased the activation of ERKs and Akt which are pro-survival signal proteins. However, this effect was blunted in the presence of ERKs and Akt inhibitors. CONCLUSIONS: These results suggest that activation of ERKs and Akt pathway might be involved in the cytoprotective effect of BRE against oxidative stress. Our findings provide new insights into the cytoprotective effects and its possible mechanism of black rice against oxidative stress.
