1.Re-standardization of the Korean- Instrumental Activities of Daily Living (K-IADL): Clinical Usefulness for Various Neurodegenerative Diseases.

BACKGROUND AND PURPOSE: Evaluating instrumental activities of daily living (IADL) is an important part of procedure to diagnose dementia. The Korean-Instrumental Activities of Daily Living (K-IADL) has been used extensively in Korea. However, its cut-off score has not been reformulated since 2002. The purpose of this study was to yield a new optimal cut-off score for the K-IADL and confirm the validity of this new cut-off score with various dementia groups. METHODS: We retrospectively collected a total of 2,347 patients' K-IADL data from 6 general hospitals in Korea. These patients had mild cognitive impairment (MCI) or dementia with various etiologies for cognitive impairment. We also recruited a normal control group (n=254) from the community. Korean-Mini Mental State Examination, Short version of the Geriatric Depression Scale, Clinical Dementia Rating, and Global Deterioration Scale were administered to all participants. Caregivers completed K-IADL and Barthel Index. RESULTS: K-IADL scores were significantly different among dementia subgroups, but not significantly different among MCI subgroups. Based on internal consistency, correlations with other scales, and factor analysis, K-IADL showed excellent reliability and validity. The new optimal cut-off score to diagnose dementia was 0.40, which gave a sensitivity of 0.901 and a specificity of 0.916. Positive predictive value for dementia using the new cut-off score was 94.2% for Alzheimer's disease, 100% for vascular dementia, and 84% for Parkinson's disease. CONCLUSIONS: Our results illustrate that the new K-IADL cut-off score of 0.40 is reliable and valid for screening impairments of daily functioning resulting from various etiologies.

Most of neuroendocrine tumors are usually found in the gastrointestinal tract. Recently, the incidence of gastrointestinal neuroendocrine tumors seems to have increased. However, only a few cases of neuroendocrine tumor arising from the minor duodenal papilla have been reported. Currently, several options are available to treat the tumors of the minor duodenal papilla. Endoscopic papillectomy is increasingly performed as a minimally invasive alternative treatment to conventional surgical resection. We present two cases of neuroendocrine tumor arising from minor duodenal papilla, which were successfully resected by endoscopic papillectomy. Although surgical resection is considered to be a standard treatment for gastrointestinal neuroendocrine tumors, our experience suggests that endoscopic papillectomy can be a minimally invasive alternative treatment for neuroendocrine tumors arising from the minor duodenal papilla.

4.Fifteen-year Experience with Telemedicine Services in Gangwon Province in Korea.

OBJECTIVES: This study attempted to identify the factors that contribute to successful telemedicine service. This was done by analyzing the operational state of successful telemedicine services offered in Gangwon Province of Korea and their outcome for the last fifteen years. METHODS: A comparative analysis was made based on reports and a thesis on the satisfaction rate of patients and providers, patient compliance to treatment, and economic assessment of Gangwon telemedicine service, which were carried out in three periods: the years 2006, 2010, and 2012. RESULTS: The satisfaction surveys in all three periods showed similar results for patients (4.46+/-0.70 point) and healthcare practitioners, including nurses (3.82+/-0.62 point) and physicians (3.60+/-0.56 point), in decreasing order from the year 2012. Through the survey of patients' compliance with treatment, it was confirmed that telemedicine services increased patients' compliance with drug administration, facilitated improvement of lifestyle habits, improved glycated hemoglobin for patients with diabetes mellitus, and enhanced the rate of blood pressure control. In the survey conducted on patients' willingness to pay for telemedicine services in 2007, it was found that those patients were willing to pay about $3.5 for services. CONCLUSIONS: The telemedicine services of Gangwon Province increased patients' compliance with drug administration, improved blood glucose control, enhanced blood pressure control for patients with hypertension, and provided economic advantage.

5.Characteristics of Scabies Patients in Goyang City and Transmission Route.

BACKGROUND: Scabies is an ectoparasite caused by the mite Sarcoptes scabiei var. hominis, an obligate human parasite. Although its incidence has been decreasing, it is not a rare dermatologic disease and is difficult to diagnosis. OBJECTIVE: To evaluate the clinical characteristics of scabies patients and find out etiologic factors in transmission. METHODS: Two hundred and ninety-five patients who were diagnosed with scabies at the Dermatologic Department, Myongji Hospital, Goyang city, Korea were enrolled. Medical records were evaluated in a retrospective manner. RESULTS: We studied 295 scabies patients who were diagnosed by the mineral oil test or skin biopsy. The mean age of the patients was 58.2 (male: 56.7/female: 61.2), and 74% of patients were over 60 years. There was seasonal variation in occurrence, and scabies was found to be most common in winter. The majority of suspected routes of infection were transmission through convalescent hospitals (38.1%) and caregivers (15.2%). Nosocomial infection and communal living were the major causes. 73.8% of scabies patients had underlying medical disease, such as hypertension (32.2%) and dementia (11.9%). CONCLUSION: Currently, scabies prevalence is still increasing in Goyang city, because of the increased number of convalescent hospitals.

Primary malignant lymphoma of the uterine cervix is very rare. Lymphomas account for 3.5% of all malignant tumors in the females. Approximately, 25% of all malignant lymphomas arise from extranodal sites, most frequently from gastrointestinal tract and skin. Although the incidence of systemic non-Hodgkin's lymphoma (NHL) and extranodal NHL has increased in recent decades, only 1 to 1.5% arises from female genital tract. Primary pelvic sites include ovary, which is the most common site, uterine corpus, cervix, vagina, and vulva. Primary cervical lymphomas account for 0.12~0.6% of all extranodal lymphomas. However, there is still no consensus on the management of cervical lymphomas due to low incidence of the disease and limited experience in the literature. We experienced two cases of primary malignant lymphoma of the uterine cervix. A 41-year-old woman, Ann Arbor stage IE, was treated with laparoscopically assisted vaginal hysterectomy and both salpingo-oophorectomy and 4 cycles chemotherapy. A second case was a 73-year-old postmenopausal woman, Ann Arbor stage III, received five courses chemotherapy without any surgery. In this article, two cases of uterine cervical lymphoma are presented with a review of the available literature.

7.Effects of Gaze Positions on Ocular Vestibular Evoked Myogenic Potentials.

BACKGROUND AND OBJECTIVES: Recent works have demonstrated the existence of ocular vestibular evoked myogenic potentials (OVEMPs), which reflects otolith-ocular reflex. The purpose of this study was to identify an appropriate gaze position to detect OVEMPs produced by air-conducted sound stimulation in healthy subjects. SUBJECTS AND METHOD: Twenty four healthy subjects (35 ears) were included in this study. Surface electromyographic activity was recorded from active electrodes placed inferior to each eye. Stimulation with 500 Hz short tone bursts was used to activate the vestibular end-organs. RESULTS: Sound stimulation evoked negative-positive biphasic responses on both ipsilateral and contralateral eyes, while responses were contralateral eye-dominant. Contralateral eye responses had higher response incidence and larger amplitudes. Altering the direction of gaze generally changed the incidence and size of the inter-peak amplitudes. The higher incidence and larger amplitudes were recorded when the eyes are directed to the superior and ipsilateral side to the sound stimulation. In that gaze position, the amplitude was 5.3 micronV, the first negative peak latency was 10.5 ms and the following positive peak latency was 15.4 ms on the average. CONCLUSION: OVEMPs can be evoked using an air-conducted 500 Hz tone burst and are best recorded contralaterally with a upward gaze towards the source of sound stimulation. Further changes of the test parameters are needed for higher and consistent responses.

8.Measurement of maxillary sinus volume and available alveolar bone height using computed tomography.

PURPOSE: To aid in determining the volume of graft bone required before a maxillary sinus lift procedure and compare the alveolar bone height measurements taken by panoramic radiographs to those by CT images. MATERIALS AND METHODS: Data obtained by both panoramic radiographs and CT examination of 25 patients were used in this study. Maxillary sinus volumes from the antral floor to heights of 5 mm, 10 mm, 15 mm, and 20 mm, were calculated. Alveolar bone height was measured on the panoramic images at each maxillary tooth site and corrected by magnification rate (PBH). Available bone height (ABH) and full bone height (FBH) was measured on reconstructed CT images. PBH was compared with ABH and FBH at the maxillary incisors, canines, premolars, and molars. RESULTS: Volumes of the inferior portion of the sinuses were 0.55+/-0.41 cm 3 for 5 mm lifts, 2.11+/-0.68 cm 3 for 10 mm, 4.26+/-1.32 cm 3 for 15 mm, 6.95+/-2.01 cm 3 for 20 mm. For the alveolar bone measurement, measurements by panoramic images were longer than available bone heights determined by CT images at the incisor and canine areas, and shorter than full bone heights on CT images at incisor, premolar, and molar areas (p< 0.001). CONCLUSION: In bone grafting of the maxillary sinus floor, 0.96 cm 3 or more is required for a 5 mm-lift, 2.79 cm 3 or more for a 10 mm-lift, 5.58 cm 3 or more for a 15 mm-lift, and 8.96 cm 3 or more for a 20 mm-lift. Maxillary implant length determined using panoramic radiograph alone could result in underestimation or overestimation, according to the site involved.

9.Renal Perfusion Image Using Harmonic Ultrasound with Microbble Contrast Agent: Preliminary Study.

PURPOSE: To compare, in terms of their feasibility and normal range, 99mTc-DTPA renal perfusion imaging and renal perfusion imaging using harmonic ultrasound (US) with a microbubble contrast agent for the evaluation of renal perfusion after renal transplantation. MATERIALS AND METHODS: During a six-month period, thirty patients who had received a renal transplant underwent both 99mTc-DTPA renal perfusion imaging and renal perfusion imaging using harmonic US with a microbubble contrast agent. Sonographic renal perfusion images were obtained before and after a bolus injection of the microbubble contrast agent LevovistTM (SH U 508A; Schering AG, Berlin, Germany) every 3 seconds for 3 minutes. Sonographic renal perfusion images were converted into a renal perfusion curve by a computer program and Tpeak of the curve thus obtained was compared with that of the 99mTc-DTPA curve. RESULTS: Average Tpeak of the 99mTc-DTPA renal perfusion curve was 16.2 seconds in the normal group and 39.6 seconds in the delayed perfusion group, while average Tpeak of the sonographic renal perfusion curve was 23.7 seconds and 46.2 seconds, respectively. Tpeak of the sonographic renal perfusion curve showed a good correlation with that of the 99mTc-DTPA curve (correlation coefficient=0.8209; p=0.0001). The cut-off value of Tpeak of the sonographic renal perfusion curve was 35 seconds (sensitivity=90%, specificity=95%). CONCLUSION: In patients who have received a renal transplant, the findings of renal perfusion imaging using harmonic US with a microbubble contrast agent show close correlation with those of 99mTc-DTPA renal perfusion imaging. The optimal cut-off value of Tpeak of the sonographic renal perfusion curve was 35 seconds.

10.A Study of the Monthly Weight Gains of Noncomplicated Pregnancies.

OBJECTIVE: Mean weight gains, standard deviations were calculated for each gestational months and the 10th, 25th, 50th, 75th, and 90th percentiles were determined. And the influence of prepregnancy body mass index and/or parity on monthly weight gain was investigated. METHODS: We analyzed the monthly weight gain data from 876 women who had healthy sigleton term pregnancy in Han-il Hospital(Jul 2001-Jun 2002). Data were categorized in three groups according to the prepregnancy body mass index. RESULTS: The monthly weight gain table has been determined and 'Sigmoid(S) shaped' monthly weight gain curve was presented. Primipara and multipara have less weight gain than nullipara in the 5th and the 7th-11th months. Monthly weight gains adjusted for parity and age were significantly different among prepregnancy body mass index groups from the 4th month. CONCLUSIONS: More careful prenatal care according to the monthly weight gain-gestational month tables is needed to prevent a lot of complications which may be associated with abnormal weight gain during pregnancy.
