1.Survival Benefit of Adjuvant Chemotherapy in Patients with Pancreatic Ductal Adenocarcinoma Who Underwent Surgery Following Neoadjuvant FOLFIRINOX

Purpose:
The benefit of adjuvant chemotherapy following curative-intent surgery in pancreatic ductal adenocarcinoma (PDAC) patients who had received neoadjuvant FOLFIRINOX is unclear. This study aimed to assess the survival benefit of adjuvant chemotherapy in this patient population.
Materials and Methods:
This retrospective study included 218 patients with localized non-metastatic PDAC who received neoadjuvant FOLFIRINOX and underwent curative-intent surgery (R0 or R1) between January 2017 and December 2020. The association of adjuvant chemotherapy with disease-free survival (DFS) and overall survival (OS) was evaluated in overall patients and in the propensity score matched (PSM) cohort. Subgroup analysis was conducted according to the pathology-proven lymph node status.
Results:
Adjuvant chemotherapy was administered to 149 patients (68.3%). In the overall cohort, the adjuvant chemotherapy group had significantly improved DFS and OS compared to the observation group (DFS: median, 13.8 months [95% confidence interval (CI), 11.0 to 19.1] vs. 8.2 months [95% CI, 6.5 to 12.0]; p < 0.001; and OS: median, 38.0 months [95% CI, 32.2 to not assessable] vs. 25.7 months [95% CI, 18.3 to not assessable]; p=0.005). In the PSM cohort of 57 matched pairs of patients, DFS and OS were better in the adjuvant chemotherapy group than in the observation group (p < 0.001 and p=0.038, respectively). In the multivariate analysis, adjuvant chemotherapy was a significant favorable prognostic factor (vs. observation; DFS: hazard ratio [HR], 0.51 [95% CI, 0.36 to 0.71; p < 0.001]; OS: HR, 0.45 [95% CI, 0.29 to 0.71; p < 0.001]).
Conclusion
Among PDAC patients who underwent surgery following neoadjuvant FOLFIRINOX, adjuvant chemotherapy may be associated with improved survival. Randomized studies should be conducted to validate this finding.
2.Effect of Different Types of Foam Rollers on Self-Myofascial Release of the Quadriceps Femoris

Purpose:
This study investigates the impact of self-myofascial release using a foam roller on the quadriceps femoris for pelvic stability.We further compare the effects of a GRID surface Foam Roller (GFR), a Non-Vibration Foam Roller (NVFR), and a Vibration Foam Roller (VFR).
Methods:
Thirty healthy adults (15 males, 15 females) participated in this study and were randomly assigned to one of three conditions:GFR, NVFR, or VFR. Participants walked at self-selected speeds with an arm sling before and after foam roller stretching. The analyzed gait parameters included pelvic tilt, pelvic obliquity, and pelvic rotation.
Results:
In the NVFR and VFR groups, there was a Significant differences were obtained in the pelvic tilt between pre-test and post-test values (p< 0.05) in the NVFR and VFR groups, but no significant difference was observed in the GFR group (p> 0.05). Comparing the amount of change between the three groups exhibited a significant decrease in pelvic tilt in the NVFR and VFR groups compared to the GFR group (p< 0.017). No significant differences were found in pelvic obliquity and pelvic rotation (p> 0.05) in all groups.
Conclusion
While walking, the use of a VFR for self-myofascial release results in pelvic alteration by reducing the anterior pelvic tilt. We propose that a foam roller can be utilized to enhance pelvic stability during gait.
3.Head-to-Head Comparison between Xpert MTB/RIF Assay and Real-Time Polymerase Chain Reaction Assay Using Bronchial Washing Specimens for Tuberculosis Diagnosis

Background:
With the introduction of Xpert MTB/RIF assay (Xpert), its incorporation into tuberculosis (TB) diagnostic algorithm has become an important issue. The aim of this study was to evaluate the performance of the Xpert assay in comparison with a commercial polymerase chain reaction (PCR) assay.
Methods:
Medical records of patients having results of both Xpert and AdvanSure TB/NTM real-time PCR (AdvanSure) assays using the same bronchial washing specimens were retrospectively reviewed.
Results:
Of the 1,297 patients included in this study, 205 (15.8%) were diagnosed with pulmonary TB. Using mycobacterial culture as the reference method, sensitivity of the Xpert assay using smear-positive specimens was 97.5%, which was comparable to that of the AdvanSure assay (96.3%, p=0.193). However, the sensitivity of the Xpert assay using smear-negative specimens was 70.6%, which was significantly higher than that of the AdvanSure assay (52.9%, p=0.018). Usng phenotypic drug susceptibility testing as the reference method, sensitivity and specificity for detecting rifampicin resistance were 100% and 99.1%, respectively. Moreover, a median turnaround time of the Xpert assay was 1 day, which was significantly shorter than 3 days of the AdvanSure assay (p<0.001).
Conclusion
In comparison with the AdvanSure assay, the Xpert assay had a higher sensitivity using smear-negative specimens, a shorter turnaround time, and could reliably predict rifampin resistance. Therefore, the Xpert assay might be preferentially recommended over TB-PCR in Korean TB diagnostic algorithm.

Background:
Remdesivir is widely used for the treatment of coronavirus disease 2019 (COVID-19), but controversies regarding its efficacy still remain.
Methods:
A retrospective cohort study was conducted to evaluate the effect of remdesivir on clinical and virologic outcomes of severe COVID-19 patients from June to July 2020. Primary clinical endpoints included clinical recovery, additional mechanical ventilator (MV) support, and duration of oxygen or MV support. Viral load reduction by hospital day (HD) 15 was evaluated by calculating changes in cycle threshold (Ct) values.
Results:
A total of 86 severe COVID-19 patients were evaluated including 48 remdesivirtreated patients. Baseline characteristics were not significantly different between the two groups. Remdesivir was administered an average of 7.42 days from symptom onset. The proportions of clinical recovery of the remdesivir and supportive care group at HD 14 (56.3% and 39.5%) and HD 28 (87.5% and 78.9%) were not statistically different. The proportion of patients requiring MV support by HD 28 was significantly lower in the remdesivir group than in the supportive care group (22.9% vs. 44.7%, P = 0.032), and MV duration was significantly shorter in the remdesivir group (average, 1.97 vs. 5.37 days; P = 0.017). Analysis of upper respiratory tract specimens demonstrated that increases of Ct value from HD 1–5 to 11–15 were significantly greater in the remdesivir group than the supportive care group (average, 10.19 vs. 5.36; P = 0.007), and the slope of the Ct value increase was also significantly steeper in the remdesivir group (average, 5.10 vs. 2.68; P = 0.007).
Conclusion
The remdesivir group showed clinical and virologic benefit in terms of MV requirement and viral load reduction, supporting remdesivir treatment for severe COVID-19.
5.Recognition of Physical Activity between Physical Therapy

Purpose:
This study was to the investigate recognition of physical activity between physical therapy students (PTS) and non-physical therapy students (NPTS) by measuring the level of physical activity using International Physical Activity Questionnaires (IPAQ).
Methods:
A cross-sectional survey was completed by 191 university students. The IPAQ with an additional question (Is physical activity necessary for your future job?) was used to evaluate the recognition and the amount of physical activity. The collected data were calculated as MET-minutes scores and were classified as walking, moderate, and vigorous level of physical activity. The students were analyzed by dividing them into those who had a part-time employment (16 PTS and 12 NPTS) and those who did not have a part-time employment (80 PTS and 83 NPTS).
Results:
In students with a part-time employment, no significances were observed between the PTS and NPTS, in terms of MET, frequency and time of physical activity, and sitting time (p > 0.05). In students without a part-time employment, the NPTS was significantly higher than the PTS for the MET and frequency of physical activity in a vigorous level (p < 0.05), and there were no significant differences in other levels of physical activity (p > 0.05). In the additional question, the PTS showed a slightly higher than the NPTS (p < 0.05).
Conclusion
The physical therapy students did not remarkable barrier to recognition of physical activity, but there was a difference in their recognition of the vigorous level of physical activity. Therefore, the understanding of physical activity for PTS would play an important role in the recognition of how physical activity can be promoted.

7.Clinical Analysis of 139 Cases of Benign Skin Tumors on the Fingers in Korea

Background:
Patients with undifferentiated masses on the finger are frequently encountered in the clinic. However, dermatology literature is scarce regarding these tumors.
Objective:
To investigate the clinical characteristics of digital benign tumors.
Methods:
We retrospectively reviewed clinical photographs and medical records of 139 patients who visited Kangbuk Samsung Hospital for digital tumors confirmed by skin biopsy between January 2013 and August 2019.
Results:
The mean patient age was 47.6 years and the male-to-female ratio was 1:1.27. The most common digital tumor was mucous cysts, accounting for 32.37% of the total number, followed by pyogenic granuloma (29.49%) and Masson’s hemangioma (7.19%). The most common digital tumors on the volar and dorsal surfaces were pyogenic granuloma (54.09%) and mucous cyst (55.12%), respectively.
Conclusion
Our study demonstrates that various digital tumors can occur. Further studies with larger patient groups are needed to better understand the incidence or site predilection of digital tumors.

BACKGROUND/AIMS: Several epidemiological studies have validated the association of interleukin gene polymorphisms with acute pancreatitis (AP) in different populations. However, there have been few studies in Asian ethnic groups. We aimed to investigate the relationships between inflammatory cytokine polymorphisms and AP as pilot research in a Korean ethnic group. METHODS: Patients who had been diagnosed with AP were prospectively enrolled. DNA was extracted from whole blood, and DNA sequencing was subsequently performed. Single-nucleotide polymorphisms (SNPs) of the interleukin 1β (IL1B), interleukin 1 receptor antagonist (IL1RN), and tumor necrosis factor α (TNFA) genes of patients with AP were compared to those of normal controls. RESULTS: Between January 2011 and January 2013, a total of 65 subjects were enrolled (40 patients with AP vs. 25 healthy controls). One intronic SNP (IL1RN −1129T>C, rs4251961) was significantly associated with the risk of AP (odds ratio, 0.304; 95% confidence interval, 0.095 to 0.967; p = 0.043). However, in our study, AP was not found to be associated with polymorphisms in the promoter regions of inflammatory cytokine genes, including IL1B (−118C>T, c47+242C>T, +3954C/T, and −598T>C) and TNFA (−1211T>C, −1043C>A, −1037C>T, −488G>A, and −418G>A). CONCLUSIONS: IL1RN −1129T>C (rs4251961) genotypes might be associated with a significant increase of AP risk in a Korean ethnic group.

Administration of a hyaluronic acid (HA) filler injection is a common aesthetic procedure widely used for facial soft-tissue augmentation. Owing to their low immunogenicity, hyaluronic acid fillers have rarely been linked with serious long-term adverse effects. Although generally safe, reports of adverse events linked to hyaluronic acid filler injection are increasing following its extensive usage globally. We describe the case of a 33-year-old woman who presented with multiple facial nodules at sites of filler injection. To the best of our knowledge, this is the first reported case of facial pseudocyst related to monophasic hyaluronic acid filler injection, in Korea.
