Background:
There has been no comparison of the determinants of admission route between acute ischemic stroke (AIS) and acute myocardial infarction (AMI). We examined whether factors associated with direct versus transferred-in admission to regional cardiocerebrovascular centers (RCVCs) differed between AIS and AMI.
Methods:
Using a nationwide RCVC registry, we identified consecutive patients presenting with AMI and AIS between July 2016 and December 2018. We explored factors associated with direct admission to RCVCs in patients with AIS and AMI and examined whether those associations differed between AIS and AMI, including interaction terms between each factor and disease type in multivariable models. To explore the influence of emergency medical service (EMS) paramedics on hospital selection, stratified analyses according to use of EMS were also performed.
Results:
Among the 17,897 and 8,927 AIS and AMI patients, 66.6% and 48.2% were directly admitted to RCVCs, respectively. Multivariable analysis showed that previous coronary heart disease, prehospital awareness, higher education level, and EMS use increased the odds of direct admission to RCVCs, but the odds ratio (OR) was different between AIS and AMI (for the first 3 factors, AMI > AIS; for EMS use, AMI < AIS). EMS use was the single most important factor for both AIS and AMI (OR, 4.72 vs. 3.90). Hypertension and hyperlipidemia increased, while living alone decreased the odds of direct admission only in AMI;additionally, age (65–74 years), previous stroke, and presentation during non-working hours increased the odds only in AIS. EMS use weakened the associations between direct admission and most factors in both AIS and AMI.
Conclusions
Various patient factors were differentially associated with direct admission to RCVCs between AIS and AMI. Public education for symptom awareness and use of EMS is essential in optimizing the transportation and hospitalization of patients with AMI and AIS.

High load is concentrated on Satba wearing area of Ssireum athletes during the tournament. Muscular injuries in this area may seriously affect the athletic performance. We report a case of a 21-year-old Ssireum athlete who was experiencing pain in his left thigh (Satba wearing area) during a tournament due to the migrating mass effect of a partial ruptured semitendinosus muscle occurred approximately 6 months before. Herein is described the clinical results after surgical intervention with an added review of the relevant literature.

PURPOSE: Chlormadinone acetate (CMA) therapy for benign prostatic hyperplasia (BPH) may lower the serum prostate specific antigen (PSA) level. However, little is known about the effect of CMA on the total or free serum PSA levels of PSA. Such information would be important since PSA testing is useful for prostate cancer screening. Thus, we prospectively studied the effect of CMA therapy on the total and free serum PSA levels. MATERIALS AND METHODS: The patients with lower urinary tract symptoms (LUTS) and BPH who were aged over 50 years were treated with 50mg CMA for 6 months. Men with a PSA level greater than 10ng/ml were excluded to reduce the likelihood of including cases of occult prostate cancer. Those with suspicious findings on the digital rectal examination and serum PSA testing were biopsied to rule out prostate cancer. alpha- blocking agents were permitted to treat the men with LUTS. Serum levels of the total and free PSA were measured at the study baseline and after approximately 3 and 6 months. The prostate volume (PV) was assessed by transrectal ultrasonography. RESULTS: The analysis included 170 patients with a mean age of 67.9 years, a baseline PV of 47.3ml and a baseline total PSA of 4.1ng/ml. The total PSA levels declined from 4.1ng/ml at baseline to 2.0ng/ml after 6 months of treatment (50.7% decrease, p<0.01). The mean percent free PSA (21% to 22% at baseline) was not significantly altered by CMA treatment. The PSA levels and PV at baseline did not affect the rate of decline of PSA. CONCLUSIONS: The total PSA serum levels decreased by an average of 50% during CMA therapy, but the percent free PSA did not change significantly. This information is potentially useful in the interpretation of the PSA data that's used for early detection of prostate cancer in the men receiving CMA.

4.Effect of autotrasfusion device affecting allotransfusion after total knee arthroplasty.

OBJECTIVES: We tried to find out the use of allogeneic blood transfusion can be reduced after total knee arthroplasty, when autotransfusion device is used. MATERIALS AND METHODS: Seventy one patients who were performed unilateral total knee arthroplasty (TKA) from January 2001 to February 2005 by one surgeon were selected. They were divided into 2 groups. First group of 29 patients were those who did not use the autotransfusion device and second group of 42 patients were those who used the autotransfusion device in postoperative period. Retrospective analysis was done. First group received allogeneic blood transfusion by a standard level of postopera - tive hemoglobin. Second group were reinfused as much blood as collected by autotransfusion suction bag and then received allogeneic blood transfusion by a standard level of postoperative hemoglobin. Postoperative hemoglobin, hematocrit, total blood loss, total amount of blood transfusion in group 1, 2 were analyzed. These categories were statistically analysed. RESULT: Total blood loss and amount of blood transfusion were almost the same in two groups. The mean amount of allotransfusion was 77.1+/-66.3 cc in group II and 440.7+/-252.7 cc in group I. It showed statistically less amount in group II (multiple regression test, P=0.04). Other factors such as age, sex and disease categories, etc did not show the statistical correlation. CONCLUSION: Using autotransfusion device reduced the mean 363.6 cc of allotransfusion after unilateral TKA. Therefore Autotransfusion device seemed to be a good method to reduce allotransfusion for TKA.

An adenosquamous carcinoma of the penis is an exceedingly rare, highly malignant tumor, with histological features of a squamous cell carcinoma and an adenocarcinoma, which originates in the penile surface epithelium, and possibly in embryologically misplaced mucus glands of the perimeatal region of the glans mucosa. We report a case of an adenosquamous carcinoma of the penis in a 73-year-old man who had been suffering from a painful ulcerative mass on his penis for 2 years.

6.Effects of Risperidone on the Schedule-Induced Polydipsia in Rats.

OBJECTIVES: This study was designed to evaluate the effects of risperidone on the schedule-induced polydipsia (SIP) which is one of animal model of obsessive-compulsive disorder in rats. We administered risperidone as a serotonin and dopamine blocking agent, fluoxetine as a selective serotonin reuptake inhibitor, and haloperidol as a dopamine antagonist to rats which showed schedule-induced polydipsic behaviour. METHODS: Sprage-Dawley rats weighing 200 - 250gm were individually housed and main-tained and allowed free access to water. The rats were placed on a restricted diet. To induce polydipsia, rats were placed in the cage where a pellet dispenser automatically dispensed 90mg pellets on a fixed-time 60 seconds (FT 60s) feeding schedule over 150 minute test session per day. Water was available at all times in the cage. After 4 weeks of daily exposure to the FT 60s feeding schedule, experimental rats met a predetermined criterion for polydipsic behavior (greater than 3 times of water per session on average). 5 groups of rats were administered risperidone (0.1mg/kg, i.p), risperidone (0.5mg/kg, i.p), fluoxetine (5mg/kg, i.p.), haloperidol (0.1mg/kg, i.p.), and vehicle (1cc/kg, i.p. ) for 3 weeks. The rats were tested once a week to access schedule induced polydipsic behavior. Water bottles were weighed before and after the 150-minute test session. The chronic effects of administration of experimental drugs on schedule induced polydipsic behavior were analyzed with ANOVA and Scheffe test as a posthoc comparison. In order to measure water consumption in non-polydipsic food-deprived rats, a separate group of rats (N=8) was individually housed and given a single bolus (14.5gm) of food per day which maintained them at their average body weight. RESULTS: The results were as follows; 1) After 4 weeks of scheduled feeding procedure, the experimental group showed significant differences than the bolus control in the amount of water consumption as compared with their average water intakes for 4 weeks. At the same periods, there were no differences between the experimental group and the bolus control in the body weight. 2) The fluoxetine group showed significant decrease in the amount of water intake at 1st, 2nd, and 3rd weeks of drug treatment as compared with their average amount of polydipsic water intakes. The risperidone 0.1mg group and the risperidone 0.5mg group showed significant decrease in the amount of water intake at the 3rd weeks of drug treatment as compared with their baseline of polydipsic water intakes. However, the haloperidol group and the vehicle control group showed no changes of amounts of water intake for 3 weeks of treatment as compared with their baseline of polydipsic water intakes. 3) The fluoxetine group (22.5+/-10.4ml) showed significantly lower amounts of water intake than haloperidol group (41.3+/-7.1ml) at 2nd weeks of drug treatment. And also the fluoxetine group (18.8+/-3.5ml) showed significantly lower amounts of water intake than the haloperidol group (35.0+/-11.7ml) and the vehicle control (34.4+/-6.8ml) at 3rd weeks of drug treatment. The risperidone 0.1mg group and the risperidone 0.5mg group showed significantly lower amounts of water intake than the haloperidol group (35.0+/-11.7ml) at 2nd weeks and the vehicle control (37.5+/-12.5 , 34.4+/-6.8ml) at 2nd and 3rd weeks of drug treatment. CONCLUSIONS: Above findings suggest that the fixed time feeding procedure for schedule induced polydipsia could be applied as an effective animal model of obsessive compulsive disorder for the evaluation of pharmacological challenge study. We confirmed that chronic treatment with risperidone revealed antipolydipsic effect as effective as fluoxetine on the schedule-induced polydipsic behaviour but the onset of effect was later than fluoxetine.

7.Two Cases of Membranous Glomerulopathy Associated with Psoriasis Vulgaris.

Membranous glumerulonephrophy was found in two men who were diagnosed as psoriasis vulgaris. In the first patient, membranous glumerulonephropathy was developed during treatment of psoriasis and deterioration of proteinuria was appeared with aggravation of skin lesion although treated with cyclophosphamide. In the second patient, psoriasis developed during treatment of membranous glomerulonephropathy and we observed the improvement of nephrotic syndrome and psoriasis with prednisolone treatment. The simultaneous deterioration and improvement of proteinuria and skin lesion of psoriasis suggests that underlying common immune abnormalities may be involved in the pathogenesis of both psoriasis and membranous glomerulopathy.

Enterococcus is a normal flora of the gastrointestinal or genitourinary tract. With the increased use of vancomycin and third generation cephalosporins, vancomycin-resistant enterococci (VRE) have become one of the major nosocomial pathogens in USA and Europe since 1986. In Korea, patients with VRE infection or colonization were increasingly reported recently and VRE may become a serious nosocomial pathogen in the near future. So we report a case of vancomycin-resistant E. faecium peritonitis in a patient on continuous ambulatory peritoneal dialysis.

9.A Study on Serum Lipid Levels of Children in Pusan.

This is a cross-sectional study to evaluate the serum lipid levels of children living in Pusan in 1996. The distribution of age is from 2 years old to 12 years old. In female children mean total cholesterol is 171.4+/-26.2mg/dl, triglyceride is 104.7+/-50.6mg/dl, HDL-cholesterol is 54.4+/-14.8mg/dl, and LDL-cholesterol is 95.4+/-32.9mg/dl. In male children mean total cholesterol is 167.9+/-25.2mg/dl, triglyceride is 90.6+/-45.5mg/dl, HDL-cholesterol is 55.4+/-11.7mg/dll, and LDL-cholesterol is 94.4+/-23.6mg/dl. The percentile of serum lipid levels is measured in children. The 95th percentile of serum total cholesterol is 210mg/dl in male children, and 214mg/dl in female children. And, the 95th percentile of serum triglyceride is 184mg/dl in male children, and 191mg/dl in female children. And, the 95th percentile of LDL-cholesterol is 133mg/dl in male children, and 135mg/dl in female children. Serum total cholesterol is positively related to age(r=0.18), height(r=0.08), weight(r=0.17), obesity index(r=0.12), and negatively related to father's education level(r=-0.13), mother's education level(r=-0.13). Serum triglyceride is positively related to weight(r=0.23), age(r=0.31) and negatively related to father's education level(r=-0.12), mother's education level(r=-0.18). Serum HDL-C was positively related to mother's education level(r=0.07) and negatively related to height(r=-0.12), weight(r=-0.09). Conclusively, the serum lipid levels of children living in Pusan is generally so high that the family and school must try to control the serum lipid levels.
