1.The diagnostic accuracy of depth prediction for lipomas by preoperative imaging with distribution according to anatomical site in Korea: a retrospective analysis

Background:
Lipomas are common benign connective-tissue tumors that usually present as slow-growing, painless, subcutaneous masses. Deeper variants, such as intramuscular, intermuscular, and submuscular lipomas, are larger and rarer. Accurate preoperative depth determination is crucial for planning appropriate surgical resection.
Methods:
We retrospectively reviewed 190 lipoma cases treated at a single medical center from January 2013 to August 2023. The accuracy of preoperative imaging techniques–ultrasonography (USG), computed tomography (CT), and magnetic resonance imaging (MRI)–in predicting lipoma depth was assessed.
Results:
USG, CT, and MRI showed accuracies of 72.5%, 56.5%, and 79.3%, respectively, with MRI showing the highest predictive accuracy. The trunk was the most common site for lipomas (49.5%), followed by the upper (20.5%) and lower extremities (13.2%). USG was more accurate for lipomas in the lower extremities and neck, whereas CT was less accurate for lipomas in the trunk.
Conclusion
MRI is preferable for the preoperative depth assessment of lipomas, especially those located in the trunk. Accurate imaging is essential for guiding surgical planning and avoiding complications. Further studies with larger sample sizes are required to validate our findings.

Purpose:
This study evaluated the postoperative quality of life (QoL) after various types of gastrectomy for gastric cancer.
Materials and Methods:
A multicenter prospective observational study was conducted in Korea using the Korean Quality of Life in Stomach Cancer Patients Study (KOQUSS)-40, a new QoL assessment tool focusing on postgastrectomy syndrome. Overall, 496 patients with gastric cancer were enrolled, and QoL was assessed at 5 time points: preoperatively and at 1, 3, 6, and 12 months after surgery.
Results:
Distal gastrectomy (DG) and pylorus-preserving gastrectomy (PPG) showed significantly better outcomes than total gastrectomy (TG) and proximal gastrectomy (PG) with regard to total score, indigestion, and dysphagia. DG, PPG, and TG also showed significantly better outcomes than PG in terms of dumping syndrome and worry about cancer. Postoperative QoL did not differ significantly according to anastomosis type in DG, except for Billroth I anastomosis, which achieved better bowel habit change scores than the others. No domains differed significantly when comparing double tract reconstruction and esophagogastrostomy after PG. The total QoL score correlated significantly with postoperative body weight loss (more than 10%) and extent of resection (P<0.05 for both).Reflux as assessed by KOQUSS-40 did not correlate significantly with reflux observed on gastroscopy 1 year postoperatively (P=0.064).
Conclusions
Our prospective observation using KOQUSS-40 revealed that DG and PPG lead to better QoL than TG and PG. Further study is needed to compare postoperative QoL according to anastomosis type in DG and PG.
3.Gynecologic oncology in 2024:breakthrough trials and evolving treatment strategies for cervical, uterine corpus, and ovarian cancers

This review summarized the results of clinical trials in 2024 that were believed to have a significant impact on clinical practice in the field of gynecologic oncology. The SHAPE trial, INTERLACE and KEYNOTE-A18 trials, and BEATcc and COMPASSION-16 trials were included in early-stage, locally advanced, and recurrent/metastatic cervical cancer, respectively. For uterine corpus cancer, updated survival data of the four trials (NRG-GY018, RUBY, AtTEnd, DUO-E) for endometrial cancer and the first survival data of LMS-04 trial for leiomyosarcoma were described. For ovarian cancer, the final overall survival results of PRIMA study were followed by DUO-O, ATHENA-combo, and FIRST-ENGOT-OV44 trial in different disease conditions. Finally, the results of DESTINY-PanTumor02, a basket trial of trastuzumab deruxtecan, were briefly addressed.

10.Mitochondria Activity and CXCR4Collaboratively Promote the Differentiation of CD11c + B Cells Induced by TLR9 in Lupus

Lupus is characterized by the autoantibodies against nuclear Ags, underscoring the importance of identifying the B cell subsets driving autoimmunity. Our research focused on the mitochondrial activity and CXCR4 expression in CD11c + B cells from lupus patients after ex vivo stimulation with a TLR9 agonist, CpG-oligodeoxyribonucleotide (ODN). We also evaluated the response of CD11c + B cells in ODN-injected mice. Post-ex vivo ODN stimulation, we observed an increase in the proportion of CD11chi cells, with elevated mitochondrial activity and CXCR4 expression in CD11c + B cells from lupus patients. In vivo experiments showed similar patterns, with TLR9 stimulation enhancing mitochondrial and CXCR4 activities in CD11chi B cells, leading to the generation of anti-dsDNA plasmablasts. The CXCR4 inhibitor AMD3100 and the mitochondrial complex I inhibitor IM156 significantly reduced the proportion of CD11c + B cells and autoreactive plasmablasts. These results underscore the pivotal roles of mitochondria and CXCR4 in the production of autoreactive plasmablasts.
