1.Study on in vitro anti-inflammatory effects and mechanisms of oblongifolins A extracted from Garcinia oblon-gifolia

OBJECTIVE To investigate the in vitro anti-inflammatory effects and mechanisms of oblongifolins A (OA) extracted from Garcinia oblongifolia. METHODS RAW264.7 cells were used as the research subject and divided into control group (0.5% DMSO), lipopolysaccharide (LPS) group (1 μg/mL), DEX group (10 µmol/L DEX+1 μg/mL LPS), and low-, medium-, and high-concentration groups of OA (7.5, 15, 30 µmol/L OA+1 μg/mL LPS). Except for the control group, the remaining groups were first stimulated with LPS for 1 hour and then mixed with drugs for 24 hours. The morphological changes of cells were observed in each group. The contents of nitric oxide (NO), reactive oxygen species (ROS), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), IL-1β, IL-4 and IL-10 were detected in cells of each group; mRNA expression levels of TNF-α, IL-6 and IL-1β were measured. The expression of key proteins in the nuclear factor κB (NF-κB) and nuclear factor-erythroid 2-related factor 2 (Nrf2) signaling pathways in each group, as well as the nuclear translocation of NF-κB p65 and Nrf2 proteins in control group, LPS group and OA high-concentration group, were detected. RESULTS Compared to the LPS group, the number of spindle-shaped and irregular cells gradually decreased in OA groups, the contents of NO, ROS (except for OA low-concentration group), TNF-α, IL-6 and IL-1β, the mRNA expressions of TNF-α, IL-6 (except for OA low-concentration group) and IL-1β as well as the protein expressions of phosphorylated NF-κB p65 (p-NF-κB p65), p-IκBα, and Kelch-like ECH-associated protein 1 (Keap1) were decreased significantly (P<0.05). The contents of IL-4 and IL-10, protein expressions of IκBα, Nrf2 (except for OA low- and medium-concentration groups), HO-1 (except for OA low-concentration group) and NQO1 were all increased significantly (P<0.05). OA of high concentration could inhibit NF-κB p65 protein nuclear translocation and promote Nrf2 protein nuclear translocation. CONCLUSIONS OA can suppress LPS-induced inflammation in RAW264.7 macrophages. The underlying molecular mechanism likely entails the inhibition of the NF-κB signaling pathway, the activation of the Nrf2 signaling pathway and the reduction of ROS and inflammatory factor release.

This paper summarises the nursing experiences with a lung cancer patient who suffered Grade 4 mixed hepatotoxicity characterised with cholestasis.Summarised key care measures include close observation of liver biochemical indexes,timely and safe administration of immunosuppressants,implementation of skincare procedure,prevention of opportunistic infections,individualised exercises,nutrition care,sleep care and psychological care.The patient was discharged with oral immune inhibitors after 29 days of hospitalisation when the hepatotoxicity had been improved and down to Grade 1.In the second month of follow-up,the patient had a well-recovered liver function.
3.Expert consensus on the whole process management of bladder perfusion for bladder cancer

Bladder perfusion is one of the main methods for the treatment of bladder cancer. In order to further improve the standardization of bladder cancer bladder perfusion operation for nursing staff, this paper, guided by evidence-based methods, formed the expert consensus on the whole process management of bladder perfusion for bladder cancer through Delphi expert consultation and expert demonstration meeting, and provided guidance for the standardization of clinical nursing practice and management institutionalization of bladder cancer bladder perfusion from seven aspects, namely, perfusion environment, operators, drug allocation, operation process, adverse reactions, health education and continuous nursing.
4.Research progress in the diagnosis and treatment of Brucella spondylitis

Brucellosis is a zoonotic disease caused by Brucella. Its typical clinical manifestations include fever, chills, fatigue, bone and muscle pain, etc. Brucellosis can affect multiple organs and tissues, of which spine is the most common affected part, forming Brucella spondylitis. Due to the clinical manifestations and imaging characteristics of infected individuals being similar to other spinal diseases, it is easy to cause misdiagnosis, missed diagnosis, and mistreatment. This article reviews the latest research progress in clinical manifestations, imaging examination, diagnosis, differential diagnosis, and treatment of Brucella spondylitis both domestically and internationally, in order to provide reference for the diagnosis and treatment of Brucella spondylitis.
5.Relationship between visceral fat related index and the severity of acute pancreatitis

Objective To investigate the relationship between visceral fat related index and the severity of acute pancreatitis (AP). Methods A total of 308 patients hospitalized with AP at the First Affiliated Hospital of Guangxi Medical University from September 2014 to October 2021 were included. They were divided into mild acute pancreatitis (MAP) ( n =186), moderate severe acute pancreatitis (MSAP) ( n =60) and severe acute pancreatitis (SAP) ( n =62) for comparison in age, hospitalization cost and days, scoring systems and body mass indexes. Comparison of normally distributed continuous data with homogeneity of variance between groups was made by one-way analysis of variance, and intergroup and intragroup pairwise comparison of data with heterogeneity of variance was made by the Kruskal-Wallis H test. The receiver operating characteristic curves (ROCs) for each index were constructed and area under the curve ( AUC ) was calculated to evaluate the performance of each index. Univariable and multivariable logistic regression analyses were used to identify the independent risk factors of MSAP and SAP. Results There were significant differences among the three groups in terms of hospitalization costs and durations, TG, HDL-C, NLR, WBC, Alb, Cr, BUN, scoring systems, CMI, LAP, WTI and CVAI. Further pairwise comparisons revealed that CMI, LAP, WTI and CVAI were significantly higher in the MAP group than in MSAP and SAP groups. We also found correlation between CMI and the severity of AP ( r =0.352, P < 0.001). By comparing the AUC s, CMI was found to be the most accurate in predicting the occurrence of MSAP and SAP. Univariable logistic regression analysis showed that CMI, LAP, WTI, CVAI and WC were the risk factors of MSAP and SAP. After adjusting for confounding factors, CMI and CVAI were identified as the independent risk factors of MSAP and SAP. The risk of MSAP and SAP with CMI ≥ 0.801 was 3.740 times that with CMI < 0.801 (95% CI : 1.983~7.056, P < 0.001). Conclusions Visceral fat is related to the severity of AP. Among the four visceral fat related indexes (CMI, LAP, WTI and CVAI), cardiometabolic index is the most valuable in predicting the severity of AP and they are positively correlated. CMI, an independent risk factor for MSAP and SAP, can be used to predict and assess the severity of AP.
6.Survey on blood donation related self-efficacy and blood donation willingness with different demographic characteristics

【Objective】 To investigate the effect of blood donation related self-efficacy on blood donation intention, in order to provide valuable reference for formulating the recruitment and service strategy for blood donors with different demographic characteristics. 【Methods】 A questionnaire was designed based on psychological self-efficacy theory, and respondents in Nanchong city were taken as the research objects. The data of demographic characteristics, blood donation related self-efficacy measurement and blood donation intention measurement of the respondents who were randomly selected by convenient sampling method were collected and analyzed by rank sum test. 【Results】 The number of previous blood donation was significantly correlated with blood donation related self-efficacy and blood donation intention, and there was a strong positive correlation between blood donation related self-efficacy and blood donation intention (r=0.618). There are significant differences in blood donation related self-efficacy or blood donation intention of respondents with different blood donation experiences (P<0.05), and respondents who donated blood twice or more showed relatively strong blood donation related self-efficacy and blood donation intention. Significant differences in blood donation related self-efficacy or blood donation intention among respondents were noticed in gender, age or education level(P<0.05), and males or 30~39 years old respondents, or respondents with bachelor degree or above have relatively strong blood donation related self-efficacy and blood donation intention. Significant differences in blood donation related self-efficacy were noticed among respondents with different occupations (P<0.05), but no significant difference was noticed in blood donation intention (P>0.05). 【Conclusion】 Blood donation intention can be increased by enhancing the blood donation related self-efficacy of respondents. We can focus on the recruitment of respondents with high self-efficacy in the areas covered by the survey during the practice of blood donor recruitment and service to achieve higher recruitment efficiency. In the meantime, special attention should also be paid to the people with relatively low self-efficacy and high-quality services should be provided t to expand blood donor team. Furthermore, blood donor recruitment and services can be appropriately expanded to the countryside as needed.
7.Practice of experience of "Internet + nursing service" mode carried out by tumor specialist nurses: a qualitative research

Objective:This study was to explore "Internet + nursing service" mode, the tumor specialist nurses nursing service experience and feelings, in the process of mining "Internet + nursing service" model of current existing problems and future development direction, discussed under the background of constructing a new mode of medical and health services, how to expand the tumor specialized subject "Internet + nursing service" the breadth and depth to improve the quality of life of cancer patients and nursing service benefit.Methods:Through the review of the literature, the focus and direction of the research were determined. A semi-structured interview was conducted with 12 oncology nurses enrolled in "Internet + nursing service" by qualitative research. Colaizzi seven-step method was used to encode, classify and extract the data.Results:The experience of "Internet + nursing service" carried out by oncology nurses can be summarized into the following five themes: Be more willing; professional identity of specialist nurses is higher (self-identity, patient identity, social identity); Multiple psychological experience (stress experience: the limitation of specialized operation and the pressure of communication; Adaptive experience: "Specialist cooperation and two-person home visit mode" is well adapted). The time and economic cost of specialist services are higher. The matching degree of service item supply and market demand is insufficient.Conclusions:In order to further expand the depth and breadth of "Internet + nursing service" in oncology, and to benefit more cancer patients in the long-term treatment and rehabilitation process, it is necessary to improve the recognition degree of nurse specialty, so as to improve the willingness to work; increase nurse training to enhance nurses' comprehensive coping ability; improving policy and economic support based on the characteristics of tumor diseases; improve the oncology "Internet + nursing services" project.
8.Research progress on the relationship between the gut microbiota and geriatric diseases

The human gut is populated by a complex community of microbiota that coexists with the host to maintain homeostasis.Macrogenomic and metabonomics analysis shows that the imbalance in gut microbiota composition and diversity, some bacterial components, metabolites and the signaling pathways mediated by them are involved in the occurrence and development of a variety of diseases, especially in the elderly.Intestinal microorganisms are one of the important factors affecting age-related diseases and can provide insights on treatment strategies for the elderly.This review focuses on the composition of the intestinal microbiota, interaction between the microbiota and the host, features of the gut microbiota in common geriatric diseases and pathogenic mechanisms.
9.Longitudinal study of limb functional exercise compliance after breast cancer surgery based on dependent curve

Objective:To investigate the level and change of compliance of functional exercise in patients with breast cancer in one year after operation.Methods:The compliance of limb functional exercise in 90 patients with breast cancer at 3, 6 and 12 months after operation was measured by the compliance scale of functional exercise after operation of breast cancer. Taking time as independent variable( x), functional exercise compliance and index of each dimension as dependent variable( y), regression curve estimation equation was used to draw compliance curve. Results:The functional compliance index of patients at 3, 6, 12 months after surgery was 67.31±14.21, 62.93±12.53, 56.62±14.75, there was significant difference( F value was 31.13, P<0.01). The physical exercise compliance index of patients at 3, 6, 12 months after surgery was 63.06±17.69, 58.03±14.86, 50.83±16.75, there was significant difference ( F value was 30.82, P<0.01). The active recommendation index of patients at 3, 6, 12 months after surgery was 58.56±19.75, 48.56±17.94, 42.94±18.69, there was significant difference( F value was 4.99, P<0.05). The post-consideration index of patients at 3, 6, 12 months after surgery was 81.95±12.70, 83.30±13.25, 78.10±16.55, there was significant difference ( F value was 38.26, P<0.01). The compliance curve of the functional exercise compliance score was y=50.22-1.08 x-0.69 x2 ( R2 value was 0.091, F value was 13.426, P<0.01); the body exercise compliance curve formula was y=23.73-0.64 x-0.39 x2 ( R2 value was 0.086, F value was 12.522, P<0.01); the actively seeking the recommendation compliance curve formula was y=11.67-2.652 x+0.35 x2 ( R2 value was 0.106, F value was 15.911, P<0.01); the postoperative cautionary compliance curve formula was y=14.82+2.22 x-0.65 x2 ( R2 value was 0.023, F value was 3.188, P<0.05). The functional compliance of breast cancer patients during the first year after operation was moderate, and then showed a downward trend. Among them, physical exercise compliance and active seeking compliance showed a low level, and a downward trend. But the postoperative attention complied with a high level. And a trend of a small increase after 6 months after surgery. Conclusions:According to the compliance curve, the overall compliance of postoperative functional exercise in breast cancer patients is poor, and the compliance has a general downward trend with time. This may be related to the low attention of patients, family members and health care workers after discharge or the patient′s own "forgotten curve".Therefore, after discharge, the nurse needs to repeatedly strengthen the patient′s memory, and follow up and supervise the patient according to the key points of the patient′s functional exercise and the reasons for the decline of compliance.
10.Effect of complete decongestive therapy in severe grade Ⅲ lower limb lymphedemabv

Objective:To investigate the effect of complete decongestive therapy in the treatment of severe grade Ⅲ lower limb lymphedema.Methods:From January 2018 to December 2018, The patients were admitted to the lymphedema clinic of the cancer prevention and treatment center of Sun Yat-sen University, seven patients with severe gradeⅢ lower limb lymphedema were intervened with complete decongestive therapy, including problem skin care, special techniques of unarmed lymphatic drainage, foam block bandage combined with air wave pressure therapy, filled elastic bandage pressure bandage, functional exercise combined with home bare-handed lymphatic drainage and weight loss management. The intervention had two courses, and 20 times was a course of treatment. Perimeter measurement and weight measurement were used to evaluate the therapeutic effect at the 0, 5, 10, 15, 20, 30, 40 times of treatment.Results:After two courses of treatment, the circumference value of each measurement point on the affected side was lower than that before treatment, and the difference was statistically significant ( F values were 7.449-23.073, P < 0.05). The circumference value of the affected side decreased by 7.10 - 24.53 cm, and the weight after treatment was 9.0 - 20.5 kg less than that before treatment. During the follow-up period, it was found that the lower extremity diameter value at 3 months of follow-up at 5 sites increased and tended to be stable compared with that at 1 year of follow-up. Conclusion:Step 6 comprehensive swelling treatment can obviously improve the symptoms of patients with lower limb lymphedema of this research, and in the subsequent follow-up found that self-therapy at home, no recurrence or aggravate the limb swelling degree, and can reduce or stability treatment effect, enhance confidence in the treatment of patients, so as to improve the quality of life of patients and treatment compliance.
