ObjectiveTo investigate the therapeutic effect of sequential administration of Dihuang Baoyuan granules (DHBY, the prescription for consolidating body resistance) and Fuling Yunhua granules (FLYH, the prescription for treating symptoms) on spontaneous type 2 diabetes mellitus (T2DM) in mice. MethodsAccording to the fasting blood glucose (FBG) level, 12-week-old db/db mice were randomized into six groups: model, DHBY (18.02 g·kg-1), FLYH (14.80 g·kg-1), sequential administration 1 (SEQ-1, DHBY 18.02 g·kg-1+FLYH 14.80 g·kg-1), sequential administration 2 (SEQ-2, FLYH 14.80 g·kg-1+DHBY 18.02 g·kg-1), and dapagliflozin (Dapa, 1.3 mg·kg-1). The m/m mice in the same litter were selected as the normal group. The mice were administrated with corresponding drugs by gavage for 8 consecutive weeks. During the 8 weeks of drug administration and 2 weeks after withdrawal, the retinal thickness, FBG, hemoglobin A1c (HbA1c), and insulin were determined, and histopathological changes of the pancreas, liver, kidney, and retina were observed by hematoxylin-eosin (HE) staining. ResultsCompared with the model group, SEQ-1 for 4 weeks lowered the FBG level (P<0.05), raised the insulin level, decreased the triglyceride (TG) level (P<0.05), increased the number of optic ganglion cells and diminished vacuolar degeneration of pancreatic islet and liver. SEQ-2 lowered FBG and HbA1c levels (P<0.05), rose the insulin level, increased the retinal thickness and the number of optic ganglion cells (P<0.05), and alleviated vacuolar degeneration of pancreatic islet and liver. Two weeks after drug withdrawal, Dapa tended to increase FBG and HbA1c compared with those at the time of drug withdrawal. However, the levels of FBG and HbA1c in the SEQ-2 group remained decreasing (P<0.05). ConclusionSEQ-1 and SEQ-2 can lower the blood glucose level and ameliorate diabetic retinopathy, and SEQ-2 outperformed DHBY and FLYH in lowering the blood glucose level. Moreover, SEQ-2 can maintain the blood glucose-lowering effect after drug withdrawal.

This article summarized the clinical experience of MEI Guoqiang in treating lung cancer of phlegm-heat obstructed in the lung syndrome with modified Xiaoxianxiong Decoction (小陷胸汤). It is believed that the key pathogenesis of lung cancer with phlegm-heat obstructed in the lung syndrome is the phlegm-heat toxin accumulation. According to the different pathogenic effects of qi stagnation, blood stasis, pathogenic toxin, phlegm-damp, qi deficiency, yin deficiency in the occurrence and development of the disease, it is advocated to clear heat and resolve phlegm, and additionally the methods of diffusing the lung and relieving cough, resolving toxins and dissipating masses, rectifying qi and activating blood, dispelling dampness, supplementing and boosting qi and yin are used if necessary. Multiple methods are used together and flexibly matched. In clinical practice, Xiaoxian-xiong Decoction with the function of clearing heat and relieving phlegm is recommended as the basic formula for further modification. For patients with mild lung symptoms, modified Xiaoxianxiong Decoction is commonly used, while for those with obvious symptoms, self-made Maxing Xianxiong Decoction(麻杏陷胸汤) in modifications is suggested. For patients with Shaoyang (少阳) diseases, modified Chaihu Xianxiong Decoction (柴胡陷胸汤) is often used.

Objective:To investigate the correlation of endoscopy findings with symptoms in patients undergoing gastroscopy. Methods:Patients who underwent gastroscopy for the first time in Renji Hospital of Shanghai Jiao Tong University School of Medicine from January to December 2017 were included in the questionnaire survey. The participants were asked to report their main symptoms of the digestive tract in details, and gastroscopy was completed within 2 weeks. Final gastroscopic diagnosis was made based on both gastroscopy and pathology, then patients were divided into the major-lesion (peptic ulcer and malignancy) group and the non-major-lesion (chronic gastritis, reflux esophagitis and others) group. The correlation of gastrointestinal symptoms with gastroscopic findings was analyzed. The risk for major gastroscopic lesions (peptic ulcer and malignant tumors) was assessed by multivariate Logistic regression analysis. Results:A total of 5 885 patients completed the questionnaire, 5 496 (93.4%) of whom completed gastroscopy. The detection rates of peptic ulcer were 12.3% (373/3 028), 12.6% (52/412), 17.9% (49/273), 9.5% (64/675) and 9.2% (102/1 108) in patients with dyspepsia, reflux, alarm, other symptoms and no symptoms, respectively, the detection rates of malignant tumors were 1.2% (36/3 028), 0.7% (3/412), 7.7% (21/273), 0.7% (5/675) and 0.4% (4/1 108), respectively in these patients. The most common symptoms was dyspepsia, accounted for 58.3% (373/640) and 52.2% (36/69) patients with peptic ulcer and malignant tumors, respectively. Alarm symptoms were found in 30.4% (21/69) patients with malignant tumors, and 15.9% (102/640) peptic ulcer patients and 5.8% (4/69) malignant tumor patients had no gastrointestinal symptoms. Compared to asymptomatic individuals, patients with dyspepsia ( P<0.001, OR=1.52, 95% CI: 1.21-1.92) and those with warning symptoms ( P<0.001, OR=2.87, 95% CI: 2.02-4.08) had significantly increased risk for major lesions (peptic ulcer and malignant tumors) detected by gastroscopy. Conclusion:Although dyspepsia and alarm symptoms are positively associated with upper gastrointestinal malignancy and peptic ulcer, they are of limited predictive value for upper gastrointestinal diseases.

In order to verify the correlation between Polygonum multiflorum-induced liver injury and HLA-B*35 : 01 alleles, six hospitalized patients diagnosed with Polygonum multiflorum-induced liver injury (PM-DILI) were selected, and their clinicopathological data were collected. Simultaneously, blood HLA-B* 35 : 01 allele detection was performed. Among the six PM-DILI cases, 4 were male, aged 38.83 ± 10.13 years old. The types of liver injury were hepatocellular injury types in all, and the severity of liver injury in five cases was grade 3. The histological presentations were acute hepatitis and acute cholestatic hepatitis. PM-DILI cases were all HLA-B*35:01 carriers, with a carrier rate of 100%. This finding indicates that PM-DILI is significantly correlated with HLA-B*35:01 alleles. Therefore, HLA-B*35 : 01 alleles can be used as an important predictive indicator for PM-DILI.

OBJECTIVE:To observe the ef fectiveness and safety of bosentan in the treatment of hypoxic pulmonary hypertension(HPH)in neonates. METHODS :From Jan. 2014 to Mar. 2019,a total of 82 HPH neonates hospitalized in the department of neonatology of our hospital were selected as research subjects. According to whether or not receiving bosentan therapy,50 cases were included into bosentan group and 32 cases into non-bosentan group. Meanwhile ,another 25 non-HPH neonates with serum sample retention time and general information such as gestational age at birth and day age matching the HPH group were selected as the control group. All neonates with HPH were given continuous intravenous infusion of Dopamine hydrochloride injection 5 mg/(kg·min)until PASP was normal. On this basis, neonates in the bosentan group were additionally given Bosentan tablets 1 mg/kg(fed after dissolving with appropriate amount of water for injection )for q 12 h,72 h. The relationship between serum ET- 1 levels of neonates with HPH and PASP was analyzed ,as well as PASP before and after treatment and therapeutic efficacy between bosentan and non-bosentan groups ,the changes of arterial blood gas indexes and ADR in 3 groups were compared. RESULTS :Before treatment ,the serum ET- 1 levels of bosentan group was (164.3±115.3)pg/mL,which was significantly higher than (41.9±3.7)pg/mL of control group and positively correlated with PASP level (r=0.864,P<0.001). Total response rate of bosentan group was 90.00%,which was significantly higher than 71.88% of non-bosentan group (P<0.05). After 72 h of treatment ,PASP of 2 groups was decreased significantly ,compared with before treatment (P<0.001),and the bosentan group was significantly lower than the non-bosentan group (P<0.05). The PaO 2,SaO2,PaCO2 and OI in 3 groups was significantly improved compared with that before treatment (P<0.001),and the PaO 2,SaO2 and OI in the bosentan group was significantly higher than that in the non-bosentan group (P<0.05). During the treatment period of bosentan and within one week after drug withdrawal, there was no significant change in serum LDH ,AST,ALT and Scr levels in neonates. There was no statistically significant difference in the incidence of feeding intolerance ,anemia, reduced WBC and reduced PLT in 3 groups(P>0.05). CONCLUSIONS: Bosentan can improve the oxygenation status of neonates with HPH, reduce PA SP,and short-termmedication is safe. com
