1.Different Effects of Fresh and Dried Dendrobium Huoshanense on Chronic Atrophic Gastritis

ObjectiveTo compare the protective effects of water extracts from fresh and dried Dendrobium huoshanense on gastric mucosa in chronic atrophic gastritis (CAG). MethodsMale SD rats (n=72) were randomly divided into 9 groups, with 8 rats in each group, which were normal group, model group, Yangwei Shu (4 g·kg-1) group, low-, medium-, and high-dose fresh D. huoshanense (3.5, 7, and 14 g·kg-1) groups, and low-, medium-, and high-dose dried D. huoshanense (0.7, 1.4, 2.8 g·kg-1) groups. The CAG rat model was successfully established by inducing with N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) and other factors for a total of 11 weeks. Then, the rats were intervened with fresh and dried D. huoshanense for 4 weeks. The serum and gastric tissues of the rats were collected. The changes in gastric juice secretion volume and gastric acid pH value in each group were observed. The gastric mucosal injury was observed by naked eyes and hematoxylin-eosin(HE) staining. The gastric mucus secretion level was determined by Alcian blue and periodic acid-Schiff staining(AB-PAS) staining. The expression levels of tight junction proteins Occludin and ZO-1 in gastric tissues were determined by immunofluorescence. The expression levels of serum pepsinogen Ⅰ (PG Ⅰ), pepsinogen Ⅱ (PG Ⅱ), gastrin 17 (G-17), interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) were determined by enzyme-linked immunosorbent assay (ELISA). The expression levels of aquaporin 1 (AQP1), aquaporin 3 (AQP3), and aquaporin 4 (AQP4) in gastric tissues were determined by Western blot. ResultsCompared with the normal group, the model group showed an obviously reduced gastric juice secretion volume (P0.05), significantly increased gastric acid pH value (P0.01), gastric mucosa with obvious atrophy, and a significantly reduced gastric mucus secretion volume (P0.01). The expression of Occludin and ZO-1 in the gastric mucosal barrier was significantly decreased (P0.01). The levels of PG Ⅰ and PG Ⅱ in the serum were obviously decreased (P0.05, P0.01), and the levels of G-17, IL-1β, IL-6, and TNF-α were significantly increased (P0.01). The expression level of AQP1 in the gastric tissue was significantly upregulated (P0.01), and the expression levels of AQP3 and AQP4 were significantly downregulated (P0.01). Compared with the model group, each drug administration group could improve the gastric mucosal atrophy of CAG model rats to varying degrees, obviously increase the gastric juice secretion volume of the model rats (P0.05, P0.01), significantly decrease the gastric acid pH value (P0.01), obviously increase the gastric mucus secretion volume (P0.05, P0.01), obviously decrease the expression levels of G-17, IL-6, IL-1β, and TNF-α (P0.05, P0.01), obviously increase the expression levels of Occludin, ZO-1, PG Ⅰ, and PG Ⅱ (P0.05, P0.01), obviously upregulate the expression levels of AQP3 and AQP4 (P0.05, P0.01), and obviously downregulate the expression level of AQP1 (P0.05, P0.01). ConclusionThe water extracts of fresh and dried D. huoshanense can exert therapeutic effects on CAG by improving gastric mucosal injury, reducing inflammation, and regulating water metabolism. Moreover, the dried D. huoshanense has a better effect.
2.Effects of retinopathy on visual function in type 2 diabetes mellitus

Objective:To study the influence of the severity of diabetic retinopathy (DR) on the visual function of patients with type 2 diabetes, to provide scientific basis for the early prevention and control of DR.Methods:This study was designed as a cross-sectional study, recruiting already-diagnosed type 2 diabetes patients in four community health service centers in Guizhou Province between February and September 2022. Employing the Chinese version of the Visual Function Index-14 (VF-14), assess the participants′ near vision, visual adaptation, subjective visual perception, and stereo vision, with higher scores indicating poorer visual function. Categorize the severity of each eye′s damage into no diabetic retinopathy (DR), mild non-proliferative diabetic retinopathy (NPDR), moderate NPDR, severe NPDR, and proliferative diabetic retinopathy (PDR), and use a 5-level DR grading system to evaluate the overall severity of diabetic retinopathy in both eyes. Employing linear regression analysis to investigate the linear relationship between DR and visual function index. Local weighted regression evaluates the nonlinear relationship between the DR composite score and the scores of visual function, with a steeper slope indicating poorer visual function for that level.Results:A total of 542 patients with type 2 diabetes were investigated, including 244 (45.02%) males, 298 (54.98%) females, and 162 (29.89%) patients with DR. After adjusting for confounders, compared with those without DR, patients with binocular DR Had overall scores ( β=0.136, P=0.003), near vision ( β=0.163, P<0.001), visual adaptation ( β=0.092, P=0.042), subjective vision ( β=0.120, P=0.009) and stereo vision ( β=0.094, P=0.044) were higher than those without DR. There were no differences in visual functions between DR And monocular DR. The local weighted regression curve showed that near vision (slope: 23.78) and overall score (slope: 58.37) increased sharply from mild to moderate NPDR in both eyes. Visual adaptation (slope: 5.37, 7.72), subjective vision (slope: 6.53, 7.93), stereovision (slope: 0.74, 0.91) increased slowly in mild to moderate NPDR in both eyes and in moderate to severe NPDR/PDR in both eyes. Conclusion:Binocular DR is associated with impaired visual function, but there is no difference between monocular DR And non-DR visual function. The early damage of DR To visual function is mainly manifested in near vision. In the prevention and control of DR, more attention should be paid to visual function, especially the change of near vision, and retinal damage should not be assessed solely by visual status.

4.Plasma D-dimer Can Effectively Predict the Prospective Occurrence of Ascites in Advanced Schistosomiasis Japonica Patients.

China still has more than 30,000 patients of advanced schistosomiasis while new cases being reported consistently. D-dimer is a fibrin degradation product. As ascites being the dominating symptom in advanced schistosomiasis, the present study aimed to explore a prediction model of ascites with D-dimer and other clinical easy-achievable indicators. A case-control study nested in a prospective cohort was conducted in schistosomiasis-endemic area of southern China. A total of 291 patients of advanced schistosomiasis were first investigated in 2013 and further followed in 2014. Information on clinical history, physical examination, and abdominal ultrasonography, including the symptom of ascites was repeatedly collected. Result showed 44 patients having ascites. Most of the patients' ascites were confined in the kidney area with median area of 20 mm². The level of plasma D-dimer and pertinent liver function indicators were measured at the initial investigation in 2013. Compared with those without ascites, cases with ascites had significantly higher levels of D-dimer (0.71±2.44 μg/L vs 0.48±2.12 μg/L, P=0.005), as well ALB (44.5 vs 46.2, g/L) and Type IV collagen (50.04 vs 44.50 μg/L). Receiver operating characteristic curve analyses indicated a moderate predictive value of D-dimer by its own area under curve (AUC) of 0.64 (95% CI: 0.54–0.73) and the cutoff value as 0.81 μg/L. Dichotomized by the cutoff level, D-dimer along with other categorical variables generated a prediction model with AUC of 0.76 (95% CI: 0.68–0.89). Risks of patients with specific characteristics in the prediction model were summarized. Our study suggests that the plasma D-dimer level is a reliable predictor for incident ascites in advanced schistosomiasis japonica patients.

5.Investigation analysis of training methods and stratification training demands in nursing staff

Objective To explore the stratification training and training methods demands for different level nursing staff, so as to provide evidence for constructing competence advanced management model for nursing staff.Methods Self-designed nursing staff′s training demands scale were arranged 256 clinical nursing staff except further study people, nursing staff working in Auxiliary Department, nursing staff with sick leave, maternity leave. Results In 256 nursing staff with different level, different level nursing staff had different demands on course demands and training methods, but it also existed some cross connections; they chose different training methods for different courses.Conclusions After investigation and analysis, the reliable plan has been made out to provide evidence for nursing staff competence advanced management model and to guide nursing team sustainable development.
6.The value of dual energy computed tomography for the diagnosis of gout

Objective To evaluate the value of dual energy computed tomography (DECT) for the diagnosis of gout.Methods Twenty-fivc consccutive patients with gout were selected as the experimental group and 20 patients with rheumatoid arthritis (RA) were selected as the control group.DECT scans were performed for every patient (all peripheral joints for the experimental group and affected joints for the control group).And every patient with gout had X-ray examination of the involved joints.x2 test and t-test were used for statistical analysis.Results All 25 patients with gout showed urate deposits on their DECT scans,whereas none of 20 controls showed urate deposits (P<0.01).DECT scans revealed a total of 184 areas of urate deposition in 25 patients,in which 107 (58.2%) were at feet and ankle,72 (39.1%) were at knees,4 (2.2%) were at hands and wrists,1 (0.5%) was at elbows,whereas physical examination only showed 64 areas of urate deposition,in which 38(59.4%) were at feet and ankle,24(37.5%) were at knees,2(3.1%) at hands and wrists,0 was at elbows(P<0.01 ).Only 6 patients with gout showed nonspecific manifestations on X-ray.Conclusion DECT scans may have potential value for the diagnosis of gout since it could produce evident colour displays for urate deposits and help to identify subclinical tophus deposits.
7.Preventive effect of artemether on schistosome infection

Objective To study the preventive effect of artemether (Art) in protecting the people from schistosome infection during flood fighting in schistosomiasis endemic area of Poyang Lake, Jiangxi Province. Methods From mid July to mid August in 1996, the water level in Poyang Lake rose due to torrential rains and 2 embankments, Zhedi and Jiangtongdi, which appeared in dangerous situation and were selected as the pilot spots. After those who went to fight against flood arrived at the pilots their sera were collected within 48 hours and were examined with indirect hemagglutination test (IHA), enzyme-linked immunosorbent assay (ELISA) and McAb-ELISA. Individuals with negative outcome in the 3 tests were then selected as the study subjects and were allocated randomly to the Art or the control group. The first dose of Art given to the individuals contacted with the infested water within 11-15 days was 6 mg/kg. If the individual continually contacted the infested water, the same dose of Art was given once every 15 days. After the individuals withdrew from the pilot, one more dose of Art was administered 7-15 days later. Placebo (starch) was given to individuals in the control group at the same period as in artemether group. Stool examinations were made in both groups 40-50 days after the last medication for evaluation of the preventive effect of artemether. Double blind method was used in the administration of both artemether and placebo. Results In Zhedi pilot, the individuals fought against flood for about 1 month. In Art group, 99 individuals receiving 3 doses of the drug completed the stool examination with egg-positive rate of 4% and no acute schistosomiasis was seen. In the control group, among 110 people who completed the observation, 44 were egg-positive with an infection rate of 40%, and 29 were identified as having acute schistosomiasis. In Jiangtondi, the studied individuals contacted the infested water for only about 4 hours. But in the control group 4 out of 102 individuals were egg-positive, while none of the 103 individuals in Art group receiving 2 doses of the drug showed schistosome infection. No apparent side effect was seen in the people treated with artemether.Conclusion After oral Art was given to the people fighting against flood in schistosomiasis endemic area of Poyang Lake, it was shown that the oral Art has a promising effect on controlling acute schistosomiasis and reducing the infection rate.
