1.Effect of thymalfasin combined with ganciclovir eye drops (gel) on herpes simplex virus epithelial keratitis

Objective:To investigate the efficacy of thymalfasin combined with ganciclovir eye drops (gel) in the treatment of herpes simplex virus epithelial keratitis.Methods:This study enrolled 80 patients with herpes simplex virus epithelial keratitis who were treated at the Department of Ophthalmology, Fenyang Hospital, between January 2019 and December 2020. These patients were randomly divided into a control group and an observation group, with 40 patients in each group using the random number table method. All patients were treated with ganciclovir eye drops (gel) for 3 weeks. The control group received conventional treatment, while the observation group received both conventional treatment and thymalfasin treatment. All patients were followed up for 2 years. The proportion of patients who achieved a significant response and an effective response within 10 days of treatment, as well as the recurrence rate, were compared between the two groups. Additionally, the changes in cellular immune function were compared between the two groups.Results:In the control group, there were 10 cases that achieved a significant or effective response within 10 days of treatment, while in the observation group, there were 28 cases. The therapeutic effect of the observation group was superior to that of the control group, and the difference was statistically significant ( χ2 = 16.24, P < 0.001). The recurrence rate of the observation group within 2 years was significantly lower than that of the control group (10.00% vs. 30.00%, χ2 = 5.00, P < 0.05). After treatment, the counts of CD3 +, CD4 +, CD4 +/CD8 +, and natural killer cells, which are indices of cellular immune function, were significantly increased in the observation group compared with both their pre-treatment levels and the levels measured concurrently in the control group ( tbetween-group= 19.27, 20.85, 11.32, 15.82, all P < 0.001). Conclusion:The combination of thymalfasin and ganciclovir eye drops (gel) for the treatment of herpes simplex virus epithelial keratitis can improve both the therapeutic effect and the immune function of patients, which helps to reduce the recurrence rate.

Goniotomy (GT) is a safe and effective type of minimally invasive glaucoma surgery (MIGS) extensively utilized in China.It is particularly suited for treating primary open-angle glaucoma and advanced primary angle-closure glaucoma.Although GT is generally safe, hyphema and postoperative intraocular pressure (IOP) spikes remain common complications after GT.Currently, there is no standardized protocol for managing these issues, which can impact clinicians' assessment of surgical outcomes and potentially affect the prognosis.Therefore, it is crucial to establish comprehensive and detailed management protocols for perioperative hyphema and IOP spike following GT.This will guide clinical practitioners in managing complications appropriately and systematically, thereby promoting the further development and refinement of MIGS.To address these concerns, several domestic glaucoma treatment experts along with members of the Glaucoma Society of Ophthalmology, Guangdong Medical Association reviewed existing literature and held recommendation meetings to develop a guideline for managing perioperative hyphema and IOP spikes following GT.It includes defining perioperative hyphema in GT, discussing its high-risk factors and outcomes, exploring methods for prevention prior to surgery and techniques to reduce bleeding during the procedure, and managing postoperative hemorrhage.Additionally, it covers defining IOP spikes after GT, investigates their causes and contributing factors, and outlines management strategies and anticipated outcomes to provide a valuable resource for clinicians.
3.Dignity therapy and hospice care for the elderly

Dignity therapy is a new and individualized psychotherapy to improve dignity for terminally ill patients and reduce psychological distress of their family and friends.In China, the practice and research concerning dignity therapy for elderly patients in hospice care are still in an exploratory stage.Here we summarize research on the implementation of dignity therapy, in order to provide a theoretical basis for its practice in hospice care for the elderly.
4.Identification of concurrent infection with Jaagsiekte sheep retrovirus and maedi-visna virus in China

Objective:
To investigate the pathological changes and conduct viral gene analysis of OPA and MVD co-occurrence in Inner Mongolia, China.
Methods:
Using gross pathology, histopathology, immunohistochemistry, ultrastructural pathology, PCR, and sequence analysis, we investigated the concurrent infection of JSRV and MVV in 319 Dorper rams slaughtered in a private slaughterhouse in Inner Mongolia, in 2022.
Results:
Of the 319 rams included, 3 showed concurrent JSRV and MVV infection. Gross lung pathology showed diffuse enlargement, consolidation, and greyish-white miliary nodules on the lung surface; the trachea was filled with a white foamy fluid; hilar and mediastinal lymph nodes were significantly enlarged. Histopathology results revealed typical OPA and MVD lesions in the lung tissue. Immunohistochemical results were positive for JSRV envelope protein (Env) in the tumor cells and MVV CA in alveolar macrophages. Transmission electron microscopy showed several virions and autophagosomes in the lung tissue, severely damaged mitochondria, and the induced mitophagy. Nucleotide sequences obtained for JSRV env and MVV gag showed the highest homology with the Inner Mongolian strains of JSRV env (JQ837489) and MVV gag (MW248464).
Conclusions
and Relevance: Our study confirmed that OPA and MVD co-occurrence and identified the pathological changes in Inner Mongolia, China, thereby providing references for the identification of concurrent JSRV and MVV infections.
5.Risk factors of drug resistance in diabetes mellitus complicated by pulmonary tuberculosis

Objective:To investigate the risk factors of diabetes mellitus complicated by pulmonary tuberculosis.Methods:The clinical data of 83 patients with diabetes mellitus complicated by pulmonary tuberculosis who received treatment in Taiyuan Fourth People's Hospital from March 2020 to March 2022 were collected. These patients were divided into sensitive group ( n = 45) and resistant group ( n = 38 ) according to the results of drug sensitivity test. Univariate and multivariate non-conditional logistic regression was performed to analyze the influential factors of drug resistance. Results:Univariate logistic regression results revealed that there were significant differences in blood CD4 +T lymphocyte count ( χ2 = 11.73, P = 0.001) and diabetic complications ( χ2 = 4.94, P = 0.026). Multivariate non-conditional logistic regression analysis was performed taking whether blood CD4 +T lymphocyte count was lower than the average level and whether patients with diabetes mellitus had complications as independent variables, and taking whether drug resistance was a dependent variable. The results showed that the OR (95% CI) value of the decreased blood CD4 +T lymphocyte count was 4.909 (1.926-12.514). It is a risk factor for drug resistance of diabetes mellitus complicated by pulmonary tuberculosis. Conclusion:The decrease of blood CD4 +T lymphocyte count is a risk factor of drug resistance in diabetes mellitus complicated by pulmonary tuberculosis, and it should be intervened early in the clinic.
6.Next-generation sequencing for the genetic characterization of Maedi/Visna virus isolated from the northwest of China

Background:
Maedi/Visna virus (MVV) is a contagious viral pathogen that causes considerable economic losses to the sheep industry worldwide.
Objectives:
In China, MVV has been detected in several regions, but its molecular characteristics and genetic variations were not thoroughly investigated.
Methods:
Therefore, in this study, we conducted next-generation sequencing on an MVV strain obtained from northwest China to reveal its genetic evolution via phylogenetic analysis.
Results:
A MVV strain obtained from Inner Mongolia (NM) of China was identified. Sequence analysis indicated that its whole-genome length is 9193 bp. Homology comparison of nucleotides between the NM strain and reference strains showed that the sequence homology of gag and env were 77.1%–86.8% and 67.7%–75.5%, respectively. Phylogenetic analysis revealed that the NM strain was closely related to the reference strains isolated from America, which belong to the A2 type. Notably, there were 5 amino acid insertions in variable region 4 and a highly variable motif at the C-terminal of the surface glycoprotein (SU5).
Conclusions
The present study is the first to show the whole-genome sequence of an MVV obtained from China. The detailed analyses provide essential information for understanding the genetic characteristics of MVV, and the results enrich the MVV library.
7.Preventive and therapeutic effects of common plant drugs on colon cancer and its mechanism

It is shown that phytochemicals have a protective effect on colon cancer. Curcumin, polysaccharides (apple polysaccharides, mushroom glucans), saponins (paridis saponins, ginsenosides), resveratrol, quercetin and other plant drugs can inhibit colon cancer cell proliferation and promote cell apoptosis through different signaling pathways. In addition, it also has anti-inflammatory, antioxidant, anti-angiogenesis, reduce the toxic side effects of chemotherapy drugs, and reverse the drug resistance of tumor cells. Understan-ding the prevention and cure effect of plant medicine on colon cancer and its possible mechanism can provide more theoretical basis and therapeutic ideas for the clinical prevention and cure of colon cancer.
8.Application of thymalfasin in non-operative treatment of cervical cancer

Objective To observe the effect of thymalfasin on non-operative treatment of cervical cancer.Methods Seventy-eight patients with advanced cervical cancer were divided into two groups according to the digital table,each group in 39 cases.The two groups were treated with concurrent chemoradiotherapy.The observation group was given 1.6 mg of thymalfasin subcutaneously each Monday to Friday.While the control group received no additional treatment.And then the two groups were compared in terms of curative effects,adverse reactions,immune states and cellular immune functions.Results The results showed that the differences of curative effects[there were 23 patients in the control group,as compared with 31 patients in the observation group,achieved a complete response(CR),x2=3.852,P<0.05],adverse reactions(grade Ⅲ or Ⅳ radiation-induced enteritis occurred in 4 patients in the observation group,and 12 patients in the control group,x2=5.032,P<0.05;Grade Ⅲ or Ⅳ leukopenia occurred in 16 patients in the observation group,and 25 patients in the control group,x2=4.165,P<0.05;Grade Ⅲ or Ⅳ nausea,vomit occurred in 6 patients in the observation group,and 14 patients in the control group,x2=4.303,P<0.05),immune states and cellular immune functions between the two groups were significant[the counts of CD+3(t=9.236,P<0.05),CD+4(t=7.445,P<0.05),CD+4/CD+8(t=7.445,P<0.05) and NK(t=9.256,P<0.05)]were significantly higher in the observation group after treatment.Conclusion In the treatment of advanced cervical cancer,concurrent chemoradiotherapy combined with thymalfasin can improve the curative effect,reduce the side effects,improve the quality of life and enhance the immunity.
9.Clinical effect of artificial tear gel combined with diclofenac sodium in the treatment with xerophthalmia

Objective To investigate the clinical effects of artificial tear gel combined with diclofenac sodium in the treatment with xerophthalmia.Methods 110 patients with xerophthalmia were chosen as study subjects,and they were divided into research group(artificial tear gel + diclofenac sodium)and control group(artificial tear gel).BUT,FS,SIT,mental statue,symptom score and clinical effects after 20 days were compared between the two groups.Results The total effective rate of research group was 90.48%,which of the control group was 75.24%,the difference was statistically significant(x2=8.582,P<0.05).The FS[(0.87±0.15)points vs.(1.53±0.84)points]of the research group was lower than that of the control group after treatment,and the BUT[(9.86±1.13)s vs.(5.71±1.27)s],SIT[(6.85±1.59)mm/5min vs.(4.38±2.07)mm/5min]of the research group were higher than those of the control group,the differences were statistically significant(t=7.926,21.352,9.697,all P<0.05).After treatment,the symptom score of the research group was lower than that of the control group[(8.39±3.24)points vs.(12.85±3.42)points],the difference was statistically significant(t=9.701,P<0.05).The SDS score [(37.16±5.63)points vs.(56.49±6.14)points]and SAS score[(35.24±5.38)points vs.(54.30±6.42)points]of the research group were lower than those of the control group after treatment,and the differences were statistically significant(t=23.777,23.317,all P<0.05).Conclusion Artificial tear gel combined with diclofenac sodium can rapidly improve the clinical symptom of xerophthlmia and treatment effect.
10.The clinical feature of glucose transporter 1 deficiency syndrome and literature review

Objective To investigate the clinical features of glucose transporter 1 deficiency syndrome(GLUT1-DS) and summarize the characteristics of GLUT1-DS through reviewing related references.Methods The clinical data including manifestation,cerebrospinal fluid (CSF) glucose,electroencephalogram,MRI and gene mutation of a patient with GLUT1-DS was collected and the related literatures were reviewed.Results The patient was a 6 years old boy.The patient,whose seizures occurred at the age of 9 month-old and prolonged to 6 year-old,attacked before breakfast.Physical examination showed microcephaly with head circumference 47.5 cm.Laboratory tests showed that CSF glucose decreased (1.87 mmol/L) and CSF-serum ratio was 0.36.And meantime the MRI was normal and electroencephalogram showed general spike and slow wave complex paroxysm.Mutation of SLC2A1 gene,c.350_385del,was found in the patient.There were 219 cases with GLUT1-DS had been reported and the age of onset was 15.69 months.In 219 patients,159 cases (72%) suffered seizures,105 cases (47%) had motor abnormalities,61 cases (27%) suffered intellectual disability.The CSF glucose values were (1.92±0.31) mmol/L,CSF-serum ratio was 0.36±0.07.SLC2A1 gene mutations were detected in 183 patients(96%)in which missense mutation was the most mutation.Conclusion A wide range of phenotypes of GLUT1-DS include seizures,motor abnormalities,mental retardation.The diagnosis is confirmed when CSF glucose and CSF-serum ratio are continuously decreased which in the absence of meningitis.The SLC2A1 gene should be detected in suspicion of GLUTI-DS patients.Early diagnosis and treatment may improve the prognosis of those GLUTI-DS patients.
